Normalcy prevails in the Central Hospital of Maracay

A calm environment prevailed Saturday on the premises of the Central Hospital of Maracay, after this hospital last Thursday was in the eye of the hurricane information.

Following the announcement by the Governor of Aragua, which denied the existence of extraneous viruses and deaths with the same symptoms in HCM, and prior to the complaint made by Angel Sarmiento, president of the College of Physicians on eight suspicious deaths , hospital users were calm and interested in more details of the investigations.

The reactions in the population have been expected according to a survey by the news crew elsiglo in the vicinity of the Central Hospital of Maracay, although there are divided opinions, respondents agreed that the results of the examinations on the deceased should be informed by authorities in Caracas and should not hide any information.

According to Wilmer Valero should make an investigation into the suspicious deaths and wait for the results before passing judgment. "Health is a very sensitive issue and the authorities must act with the seriousness it warrants."

He said he does not know if they are true deaths from the same cause in a short period, however he said that he had taken certain precautions.

For Jos? Rafael L?pez, "pointed out by the Governor of Aragua is true, were random deaths, nevertheless think every death should be investigated." He stressed that he is not afraid of getting a virus, has a week since entering the Central Hospital of Maracay and not suffering from any illness.

Although Roger Boyer said there is normalcy in the Central Hospital of Maracay, said he does not know if you really have arisen deaths with the same symptoms, for it urged the Health Ministry to take the necessary corrective and report the results as soon as possible .

He also said that the status of inputs and resources within the HCM is quite poor, so he invited the national government to invest in health.

It is noteworthy that several people indicated that they preferred not to comment he said, but requested to investigate all deaths that occur Maracay Central Hospital, to prevent alarm.

This is becoming a political mess, but I can't find further test results. Notably, it has been 3 days since the first report, and no one else has died (including any suspected cases under treatment at the time of the first report).

It would be informative to know what treatment those people were given. Antibiotics?

They also called for a general assembly next Monday, September 15, at 5:00 pm, in the spaces of the College of Physicians to specify actions to take to the accusations made against Sarmiento.

"From responsibly complying with the code of ethics, union president alerted the public about several suspicious deaths, clarified that there was some statement of a particular disease, much less bacteria in the Central Hospital of Maracay, so the guild and intertrade support Dr. Angel Sarmiento "he Feder Alvarez.

"As physicians we fulfill the responsibility to warn people about death with the same symptoms and we place the order to address the situation, indeed, there is a situation room at the College of Physicians to cover ten specific points and providing health as is our duty, "they said spokespersons.

We are fighting against a vicious attack over 15 years ago, which has intensified in recent days. "

"Enough that we have to leave the country ... I am ready to assume my responsibilities, but I respect the due process and not the abuse of power." We will require the truths as truths only by requiring me ... criminalizes union Unity, there's no need to fear, no prisons for Aragua 8000 physicians, "he said.

"Fear can it be but the coward can die a thousand times, the brave only once. Me are now criminalized for defending life. Patients, relatives, neighbors, doctors must demand an investigation comes to light" for added.

Sarmiento the question arose over statements by the Governor of Aragua, "a number of medical histories were chosen, I doubt that 8 people were killed in the Central Hospital of Maracay in 15 days, today I are criminalized for alerting the population investigated , take corrective and tell the truth. "

Moreover, the spokesmen of the College of Physicians ruled that there is no intimidation of the relatives of the deceased alleged by the same symptoms. They also requested respect for the profession, as it would be respectful to the treating physicians were victims of persecution.

A ninth death, and a new symptom. Coughing blood combined with all the previous symptoms might suggest plague, which should be the next thing tested for. Those dark purple blisters could be buboes.

I am not aware of any previous plague cases in Venezuela.

This is looking less like a viral hemorrhagic fever.

There are several large black swellings on the child that certainly look like buboes.

Notably, this also stretches the timespan of this outbreak back several weeks.

I have found many articles indicating large amounts of dengue and chikungunya circulating in Aragua state right now, independent of this mystery illness.

Without a diagnosis, it is unclear whether additional cases of the mystery illness are being missed in the dengue and chikungunya outbreaks. At this point, all nine people suspected of having the mystery illness have died. Of course, if the mystery illness is indeed plague, those 1000 other people obviously don't have it.

Nine sporadic cases in what might be eight weeks does not seem like a high frequency.

Determing frequency is relevant to the exposure. For pnemonic plague - would be many more cases in eight weeks. Bubonic plague would be determined by exposure to the vector. Nine cases in several weeks is quite frequent and worrisome considering there would have to be multiple environmental exposures.

On the other hand - considering the possibility plague is to blame - the known cases may only be those with bubous. The pneumonic plague victims would not have such telltale signs but would die very quickly from sepsis and respiratory failure.

Venezuelan doctors alarmed unknown and deadly disease

Venezuelan doctors launched Thursday a wake-up call after the appearance of a mysterious disease that kills within 72 hours and that the hospitals in the country do not know how to deal.

