Pandemic Influenza (H1N1) 09 Swine Flu - Update 124

The number of GP consultations for influenza-like illnesses appears to have decreased slightly but it is too soon to determine whether this is a temporary dip or the start of an ongoing fall in swine flu cases.

Health Minister Tony Ryall says it is common for influenza numbers to rise and fall over several months and this pandemic may follow a similar pattern.

The next few weeks will be critical in better understanding the future path of the pandemic.

The general practice sentinel surveillance system shows a general drop in consultation rates in the week to 26 July, however a number of District Health Boards have not followed this trend. In the same calendar week ILI consultations increased in Taranaki, MidCentral, Wairarapa, South Canterbury, Canterbury, Otago and Southland. GP consultation rates for school age children with flu symptoms declined during the July school holidays and have since increased slightly in line with previous years.

Mr Ryall says the pandemic influenza had an early and rapid impact on Auckland, Wellington and Christchurch, before spreading to neighbouring regions including South Canterbury, Hawkes Bay, Northland and Waikato.

Figures suggest that in Auckland and Wellington, pandemic influenza may be slowly declining, but this could simply be part of the normal fluctuation you would expect, particularly with a new virus.

Mr Ryall says "We are seeing different pictures of swine flu activity across different regions. For instance, the numbers of confirmed H1N1 hospitalisations in Lakes, MidCentral, Nelson Marlborough, Otago, South Canterbury, Southland, Tairawhiti, Taranaki, Wairarapa, West Coast, and Whanganui DHBs are still low but it may be the virus has not reached its peak in these regions."


Latest Numbers


Numbers of people in hospitals with the Pandemic Influenza (H1N1) 09

As at midday today, a total of 52 people are reported to be in hospital with Pandemic Influenza (H1N1) 09 or its complications, including 20 in intensive care.

The number of patients currently in intensive care with confirmed Pandemic Influenza (H1N1) 09 broken down by region is as follows:
Northland (3) Auckland (8), Counties Manukau (4), Lakes (1), Nelson Marlborough (1) Canterbury (1), Otago (1), Southland (1)

There are a total of 2872 confirmed cases of Pandemic Influenza (H1N1) 09 swine flu, up from 2855 yesterday.

The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms are being tested.

This is because for most people, it's a mild illness and they will recover readily at home without needing medical treatment.

The number of deaths from Pandemic Influenza (H1N1) 09 swine flu is 14.

Please note these 14 deaths reported by the Ministry are deaths from swine flu, where swine flu was a primary cause of death.


School absences


Early responses to a Ministry of Education "snapshot" survey of all schools on Friday 31 July show that in general schools are reporting usual levels of absence for this time of year, while only a minority of the of schools report either lower or higher levels.

Deputy Director of Public Health Dr Fran McGrath says this may be because people have been taking note of the message to stay home if they are sick, to help slow the spread of influenza.


Healthline Calls


The high number of influenza-related calls to Healthline has continued over the past week, although numbers have decreased since peaks in mid-June and early July, when calls to the Healthline 0800 number exceeded 6000 a day. The number of calls answered by Healthline nurses continues to be at least 20 percent above normal levels.


GP Consultations


The data below is from ESR's sentinel general practice surveillance system and shows the highest weekly influenza-like illness (ILI) consultation rates are nearly three times higher than the winter peak experienced in the last two years.

Pandemic Influenza (H1N1) 09 Swine Flu - Update 125

Efforts by the health sector to treat influenza and delay its spread in New Zealand communities, continues.

As reported by the Ministry yesterday, the number of people visiting their GP with influenza-like illnesses has slightly decreased in recent days, but it remains unclear as to whether this will be a temporary dip or not.

Deputy Director of Public Health Dr Fran McGrath says people who have influenza-like symptoms which worsen, should phone for medical advice. Symptoms which may require further medical advice include difficulty breathing or chest pain, a high temperature, drowsiness, severe vomiting or a cough with blood or green phlegm

"Pregnant women and people with heart problems, lung conditions, and some other medical conditions are at greater risk of severe illness from influenza and should also seek medical advice early if they have flu-like symptoms."

Latest Numbers


The total number of confirmed cases of Pandemic Influenza (H1N1) 09 swine flu is 3021, up from 3000 yesterday.

These are people who have returned a laboratory positive test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms are being tested to better inform their treatment. This is because for most people, it's a mild illness and they will recover readily at home without needing medical treatment.

