There was poor sanitary condition in the affected area. All the cases were normal. Age group 12 to 60 were mostly affected. Males were mostly affected than females. Source of safe drinking water was hand pump. Water sample collected and tested. Metronidazole-I.V., R.L.-I.V., DNS-I.V., NS-I.V., Ciprofloxacin-I.V., Gentamycine inj., Metronidazole tab (400mg.), Metrochlopramide inj., Pantoprazole tab. Investigated by RRT. Most of them villagers are used the local hand pump which is situated near the pound. Most of the villagers were affected at that area. Total person have been suffered from diarrhoea. All were admitted in CHC sahar, District Auraiya three water samples collected from the village. (one india marka hand pump and two local hand pump) District RRT investigated the out break, 03 well disinfected50 ORS packets and 2000 chlorine tablet were distributed Investigation Closed Normal condition of the village.

Viral Haemorragic Fever Manigah, Kupwara 130 cases 2 deaths 01/09/2012 2012-09-03 14:24:32 10000 We used standard clinical case definition of Viral Haemorragic fever (an acute febrile illness of 2-7 days duration with 2 or more of the following symptoms---Headache, Retro-orbital pain,Myalgia,Arthralgia.Rash,Haemorragic manifestation.Leucopenia) . A team consisting of a Epidemiologist, Physician,Microbiologist and, Senoir Health Inspector and support staff visited the village. All the symptomatic patients were examined by the physician and febrile patients showing signs and symptoms of the disease were put on anti pyretics(Paracetemol). We described the outbreak by time place and person. 12 blood samples were taken for IgM Serology for Dengue,Typhi Dot and Malaria card test. 2 blood samples were taken for culture and sensitivity for Salmonella typhi. 2 water samples were taken for MPN Count. Results awaited. Symptomatic treatment was given to the patients (Paracetemol). Health Education imparted Investigation Open

