Peru - outbreak of pneumonic plague

Re: Peru - outbreak of pneumonic plague

It looks like the 2 girls above are the "suspected" pneumonic plague cases, and they appear to be case #7, the 2 year old sister of the fatality, and the 7-year-old niece of case #1. That is, unless we are still counting the third HCW as suspected.

He recalled that correspond to a 29-year-old from Chocop district, which spread to a resident physician who attended as a student of fourth year medical career. While the fourth case of a child under 5 years living with the infected.

The student's health status would have been complicated in recent days into a coma and multiorgan failure to present that reduce their chances of recovery, medical sources.

In addition, there are 15 probable cases of bubonic plague, including a child under seven years, and two possible cases of pneumonic plague (a child under 14 years died).

Samples of these people were sent to the National Institute of Health (NIH) to confirm or exclude the disease.

In this situation, the Regional Health Management, in coordination with the municipalities of the province of Ascope, makes the insect more than 10,000 thousand homes.

This is because the studies in the area revealed that 85 percent of urban rodents (rats, guinea pigs and rabbits) are infected with the bacterium Yersinia pestis, which causes the disease.

Eleven cases of bubonic plague and pneumonic plague in Ascope
Saturday, July 31, 2010 14:03
TRUJILLO-seven cases of bubonic plague and pneumonic four have occurred in rural areas in the north revealed a medical epidemiologist Pedro Diaz Camacho, chairman of the Regional Operational Technician Pest Control.

The epidemiologist formed to date, has identified eleven confirmed cases of plague in the north particularly in Ascope.

Likewise, the Regional Health Management Freedom disinsection began more than 10 thousand homes in the province of Ascope, said Victor Chavez Peralta, head of the institution

Dr. Diaz Camacho said date, this year, have registradoi seven confirmed cases of bubonic plague, two confirmed pneumonic plague secondary, or have been acquired in the community, and two cases of primary pneumonic plague, which indoor patient were acquired.

Re: Peru - outbreak of pneumonic plague

This article seems to indicate the confirmed pneumonic cases are the woman (#1) and the three HCW (2, 3, 4), whereas the above one indicates it is the woman and cases #2,3, and 5. It is also conceivable that the reporter here makes the mistake because the student is a medical student, so he is actually being double counted as both a student and a doctor.

Septicemic plague takes its first victim
Saturday, July 31, 2010
Public Enemy. The National Institute of Health (NIH) in Lima confirmed yesterday that the 14-year-old, identified with initials Ohura, a native of the Cartavio (Ascope), who ceased to exist last Monday afternoon, died of the worst plague of all: septicemic.

Thus, the child became the first fatality of this disease in La Libertad.

This was confirmed yesterday by the Regional Technical Committee Chairman Pest Control Operational and epidemiologist at Belen Hospital, Trujillo, Pedro Diaz Camacho, who acknowledged that this is the first case sparked by the disease. Although when asked, the physician denied that the deceased has died of pneumonic plague, the truth is that septicemic plague is the ultimate complication of plague.

"The boy died with septicemic plague because their parents refused to intern at a local hospital. Therefore, evil is complicated and when doctors ordered his transfer to hospital San Francisco (Cartavio), was dead, "said the Industry.

In the same vein, the health professional today indicated that the INS will confirm whether the 15 suspected cases of bubonic plague are true, like the two of pneumonic plague. It also recommended that families apply Chicama Valley preventive health measures for breeding of rodents (guinea pigs or rabbits) and surveillance of fever cases.

Student remains grave
Meanwhile, on the health of medical student in critical condition for pneumonic plague, Diaz Camacho said his health is still care and that it remains stable.

The 21-year-old was one of three people (the other two are doctors and were also infected) who attended at the Regional Hospital, the woman who came Chocope victim of pneumonic plague.
The student, a woman and two doctors are isolated

[Man] Sick with [plague] contracted pseudomonas
Sunday, August 1, 2010
The cure was worse than the disease. As if that were not enough to have to fight for his life because of the terrible suffering from pneumonic plague, now that the young student is the fourth year of Human Medicine, Universidad Nacional de Trujillo (UNT), identified as Felix UL (21), just getting a fatal lung infection, nothing more and nothing less than caused by pseudomonas present in the same intensive care unit (ICU) of the Regional Hospital.

