Dungu is notable for reporting one fatal suspect VHF case last year that was actually confirmed to be due to Lassa:

It might be safe to assume that Marburg is also being tested for and hopefully being excluded, as it would also spread like Ebola.

Now the hard work begins. What is causing these cases? The lone Lassa fever case in 2011 took over two weeks to confirm, mainly because no one would even think to test for that virus in this area, as Lassa is endemic in West Africa, not Central Africa. Lassa is contagious, however, and could easily spread to HCW, although it did not in 2011. Spread of Lassa to HCW has been reported in West Africa.

Crimean-Congo Hemorrrhagic Fever (CCHF) is also native to Central Africa, hence its name.

The above article indicates that "bloody diarrhea" (likely dysentery) is also being reported from Orientale. Could the most severe cases of this be mistaken for VHF? Or might these suspected VHF cases be caused by something non-infectious?

I don't see any evidence from locations reported of any clustering (things like reports of contacts of cases taking ill), and these cases do seem spread over a somewhat large area. The above article also indicates that the six cases in Isiro have been reported over a period of seven weeks, which is a bit long to be a single cluster.

This is a BAD article. Note the bold, underlined text. Ebola has NOT yet been confirmed. There are a lot of other things that may be responsible for these cases.

It is irresponsible reporting to call this outbreak "Ebola" until laboratory confirmation is obtained.

Note also a slight discrepancy in the reporting: This article misses the two reported surviving cases in Dungu (have they been discarded?) and reports 3 of the 6 in Isiro as alive, whereas the weekly ReliefWeb report lists only 2.

A little article dated Aug. 15, from Radio Opaki also mentions suspected cases in the Ituri district, which is on the Sudan and Ugandan borders. That puts it between Isiro and Uganda. There's a big difference between suspected and confirmed but it's worth watching to see if anything more comes of this report.

The article in post 8 quotes Dr. Madindi who is the provincial medical inspector of the Eastern Province of DRC. I think he probably knows an ebola case when he sees it.

As to the ProMED article above, I do not see any link or substantive information to support the origin of any ebola cases in RDC.

I do not personally believe the animal bite story as referenced for the index case in the ebola outbreak in Uganda.

Might the reduction in case counts indicate some of the cases have been excldued due to alternative diagnoses? I don't like that only one sample (from a surviving case?) seems to have been taken, unless it is a pooled sample, as it runs the risk that single case might have an alernative diagnosis than some of the others.

Ebola Hemorrhagic Fever Outbreak in DRC

17 AUGUST 2012 - On 17 August 2012, the Ministry of Health (MoH) of the Democratic Republic of Congo notified the World Health Organization (WHO) of an outbreak of Ebola Haemorrhagic fever in the Isiro and Dungu Health Zones of Province Orientale in Eastern DRC. A total of 10 suspected cases (9 in Isiro and 1 in Dungu) and 6 deaths (5 deaths in Isiro and 1 in Dungu) have been reported.

Laboratory investigations conducted at the Uganda Virus Research Institute (UVRI), Entebbe, Uganda, confirmed Ebola virus (Bundibugyo species). Three samples taken from two patients turned out positive for Ebola. A National Task Force convened by the Congolese Ministry of Health, is working with several partners including WHO, MSF and CDC. A joint MoH, WHO and MSF emergency response team are in the field to conduct a detailed epidemiological investigation and case management.

WHO is supporting the Ministry of Health in the areas of coordination, surveillance, epidemiology, laboratory, case management, logistics for outbreak, public information and social mobilization. An additional team of experts from Congo, DRC and IST/Gabon comprised of an epidemiologist, a logistician, an anthropologist and social mobilisation officers are being mobilized for possible deployment in the field. Control activities that are being carried include active case finding and contact tracing, enhanced surveillance, case management, public information and social mobilization and reinforcing infection control practices.

WHO does not recommend that any travel or trade restrictions be applied to Democratic Republic of Congo.

twelve cases including seven deaths were recorded in Eastern Province.

Since this is a quote from the minister of public health I am going to change the thread to reflect this number.

The U.S. Embassy in Kinshasa informs U.S. citizens that the provincial medical inspector of Orientale Province has announced that there are suspected cases of the Ebola Virus in the towns of Isiro and Dungu. The Center for Disease Control (CDC) has laboratory confirmation of two cases. The Ebola Virus is one of the hemorrhagic fever illnesses. The virus is transmitted through contact with infected blood, bodily fluids, and handling or washing of infected corpses or through contact with objects that have been in contact with infected secretions. Avoid consumption of meat derived from primates. If you have any concerns, you should contact your healthcare provider.

Very interesting. So the outbreak is confirmed as the Bundibugyo ebolavirus, making this only the second outbreak ever confirmed due to this strain.

The original outbreak was in Bundibugyo district of Uganda, in 2007:

It would be interesting to know if there are any links between the cases in Isiro and Dungu. Having only 10-12 sporadic cases over nearly two months seems a bit odd.