But the warning was minimized by spokesmen for the regime Nicolas Maduro who described efforts to notify the public as a "campaign of disinformation and terrorism", even when the disease in ten days has left eight people dead in the central state of Aragua .

Despite official indifference, the country's doctors said there is reason for concern against a highly dangerous and contagious disease of unknown origin.

"We do not know what it is," he admitted in Caracas Duglas Le?n Natera, president of the Venezuelan Medical Federation.

"There is a full alarm in Aragua state, because there is absolute silence from the health authorities of the state, I understand that they know nothing, because people who are in charge of these offices are not doctors and do not know health public, or epidemiology, "he added.

The first warnings were issued Thursday by the College of Physicians of Aragua state, who called a press conference to warn the public about the disease, which has killed four children and four adults.

The president of this collegiate body, Angel Sarmiento said in a telephone interview that the fact that doctors are unable to treat the condition of great concern.

....

"The only thing common is the way the disease occurs," he said in a telephone interview.

The eight victims, from different points of Aragua, were transferred to the state capital, Ottawa, with symptoms including malaise, fever, skin spots and "bubbles" in the mouth.

Those spots, then became blisters and soon patients began presenting internal and external bleeding.

"After the bleeding starts, is respiratory failure, liver failure, kidney failure and death within 72 hours," said Sarmiento.

....

"People have gotten very ill and died the next day, hours, in hospitals," Sarmiento said.

Sarmiento said the College of Physicians of Aragua state decided to declare an emergency and health warning.

...

But the offer to collaborate with the government so far does not seem to be being taken seriously by the authorities of the state, whose governor, Tarek El Aissami, described the ads as a campaign to generate anxiety undertaken by the opposition.

Ninth patient died of HCM with "hemorrhagic fever"

The information was confirmed by relatives who remain on medical grounds pending autopsy

Airam Fern?ndez .- Fossil Franklin, the ninth patient had symptoms of fever associated with viruses these days dismays Aragua and the country, died at 6:00 am Sunday in the Hall of the Emergency Resuscitation Adult Maracay Central Hospital (HCM) .

The information was confirmed to Counterpoint by relatives who remain in the medical facility while awaiting results of the autopsy, performed earlier.

On Wednesday, Fossil was admitted to the emergency room to present joint pains, coughing bleeding, red spots on the skin and fever of 40 degrees. He was under observation for five hours, until the temperature subsided. He was diagnosed with chikungunya, not that he have a blood test. On Thursday returned with the same symptoms and a further chest pain. He was hospitalized and intubated Friday night, after being subjected to a routine that showed a high number of leukocytes. They also practiced a rib plate where doctors confirmed a "severe" infection in the lungs.

Yesterday, on the outskirts of HCM, the family said he was "in limbo" for the bickering that since Thursday kept the College of Physicians of Aragua, Corposalud and Governor Tarek El Aissami. "Until the doctors did not give us a diagnosis will not be quiet, what is left is to pray," he said.

On Friday, El Aissami confirmed the deaths of eight patients for various reasons associated with diseases like cancer and diabetes. Denied that was due to a "rare virus" or syndrome with symptoms of fever and bleeding, as noted by the President of the College of Physicians of Aragua, Angel Sarmiento, who was also denounced by the government of the entity before the Public Prosecutor, "run a terrorist campaign."

On the other hand - considering the possibility plague is to blame - the known cases may only be those with bubous. The pneumonic plague victims would not have such telltale signs but would die very quickly from sepsis and respiratory failure.

My suspicion is that the bacterium may be fairly widespread in rodents in the area, and that they have had both bubonic and septicemic plague cases from contact with rats. My suspicion on that baby is that he had a case of bubonic plague, which developed into septicemic plague, as I see several large black blisters, but also black spots under the skin that do not look like any swelling.

Septicemic plague would also explain the reports of DIC occurring right before a patient dies (something that is not consistent with a hemorrhagic fever).

Fortunately, they don't appear to have had any primary pneumonic plague cases yet, because that would likely result in the infection of a large number of HCW.

While I am not aware of any previous plague cases in Venezuela, and cannot find any reports of such on ProMED, the local forum above has someone report that there was a plague outbreak 15 years ago. Plague is also enzootic in Peru and Ecuador, having caused outbreaks there in the past.

They must have been treating with antibiotics on a few of the cases from onset. Yrsenia Pestis is easily identified and susceptible to antibiotics - with the exception of a few reports of antibiotic resistant strains. Any of the antibiotic resistant strains ever reported in South America? Would be interesting to find the illnesses described are in fact plague.

I agree with you on the baby pic. The neck and groin did present with buboe like swellings.

The patients have been presenting late in illness. The most recent one was misdiagnosed as (or co-infected with?) chikungunya. It's not clear that antibiotic resistance is the problem; it might be late treatment.

It might also be an issue of the wrong antibiotic. The only bacteria in the list of excluded hypotheses is staph (and they might specifically be referring to MRSA). I believe the antibiotic of choice for plague would be doxycycline or tetracycline, something that isn't commonly used for MRSA.

I think they have also been (mistakenly) leaning toward VHF as a diagnosis, something looking increasingly incorrect.