Responsible Wanted
This was due to inadequate sterilization of respiratory therapy equipment or improper hand washing of the doctors responsible for care. This dismal situation was revealed yesterday by the Regional Technical Committee Chairman Operational Pest Control, Pedro Diaz Camacho, who said the bacteria pseudomonas or contaminated was acquired because the patient underwent invasive procedures such as intubation, artificial respiration, placement sera or dextrose.

"These are infectious outbreaks affecting the person due to the mechanical ventilator. Unfortunately, in all intensive care units in Peru and the world there is a 66 per cent of ongoing risk. Even having intubation, the risk taken is total, "he said denying any negligence of hygiene at the health facility. He also argued that the prognosis of the young is quiet and avoided talking about the likelihood of mortality.

"Doctors do not lose faith because everything is in God's hands. We hope that with your help can be saved. For our part, we are making every effort to avoid further complications, "he said recalling that the resident physician and women infected with pneumonic plague are recovering slowly.

Adding insult
However, to know the truth further, industry journalists contacted the adviser to former Ministry of Health and Health manager Libertad, Henry Rebaza Iparraguirre, who said that the lung infection (IHN) is an infection of the parenchyma (tissue) lung presented during the patient's stay in ICU, however, made clear that this germ attacks when there is proper hygiene in the sanitary environment.

"This may be due to poor hand washing technique, improper sterilization of UCI, mismanagement of hospital safety equipment and pollution. Even the elderly or weak patients who do not respond to treatment may die from acute respiratory failure caused by pneumonia, "he declared.

Lima, ago. 02 (ANDINA). The outbreak of plague reported in the province of Ascope, La Libertad, has been controlled by the actions implemented and has identified 100 percent of household contacts and hospital receiving the necessary treatment, exceeding the incubation period, said Today Health Minister Oscar Ugarte Ubilluz.
However, it considered necessary multi-sectoral and inter-governmental intervention to prevent the spread of the disease in the other provinces.

Since April, the Ministry of Health (MoH) has recorded 31 cases, including confirmed and probable locations of pest in the valley of Chicama, in Ascope.

At a press conference explained that the total of 25 cases correspond to bubonic plague, pneumonic four and two septicemic
. He said that there are 11 confirmed cases, among whom are a child under 14 died of bubonic plague. All patients receive preventive treatment.

"Of all the deceased presented there corresponds to a child under 14 who suffered from Down syndrome, who was taken to hospital but was dead Cartavio, no diagnosis was made on time. Not detected by the family itself, "he said.

According to the report submitted by the Regional Health Management (Geres) of La Libertad, the majority of cases reported in the towns of Chicama, Chicamita Chiclín and Pascana in Chicama district.

Also in the districts of Casagrande, Chocop, Paiján and Santiago de Cao, while in the district of Huanchaco, Trujillo, has reported a case from the Chicama Valley.

"The spread of plague occurs through the bite of infected rodent flea (with the bacteria Yersinia pestis). Pneumonic plague is a form of complication of the disease, it is no longer transmitted by the flea but through saliva droplets, "he said.

Ugarte noted that the latest outbreak of plague in the north of the country was registered in 1994 with 1,104 cases and 35 deaths.

The minister denied that the outbreak of the disease in the case of a regression in the status of the country but their presence is due to the economic advancement of the sugar sector.

"After an analysis determined that this outbreak was due to the expansion of cane cultivation and the practice of burning it, causing the flight of rodents to the surrounding towns," he declared.

Another risk factor identified is that housing in which people spend the night working in the sugar cane harvest.

"On this occasion we relate this outbreak of plague with some farming practices such as burning cane, for when this occurs and the wild rodents invade homes and flee down the contagion," he declared.

On the other hand, noted that the outbreak was confined to the Chicama Valley area and not necessarily the north is in potential danger of plague spreading.

