The woman died after developing breathing problems in the Kirov region, about 850 kilometers (530 miles) east of Moscow, said Lyudmila Ovchinnikova, a local health official.

?We are trying to rule out SARS, pneumonia and other infectious diseases as the cause of death,? Ovchinnikova said by phone today.

SARS, or Severe Acute Respiratory Syndrome, claimed hundreds of lives in China and Taiwan in 2003.

Fifty-one Chinese passengers and two train conductors were quarantined in the infectious disease ward of a local hospital and blood and tissue samples from the deceased were sent to Moscow for tests, Ovchinnikova said.

The results may not be known for a couple of days, she said.

carriage in which the woman was travelling was disconnected from the
rest of the train. The train then continued on its way to Moscow.
Russia's [public health] watchdog spokesman was unable to confirm
that the woman had died from SARS. "Doctors are currently
establishing a preliminary diagnosis," he said.

Kirov Region administration officials said that the final results of
the analysis would be ready in 24 hours' time.

A body of a dead Chinese woman has been taken off a Moscow-bound
train travelling from Russia's Far East. The woman has allegedly died
of pneumonia. 51 passengers, who were also from China, plus 2 train
attendants traveling aboard the same carriage were taken to hospital
in Kirovsky region in Volga district and the carriage has been
disinfected. Russia's Health Ministry reported the passengers were in
no danger but were kept under observation.

The train was traveling to the Russian capital from the city of
Blagoveshensk and got stuck for several hours in Kirovsky region [a]
thousand kilometers east of Moscow.

"Isolation is necessary because we still don't know the cause of the
girl's death. It could have been a heart attack, or other disease,"
stated the press office of the Kirovsky region's government.

"We've taken all measures to prevent the spread of infection and
taken samples from all people who have been hospitalized. We'll know
preliminary test results tomorrow. In several weeks, doctors will be
able to draw final conclusions," added the officials.

The Chinese embassy hasn't allowed an autopsy of the body to be
carried out but doctors say a medical examination showed the woman
allegedly had fluid on her lungs. It is also known, that 2 hours
before her death, the woman asked the train attendant for medical
help as she wasn't feeling good. The situation is being closely
monitored by medical professionals, says Russia's Chief Medical
officer of Health Gennady Onischenko: "The situation is under total
control. There are no complications that people of the Russian
Federation should worry about," said Onischenko.

[Severe acute respiratory syndrome (SARS) is a viral respiratory
illness caused by a coronavirus, called SARS-associated coronavirus
(SARS-CoV). SARS was 1st reported in Asia in February 2003. Over the
next few months, the illness spread to more than 2 dozen countries in
North America, South America, Europe, and Asia before the SARS global
outbreak of 2003 was contained. The disease has not recurred
subsequently except in some cases presumed related to laboratory
exposure. It is unlikely that the death of the passenger on board the
train en route from the Russian Far East to Moscow will be attributed
to SARS virus infection. The only reason to consider the diagnosis of
SARS in this case appears to be the patient's origin from China and
the possibility that pneumonia contributed to the fatal outcome.
There is no indication of spread of the illness to other passengers
or attendants on the train (during the course of a journey probably
extending over several days). Any additional information as to the
etiology or other details would be appreciated.

SARS begins with a high fever. Other symptoms may include headache,
an overall feeling of discomfort, and body aches. Some people also
have mild respiratory symptoms at the outset. About 10 percent to 20
percent of patients have diarrhea. After 2 to 7 days, SARS patients
may develop a dry cough. Most patients develop pneumonia. The virus
that causes SARS is thought to be transmitted most readily by
respiratory droplets (droplet spread) produced when an infected
person coughs or sneezes. The virus also can spread when a person
touches a surface or object contaminated with infectious droplets and
then touches his or her mouth, nose, or eye(s). In addition, it is
possible that the SARS virus might spread more broadly through the
air (airborne spread) or by other ways that are not now known.

Russian health authorities have quarantined 53 people after a woman died of an unknown illness on board a train headed to Moscow, the World Health Organisation (WHO) said on Thursday.


The woman, from mainland China, had been on a train heading from the Far Eastern city of Blagoveshchensk on Wednesday when she died suddenly, causing alarm.

Earlier on Thursday, Radio Television Hong Kong reported that the woman may have died from Severe Acute Respiratory Syndrome or SARS, a contagious disease that spread worldwide and killed at least 774 people in 2002 and 2003.

WHO spokeswoman Sari Setiogi said the U.N. health agency had not received any information to support the report, nor have Russian authorities identified SARS as the cause of the death.

"At the moment there are 53 contacts of the woman in quarantine in hospital," Setiogi told reporters.

"Samples have been taken from the woman and her contacts and laboratory investigations are ongoing."

The WHO could not confirm her age.

WHO authorities are working with Russian authorities to track the case and ensure it did not represent an international public health threat, Setiogi said.

It can take days or longer to establish causes of death and many illnesses and infections have similar symptoms, making it very difficult to reach such conclusions quickly.

WHO authorities are working with Russian authorities to track the case and ensure it did not represent an international public health threat, Setiogi said.

The above comments on the train passengers quarantined in Russia raise concerns that the atypical bilateral pneumonia in the fatal case was caused by H5N1. China had issued an alert in January for atypical pneumonia cases because of the spate of H5N1 infections. These human infections were not linked to H5N1 confirmed poultry outbreaks and the reported cases ceased in February. Prior to the outbreaks in patients, there were poultry outbreaks in Jiangsu which were caused by clade 7 H5N1.

The first reported case was in Beijing, A/Beijing/01/2009, which was also likely caused by clade 7. The first confirmed H5N1 cases in China was a patient with atypical pneumonia who was misdiagnosed as SARS, but was subsequently found to be infected with clade 7 H5N1, A/Beijing/01/2003.

The victim on the train was a Chinese citizen who boarded the train at the Russian border with northeastern China (see map). The patient was symptomatic for four days on the train before dying, raising concerns of spread to the relatives on the train, who have a temperature.

Passengers in other cars, who were released, were given Ribavarin, signaling a suspected viral etiology.

The involvement of WHO in the investigation increases concerns that the virus is H5N1.

A Chinese women who suddenly died on a Russian train this week in the country's Far East was not struck down by the SARS virus, Interfax news agency quoted a top Russian health official as saying on Friday.


Russian health authorities have quarantined 53 people from the same train while they investigate the cause of the illness after the woman died on the way to Moscow from the city of Blagoveshchensk on the border with China.

On Thursday Radio Television Hong Kong sent alarm bells through the health community when it reported the woman may have died from Severe Acute Respiratory Syndrome or SARS, a contagious disease that spread worldwide and killed at least 774 people in 2002 and 2003.

Preliminary test results using material from the woman's lungs, intestines and brain showed no trace of SARS or other dangerous viruses, said Gennady Onishchenko, Russia's chief public health official.

"Investigations are continuing," he said, adding that more data would be available later on Friday.