The WHO reports that transmission of the pandemic influenza (H1N1) 09 virus is declining globally, especially in temperate regions of the southern hemisphere (with the exception of South Africa). The virus continues to spread in the northern hemisphere, although areas first affected by the virus are seeing less activity. Tropical areas are seeing a peak in pandemic influenza cases at a time when the usual seasonal influenza peaks would occur. The WHO warns of the risk of further spread in highly populated areas as community spread begins in Asia and Africa.

The H1N1 virus is now the dominant influenza strain is most parts of the world, and is expected to remain so during the coming northern hemisphere influenza season.

The number of deaths from Pandemic Influenza (H1N1) 09 swine flu is 16, following the death of a Waikato woman with underlying medical conditions, in a hospital in Auckland. The family have asked for privacy at this time.

Pandemic Influenza (H1N1) 09 Swine Flu - Update 143

The amount of influenza in the community continues to decline as winter draws to a close, but health officials say people should continue the good hygiene habits they have developed in order to reduce their chance of getting and spreading influenza as well as a myriad of other infections which circulate all year.

Pandemic influenza (H1N1) 09 swine flu has been the dominant influenza strain in New Zealand this winter and although it looks to be abating as the weather warms up, it is unlikely to disappear completely.

Deputy Director of Public Health, Dr Fran McGrath says this virus will continue to cause influenza because most of us have not been exposed to the virus and have no immunity to it.

She says, "People have really taken heed of the tried and true hygiene messages - cover coughs and sneezes, wash your hands often and stay home if you are sick. These simple action appear to have helped delay the spread of this virus and lessened the impact of this pandemic on our communities.This really is an example of people being concerned for each other and working together as a community to protect one another."

The hard work of health practitioners, District Health Boards (DHBs), border control agencies and wider communities helped contain this pandemic influenza and delay its initial spread by six or seven weeks. This enabled plans to be put in place to ensure people would get the services they needed, and health services would be able to manage the increased patient load.

Dr McGrath says,"The health sector has done that very well overall. DHBs report there has been less gridlock in hospitals than expected, and we hear there are fewer absences from schools and work places."

Continuing good hygiene habits of covering coughs and sneezes, hand washing and staying at home while sick will limit the spread of most communicable diseases, including measles, meningitis and skin infections.

Dr McGrath says the focus is now on planning to be ready for any increase in cases in the coming months, and ensuring health services can respond rapidly if the virus becomes more widespread or more virulent.

These are people who have returned a positive laboratory test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms are being tested. This is because for most people, it's a mild illness and they recover readily at home without needing medical treatment.

As mentioned above, the number of deaths from Pandemic Influenza (H1N1) swine flu is now 16. These are deaths where swine flu was a primary cause. Other deaths are being investigated by the Coroner's office.

Numbers of people in hospitals with the Pandemic Influenza (H1N1) 09


As at midday today, a total of 10 people are reported to be in hospital with Pandemic Influenza (H1N1) 09 or its complications, including six people in intensive care.

The number of patients currently in intensive care with confirmed Pandemic Influenza (H1N1) 09 broken down by region is as follows:

Auckland (2), Waitemata (1), Wellington (1), Canterbury (2).

School absences


In general, schools are reporting usual levels of absence for this time of year, while only a minority of schools are reporting either lower or higher levels.

Healthline Calls


Healthline has continued to receive a high number of influenza-related calls over the past week, although numbers have decreased since peaks in mid-June and early July. The total number of calls answered by Healthline nurses continues to be about 20 percent above normal levels.

GP Consultations


The data below are from Environmental Science and Research's (ESR) sentinel general practice surveillance system. It shows that at its peak, the weekly influenza-like illness (ILI) consultation rates this year were nearly three times higher than the winter peak experienced in the last two years. The number of ILI consultations has decreased over recent weeks, but there are different pictures of influenza activity across regions and across some age groups.

Swine flu' vaccine:


The Ministry of Health has been advised by Baxters Healthcare Ltd that the first shipment of the pandemic influenza vaccine will not arrive in New Zealand until early next month.

The Government also has an option arrangement in place with CSL for their pandemic vaccine, but is yet to consider whether it is necessary to purchase any supplies of this vaccine.

Both vaccines will undergo clinical trials and would only be used once licensed by Medsafe. If a vaccination campaign were to proceed, frontline health workers would likely begin to be vaccinated early in 2010.

