Water sample: 5 nos of H2S Strip Water bottles have been collected on02112012 at 1.05 P.M. from the house of deceased and his neighbor . two nos found contaminated after 12 hrs. 2. Stool sample: Not taken. as there is no other jaundice cases around the area. 3. Total biliribin found 10.1 in Dispur Polyclinic & Nursing Home, Ghy. 4. S.G.O.T 9891 ul ,S.G.P.T 3423 ul ,HAV IgM 1.8, Dengue IgM reactive found in International Hospital 1. DSO, Epidemiologist , IDSP, ANM of Keokuchi SC visited the house of deceased and adjacent houses on 21112. 2. Health Education about safe drinking water, food hygine, personal hygine and environmental sanitation. 3. Suggestion for boiling water. 4. Distribution of Helogen tab for water purification. Investigation Open The deceased worked in one Central NGO, Mukalmua Branch in Nalbari District. The deceased was treated locally at keokuchi on 19th Oct.’12. There after he was treated at F.A.A. Medical College Hospital , Barpeta .On 21st Oct. ’12 he was admitted in Dispur Polyclinic & Nursing Home, Ghy.He was then admitted in International Hospital ,Ghy 0n 26th Oct. ’12. He expired on 30th.Oct.’12 at about 11 P.M. at International Hospital due to fulminant hepatic failure ( HAV related).

Suspected Hepatitis Block : PandanadPHC : AlaSC : AlaVillage : Ala 5 0 03/09/2012 2012-11-01 13:18:47 14498 Ward 2 identified with clustering of cases.the index case is identified with jaundice symptoms.DRRT Which includes DSO and epidemiologist, HS, Pandanad and local health workers visited Ala ward 2 for investigating the case on 20/10/12On taking the history from the cases it was identified that promiscuity might be the associated risk factor in this present clustering. No history of injections, blood examinations, accidents with blood reception, dental extraction and treatment history. On taking history from the Lathika the home nurse revealed unhealthy sexual relations even from outside areas of the surrounding which include Alappuzha .Rahul and his friends gave history of usage of barber shops periodically. The available lab results are HBsAg card test positives (private lab City lab, Chenganoor). Random blood samples taken and sent to medical microbiology department of Medical College ,Alappuzha for Elisa Igm on 201012 (for standardization of results) under guidance of Distict RRT and awaiting results . Health education given to the field level officers and ASHA workers by DSO and Epidemiologist. Health education talk given in the ward on 201012 by HS Pandanad. Information regarding passing of IEC materials at local labs in the area and also inspection of barbershops advised for effective usage of disposable blades after every customer. Hepatitis B vaccination to risk contacts advised for effective prevention in the ward. Investigation Open Area under surveillance by DSU. Hepatitis B vaccination to risk contacts ensured. The index case didn’t have sufficient lab confirmation report since the cases depended on traditional systems for treatment.

Later the rest of the cases had similar symptoms on 24-10-12.The cases are primarily Class 4 students with average age of 9 yrs. - Isolation of cases strictly advised. PTA meeting conducted under guidance of MO, Vayalar and local health staff and effective health education given. House quarantine for 10 days advised and avoiding playing among neighborhood children strictly advised. School assembly temporarily withheld and also break sessions are cancelled in the school for preventing intermingling among students .The school and wards are under surveillance by the local surveillance team daily for notifying any further cases. Mumps Block :Veliyanad PHC :Kavalam, Neelamperoor 23 0 08/10/2012 2012-11-03 13:02:42 281 The incident case Ramadas developed fever on 81012 and later had developed swelling in the parotid region. Later clustering of cases was noticed from 241012 to 301012 in the school premises principally among children in the same school but from 2 different Panchayaths. Later a female 43 yrs developed similar symptoms (301012) in the household of a case. MMR vaccination is nil among the cases except for as single case - School under surveillance. House quarantine and isolation advised strictly for 14 days for the cases. PTA meeting conducted in school for giving HE under guidance of HI, Neelampeeroor on 3110112. Mobile medical camp conducted for detection of more cases in the respective wards in Kavalam, HE and notice distribution done in affected wards of Kavalam. School assembly and breaks with held to prevent intermingling among children and other members School authorities informed and local health staff posted every day for surveillance of cases in the school. Suspected Hepatitis Block : PandanadPHC : AlaSC : AlaVillage : Ala 5 0 03/09/2012 2012-11-01 13:18:47 14498 Ward 2 identified with clustering of cases.the index case is identified with jaundice symptoms.DRRT Which includes DSO and epidemiologist, HS, Pandanad and local health workers visited Ala ward 2 for investigating the case on 20/10/12On taking the history from the cases it was identified that promiscuity might be the associated risk factor in this present clustering. No history of injections, blood examinations, accidents with blood reception, dental extraction and treatment history. On taking history from the Lathika the home nurse revealed unhealthy sexual relations even from outside areas of the surrounding which include Alappuzha .Rahul and his friends gave history of usage of barber shops periodically. The available lab results are HBsAg card test positives (private lab City lab, Chenganoor). Random blood samples taken and sent to medical microbiology department of Medical College ,Alappuzha for Elisa Igm on 201012 (for standardization of results) under guidance of Distict RRT and awaiting results . Health education given to the field level officers and ASHA workers by DSO and Epidemiologist. Health education talk given in the ward on 201012 by HS Pandanad. Information regarding passing of IEC materials at local labs in the area and also inspection of barbershops advised for effective usage of disposable blades after every customer. Hepatitis B vaccination to risk contacts advised for effective prevention in the ward. Investigation Open Area under surveillance by DSU. Hepatitis B vaccination to risk contacts ensured. The index case didn’t have sufficient lab confirmation report since the cases depended on traditional systems for treatment.

Later the rest of the cases had similar symptoms on 24-10-12.The cases are primarily Class 4 students with average age of 9 yrs. - Isolation of cases strictly advised. PTA meeting conducted under guidance of MO, Vayalar and local health staff and effective health education given. House quarantine for 10 days advised and avoiding playing among neighborhood children strictly advised. School assembly temporarily withheld and also break sessions are cancelled in the school for preventing intermingling among students .The school and wards are under surveillance by the local surveillance team daily for notifying any further cases. Later clustering of cases was noticed from 241012 to 301012 in the school premises principally among children in the same school but from 2 different Panchayaths. Later a female 43 yrs developed similar symptoms (301012) in the household of a case. MMR vaccination is nil among the cases except for as single case - School under surveillance. House quarantine and isolation advised strictly for 14 days for the cases. PTA meeting conducted in school for giving HE under guidance of HI, Neelampeeroor on 3110112. Mobile medical camp conducted for detection of more cases in the respective wards in Kavalam, HE and notice distribution done in affected wards of Kavalam. School assembly and breaks with held to prevent intermingling among children and other members School authorities informed and local health staff posted every day for surveillance of cases in the school.