It all depends on the responsiveness and implementation of preventive measures not only the health sector, but also of regional government authorities, provincial and local, he said.

Distributed nearly 6 tons of insecticide to eradicate fleas in La Libertad

Lima, ago. 02 (ANDINA). About six tons of insecticide (carbaryl) to combat the fleas that transmit bubonic plague were sent to the department of La Libertad to deploy the insect, mainly in areas where the outbreak began, reported the Health Minister Oscar Ugarte Ubilluz.
He said also managed to send 20 teams dusters for the implementation of vector control.

He said that to date has protected homes in 1.992 Chicama district, Chocop and Santiago de Cao, and perform the work of eradicating insects in other 1,500 homes in other affected districts in the valley of Chicama, in coordination with local governments .

By taking knowledge of the outbreak, the Ministry of Health (MoH) issued an alert recommending strengthening epidemiological respiratory protection in health services to prevent transmission by direct route, he said.

It also provided for the notification and treatment of all cases. Also recommended a census and contact tracing, and the taking of samples of suspected cases, immediate notification of cases in animals and updating the health situation room with information about the risk of transmission of plague.

Among other measures, macro-regional workshop was conducted epidemiological bases for monitoring, control and prevention in the north, in order to optimize plans for pest control. Participants Piura, Lambayeque, La Libertad and Cajamarca.

Through the various Directorates-General and the National Institutes of Health (NIH) of the MoH was decided to shift multidiscplinarios equipment, in order to support Regional Health Management (Geres) of La Libertad in controlling the outbreak.

On the other hand, sought support of the Peruvian Army personnel has been trained in vector control. Also, warning posters have been produced on the subject and giving recommendations to the public to avoid infection and spread of the plague.

Recommendations
In order to prevent the spread of cases, people should join the control measures and follow the following recommendations:

"No rodents should be removed during the presentation of outbreaks of plague since the release of their fleas increases the risk of spread of the disease. Therefore, you must first remove the fleas.
"Proper storage of crops in barns or silos rodent-proof.
-Provision of adequate waste (sanitary burial).
"Proper storage of food in rodent-proof containers.
"Improvement in the structures of the rodent-proof homes (remove holes, crevices, passageways income ceilings, etc).
"Proper disposal of excreta (silos, septic tank).
Having properly-products of agricultural activity (storage, recycling, preparation of humus, sanitary burial, etc.)..
-Elimination of bushes, shrubs and bushes around houses and irrigation canals and drainage.
-Breeding of domestic animals (guinea pigs, rabbits, etc..) In pens outside homes. :tiphat:http://www.andina.com.pe/Espanol/Noticia.aspx?id=0DoKUx1RyG8=

Fever infection control in Freedom
Mon, 02/08/2010 - 17:29

To implement that action has been identified to one hundred percent of those infected contracted a plague outbreak in the province of Ascope, La Libertad, said the Health Minister Oscar Ugarte Ubilluz.

Ugarte Minister said during a conference at the headquarters of the MoH, which so far are 31 plague cases presented at the Liberty, which have been detected early and controlled.

Of the total cases found in this northern city and 25 were identified with bubonic plague, pneumonic plague, 4 and 2 of septicemic plague, as deemed necessary and inter-sectoral intervention to prevent the spread of the disease in the other provinces.

The spread of plague occurs through the bite of infected rodent flea. One dead and 31 infected by plague outbreak
Mon, 02/08/2010 - 17:11

An outbreak of plague killed up on Monday killed and 31 infected in northern Peru, where authorities declared an epidemiological alert with revisions in ports to stop the virus, said Health Minister Oscar Ugarte.

"The case (fatal) is a 14 year old boy with Down syndrome later taken to hospital," Ugarte said Monday at a conference after stating that doctors working in the area and the Peruvian Naval Medical Research Detachment United States. The death occurred on 26 July.

Ugarte said that until Saturday there were 25 cases of bubonic plague, pneumonic plague, 4 and 2 and added septicemic plague has been detected also an unspecified number of "dogs, cats and guinea pigs (guinea pigs) contaminated."