In late September, the WHO will also consider whether to include the pandemic strain in next year?s seasonal influenza vaccine for the Southern Hemisphere, and this will have an impact on Ministry considerations.

International Update from the World Health Organization


The WHO reports that transmission of the pandemic influenza (H1N1) 09 virus is declining globally, especially in temperate regions of the southern hemisphere (with the exception of South Africa). The virus continues to spread in the northern hemisphere, although areas first affected by the virus are seeing less activity. Tropical areas are seeing a peak in pandemic influenza cases at a time when the usual seasonal influenza peaks would occur. The WHO warns of the risk of further spread in highly populated areas as community spread begins in Asia and Africa.

The H1N1 virus is now the dominant influenza strain is most parts of the world, and is expected to remain so during the coming northern hemisphere influenza season.

The number of deaths from Pandemic Influenza (H1N1) 09 swine flu is 17. The latest person to die from swine flu was a man in his 30s who died in Auckland yesterday. He had an underlying condition.

Please note these 17 deaths reported by the Ministry are deaths from swine flu, where swine flu was a primary cause of death. Other deaths are being investigated by the Coroner's office.

There are a total of 3130 confirmed cases of Pandemic Influenza (H1N1) 09 swine flu, up from 3127 on 28 August. These are people who have returned a positive laboratory test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms are being tested. This is because for most people, it's a mild illness and they will recover readily at home without needing medical treatment.

NZ experience reflected in WHO advice

WHO is monitoring outbreaks from different parts of the world to make some tentative conclusions about how the pandemic might evolve in the coming months. The clinical picture of pandemic influenza is largely consistent around the world - the majority of patients continue to experience a mild illness, Although the virus can cause more severe and occasionally fatal illness in young and healthy people, pregnant women and those with chronic underlhing health conditions, the number of such cases remains small.

The experience of New Zealand hospitals and intensive care units (ICUs) is reflected in an update on the impact of pandemic influenza released by the World Health Organization. New Zealand ICUs have reported that between 12 - 15% of patients admitted to hospital with pandemic influenza require treatment in an intensive care unit and this is similar to the rates experienced in other countries.

WHO is advising countries in the northern hemisphere to prepare for a second wave of pandemic spread. They advise countries in temperate parts of the southern hemisphere to remain vigilant. International experience has shown that localised "hot spots" of increasing transmission can continue to occur even when the pandemic has peaked at the national level.

There are a total of 3143 confirmed cases of Pandemic Influenza (H1N1) 09 swine flu, up from 3130 on 31 August. These are people who have returned a positive laboratory test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms have been tested.

The number of deaths from Pandemic Influenza (H1N1) swine flu is 17. These are deaths where swine flu was a primary cause. Other deaths are being investigated by the Coroner's office.

The number of swine flu cases appears to be abating in New Zealand but the virus will not disappear altogether. It is important people maintain good hygiene habits : covering coughs and sneezes, hand washing and staying at home while sick.


Numbers of people in hospitals with the Pandemic Influenza (H1N1) 09

As at midday today, a total of seven people are reported to be in hospital with pandemic Influenza (H1N1) 09 or its complications. DHBs are no longer required to separately report the number of people with pandemic influenza who are in Intensive Care Units (ICU). This is because the numbers are now low and the pressure on ICU beds is reduced.

Healthline Calls

Healthline has continued to receive a high number of influenza-related calls over the past week. The total number of calls answered by Healthline nurses continues to be about 20 percent above normal levels.

GP Consultations

The data below are from the Institute of Environmental Science and Research's (ESR) sentinel general practice surveillance system. It shows that at its peak, the weekly influenza-like illness (ILI) consultation rates this year were nearly three times higher than the winter peak experienced in the last two years. The number of ILI consultations continues to decrease.

The World Health Organisation (WHO) advises evidence from multiple outbreak sites demonstrates that the H1N1 pandemic virus has rapidly established itself and is now the dominant influenza strain in most parts of the world. The pandemic will persist in the coming months as the virus continues to move through susceptible populations.

Close monitoring of viruses by a WHO network of laboratories shows that viruses from all outbreaks remain virtually identical. Studies have detected no signs that the virus has mutated to a more virulent or lethal form.

Likewise, the clinical picture of pandemic influenza is largely consistent across all countries. The overwhelming majority of patients continue to experience mild illness. Although the virus can cause very severe and fatal illness, including in young and healthy people, the number of such cases remains small.

