District Commissioner Freddy BosombaBUNIA, 21 August 2012 (IRIN) - Inter-ethnic tensions have again turned violent in north-eastern Democratic Republic of Congo?s Ituri region, compromising the delivery of humanitarian aid to more than 100,000 people, according to officials there.

Some 13 civilians have been killed in recent weeks, according to civil society sources.

An estimated 60,000 people, mostly civilians, were killed in Ituri between 1998 and 2003 as ethnically aligned armed groups vied for control of power and resources, notably gold deposits.

According to a 27 July memo by a coalition of civil society organizations, the decomposed bodies of five members of the Hema community were found in Kapuru, a settlement on the shore of Lake Albert in an area under the control of the Front de la resistance patriotique en Ituri (FRPI) armed group, about 100km south of Bunia, Ituri?s main city.

?The rebels captured six of them as they were heading to Uganda to visit a sick relative, demanding money,? recounted the brother of one of the victims.

?The rebels said, ?You have refused to join our group, we?re going to kill you,? and they opened fire. One of them escaped after the rebels cut off his hand with a machete,? he added.

The spokesperson for the Hema-sud community, Aliegera Kwonke, said, ?We in the Hema community feel targeted. The FRPI don?t allow us to go about our business, our roads are blocked, our crops looted. Our people are massacred under the noses of the army. We don?t feel safe.?

A few days after the bodies were found, a member of the Ngity community disappeared in the same area, prompting demonstrations in the lakeside villages of Nyamavi and Soni by Ngity youths brandishing machetes and other weapons.

On 2 August, the population of Ndimo, 160km southwest of Bunia, barricaded the road leading to North Kivu Province and displayed the corpses of five people killed by unidentified assailants.

With the civil unrest and roads being barricaded - it might be that many people are fleeing/traveling in the remote backwoods. Thus allowing more contact with the virus.

In 1998 the Orientale villages of Durba and Watsa were the center of an outbreak of Marburg hemorrhagic fever among gold mine workers, so we know the virus is endemic.

Commenting on the 1998 outbreak Dr. Bausch notes that the outbreak was "consistent with multiple parallel primary introductions rather than a single one amplified by secondary spread" - so the bug is out there in the wild.

KINSHASA, DR CONGO (BNO NEWS) -- Three health care workers in the northeastern region of the Democratic Republic of the Congo are believed to have died from Ebola, raising the number of fatalities from the outbreak to at least nine, local and international health authorities said on Tuesday.

The Congolese Ministry of Health said it is aware of at least fifteen suspected Ebola cases, including nine fatal cases. The initial death toll given on Friday was risen to nine after three health care workers died in the town of Isiro, located in the province of Orientale which borders the Central African Republic and South Sudan to the north and Uganda to the east.

In addition to the nine deaths in Isiro, three other suspected Ebola cases have also been reported in the town. Two suspected cases have further been reported in Pawa, about 42 kilometers (26 miles) south of Isiro, and one fatal case has been reported in Dungu, some 139 kilometers (86 miles) northeast of Isiro...

Ebola outbreak in DR Congo: ECDC rapid risk assessment

An outbreak of Ebola haemorrhagic fever is ongoing in the Democratic Republic of Congo (DRC) with 15 cases reported, including nine deaths in Orientale province in eastern DRC. ECDC has published a rapid risk assessment to assess the risk at an EU level associated with this outbreak.

The risk of the outbreak spreading to the EU is extremely low.

It is unlikely, but not impossible, that travellers infected in DRC could arrive in the EU while incubating the disease and develop symptoms while in the EU. These cases would most likely seek medical attention and be isolated, preventing further transmission.

It is the first time that the Ebola species Bundibugyo has been laboratory confirmed as the cause of an Ebola outbreak in DRC. This finding indicates that the current outbreak in DRC is not related to the recently ended outbreak in Uganda, which was caused by the Ebola species Sudan.

As the incubation period can last up to three weeks, it is likely that additional cases will be identified in the coming weeks.

However, control measures currently implemented in DRC with the support of international partners, e.g. isolation of cases and active monitoring of contacts, should prevent further spread of the disease.

The WHO is currently not recommending any travel or trade restrictions be applied to DRC.

This IRIN article, dated today, discusses the death of the hunter who found the dead antelope. But what is interesting is the reference to cases beginning about a month ago.

WHO supervised labs have determined that the ebola outbreaks are 2 different strains.