Yersinia pestis
The three events are produced by the bacterium Yersinia pestis depending upon the mode of transmission. The plague occurs through the bite of fleas and bacteria infect the lymph glands, the pneumonic form occurs when the bacterium is inhaled and lodges in the lungs and the last is when the bacterium reaches the bloodstream.

The outbreak occurred in the province of Ascope, an area with extensive sugar cane crops where rodents are abundant and close to the ports of Salaverry and Malabrigo, 520 miles northwest of Lima.

Of those infected with pneumonic plague, two are medical doctors who were infected after diagnosing the disease to a female patient.

The arrival of evil
The NIH believes that the plague came to Peru with the arrival of the Spanish in the sixteenth century, but the first national registry data from 1903 and the last outbreak in 1994 killed 35 people.

In the fourteenth century the bubonic plague, known as Black Death, killed an estimated 25 million Europeans. :tiphat:http://www.larepublica.pe/regionales/02/08/2010/un-muerto-y-31-infectados-por-brote-de-peste

Take action to prevent the spread of evil. (USI)

The outbreak of plague in La Libertad worsens. Despite the actions taken by health authorities in the region, the incidence of the disease progresses and soon could propagate to the neighboring departments and extended throughout the northern macro-region.

So warned the president of the Regional Committee Technical Operational Pest Control, Pedro Diaz Camacho, who said that evil could come to Piura, Lambayeque and Cajamarca.

"We are in imminent risk of a resurgence of bubonic plague in the area of the country macronorte. This is because, unfortunately, their watersheds, Chira-Piura, Quiroz river, the rivers of Cajamarca, the Jequetepeque and Chicama, wild rodents are infected carriers of Yersinia pestis (the bacteria that renders the poor), "he said.

The specialist added that even the plague could reach the capital, if any person is "involuntarily" an infected flea to Lima. However, he stressed that it is very unlikely to happen.

ON ALERT. On the possibility of spreading the disease, the regional director of Health Piura, Manuel Arrunategui said already is working jointly with the municipalities, to avoid increasing the population of rodents and other sites of infection as landfills.

"Certainly you can not wipe out the rats, but it is important to prevent such animals do not proliferate and come into contact with the population", he said.

In the same vein, the regional health director of Lambayeque, Victor Echeand?a said they already have a surveillance plan, which consists of going to endemic areas and examine the animals to prevent the bacteria from spreading.

For its part, the dean of the Medical College of Peru, Ciro Magui?a, said that the spread of bubonic plague is caused by the negligence of health authorities in the regions.

The physician said that the epidemic can be contained with relatively simple measures such as rodent control, fumigation and a campaign to maintain sanitary conditions :tiphat:http://peru21.pe/noticia/616870/peste-ratas-norte-se-agrava

Outbreak of fever controlled
One child with Down syndrome died. They urge people to remove fleas from pets and maintain cleanliness.
One child died and another 31 cases of plague have been recorded so far in the province of Ascope, La Libertad. The latter have been detected early and monitored, said yesterday at a press conference the Health Minister Oscar Ugarte. However, deemed necessary multi-sectoral and inter-governmental intervention to prevent the spread of the disease. Of the total cases, he added, 25 are for bubonic plague, which is swelling of the lymph; five pneumonic plague (including the deceased), which affects the lungs and is the most complicated because it is transmitted through saliva droplets and two septicemic plague, which invades all organs.

"The transmission of plague occurs through the bite of infected rodent flea. But pneumonic plague is more complicated because it is processed through saliva droplets, "he said. This is the case of a woman of 29 years treated at a hospital in Chicago, which infected the health personnel, they have received adequate treatment and its prognosis is favorable.