While these trends are encouraging, large numbers of people in all countries remain susceptible to infection. Even if the current pattern of usually mild illness continues, the impact of the pandemic during the second wave could worsen as larger numbers of people become infected.

WHO is advising countries in temperate parts of the southern hemisphere to remain vigilant. As experience has shown, localized ?hot spots? of increasing transmission can continue to occur even when the pandemic has peaked at the national level. Countries in the northern hemisphere are advised to prepare for a second wave of pandemic spread. Countries with tropical climates, where the pandemic virus arrived later than elsewhere, are also advised to prepare for an increasing number of cases.

Pandemic Influenza (H1N1) 09 Swine Flu - Update 150

Latest Numbers


The number of deaths from Pandemic Influenza (H1N1) 09 swine flu is 17.

Please note these 17 deaths reported by the Ministry are deaths where swine flu was a primary cause of death. Other deaths are being investigated by the Coroner's office.

There are a total of 3151 confirmed cases of Pandemic Influenza (H1N1) 09 swine flu, up from 3050 on 9 September. These are people who have returned a positive laboratory test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms are being tested. This is because for most people, it is a mild illness and they will recover readily at home without needing medical treatment.

New Zealand's Health services are managing well and the number of swine flu cases appears to be abating in the country. However, the virus is still circulating and it is important people maintain good hygiene habits: covering coughs and sneezes, regular hand washing and staying at home while sick.

International Update from the World Health Organization


NZ experience reflected in WHO advice

WHO is monitoring outbreaks from different parts of the world to make some tentative conclusions about how the pandemic might evolve in the coming months. The clinical picture of pandemic influenza is largely consistent around the world - the majority of patients continue to experience a mild illness, Although the virus can cause more severe and occasionally fatal illness in young and healthy people, pregnant women and those with chronic underlying health conditions, the percentage of such cases remains small.

The experience of New Zealand hospitals and intensive care units (ICUs) is reflected in an update on the impact of pandemic influenza released by the World Health Organization. New Zealand ICUs have reported that between 12 - 15% of patients admitted to hospital with pandemic influenza require treatment in an intensive care unit and this is similar to the rates experienced in other countries.

WHO is advising countries in the northern hemisphere to prepare for a second wave of pandemic spread. They advise countries in temperate parts of the southern hemisphere to remain vigilant. International experience has shown that localised "hot spots" of increasing transmission can continue to occur even when the pandemic has peaked at the national level.

The H1N1 virus is now the dominant influenza strain is most parts of the world, and is expected to remain so during the coming northern hemisphere influenza season.

Pandemic Influenza (H1N1) 09 Swine Flu - Update 151



The number of swine flu cases appears to be abating in New Zealand but the virus is still circulating. It is important people maintain good hygiene habits: covering coughs and sneezes, regular hand washing and staying at home while sick.



Latest Numbers

There are a total of 3152 confirmed cases of Pandemic Influenza (H1N1) 09 swine flu, compared with 3151 on 11 September. These are people who have returned a positive laboratory test for pandemic influenza (H1N1) 09. The actual number of cases of Pandemic Influenza (H1N1) 09 will be significantly higher, as only a small proportion of people with symptoms have been tested.

The number of deaths from Pandemic Influenza (H1N1) swine flu where swine flu was a primary cause remains at 17. Other deaths are being investigated by the Coroner's office.

Numbers of people in hospitals with the Pandemic Influenza (H1N1) 09

As at midday today, only one person was reported to be in hospital with pandemic Influenza (H1N1) 09 or its complications.

Healthline Calls

The number of influenza-related calls to Healthline has reduced over recent weeks but remains significantly higher than at the same time last year. The total number of calls [of all call types] answered by Healthline nurses is about 15 percent above normal levels.

GP Consultations

The data below are from the Institute of Environmental Science and Research's (ESR) sentinel general practice surveillance system. They show that at its peak, the weekly influenza-like illness (ILI) consultation rates this year were nearly three times higher than the winter peak experienced in the last two years. The number of ILI consultations continues to decrease, although still remains higher than at the same time in previous years.

In the temperate region of the southern hemisphere (represented by countries such as Chile, Argentina, Australia, New Zealand, and South Africa), influenza activity continues to decrease or return to baseline.

