Indonesia reports 103rd bird flu death




JAKARTA - A 29-YEAR-OLD Indonesian woman has become the country's latest bird flu fatality, the health ministry confirmed on Monday, bringing the toll to 103 in the worst-hit nation.

'We are now at 126 infection cases, 103 of them fatal,' the ministry's director general for contagious disease control, I Nyoman Kandun, told AFP.

A 28-year-old woman has also been confirmed as infected but is still alive, he added.

The health ministry's website identified the latest fatality, who died over the weekend, as SU, and said she came from the Jakarta satellite city of Tangerang, where she was admitted to hospital on Jan 28, six days after falling ill.

She was referred to Jakarta's Persahabatan hospital a day later but died on Saturday, the site said.

Nine people have died from bird flu in Indonesia so far this year, all of them from Jakarta and its surrounding sprawl.

The ministry site identified the latest bird flu patient as SA and said she comes from West Jakarta's Kalideres district, next to Tangerang.

The ministry said she fell sick on Jan 24, was admitted to a hospital in Tangerang two days later and eventually transferred to Jakarta's Persahabatan last Friday, where she is still being given respiratory aid.

The woman who died, SU, had recently visited her parent's home, where neighbours kept ducks, the site reported, but did not say where the village was located. The infected patient, SA, lived in a neighbourhood where backyard poultry are kept.

Experts fear that the virus, which is usually spread directly from a bird to a human, could mutate into a form easily transmissible between people, sparking a deadly global pandemic.

The concern stems from past influenza pandemics. A pandemic in 1918, just after the end of World War I, killed 20 million people worldwide.

Bird flu is now endemic across nearly all of Indonesia, which was initially criticised for being slow to act against the deadly virus. -- AFP

Muhamad Ibnu Yusuf (MIY) was a 9 year old child who died on Jan 27, 2008. As noted in the WHO update, he lived near a wet market where poultry was sold. The above post suggests that he may have infected from some other reservoir besides poultry.

Conclusions

We found that 2 different AI virus (H5N1) genotypes that were isolated in 2004 and 2007 can replicate in the feather epidermal cells of domestic ducks and geese. To our knowledge, this is the first report of in vivo ultrastructural observation of AI (H5N1) replication in waterfowl.
The important finding is that the histologic feather finding and virus isolation from the skin were found in inoculated birds that did not exhibit apparent clinical signs. Although 1 goose inoculated with Ck/Yama/7/04 was negative for all examinations, this might have resulted from individual differences in susceptibility or the limited area of the skin used for the examination. Nevertheless, our data indicate that recent AI (H5N1) strains are likely to replicate in feather epidermal cells of domestic ducks and geese. All birds inoculated with Ck/Miya/K11/07, which belongs to the current lineage spreading to Europe and Africa, tested positive for virus isolation, compared with the results with Ck/Yama/7/04.

Feathers can easily drop off, blow away, or be reduced to dust, suggesting that affected feathers of waterfowl infected with influenza (H5N1) virus can be potential sources of infection, along with their feces and respiratory secretions
(7,8).

At this time, it is unclear to what extent affected feathers contribute to the epidemiology of AI (H5N1) field outbreaks. However, more attention needs to be paid to persons who handle domestic waterfowl possibly infected with AI virus (H5N1).

'Mysterious' bird flu baffles Indonesian scientists

Indonesian scientists and officials said they were baffled by the "mysterious" behaviour of the bird flu virus here, which has already claimed nine lives this year in the world's worst-hit nation.

Indonesia has reported 126 cases of H5N1 bird flu, 103 of them fatal, since 2005. This year's victims have all come from the capital Jakarta and its satellite cities.

Officials from the ministry of agriculture's bird flu control unit told a media briefing that the risk factors for human infection remained unclear after studies were conducted around victims' homes.

"In some of the cases we found the virus in the water and chickens, but in many other cases the studies showed no signs of the virus in the surroundings," said the unit's Tjahjani Widjastuti at the briefing late Tuesday.

The usual mode of transmission of the deadly H5N1 strain of bird flu is directly from an infected bird -- typically poultry -- to humans.

"The behaviour (of the virus in Indonesia) is mysterious and we are competing with the dynamics of the virus. There needs to be deeper study on why there are more cases in humans, what are the risk factors... so we can cut the chain of infection to humans," Widjastuti said.

Globally, scientists fear that the virus will eventually mutate into a form easily spread between people.

Indonesia has been sharply criticised for being slow to act in its fight to control bird flu, which has spread easily in a nation where many people keep chickens and other birds in their gardens and homes.

Bird flu is endemic in all of Indonesia's 33 provinces except for Gorontalo on Sulawesi island and in North Maluku, said Widjastuti.

