My suspicion is that the bacterium may be fairly widespread in rodents in the area, and that they have had both bubonic and septicemic plague cases from contact with rats. The patients have been presenting late in illness. The most recent one was misdiagnosed as (or co-infected with?) chikungunya. It's not clear that antibiotic resistance is the problem; it might be late treatment.

It might also be an issue of the wrong antibiotic. The only bacteria in the list of excluded hypotheses is staph (and they might specifically be referring to MRSA). I believe the antibiotic of choice for plague would be doxycycline or tetracycline, something that isn't commonly used for MRSA.

I think they have also been (mistakenly) leaning toward VHF as a diagnosis, something looking increasingly incorrect.