Active transmission persists in tropical regions of the Americas and Asia. Many countries in Central America and the Caribbean continue to report declining activity for the second week in a row. However, countries in the tropical region of South America (represented by countries such as Bolivia, Ecuador, and Venezuela) are reporting increasing levels of respiratory disease. In the tropical regions of Asia, respiratory disease activity remains geographically regional or widespread but the trend is generally increasing as noted in India, Bangladesh, and Cambodia.

In the temperate regions of the Northern Hemisphere activity is variable. In the United States, regional increases in influenza activity are being reported, most notably in the south eastern states. Most of Europe is reporting low or moderate respiratory diseases activity, but parts of Eastern Europe are beginning to report increases in activity.

Pandemic (H1N1) influenza virus continues to be the predominant circulating virus of influenza, both in the northern and southern hemisphere.

While numbers of swine flu cases in New Zealand appears to be declining the virus is still circulating. It is still important that good hygiene habits are kept up to dampen down spread of the virius from one person to another - that means covering coughs and sneezes and regular, thorough washing and drying of hands. It is also important people stay at home if they become unwell - in other words don't take viruses out of the house - and seek medical help if they have underlying health conditions or symptoms worsen.

Pandemic Influenza (H1N1) 09 Swine Flu - Update 153

As at midday today two people are reported to be in hospital with Pandemic Influenza (H1N1) 09 or its complications.

Since May 2009 the cumulative total of people in New Zealand who have returned a positive laboratory test for pandemic influenza (H1N1) 09 is 3163, compared with 3161 reported on Wednesday last week.

The number of deaths from Pandemic Influenza (H1N1) swine flu where swine flu was a primary cause is 18, following the death this week in Auckland of a 67 year old woman reportedly without the underlying health conditions that have been associated with more severe illness. Some deaths are still being investigated by the Coroner.

Border control

The Ministry of Health intends to today suspend inflight cabin announcements around Swine flu, and return to the normal levels of health screening of flights in and out of New Zealand.

The reported level of influenza has declined to levels similar to the level at this time in the past two years. Influenza is still circulating, but this measure better reflects the reduced level of risk.

A public health presence will be maintained at international airports. Help is still available for passengers identified as potentially sick by aircrew or who self-identify. Anyone needing assistance will be assessed by a paramedic then transported to hospital or advised how to seek medical help.

Tamiflu

Medsafe - the Ministry of Health unit responsible for assessing medicines and vaccines - has approved a two year extension to expiry dates currently shown on boxes of Tamiflu in New Zealand.

This extension follows Medsafe's evaluation of data supplied by Roche, the manufacturer of Tamiflu, demonstrating that the product continues to meet its safety and quality specifications for two years beyond the original date. Medsafe reviewed this data and approved extending Tamiflu?s shelf life from five years to seven years, meaning two years can be added to the expiry date shown on packet and foil packs containing Tamiflu capsules.

The move follows a decision by regulatory authorities in Australia, Canada, Hong Kong, the US and the European Union which have also extended Tamiflu?s shelf life by two years.

There are few changes to report and influenza activity including swine flu is now the same as would be expected at this time of year.

As at midday today four people are in hospital with confirmed swine flu (H1N1) 09 or its complications ? up from two last week.

The cumulative total of people in New Zealand who have returned a positive laboratory test for swine flu (H1N1) 09 is 3174, up 11 from last week

The number of deaths from Pandemic Influenza (H1N1) swine flu is unchanged from last week at 18. Some deaths are still being investigated by the Coroner.

Vaccination

Health Minister Tony Ryall says the first of the 300,000 doses of swine flu vaccine for the protection of frontline health workers on order from Baxter Healthcare Ltd arrived in September. He says 72,000 doses are now in the country and the remainder will be here by December.

"The vaccine will be available for use following assessment by Medsafe. This is expected to be in February 2010. If circumstances change the vaccine could be assessed and potentially used more quickly" says Mr Ryall.

"The WHO has recommended the seasonal flu vaccine for the southern hemisphere 2010 flu season should contain the swine flu (H1N1) 09 strain " says Mr Ryall.

The seasonal influenza immunisation programme usually runs from 1 March to 20 June.

Seasonal immunisation is available to everyone through their health provider and to some people through their employer. It is subsidised for those aged 6 months to 64 years with certain pre-existing health conditions, and for all those over 65.

The Minister says New Zealand health authorities are continuing to monitor international developments but are not at this stage recommending swine flu vaccination for the general population.
