Ebola outbreak in Democratic Republic of Congo – update

14 September 2012 - Retrospective epidemiological investigation recently identified additional confirmed and probable cases with Ebola haemorrhagic fever (EHF) in the Democratic Republic of Congo (DRC), according to the Ministry of Health (MoH).

As of 12 September 2012, 41 cases (9 laboratory confirmed, and 32 probable) have been reported from Haut-Uélé district in Province Orientale. Of these cases, 18 have been fatal. (5 confirmed and 13 probable). 18 healthcare workers are included among the probable cases. 28 suspected cases have also been reported and are being investigated.

Since the last update, an additional 27 cases (1 laboratory confirmed and 26 probable) of EHF were identified through retrospective epidemiological investigation in the two health zones of Isiro and Viadana. The retrospective epidemiological investigation was done to identify cases of illness which might have been EHF between the end of April 2012 and August 2012, and thus help clarify the possible origins of the outbreak and provide additional information on chains of transmission. In addition, active field epidemiological investigation is continuing to help identify all possible chains of continuing transmission of the illness, to ensure that appropriate measures are immediately taken to interrupt the transmission, and stop the outbreak.

The death total is different than what Reuters quoted the health minister above in post #72 above (and then was picked up by google and other news re-packagers).

After a flurry of news reports yesterday ? some suggesting that the Ebola outbreak in the Democratic Republic of the Congo was `out of control? - we?ve an update from the World Health Organization this morning that endeavors to clarify the situation.

Ebola outbreak in Democratic Republic of Congo ? update

14 September 2012 - Retrospective epidemiological investigation recently identified additional confirmed and probable cases with Ebola haemorrhagic fever (EHF) in the Democratic Republic of Congo (DRC), according to the Ministry of Health (MoH).

Ebola was first discovered in Zaire and Sudan in 1976 and since then has become almost legendary for its incredibly high fatality rate and gruesome hemorrhagic symptoms.

As a public health threat, Ebola ? which has been blamed for fewer than 2,000 deaths over the past 30+ years - pales in comparison to most of the world?s less feared infectious diseases.

Simple childhood pneumonia, claims 1.8 million lives each year (cite) and Malaria claims between one half, to one million lives a year (cite).

Despite its relative rarity, movies like 1995?s Outbreak with Dustin Hoffman, and books like Tom Clancy?s Executive Orders and The Hot Zone by Richard Preston, have helped to turn Ebola into the ultimate nightmare disease in the eyes of the public.

There are currently five known strains of the disease, of which four are highly pathogenic in humans.; Ebola-Zaire, Ebola-Sudan, Ebola-Ivory Coast and Ebola-Bundibugyo.

This outbreak, in the DRC, is caused by Ebola-Bundibugyo.
The odd virus out - Ebola Reston - which can infect and kill non-human primates, can infect humans, but has not been shown to produce disease.

While the zoonotic reservoir for the Ebola virus has yet to be firmly established, bats are considered to be a likely candidate.

For more background on the Ebola virus the CDC?s Special Pathogens Branch has an extensive webpage on the virus.

Ebola: 30 deaths since the declaration of the epidemic in Eastern Province

published 5 hours ago, 48 minutes | Last update September 14, 2012 at 9:13 | under News, National, Health.

Sixty-nine cases of Ebola, thirty deaths have been recorded since the outbreak of the epidemic in August last in the Eastern Province, Thursday, Sept. 13 said the Congolese Minister of Health, Felix Kabange Numbi. Twenty eight cases including fourteen deaths were listed on 3 September. According to the same source, more than half of the cases occurred before the official declaration of the epidemic.

"After compiling and analyzing data and retrospective to date, the International Coordinating Committee of Scientific and Technical struggle believes that the epidemic had started in May 2012. And to date, 69 cases have been recorded including health personnel. More than half of the cases occurred before the official declaration of the epidemic, "said the minister.

During a press conference held jointly with the World Health Organization (WHO), the Minister Felix Kabange Numbi is concerned about the increasing incidence of this epidemic that has no treatment.

He said that the Ebola virus circulating in the Eastern Province since last May and more than five hundred people have already been in contact with sick people.