Age Sex distribution of affected persons is : a. (0-4)yrs4 (M2,F2) b. (5-9)yrs19(M6,F13) c. (10-14)yrs13(M7,F6) d. Above 15yrs54(M20,F34) 1. Three Rectal Swab samples of three patients are collected on 06.09.2012 and are sent to Deptt., of Microbiology, AMC, Dibrugarh for investigation. 2. Two water samples are collected from the from the water source of the household for testing. 3. Water sample is also taken for H2S Test Kit and the result shows water contaminated. 1.. Epidemiologist along with the IDSP staffs visited the Gogamukh BPHCCHC where the patients were treated and admitted. The team also visited the household and collect information and water sample for testing. 2.. One Medical Specialist has been also deputed from Dhemaji Civil Hospital to Gogamukh BPHCCHC for emergency and control measures. 3.. 11 patients have been referred to Lakhimpur Civil Hospital on 060912 of which 3 Patients were already discharged after symptomatic treatment. 4. Information about contaminated water is given to Exe, Engg(PHE) , Dhemaji for taking further necessary action in the adjacent area. Investigation Open 1.. The affected persons are young adults, senior citizens, and small childrens. Investigation open. 86 blood smears have been made. The result is awaited Cases of high grade fever, loose motions and vomiting reported from village Jairala Pasi, CHC Amawan, Block Amawan of district Raebareli. The cases were investigated by the epidemic control unit of district Raebareli. House to house survey of fever was done extensively. All the cases were treated symptomatically. Adequate amounts of antipyretics, antidiarrheals, and antiemetics were given to the affected persons. IEC done. Health education given regarding precautions to be taken in high grade fever. 86 blood slides have been made. The villagers were given instructions to use mosquito nets and use antirepellents. Source of Infection is suspected to be from drinking water. Two (2 ) Nos. of death cases occurs in two adjacent households in one campus. One cases died within 5 hours after passing two loose watery stools, died at 1 A.M. on 26-8-12. and another patient died in 5 hours after passing 3 Nos. of watery stools at 6 A.M. on 26-8-12. 14 Nos. of H2S samples collected. One sample collected on 26-8-12 from the house of deceased and found it normal, another 13 Nos sample collected on 27-8-12 . Out of 13 Nos. of H2S 3 Nos. samples failed and disinfection done by chlorination. 14 Nos. of H2S samples collected. Dengue Fever Kalyan Corp.,Dombiwalli,Thakurwadi Arogya Kendra,Shridhar Mhatre Chauk 11 cases 1 fatality 18/06/2012 2012-09-12 17:56:17 2150 There was a death due to fever having the signs and symptoms of Dengue.Sandesh Krishna Bhave,27 years,residing at Raghunath Patil Chawl,Garibachapada,Shridhar Matre Chawk,Dombiwalli was admitted at ICON hospital,Dombiwalli.The RDK test done at the hospital was positive to Dengue.Patient died on 18th June 2012 at 7.30 am.The ICON Hospital had given cause of death as Multi organ dysfunction syndrome due to disseminated intravascular coagulation because of Dengue hemorrhagic fever The RDK Test of the deceased was positive for Dengue positive.3 blood serum samples sent to NIV Pune for further testing.The result from NIV Pune received on 2nd July stated that the deceased patient was ELISA Dengue IgM antibodies positive. IEC activities and Control measures(Spraying and fogging) and Active Daily Surveillance for new fever cases, Breeding places of mosquitoes and destroying the larvae is going on . Malaria PV Shahapur,Vashind PHC,Asangaon SC,Taspada 26 0 25/06/2012 2012-09-13 11:52:13 112 A death of 5 year 11 months old boy occurred at Taspada,Asangaon SC area on 25th morning 1.30 am.The boy had a history of fever with chills and cough.On 22nd June 2012 went to private hospital and the next day MPW of that area took him to PHC and tested for Malaria and found Malaria PV.Given treatment and send home.On 25th morning 1.30 am,he expired at home He had high grade fever ,cough and convulsions during this time(relatives responded).His brother is also found PV malaria positive and is icterus .He is under treatment at SDH Shahapur.The houses are located near the bank of river and waterlogged area around the house. 26 Blood samples taken from that area and tested for Malaria PV & PF and 3 found positive to Malaria PV.Dengue RDK tested for 1 sample and is negative.Lepto RDK also tested and is negative. IEC activities and Control measures(Spraying and fogging) and Active Daily Surveillance for new fever cases, Breeding places of mosquitoes and destroying the larvae was done .District RRT visited. Investigation Closed The death committee(on 5th July 2012),Thane district reviewed on the death of 5 yr 11 month old boy( suspected PV) and confirmed as a death due to ARDS,So the death is not declared as death due to PV Malaria.The Outbreak was reported through SSU to CSu by Email on 27th June 2012

Fever KalyanCorp.Shantinagar Gogarswadi 14 0 27/06/2012 2012-09-13 12:44:00 1952 There was a death due to fever and symptoms of dengue fever at Kalyan corporation.the patient was admitted at AIMS Private hospital Kalyan and blood serum test at a private lab is RDK positive to Dengue. 2 Blood serum sample sent to NIV Pune for further lab investigation is negative to Dengue Fever Active Daily Surveillance for new fever cases, Breeding places of mosquitoes and destroying the larvae done. Investigation Closed The death committee(on 5th July 2012),Thane district reviewed on the death of the patient and decided not to declared it as death due to Dengue since not yet had any confirmed evidence.This Outbreak was reported 3rd July 2007 to SSu and SSU reported it to CSU by Email

