"Right now the H1N1 may be in Ciudad Ju?rez, El Paso and elsewhere in the world because it appears climate change with the season," he said.

According to the doctor, the exact moment of contagion is not always possible to specify with certainty: "It can happen on a bridge in a mall, a bus in the street, without the person realizing."

And he reiterated: "The flu is everywhere, whether seasonal or H1N1 (which currently is seasonal)."

But how the virus is passed?

Castellanos said that only three factors must match "(1) an infected person, (2) otherwise healthy person, and (3) that both coincide at the same time so that the patient will pass the virus to her healthy."

He argued that the main route of infection is aerosol droplets expelled with secretions of patients with H1N1 to the healthy person, "a handshake, a sneeze, a laugh, among others."

The statement made by the expert in the above post may be too much optimistic about the level of herd immunity in Mexicans (or in general world poupulation) against H1N1 (2009):

1) the level of immunity in the population may have decreased against H1N1 (2009);
2) the low viral circulation for almost an year may have further increased the susceptible population ratio;
3) trivalent seasonal vaccines, including H1N1 (2009) strain, although represent a good option for controlling winter outbreaks, demonstrated in some epidemiological studies a lower than expected effectiveness in preventing disease in vaccinated people;
4) H1N1 (2009), as all other influenza viruses, undergoing small changes, with selection of drift variants, and the susceptible portion of population may fall ill even though the virus is not longer in the 'pandemic' status; further, people who had the flu during 2009-2010 could have a reduced immunity against new drift variants.

Of those confirmed, 11 have been reported by hospitals in the Health Services Chihuahua, two in hospital belonging to the Chihuahua Institute of Health, two by the IMSS and two in private practice.

OK. Now that's a bit unusual. Two fatalities plus one in the ICU. It appears that at least four individuals from that station have been confirmed as H1N1 cases (plus the original fatality they could not confirm), so it is unlikely the outbreak is due to some other agent.

Perhaps the officers were co-infected with something such as MRSA? Perhaps something else was impairing their immune system? Could they all have had some pre-existing condition? While it is possible that the virus could have mutated, I am hesitant to suspect that because no severe illness has been reported (that we know) in family members or contact HCW. I do note that I haven't seen any mention of Tamiflu in this outbreak, which certainly could save lives.

There have been a number of media reports referring to a mutated pH1N1 in Mexico, but that could just be heresay and paranoia. However, the EuroFlu Bulletin for Week 12 indicated that about two-thirds of the pH1N1 isolates were sub-clades that had departed from the A/California/7/2009 H1N1 Pandemic strain. Furthermore, one of the subclades (from England) with a mutation at the receptor binding domain - S185T - outnumbered the California clade. This mutation was associated with more severe cases in England.

So, it's very plausible that the virus circulating in Mexico, and possibly Venezuela, is one that can evade prior immunity and cause more serious illness. But, we won't know until some sequences are released. For some reason there have been no pH1N1 sequences released of specimens collected in the U.S. since December. It sounds like the outbreak in Mexico was related, in part, to contacts in Texas, so even release of some U.S. sequences would be helpful. It is even possible, based on the isolates data at the state level and the severity of illness, that two strains of pH1N1 are circulating in different parts of the U.S.; one similar to A/California/7/2009 and one like one or more of the subclades detected in Europe. That could explain why some states that have had pH1N1 co-circulating with seasonal varieties since before December have not had much serious illness, and other states started experiencing severe illnesses and deaths as soon as pH1N1 began showing up.

Anyway, we need data from North America.

Genetic characterization is pointing toward an increase in genome heterogeneity among H1N1 (2009) influenza viruses.

However, to date, the antigenic properties of recent isolates belonging to different clades seem to be much more homogeneous - ie the vaccine should works against circulating strain (at least until they don't suffer for major antigenic drifts).

It remains to be established the effectiveness on the field of trivalent vaccines composition after some studies pointed toward a lower than expected protection (it seems to range between 45% to 65%...).

Confirmation of a fifth fatality, one that has been previously reported. Of the now 21 confirmed cases, 5 have died, 4 are hospitalized (1 grave), and the other 12 are recovering or have recovered at home. No indication which category the three surviving confirmed officers fall into.

Of note, there are currently only two unconfirmed deaths (the original discarded officer, and one more recent death still pending testing), and at most one severe unconfirmed case (combining this article with the above one - and it might indicate a confirmed case that has recovered somewhat in the past day), so the speculation that they are having trouble confirming cases seems extremely unfounded.

ACCORDING TO THE OFFICIAL REPORT OF 01 APRIL DAY OF THE MINISTRY OF HEALTH OF CHIHUAHUA: 21 people have tested positive for influenza A virus H1N1, of whom five have died.


Based study of the universe according to epidemiological criteria to track the H1N1 influenza outbreak, which is 92.21 have been confirmed, 23 dismissed, 18 are considered suspects without sample and 29 are suspects in the study.

Importantly, the 7 deaths have been subjected to laboratory studies. Of them exists only official confirmation of the presence of H1N1 virus in 5 people, 1 is in laboratory study and 1 is discarded. 5 deaths occurred in Ciudad Juarez and in Chihuahua 2.

The Health Ministry stated that the number of cases confirmed as positive for H1N1 virus is 21, in the period from day 22 to April 1, 2011.

Of those confirmed, 3 were found in clinically stable, 1 is severe and 5 people have died. Of those 4 are confirmed and 12 hospitalized more convalescing at home.

Confirmed cases of influenza, by age group, under 4 to range from 15 to 24 years, 11 to group 25 to 44 and 6 more at an age ranging between 45 and 64 years of age.

Of those confirmed, 11 have been reported by hospitals in the Health Services Chihuahua, 2 in hospital belonging to the Chihuahua Institute of Health, 6 by the IMSS and 2 in private practice.

By Municipality, people confirmed H1N1 influenza occurred, 8 in Chihuahua and 13 in Ciudad Juarez.