The WHO estimates that the lifestyle of the population facilitates the spread of the disease in this province. Felix Kabange Numbi said that the province has already reached "the peak of the epidemic."

"We can not say that we are in the downswing as contact with the animal persists. There may be new infections which may cause a second peak and we prefer to remain on this phase of alert in the region for the ongoing education "Does he added.

For his part, the representative of WHO, Dr Léodégal Bazira, invited all international partners to get involved in the fight against this epidemic, according to him, could pose a real public health problem throughout World.

Re: Dem. Rep. Congo - Ebola: 30-31 deaths, 457 contacts traced, 69 confirmed/probable/suspected cases since May 2012

The numbers here are a bit odd, and possibly revealing:

There are 9 confirmed cases. Five of the confirmed cases have died (55.5% CFR). There is no evidence any of these 9 cases are HCW.

There are 32 probable cases, 13 of whom have died (40.6% CFR). Of those 32, 18 are HCW, of whom 5 died (27.7% CFR). That means there are 14 non-HCW probable cases, of whom 8 died (57.1% CFR).

Eighteen of the 41 probable/confirmed cases (44%) are HCW, reflecting significant nosocomial spread.

There are thus 28 suspect cases, 12-13 of whom have died (42.8-46.4% CFR).

Obviously, all these counts assume that everyone still alive will recover, but in the four month long outbreak, the number of cases still ill right now would be expected to be very low, perhaps almost zero.

Notably, the HCW have fared significantly better than the surrounding population. Could this be the result of earlier treatment? Maybe, but I really don't know how much treatment would help here. Perhaps wider surveillance among HCW is picking up milder (or unrelated?) cases that would be missed in the general population?

Except for the HCW, the CFR in all groups is significantly higher than the 25-35% reported for the only previous outbreak of the Bundibugyo strain, in Uganda in 2007. Considering that the CFR depends on many things (age of affected population, pre-existing conditions, etc.), perhaps the media should stop reporting that the Bundibugyo virus is "milder" than the other Ebola strains.

Ebola toll rises, health system on its knees

Report
?
Missionary International Service News Agency

Since the start of the month the death toll of the Ebola outbreak in the north-east has climbed from 15 to 31, the World Health Organization (WHO) said, defining the situation ?serious?. Fadela Chaib, a spokeswoman for the Geneva-based agency, stressed that ?it is unusual that the first person to be infected was a health worker?. The areas affected by the outbreak are Isiro and Viadana (70km apart), in the Eastern Province (north-east).
...
?Congo?s health sector has for decades been left to its own demise, with the result of an increasing critical degradation. There is a lack of political will, incapacity of the State and lack of funds that have led to a two track health system?, denounces a civil society source of the Eastern Province. ?Because current, today?s problem is Ebola, but how many people have died in silence from cholera, malaria, malnutrition and diarrhea, all easily treatable?, added the source, who prefers to remain anonymous.

There is a lack of health facilities in the former Belgian colony, especially in the small towns and remote villages, but also medical supplies, drugs and doctors. The private structures and those run by the church work better, but often at prohibitive costs for the majority of the people.

...?The multiple conflicts in the East, with thousands of displaced seeking refuge in neighbouring countries, complicate the situation of the health sector further and render humanitarian interventions more complicated?, stressed workers of the MSF (Doctors Without Borders). ?Those who manage to reach a refugee camp and receive treatment from the WHO, Red Cross or MSF are lucky: the medical assistance is significantly better than that in the villages they fled from?, concluded a missionary source from Isiro. I don't see any conflict between the two reports. Both reports give 14 confirmed, 32 probable, and 26 suspect cases, for a total of 72. Presumably, 19 of the confirmed/probable and 13 of the suspected cases have died.

The only difference is whether the suspected deaths are included in the fatality count.

Ebola outbreak in Democratic Republic of Congo ? update

18 September 2012

As of 15 September 2012, 46 cases (14 laboratory confirmed, 32 probable) with Ebola haemorrhagic fever (EHF) have been reported in the Democratic Republic of Congo (DRC). Of these 19 have been fatal (6 confirmed, 13 probable).

The MoH continues to work with partners to control the outbreak. Active epidemiological investigation is being done to identify all possible chains of transmission of the illness, and ensure that appropriate measures are immediately taken to interrupt the transmission, and stop the outbreak.