Meanwhile, early partial results of a study conducted by the UN's Food and Agriculture Organisation bird flu programme in Indonesia in traditional markets in western Java -- which covers Jakarta and its surrounds -- showed a high level of penetration by the virus.

About half of all markets tested were positive for the virus, said James McGrane, leader of the FAO programme.

"It's a very difficult virus to control," he said, adding that the team could not say why the number of cases had spiked here.

Medan - Kasus flu burung terjadi lagi di Sumatera Utara (Sumut). Seorang anak berusia 10 tahun terpaksa dirawat di Rumah Umum Pusat (RSUP) Adam Malik Medan karena diduga suspect flu burung.

Khairul, 10 tahun, penduduk Jl. Abdul Hamid, Desa Sungai Sigiling, Kec. Padang Hilir, Kota Tebing Tinggi, Sumatera Utara, saat ini dirawat intensif di ruang khusus suspect flu burung RS. Adam Malik Medan, Jl. Bungalaw, Medan.

Humas RS. Adam Malik Medan Sinar Ginting mengatakan, sejak di bawa ke RS Adam Malik, Senin (11/2/2008) pagi, kondisi putra pasangan Enden dan Ani ini mulai membaik.

"Secara umum kesehatan pasien semakin membaik. Kita telah memeriksa sampel darah, semuanya normal," kata Ginting, Selasa (12/2/2008).

Namun menurut Ginting, untuk memastikan darah Khairul, pihaknya RS Adam Malik telah mengirim sampel darah ke Pusat Penelitian dan Pengembangan Kesehatan (Puslitbangkes) Departemen Kesehatan di Jakarta.

"Kita masih menunggu hasil laboratorium untuk memastikan hasil sampel darah pasien. Bila hasil laboratorium menunjukkan sampel darah baik, pasien akan diperbolehkan pulang," kata Ginting.

Sementara ibu Khairul, Ani mengatakan, anaknya mengalami demam tinggi setelah kontak dengan empat ekor ayam mereka yang mati secara tiba-tiba pada Sabtu lalu (9/2/2008).

"Saya sempat memberinya obat demam beli dari kios. Tapi demamnya tetap tinggi," kata Ani.

Karena menduga demam biasa, Ani membolehkan Khairul pergi ke sekolah pada Senin kamarin. Namun menjelang siang, Khairul pulang diantar oleh gurunya karena demam tinggi. Kemudian Ani melapor ke Kepala Desa
setempat dan Luruh. Pihak Dinas Kesehatan Kecamatan Padang Hilir kemudian datang dan memeriksa sample ayam dan menyatakan ayam tersebut positif terjangkit flu burung.

Medan - The case of bird flu happened again in North Sumatra (North Sumatra).

But his fever stayed high, said Ani.

Because of expecting the common fever, Ani allowed Khairul to go to the school on Monday kamarin.
However gazed at the afternoon, Khairul came home was delivered by his teacher because of the high fever.
Afterwards Ani reported to the Desa Head local and Luruh.
The Dinas Kesehatan side of the Padang Hilir Subdistrict afterwards came and checked sample the chicken and stated this chicken was positive terjangkit bird flu.

Avian influenza ? situation in Indonesia ? update 39

The Ministry of Health of Indonesia has announced a new case of human infection of H5N1 avian influenza. A 15-year-old female from West Jakarta, Jakarta Province developed symptoms on 2 February, was hospitalized on 8 February and is currently in hospital in a critical condition. The case is the daughter of a previously confirmed case, the 38-year-old female from West Jakarta, Jakarta Province who developed symptoms on 23 January.
Investigations into the source of her infection are ongoing. However, she was exposed to her sick mother on 27-28 January and spent time in a neighbourbood where chickens and other birds were found. Samples from these birds have been taken and are undergoing tests to determine whether they may have been the source of infection.
Of the 127 cases confirmed to date in Indonesia, 103 have been fatal.

Likely Human to Human H5N1 Transmission in Jakarta Cluster

The case is the daughter of a previously confirmed case, the 38-year-old female from West Jakarta, Jakarta Province who developed symptoms on 23 January.

Investigations into the source of her infection are ongoing. However, she was exposed to her sick mother on 27-28 January and spent time in a neighbourbood where chickens and other birds were found.


The above comments from the WHO update describe a confirmed cluster in West Jakarta. The gap in the disease onset dates, as well as the timing of the exposure indicates this is a case of human to human transmission. Although H5N1 infections of humans have been high in Indonesia, and highest in the Jakarta area, the likelihood to two family members being independently infected is remote. However, the likelihood of transmission within a family is high, because of close contact that is common between family members.

The ?exposure? of the 15F to her 38F mother probably involved visits to her mother, who was hospitalized on January 26 in Tangerang and was subsequently transfered and was in critical condition.

In this instance, sequencing of H5N1 from the two patients would be useful.