The ebola strain in Dem. Rep. of Congo is Bundibugyo.

The ebola strain in Uganda is Sudan.

This actually appears to be two spearate outbreaks. The case in Dungu got the virus from an animal (assuming he is confirmed), whereas the cluster in Isiro appears to be the result of H2H.

Actually, I see that the article doesn't even mention the hunter cooking the animal, making me wonder whether he could have died of something else. Is his one of the confirmed cases?

"The death case we got in Dungu was a hunter. Once in the bush he saw a dead antelope and did not care to ask why it had died. He immediately took [the carcass] and went to eat it... He fell sick and passed away," said the DRC's Health Minister Kabange Numbi.

I was also wondering if the case in Dungu has been confirmed. I can't find anything definite on that except this, from a ReliefWeb article posted earlier in this thread.

The machine translation makes it a little iffy. Perhaps someone who reads French better than I can provide a more accurate translation. But to me, I read it as something like, "...confirmed 2 positive cases of Ebola Bundibugyo strain in Isiro a town of more than 200 thousand...".

If this is correct, then it would seem that the hunter in Dungu has not been confirmed.

The WHO report of Aug. 17 lists the case in Dungu as suspected:

A total of 10 suspected cases (9 in Isiro and 1 in Dungu) and 6 deaths (5 deaths in Isiro and 1 in Dungu) have been reported.

So it would seem that unless some confirmation came in since Aug. 17, the Dungu case is only suspected.


A WireUpdate article, also posted here earlier, gives a better geographic picture.

Combined, the info definitely does make it seem that the Dungu case is kind of out there on its' own. But was there ever anything more found about the earlier report you posted regarding 3 cases of suspected VHF in Dungu?

In an article you posted earlier from http://rdc-humanitaire.net/images/do... 13 vf.pdf, those cases seem to have disappeared.

The translator is fouling us up there. The original French is:

I think that means 2 cases, 2 deaths in Pawa, and one fatal case in Viadana.

So I think the total number of suspected deaths is 14 only if you count the hunter in Dungu, who is not in that report. Otherwise, it's 13 (10 in Isiro, 2 Pawa, and 1 in Viadana).

As far as suspected cases, it looks like 16 in Isiro, plus the 3 above, plus the 1-3 in Dungu, for a total of 20-22.

In a report posted yesterday, MSF has different numbers. I'm wondering if this is info from an MSF team on-site in Isiro and so only includes cases actually in Isiro. From what I can find, there are still only 2 lab confirmed cases. So if one of those is in Isiro, where is the other one?

Ebola outbreak in Democratic Republic of Congo ? update

The Ministry of Health (MoH) of the Democratic Republic of Congo (DRC) continues to work with partners to control the outbreak of Ebola haemorrhagic fever.

As of 28 August 2012, a total of 24 (6 probable, 6 confirmed and 12 suspected) cases and 11 deaths had been reported from Province Orientale.

The reported cases and deaths occurred in 3 health zones as follows: 17 cases and 9 deaths in Isiro; 6 cases and 2 deaths in Viadana; 1 case and 0 death in Dungu. The fatal cases in Isiro include three health-care workers.

All alerts have been investigated and so far there has been no Ebola suspected or confirmed case reported from outside Province Orientale.

Initial samples were tested by Uganda Virus Research Institute (UVRI) in Entebbe, Uganda, and three samples taken from two patients were confirmed for Ebola virus (Ebola subtype Bundibugyo).

US Centers for Disease Control and Prevention (CDC) has established a field laboratory in Isiro to support case management and epidemiological investigations. A National Task Force convened by the Congolese Ministry of Health is working with several partners including WHO, M?decins Sans Fronti?res (MSF), International Federation of Red Cross and Red Crescent Societies (IFRC) and CDC to control the outbreak.

A joint MoH, WHO and MSF emergency response team is in the field to conduct a detailed epidemiological investigation and case management. WHO has deployed epidemiologists and logisticians from AFRO regional office (including the Inter-country Support team) and WHO headquarters to support the MoH and is working closely with partners in the areas of coordination, surveillance, epidemiology, logistics for outbreak, public information and social mobilization.

WHO?s Global Outbreak Alert and Response Network (GOARN) has deployed an anthropologist from the Museum National d?Histoire Naturelle (MNHN), France to assist with a clearer understanding of the social and anthropological issues among the affected population that could impact the on-going response efforts.