There were several valve leakages in this area which lead to mixing of drainage water into the drinking water which results to an outbreak. 3 stool samples & 4 water samples were sent for evaluation to DPHL.3 stool samples were sent to GMC Akola. Stool samples came negative for v.cholarae. RRT has investigated an outbreak.Valve leakages were removed on urget basis. Malaria Shahapur,Kinhavali,Vachkole,Katkari wadi 2 cases 1 fatality 08/08/2012 2012-09-14 13:18:24 116 Renuka Balu Mukane,16year old girl expired at Civil Hospital Thane on 12th Aug 2012.The patient had a two days history of fever and had treatment from a private clinic.On 11th aug at 6.30 pm, the patient came to Kinhavali PHC with fever and head ache and referred to SDH Shahapur and from there referred to Civil Thane.The patient expired on 12th aug 2012 at 8.25 am.After surveillance total fever cases found is 47 47 Blood samples taken from that area and tested for Malaria PV & PF and 2 found positive to Malaria PF.RDK leptospirosis test done and is negative. IEC activities and Control measures(Spraying and fogging) and Active Daily Surveillance for new fever cases, Breeding places of mosquitoes and destroying the larvae is going on .District RRT investigated Investigation Closed The death committee,Thane district reviewed on the death of 16 old girl and confirmed as a death due to Plasmodium Falciparum Malaria(The patients Blood Smear Slide Test at Civil Thane is positive to Malaria PV and when cross verified the Slide by DMO Staff and found PV and PF mix).This outbreak was reported to CSU through SSU via Email on 21st Aug 2012

Dengue Fever Bhiwandi Nijampur Municipal Corp.,Aazmi Nagar Health Post,Kaneri 1 case 1 fatality 06/08/2012 2012-09-14 13:23:58 460 There was a death due to fever having the signs and symptoms of Dengue.Ms.Manashree Shamal,9 years had fever from 4th august and admitted at Meher Pvt hospital Bhiwandi on 6th Aug 2012 ,shifted to Metro hospital Manpada Thane on the same day evening.The patient expired on 7th August 2012 morning 4.45 am.HI-10.86,CI-8.47BI-10.86.Fever patients-3 The deceased patients blood serum(Elisa Dengue IgM) tested at Pooja Path Pvt lab Thane is positive.BS tested for 2 more fever patients in that area and found negative so Blood serum sent to Civil Thane for further test and is negative IEC activities and Control measures(Spraying and fogging) and Active Daily Surveillance for new fever cases, Breeding places of mosquitoes and destroying the larvae done Investigation Closed The death committee declared it as a confirmed Dengue Death .The surveillance activities started from 7th August onwards.The outbreak was reported to CSU through SSU via Email on 23rd Aug 2012.

Fever Omerga,Alur,Kesarjavalga,Kesarjavalga 23 cases 1 fatality 20/03/2012 2012-09-15 13:18:31 4843 A single isolated case of fever since 10-03-2012 Female age 3 years treated by private practioner & afterwards admitted in private Hospital in Omerga. She detrained even after treatment.& Expired on 20-03-2012.The news flash in Sakal news paper on 21-03-12 Immediately RRT team visited the village. Rapid survey was done there was not a single case of fever in the village. Entomological survey was also done which reviles House Index to 5.8 & Contenor Index 3.2 , BR index 8.8. Blood smears of contacts & neibours were taken on 2324 -03-12 total 23.All the smears are negative for MP.Five seream samples were collected from the contacts & neigbors send to Referral lab at Beed. Result Awaited Rapid Survey, Abating has done Medical Team was kept at the village. Investigation Closed Reports of the Private lab of Vaishnavi Balappa Hadape , Female Age 3 years.( Death Case) is attached herewith.

A big pound situated near the villager named Mr. Sameem where causality has happened. The resident of that locality throwing garbage in discriminately in to the water. So water gets polluted. Street filled with mud and animal dunk. Under process (20 BS from village and these BS checked by Malaria department, Result: - under process. 65 patients treated on the spot, 50 ORS, 500 chlorine tablet distributed. Investigation Closed Epidemic team of PHC level continuously watching on that village, for 6 days.

All cases treated symptomatically 3 bottle water samples collected for biologicalcontamination purpose. Result waited. Outbreak reported from village Hijiya, CHC Chakradharpur due to consumption of contaminated water. 2 patients were referred to the Subdivisional Hospital Chakradharpur for better treatment. Block team also reached to the affected locality. All cases treated with anti diarrhoeal drugs and ORS. Active surveillance done. IEC activity undertaken and health education given regarding hygienic and sanitation measures. Situation is under control. Investigation Open 1. Follow up continued. 2. Health education camp held. 3. Surveillance by paramedical going on.
