JAKARTA, Feb. 12 (Xinhua) -- A 15-year-old Indonesian girl from West Jakarta has been infected by avian influenza, bringing the total cases to 127 with 103 of them fatal, Indonesian health ministry said here Tuesday.
The girl was a daughter of a 38-year-old woman who was previously infected by the disease and first developed the symptoms of the disease on Jan. 23, said the spokeswoman of the ministry Lily Sriwahyuni Sulistyowati.
The girl was now in critical condition at a hospital, said Sulistyowati.
"Both of her laboratory tests showed today that the girl is positive on bird flu," she told Xinhua.
She said that it was not clear yet whether the girl had historical contacts with fowls.
"Investigations into the source of her infection are ongoing. However, she was exposed to her sick mother on Jan. 27-28 and spent time in a neighborhood where chickens and other birds were found," said Sulistyowati.
"Samples from these birds have been taken and are undergoing tests to determine whether they may have been the source of infection," she said.
Contact with fowls has been the common source of the infection of the disease in Indonesia.
The spokesperson said that the girl first showed the symptoms of the H5N1 virus on Feb. 2 and was hospitalized on Feb. 8. Experts have said that improper initial medical treatment and poor hygiene of the traditional poultry market have been blamed for the highest fatality rate of bird flu in Indonesia, the world's fourth most populous country with a population of over 240 million. Indonesia has been grappled with major problem on its effort to stop the spread of the virus spread, including huge territory, traditional way of rising chickens on back yard and lack of obedience of provincial administration in implementing the Jakarta decision to stop the virus spread.

H5N1 H2H Transmission in Tangerang Raises Concerns

Recombinomics Commentary 16:25
February 12, 2008

The confirmed H5N1 familial cluster in the Tangerang area raises pandemic concerns. Prior to this familial cluster, there was a geographic cluster in the Tangerang area. This type of cluster signals more efficient H5N1 transmission to humans.

Now the geographic cluster has been overlaid with a familial cluster, which has been confirmed and is almost certainly human to human transmission based on disease onset dates and acknowledged association between the mother and daughter.

H5N1 data out of Indonesia has been difficult. Sequences have been withheld for over a year, and the liberal use of Tamiflu blankets limits the number of lab confirmed H5N1 cases.

However, in spite of these limitations, a confirmed human cluster has emerged on the heals of clustering in the Jakarta area. The withholding of the sequence data is hazardous to the world?s health, and the denial of human to human transmission increases the likelihood of familial transmissions.

In the current cluster, the exposure of the daughter to her mother appears to be after the mother was hospitalized, raising concerns about H5N1 containment in hospital settings. In the past, familial exposures were linked to the misdiagnosis of patients, which may be the case here because the index case was transferred on January 29, which was after the daughter's exposure when her mother was hospitalized in Tangerang on January 26.

More information on the exposure, as well as sequence data from the mother and daughter, would be useful.

Daughter of Indonesian H5N1 patient infected

Feb 12, 2008 (CIDRAP News) ? The World Health Organization (WHO) today reported that a 15-year-old girl from Indonesia is hospitalized in critical condition with an H5N1 avian influenza infection, a week after her mother's H5N1 illness was announced.
The girl, who is from West Jakarta, got sick on Feb 2 and was hospitalized 6 days later, according to a statement from the WHO. She is listed as Indonesia's 127th case-patient.
An investigation into the girl's illness is ongoing, though the WHO said she was exposed to her sick mother on Jan 27 and 28 and also spent time in a neighborhood where chickens and other birds were present. The WHO said samples from the birds have been taken to determine if they were the source of the girl's infection.
The girl's mother fell ill on Jan 23 and was hospitalized 3 days later. The WHO's Feb 5 statement on the woman, who is listed as Indonesia's 126th case-patient, said she was in critical condition. The report gave no details about the source of her infection.
Indonesia's last known family cluster of H5N1 cases was reported in January 2007, when an 18-year-old boy's H5N1 illness was confirmed after his mother died of the disease. The mother was from Tangerang, a suburb of Jakarta. The son's place of residence was not listed, but a Jan 15, 2007, WHO statement on his illness said the son and his mother probably had the same environmental exposure to the virus.
With the latest case, the WHO's global H5N1 count has reached 360 cases with 226 deaths.

Bird flu, Teror Diam

Tajuk Rencana Add comments

the Death resulting from bird flu in Indonesia has achieved the worrying figure.
From 124 cases that have happened, totalling 102 including dying.
That meant the level of this illness fatalism has reached more than 80 percent.

Meaning that, only 2 could be from his 10 sufferers ascertained will be safe.
The rest of them will statistically die.

The spreading of the bird flu illness in humankind in the beginning of 2008 quite extraordinary, against the period that was same in the year beforehand.
After the case incident could descend in knew then, currently just in the beginning of the year of several fatal cases (11/2) of a child from T. Tinggi has happened on Monday terindikasi this illness was carried to RS HAM Medan.
The government personally still continue to by means of for a long time faced this problem.

Seemed to suffer from vertigo.