Deceased
The deceased, said Ugarte, is a minor of 14 years with Down syndrome, who died in the hospital because Cartavio was taken to hospital late. The disease is caused by a bacterium that lives in wild rodents. When these marauding urban areas, its fleas can reach the homes and pets and biting people. The burning of sugar cane pushed the species to leave these places and get closer to homes. About two thousand homes have been protected with carbaryl (insecticide to control fleas) which have been more than 5,800 kilos to the area. Recommend that people eliminate fleas from pets and maintain cleanliness. Not expected to reach Lima. :tiphat:http://www.expreso.com.pe/edicion/index.php?option=com_content&task=view&id=110065&Itemid=35

Re: Peru - outbreak of pneumonic plague

I don't know what the red text refers to; that is not the hospital mentioned above in this thread, and there is no indication of four HCW infected before this article. If a separate cluster of pneumonic plague had occurred in an additional hospital, there would be more than four or five reported pneumonic plague cases.

Something is not right in this report.

The discrepancy between four or five pneumonic plague cases seems to be whether to count the fatality, which was a bubonic or septicemic plague case, but may have become a secondary pneumonic plague case when the bacteria spread to his/her lungs.

LIMA | The dreaded black or bubonic plague, which hit our country in the early twentieth century and is currently presented in La Libertad, has become a threat to Lima and other coastal cities. The resurgence of the pandemic in the country's north, where there is one dead and 32 people infected, has led the Ministry of Health (MoH) to take action to prevent the spread of other major cities.

These measures focus on preventing and Salaverry Malabrigo ports located in the region, are sources of spread of the epidemic, caused by the bacterium Yersinia pestis, which are transmitted via fleas which have voles.

"We work with the Port Captain in order that through fumigation or other procedures to prevent the plague from entering large cities such as Lima, Piura, Lambayeque and Cajamarca," said Health Minister Oscar Ugarte.

Given this risk, the MoH in epidemiological alert said the city affected and coordinates with district and provincial authorities to eradicate this evil.

"The risk of the plague from spreading there, pending compliance with preventive measures identified," he said Ugarte.

ONE DEAD. According to the MoH, to July 31 were reported in 33 affected Freedom of plague, of which a 14-year-old died because they did not receive immediate attention.

Ascope province hardest hit, recording 29 cases. It is followed Trujillo, where four people have the disease.

The MoH reported that in the cases reported, 26 people suffering from bubonic plague, pneumonic plague suffering four and two septicemic plague, the mortality rate is higher. It was learned that a woman infected with this type of plague spread to four aid workers Lambayeque Regional Hospital.

THE CAUSE. Ugarte said the outbreak of plague in La Libertad, which intensified in April, is the result of the expansion of sugarcane crops over populated areas.

ActualidadEn risk 7:57 40 000 by the plague of rats
Chicama Valley (La Libertad) is an emergency. Health Minister said that disease could arrive in Piura, Lambayeque and Cajamarca.

The spread of bubonic plague occurs through the bite of fleas from rodents. (USI)
Alarm in the north. At least 40 000 people are at risk of contracting the bubonic plague in the Chicama Valley in the province of Ascope. This was announced by Health Minister Oscar Ugarte.

It therefore considered necessary multi-sectoral and intergovernmental intervention to prevent spread of the disease in other provinces and in Lambayeque, Piura and Cajamarca.

Also, Ugarte showed that since April, the Ministry of Health (MoH) has recorded 31 cases, including confirmed and probable, of plague in the Chicama Valley. The minister explained that the total cases, 25 relate to bubonic plague, pneumonic four and two septicemic.

He said that we know of 11 confirmed cases, among whom are a child under 14 years with Down syndrome died of bubonic plague in Cartavio. Ugarte says the spread of the fever is due to the expansion of cane cultivation, which drives away the rodents to people.

For its part, the regional manager of Health of La Libertad, V?ctor Peralta, said that the state of medical student 21 years who contracted pneumonic plague in the regional hospital is critical.

Re: Peru - outbreak of pneumonic plague

The warning below is NOT an alarm about plague and H1N1 combining; that is impossible as one is bacterial and the other is viral. IMO, that fact cannot be repeated enough, especially in light of previous events last year. It also is NOT an alarm about a co-infection (although that certainly could happen, but probably wouldn't be that much worse than a plague infection alone). It is an alarm about confusing one disease for the other - although symptoms would be similar, treatment would be very different.