Control activities that are being carried out include active case finding and contact tracing, enhanced surveillance, case management, public information and social mobilization and reinforcing infection control practices.

WHO does not recommend that any travel or trade restrictions be applied to Democratic Republic of Congo.

There is currently no indication that this Ebola outbreak is related to the recent Ebola outbreak in Uganda occurring in Kibaale district.



General information on Ebola subtypes

There are five identified subtypes of Ebola virus.

The subtypes have been named after the location they have been first detected in Ebola outbreaks.

Three subtypes of the five have been associated with large Ebola haemorrhagic fever (EHF) outbreaks in Africa. Ebola-Zaire, Ebola-Sudan and Ebola-Bundibugyo.

EHF is a febrile haemorrhagic illness which causes death in 25-90% of all cases.


The Ebola Reston species, found in the Philippines, can infect humans, but no illness or death in humans has been reported to date.

I've been watching and haven't seen much of anything on this from any media for several days and was pretty curious about why the info dried up.

And now this blows my mind:

If you notice the wording of the WHO report above, it now refers to "1 case and 0 death in Dungu". All previous reports have mentioned the Dungu case as fatal. Was that a mistake? Or has the original fatal case in Dungu been discarded and replaced with a non-fatal one? The original posts did indicate three suspects in Dungu, one of whom had died.

As of 28 August 2012, a total of 24 (12 suspected, 6 probable and 6 confirmed) cases and 11 deaths had been reported in Province Orientale.

The reported cases and deaths (suspected, probable and confirmed) have occurred in 3 health zones as follows: 17 cases and 9 deaths in Isiro, including three (3) health care workers who have died; 6 cases and 2 deaths in Viadana; and 1 fatal case in Dungu. One alert was received from Gombari health zone, yet to be verified.


reports the case as fatal, but the numbers do not add up. The sum of the deaths for the three zones (9+2+1) is 12, not the 11 reported.

So what gives?

I noticed that also. I've been looking this evening and so far can't find anything to clarify it.

The version of the report on the GAR site (http://www.who.int/csr/don/2012_08_30/en/index.html ) shows the same info, so it isn't just the version posted to the afro.who site.:

"...30 August 2012 - The Ministry of Health (MoH) of the Democratic Republic of Congo (DRC) continues to work with partners to control the outbreak of Ebola haemorrhagic fever. As of 28 August 2012, a total of 24 (6 probable, 6 confirmed and 12 suspected) cases and 11 deaths had been reported from Province Orientale. The reported cases and deaths occurred in 3 health zones as follows: 17 cases and 9 deaths in Isiro; 6 cases and 2 deaths in Viadana; 1 case and 0 death in Dungu. "


Not only does the death toll not add up, but AFAIK, the only case in Dungu was that of the hunter who died after supposedly eating the antelope meat. So if his death isn't being counted, has there been another case in Dungu? I'm thinking that this is just a typo and the 1 counted in Dungu should have been listed as a death and not a case. If not, then things are really screwy.

The other thing I noticed is this:
"...Initial samples were tested by Uganda Virus Research Institute (UVRI) in Entebbe, Uganda, and three samples taken from two patients were confirmed for Ebola virus (Ebola subtype Bundibugyo)."

In this entire outbreak, of supposedly 24 cases, there have been only 2 cases that were confirmed by testing? We were discussing those 2 confirmed cases over a week ago. There's been no further confirmations? Or no further samples? Or all other samples tested negative for Ebola? Really?

I know things are bad in the Congo so maybe this confusion is not coming so much from the WHO as it is from local authorities. But someone needs to get the facts straight, or the WHO will lose what credibility it has.

The health zone of Dungu reported three suspected cases and one death of suspected VHF during the month of August. The results of the investigation and charges are expected for better learning of the situation.

This was earlier in the thread.

My guess is that the fatal case in the hunter has been discarded, and some other surviving case is the current suspect in Dungu. Either that or the hunter never died, and the report of his death was incorrect.

I'm not that concerned about that only two cases have been confirmed. Comfirmation requires sending samples to Kinshasa or Kampala, something that has some cost and effort. I don't think too many other samples were taken. It appears that about 8 of the deaths occurred before the first report, so there may have been no sample to take.

Good catch on that, thanks! So the difference between the confirmed case count (both 2 and 6 being mentioned in the same WHO article) could be due to a time lag. If the cases were confirmed at about the same time that the latest WHO report was being put together, I can see how the confusion occurred.