The department of the Health said that indeed was not yet proven by the existence of the spread from humankind to humankind.
But with even more of them the case that was kluster, this indicated that the possibility of the spread not was not available.
Only problems of time to prove that.

In the meantime, the Department of Agriculture personally also stated that they could not act on the incident that struck humankind.
They only were assigned to his region, that is the poultry.

However the problem that really could not be resolved by the government was how that the poultry that became spread means of this case no longer is directly touching with humankind.

In various places, the poultry indeed really close to the settlement.
This was caused because of his value of economics that was very important for the community.

Indeed resulting from from this case was very big.
Pembiaran the occurrence of this case without taking the real action of appearing to allow the occurrence of still terrorism (silent terrorism) in the middle of the community.

The mutation of this illness virus will be easy to happen because of the poultry that became the source of the spread always was available.
Now his characteristics that increasingly resisten will be easy to happen also because of the occurrence of relations that were easy between various other animals that was around the poultry region.

And that was most worrying was the guarantee about medicine that currently is owned by the government.

There was the concern that because of the case emergence for the sake of the case, the community had finally chosen to consume personally medicine for this illness.

Worried that if the actual problem happened, his illness precisely can be not cured. Efficiency of the donor agency not was not available.

The Kesehatan body of the World has given consultations. Moreover the laboratory of bird flu investigation in North Sumatra then already didanai.

The European union has also provided the fund.

Currently only how moved all the resources so as to be able to be maximal.
The government necessarily more must intensify various promotion means of the health to the community to the local level.

The change in the behaviour of the community to want to take care of himself to live healthy was a long process.

Supposing that since this case happened very first in 2005 the government at once took the fast movement action of taking the form of empowerment of the community, then certainly this case will not continue

Flu Burung, Teror Diam

Tajuk Rencana Add comments

Indonesian girl contracts bird flu in new family cluster: WHO

JAKARTA, Indonesia (AP) -- The daughter of an Indonesian women who tested positive for bird flu has also contracted the virus, the World Health Organization said Wednesday, in what could be a case of human-to-human transmission.

The 14-year-old girl was in critical condition at a Jakarta hospital, the WHO said on its Web site. The girl's 38-year-old mother has been hospitalized with the bird flu virus since Jan. 26.

"She was exposed to her sick mother... and spent time in a neighborhood where chickens and other birds were found," the WHO statement said, adding the source of infection was still under investigation.

Further tests will be needed to establish whether the daughter contracted the virus from contact with her mother or from an infected chicken or its droppings - the source of most of Indonesia's 103 fatal cases.

Cases of possible human-to-human transmission are watched closely because they increase the chance of the virus mutating into a form that is easily passed between humans, which could possibly trigger a global pandemic.

The World Health Organization said the virus passed between humans, although in a limited way, in one large cluster of cases in a single family on Indonesia's Sumatra Island in 2006.

The H5N1 strain of bird flu has killed at least 226 people worldwide since it started ravaging poultry stocks in Asia five years ago, according to WHO.

The teenager, the nation's 127th case, was in a critical condition at Jakarta's Persahabatan hospital, where her 38-year-old mother was admitted for treatment on February 1, the ministry and doctors said.

The often deadly H5N1 strain of bird flu has claimed 103 lives in Indonesia, making it the worst-hit nation.

"She tested positive in both test results," health ministry spokeswoman Lily Sulistyowati told AFP, referring to the girl.

Two positive results of tests on blood and tissue samples are needed before Indonesian authorities confirm a human bird flu infection.

Muchtar Ikhsan, a doctor treating the latest case, told AFP that the teenager was admitted on February 8 and was gravely ill, but her mother was improving.

"The girl... is in an ICU (intensive care unit) and is being assisted by a ventilator," he said.

The mother and daughter came from West Jakarta's Kaliderest district, which lies next to the satellite city of Tangerang.

Nine people have died from bird flu in Indonesia so far this year, all of them from Jakarta and its surrounding sprawl.

Earlier, the ministry said that the infected mother lived in a neighbourhood where backyard poultry are kept, but the health ministry spokeswoman said Wednesday that the pair had visited relatives where neighbours kept poultry.

Experts fear that the virus, which is usually spread directly from a bird to a human, could mutate into a form easily transmissible between people, sparking a deadly global pandemic.

The concern stems from past influenza pandemics -- one in 1918, just after the end of World War I, killed 20 million people worldwide.

JAKARTA, February 13 (RIA Novosti) - Indonesia's health ministry confirmed that a 14 year-old girl from west Jakarta has contracted bird flu, the nation's 127th case of the disease, the national ANTARA news agency said on Wednesday. Tests revealed that the girl was infected by the deadly H5N1 virus and is currently in intensive care. The girl's mother, who is also infected with bird flu, is also in hospital, but her condition is said to be improving.
The source of the infection has yet to be established. The health ministry earlier said that the family kept poultry in their backyard, while latest reports say they were infected after visiting relatives.
Indonesia has the world's highest death rate from bird flu with 103 people or one third of the global death toll, with 80% of cases in Indonesia being traced to the Jakarta region. The number of infections traditionally rises during the monsoon period and floods.