Post #2 in this thread indicates that the first pneumonic plague case was originally treated as suspected H1N1.

Bubonic plague worsens
Alert Piura, Lambayeque and Cajamarca

Health Minister Oscar Ugarte.

The outbreak of Bubonic Plague in La Libertad worsens. Despite the actions taken by health authorities in the region, the incidence of the disease progresses and soon could propagate to the neighboring departments and extended throughout the northern macro-region.

Lima (Peru 21) .- It warned the president of the Regional Committee Technical Operational Pest Control, Pedro Diaz Camacho, who said that evil could come to Piura, Lambayeque and Cajamarca.

Piura
On the possibility of spreading the disease, the regional director of Health Piura, Manuel Arrunategui said already is working jointly with the municipalities, to avoid increasing the population of rodents and other sites of infection as landfills.
"Certainly you can not wipe out the rats, but it is important to prevent such animals do not proliferate and come into contact with the population", he said.

Lambayeque
In the same vein, the regional health director of Lambayeque, Victor Echeandía said they already have a surveillance plan, which consists of going to endemic areas and examine the animals to prevent the bacteria from spreading.
For its part, the dean of the Medical College of Peru, Ciro Maguiña, said that the spread of bubonic plague is caused by the negligence of health authorities in the regions.
The physician said that the epidemic can be contained with relatively simple measures such as rodent control, fumigation and a campaign to maintain sanitary conditions in homes.

It can be confused with H1N1 influenza
Health Director asked to be alert

After the outbreak of cases of bubonic plague in the region of La Libertad, the regional health director, Manuel Arrunategui, yesterday asked the doctors to take greater care when diagnosing patients, since it could be confused with a H1N1 influenza, which could be fatal to humans.
"Physicians can be confused with influenza, so they must be careful. Patient who comes from La Libertad, with cold, cough, be careful, we may be in the presence of a case of bubonic plague, "he said.
For that said it will coordinate with Dr. César Morón Pastor, to train doctors in health facilities in Piura in order to avoid possible complications.
He also reported he held an emergency meeting with key officials of his sector, and the first concrete step will be to send a team of specialists to Huancabamba Tuesday, as it is an endemic, and where he presented the last case of this disease 1999.
Arrunategui stressed that the solution to this disease is not to kill rats, the main transmitter of the disease, but the fleas bite the rodents, they are the real perpetrators of such evil, for them to specialists, will live to Lima to rodents analysis.
He also said he will use the trip to send penicillin that area, due to low temperatures affecting much of the country and threatening to children under 5 years.
In this regard revealed that the date in our region has made 11 000 cases of Acute Respiratory Diseases (Eras), figures similar to last year.

Peru's health minister says an outbreak of plague has killed a
14-year-old boy and infected at least 31 people in a northern coastal
province. Health Minister Oscar Ugarte says authorities are screening
sugar and fish meal exports from Ascope province [La Libertad
region], located about 325 miles (520 km) northwest of Lima. Popular
Chicama beach isn't far away.

Ugarte says the boy, who had Down syndrome, died of bubonic plague 26
Jul 2010. He said Monday, 2 Aug 2010, that most of the infections are
bubonic plague, with 4 cases of pneumonic plague. The former is
transmitted by flea bites, the latter by airborne contagion. The
disease is curable if treated early with antibiotics.

The 1st recorded plague outbreak in Peru was in 1903. The last, in
1994, killed 35 people.

[Ascope province is located in the region of La Libertad, in
northwestern Peru. It shares a border with Trujillo province where
the disease has also been recently reported. It is not clearly stated
in the posting whether the pneumonic cases were directly acquired
from person-to-person spread or were a complication of bubonic
plague. Of note, there was an outbreak of bubonic plague reported in
Chicama district of Ascope province in April 2010, suggestive that
these current cases may be part of this previously reported outbreak
as proximity to Chicama beach is mentioned in the above newswire (see
prior ProMED-mail posting Plague, bubonic - Peru: (LL) RFI