A National Task Force convened by the MoH is working with partners including M?decins Sans Fronti?res (MSF), the International Federation of Red Cross and Red Crescent Societies (IFRC), US Agency for International Development (USAID), US Centers for Disease Control and Prevention (CDC), United Nations Children?s Fund (UNICEF) and WHO, to control the outbreak.

WHO and the Global Outbreak Alert and Response Network (GOARN) are providing support by deploying experts to the field to work with partners in the areas of coordination, infection prevention and control (IPC), surveillance, epidemiology, public information and social mobilization, anthropological analysis and logistics for outbreak response.

With respect to this event, WHO does not recommend that any travel or trade restrictions be applied to the DRC.

General information on Ebola subtypes

There are five identified subtypes of Ebola virus. The subtypes have been named after the location they have been first detected in Ebola outbreaks. Three subtypes of the five have been associated with large Ebola haemorrhagic fever (EHF) outbreaks in Africa. Ebola-Zaire, Ebola-Sudan and Ebola-Bundibugyo. EHF is a febrile haemorrhagic illness which causes death in 25-90% of all cases. The Ebola Reston species, found in the Philippines, can infect humans, but no illness or death in humans has been reported to date.

DRC: a suspected case of Ebola reported in Kisangani

A suspected case of Ebola was reported in Kisangani, capital of Orientale province (north-eastern DRC), located approximately 600 kilometers from the city of Isiro, considered the epicenter of disease, announced Tuesday the medical director of the general hospital Isiro Dr. Bwengadi

"There is a suspected case of a 16 year old girl who was recorded last Friday. It fevers and nose bleeds. Samples were made and sent to Isiro," continued Dr. Bwengadi, adding that analysis results will be announced on September 19.

According to Dr. Bwengadi, we can not yet confirm that there is the Ebola outbreak in Kisangani, but steps are being taken to prevent the spread of this disease.

According to the latest report, published on 13 September by Dr. Felix Kabange Numbi, DRC Minister of Health, the Ebola epidemic raging in the north-eastern DRC had 31 deaths since May 2012.

The Ministry of Health, the World Health Organization (WHO), the Center for Disease Control (CDC-Atlanta) and M?decins Sans Fronti?res (MSF) are working closely to fight against the disease.

The medical director of the general hospital of Makiso in Kisangani, called Tuesday, September 18 population quiet Friday after the discovery of a suspected case of Ebola fever hemorrhage. A teenager with a high fever with bleeding from the nose was taken to the hospital. Dr. Dieudonné Baengandi says that it is probably not a case of Ebola hemorrhagic fever. However, he called the people to respect the rules of protection against this disease.

According to the medical director, the sample taken from the young 16 year old girl was sent to a specialized laboratory in Isiro and the result is expected to arrive Wednesday, September 20.

According to Dr. Baengandi, this case seemed suspect because the patient had previously consumed meat bat.

"We thought, as we are in the context of an epidemic, it is better to isolate the hospital time to investigate and find out if this is a case of Ebola haemorrhagic fever," has he said.

The doctor says that the clinical, health status of the patient changes positively.

"We trust that this is probably not a case of Ebola hemorrhagic fever. Time to get results, we will confirm or not, "he said.

Dr. Dieudonné Baengandi then urged people to take precautions to avoid contracting the disease, among others, avoiding eating fresh meat of wild animals and greet hand.

According to a new survey released Tuesday by the World Health Organization (WHO), the Ebola outbreak was 32 dead, including a woman who had just given birth. Baby, by cons have survived, according to the same source.

Speaking to AFP, Dr. Léodégal Bazira, WHO Representative in Kinshasa, said that "the analysis of the situation today shows that the epidemic seems dammed in its epicenter and that Isiro there were more new infections among health personnel.

The Ebola virus continues to strike people in the Democratic Republic of Congo. Since May, the World Health Organization has counted 72 confirmed, probable or suspected cases and 32 deaths.

As usual, a disproportionate share of those cases are health care workers ? 23 of them, almost a third.

That's because, despite elaborate protective garb and other precautions, it's very hard for doctors, nurses and health aides to avoid virus-laden bodily fluids of Ebola patients or accidental needle sticks. That's especially true at the beginning of an outbreak, when Ebola symptoms might be mistaken for malaria or something else.
...
Apart from the tragedy of caregiver deaths, this has a ripple effect that helps keep an outbreak going.

