Jamunamukh Block PHC: Kathiatoli District: Nagaon 37 0 05/10/2012 2012-10-06 11:58:11 450 All the affected persons took common food (Mah prasad and payas)in a religious function in a nearby temple at about 3 PM of 4102012. Following the event some people felt abdominal discomfort and the 1st case with SS of Loose motionvomiting & Fever came to Jamunamukh SD at about 1 AM of 5102012. In the morning hours of 51012 few number of cases with similar complaint came to Jamunamukh SD. All together 37 number of cases admitted in the hospital out of which 22 numbers of patient were referred to the District Hospital.All patients are in recovery state and no death. Stool samples were not taken as patients were already under antibiotic therapy. 1.Patients were treated immediately. 2.Food Inspector collected the sample of food. 3. Health education imparted by the Local Medical Officer. Investigated by RRT Investigation Open Situation is under control. No more cases report at the local hospital

Diphtheria Block-Niphad, PHC-Deogaon, Village-Deogaon, Dist-Nashik. 1 case 1 fatality 25/08/2012 2012-10-06 18:15:33 4945 The Index case(Miss Ashwini Sable, Age-7yr, Sex-Female)had treated by general practionar and ENT given sysptomagtic treatment and immediatly refered to MVP Medical Collage, Nashi. On 230812 patient consulted in Medical Collage, diagnosed as peritonsillar abscess with membrane and taken swab for microscopy and culture, diagnosed as Gram Possitive Bacilli at an angle Suggestive of KLB(C.Diphtheria and immediatly refferd to Civil Hosp. admitted in Infection ward where and adviced to administred ADS.(Anti Diptheria Serum).On 25812at 2.30pm patient declared as dead due to septicemic shock Type of Sample-Membrane Over Left tonsil, Peritonsillar Abscess What test-Microscopy, Gram Stain Lab-Microbiology Dept. MVP Collage, Nashik Result-Gram Possitive Bacilli at an angle Suggestive of KLB(c.Diptheria) Investigated by DRCHO 1)Prophylactic measures: a)Close contact are under observation and administered with erythromycin.(Total Close Contacts 37) b)Total 133 children of age 5 to 6yr has been admistered with DPT B. dose and are under close monitoring 2)Surveillance in school and affected area: Regular house to house surveillance and close monitoring in school Investigation Open No new Patient till date
Gastroenteritis PHC-Borgaon, SC-Borgaon, Village-Borgaon, Block-Surgana. 46 0 28/09/2012 2012-10-06 18:23:17 2501 1. Irregular and Improper techniques used for Chlorination of all the sources. 2. Suspected cause of outbreak may be the pipeline leakage leading to contamination of water. 3. Poor sanitary & disease preventive practices was going in village which may be one of the cause of outbreak. 4. Last 3 months data indicates the presence of disease factor in the affected area. 5. Tribal community having low awareness about safe drinking water & personal hygiene. Water sample-4, Stool sample-2, TCL sample-1 collected and sent to DPHL Result awaited. 1.Emergency Medical unit in PHC 24 *7 for next 7 days 2. Regular chlorination of all sources with double dose of TCL and proper OT monitoring. 3. House to House surveillance for ADD case especially children 0 to 5 yrs and high risk population. 4. Health Education to tribal community focusing on safe water and hygiene practices through gram sabha, banners and miking. 5. Food Poisoning Block-Baglan, RH-Nampur, Jai Bajrang Bali Hostel 30 0 11/09/2012 2012-10-06 18:17:33 21000 On 10 Sept.2012 for dinner Khichadi and on 11 Sept. for breakfast Poha with Tea was consumed by students. After that developed symptoms of nausea, abdominal pain, fever and vomiting. Immediately admitted in RH: Nampur of which 11 patient referred to Civil Hospital Malegaon. 3 vomit samples are sent for bacteriological examination at DPHL Nasik. 1. Isolation ward at RH: Nampur for emergency treatment. 2. Collection of prepared food sample, raw food sample and vomitus sample for laboratory investigation. 3. Health education given to all staff and cooks about personal hygiene, safe practices for fresh & clean food preparation, use of clean chlorinated water etc. Investigation Closed Outbreak Closed on 11092012.

Sample not available The children were treated at Sadar Hospital Sasaram Investigation Closed During mid day meal distribution a wall lizard was shotted in the pulse before that around 25 children had already eaten the meal. The health department, education department and administration were involved. Civil surgeon, District development commissioner, district education officer, teachers of the school made the situation normal and all children are discharged within 4-6 hours.