Dr. Armand Sprecher of Doctors Without Borders says that's because when health workers don moon suits and avoid all unnecessary contact with Ebola victims, that reinforces the community perception that the hospital is just the place people go to die.

"If you don't hang IV lines and do things that look medical, if you just put people in beds and walk around in protective gear and don't touch anybody, well, why would they want to come there?" Sprecher said in an interview with Shots from the Doctors Without Borders operations center in Brussels.

The perception is only fueled when people see health care workers die of Ebola in hospitals.

"We have a horrible time marketing our treatment unit because patients are not seeing a benefit to come in when we don't produce a lot of survivors," Sprecher says.

And if infected people stay away from hospitals, that just allows the virus to spread out in the community.

But the grim truth is doctors can't do anything for Ebola patients except give them fluids and other supportive care. What's needed, Sprecher says, is an effective treatment.

76 cases of fever "Ebola" with 32 deaths in Haut-Uele to September 19, 2012

76 cases of Ebola haemorrhagic fever, including 32 deaths, have been recorded since the beginning of the epidemic in May 2012 in the Haut-Uele district (Orientale Province - Eastern DRC) until 19 September, reported Wednesday in Kinshasa Office for the Coordination of Humanitarian Affairs (OCHA).
69 cases, including 31 deaths, were registered from 18 May to 11 September, specifically in the areas of health and Isiro Viodana located in the health district of Haut-Uele. Twenty health professionals also contracted the disease and five died so far. Isiro is the epicenter of the epidemic, while the city of Viodana, located 75 km southwest of Isiro is considered a secondary source. The International Coordinating Committee Technical and Scientific fight against this epidemic in the district was estimated recently, after compilation and analysis of data and retrospective September 13, that the epidemic had started in May 2012. More than half of the reported cases occurred before the official declaration of the epidemic on 17 August. The World Health Organization (WHO) and the United Nations Fund for Children (UNICEF) received $ 1.75 million to strengthen the fight against this epidemic.
According to OCHA, the Pooled Fund with a contribution of U.S. $ 1 million to the fund, provided 50% of the fundraising worth $ 2 million jointly launched by WHO and UNICEF earlier this month. The other 750,000 dollars have been granted by the Central Fund for Emergency Response Fund (CERF). The project of all these international structures to strengthen this struggle, about 700,000 beneficiaries and target is to support the efforts of the Government of the DRC to reduce mortality from the epidemic. This full support several components, including active case search, communication and social mobilization, psychosocial care and logistical support.
OCHA said that UNICEF, which deals with three aspects, has already started some activities in this framework since September 10.

Ebola Hemorrhagic Fever Outbreak in Democratic Republic of Congo (Updated September 24, 2012)

According to the DRC Ministry of Health report on September 17, 2012, a total of 18 confirmed cases (7 of them fatal), all from the Isiro area in DRC's Province Orientale, were reported. The case count also includes 32 probable and 32 suspect cases, with 26 deaths in these 2 categories. CDC teams are presently on the ground in DRC to assist the Ministry of Health in the epidemiologic and diagnostic aspects of the investigation. Laboratory support is provided both through CDC's field laboratory in Isiro, and through the CDC/UVRI lab in Uganda. The outbreak in DRC is not linked epidemiologically to the recently-ended Ebola outbreak in the Kibaale district of Uganda.

KINSHASA, Sept 27, 2012 (AFP) - An outbreak of Ebola fever in the Democratic Republic of Congo may have killed up to 33 people, while the number of suspected cases has risen, the health ministry said on Thursday.

A total of 79 cases was recorded, including 19 confirmed positive after laboratory analysis, 32 probable cases and 28 suspected cases, while 33 deaths were registered by September 25, the ministry announced. Seven of the deaths were proven to be caused by Ebola.

The World Health Organisation (WHO) said that the fatality rate was almost 42 percent.

The WHO added that a baby born prematurely in the isolation centre in Isiro -- the epicentre of the epidemic in northeastern DR Congo -- to a mother infected by the Ebola virus had died on Thursday, the statement said.
