Guwahati, June 21: The knee-jerk reaction of Assam government to yearly outbreaks of Japanese Encephalitis (JE) seems to go by the old saying ?Chor gole buddhi, boroxun gole jaapi?. This vain groping around for a preventive measure long after the incident has occurred, is true this year as well for JE.

Already 16 lives have been lost in the State as the JE outbreak season got going from May last. As usual, the State Health department is scrambling for vaccinating the adult population, with proposals sent recently to the Centre for vaccination drive in three districts.

However, the proposal has been nixed due to shortage of funds. The JE vaccine, after all, has to be imported from China.

Meanwhile, Acute Encephalitis Syndrome (AES), caused by a variety of viral, bacterial and other parasitical infections as well as chemicals and toxins, has also raised its head this year. Already 366 AES cases have been reported from different parts of the State. The number of AES deaths has risen to 49 by the latest count.

Comment: In previous years, media reports from Assam have confused the issue as to whether the government reported JE positive figures were a subset of or in addition to the government reported number of AES cases. Five test positive for encephalitis

Guwahati, June 22: Five persons have tested positive for Japanese encephalitis in Kamrup (metro) district so far while the number of acute encephalitis syndrome cases has reached 35. One person suffering from encephalitis has died.

Health officials, however, claimed the number of Japanese encephalitis and acute encephalitis syndrome cases in Kamrup (metro) are comparatively low compared to other districts. The disease is more prevalent in the Upper Assam districts.

According to the latest data, 72 cases of Japanese encephalitis and 391 cases of acute encephalitis syndrome have been detected in the state. While the total number deaths owing to acute encephalitis syndrome is 55, 17 people Japanese encephalitis patients have lost their lives.

Tinsukia, June 22: Fingers were pointed on central government much hyped NRHM and state governments health system after a 12 year old child of a businessman here in Tinsukia town lost his battle for life at around 11.00 am at a private hospital at Dibrugarh on Wednesday due to encephalitis. Doctors diagnosed Mulund Birmiwal for acute encephalitis and also doubted that it can be a case of Japanese encephalitis (JE), the same is yet to be confirmed by the officials of the health department. However, in past one month, 5 cases of Japanese encephalitis has been detected in Tinsukia district out of which 2 youths below 15 - years died, out of which one Priya Orang was vaccinated with JE immunization. Hence, the probability of JE breakthrough cannot be ruled out.

Three dead of suspected encephalitis in Gaya in four days

Three people with symptoms of encephalitis have died at the Anugrah Narayan Magadh Medical College and Hospital (ANMCH) in Gaya in the last four days, and five others are being treated.
Confirming the three deaths, a senior doctor at ANMCH said the patients were brought to the hospital from remote areas in serious condition.
?If patients are brought to the health facility soon after they develop AES (acute encephalitis syndrome) symptoms, such as high fever and body ache, the chances of their survival are high. But delayed arrival only complicates the cases, which sometimes go beyond our control,? he said.

?All the three patients who died had tested negative for JE despite some symptoms of the disease. Other reports are still awaited, but we have taken it as cases of AES in view of initial symptoms,? Dr Sinha said.

Panel raps district over anti-virus drive

An expert team from National Vector Borne Disease Control Programme, New Delhi, has expressed concern over the tardy pace in which immunisation drives against Japanese encephalitis are being carried out in the district.

National Vector Borne Disease Control Programme joint secretary Dr R.K. Das is leading the team along with epidemic intelligence officer Dr Janardhan Nayak. Eight cases of Japanese encephalitis (JE) have been reported in Muzaffarpur with another three in East Champaran district.

The team arrived on Saturday and visited Gangapur village on Sunday afternoon. They met Suraj Kumar (8) who was diagnosed with symptoms of JE twice within a span of 14 days. He was discharged from Shri Krishna Medical College and Hospital on June 24.

Fresh encephalitis cases reported in Gaya district

Gaya: Despite official claims of over two rounds of mass immunisation against encephalitis, fresh cases have been reported in the district. In the last round, about 14,82,887 children were officially immunised against the deadly disease, Mahadalaits being its most easy prey.
According to Anugrah Narain Magadh Medical College Hospital(ANNMCH) superintendent Dr Sudhir Kumar Sinha, till date five children have lost their lives. "Altogether 11 patients were admitted to the children's ward of the hospital with symptoms of the disease. Of the total 11 admitted patients, the disease was diagnosed in seven cases and investigation reports of the remaining four are awaited," said Dr Sinha.

Indo-US conference on combating viral infections of brain

Kolkata, June 29: Over 80 researchers from India and the US will discuss cutting edge research and exchange ideas in November in West Bengal to combat viral infections of the brain, against the backdrop of globalisation, immigration and the increasing incidences of such infections.

The 'Indo-US Symposium on Central Nervous System Virus Infection and its Therapy' will run from November 14-17 in Jalpaiguri in the hills of West Bengal.

"Lots of people die from meningitis and Japanese encephalitis, West Nile viral infection and other viral infections of the brain. Many harbour viruses in the brain and we may not even know about it," organiser Jayasri Das Sarma, a scientist at the Indian Institute of Science Education and Research (IISER), Kolkata, told IANS.

Given a cfr of around 15% it is odd that the 461 cases reported from Odisha, Tamil Nadu and Tripura have not resulted in a single recorded fatality while 468 cases from Assam have resulted in 62 deaths. A case of under reported deaths or a different disease? - Ro.

Expert teams to probe kids' deaths

Gaya, July 2: A 28-member team of the health department will arrive in Gaya in a couple of days to conduct a survey of the areas from where as many as eight children have died since June 25.
... At ANMCH, 12 children in the age group of two to 10 years have been admitted since June 23 out of which eight died. Among the children who have died include five from Gaya, two from Nawada and one from Aurangabad district.
None among the children admitted at ANMCH have tested positive for Japanese encephalitis (JE) after pathological tests were conducted at the microbiology department of the medical college here.

45 yr old Subhash Kumar, a farmer from Samastipur, was brought to the hospital emergency unit with high fever. His reports from a local hospital highlighted that he was suffering from persistant fever for the last 1 week. Dr. Abhishek, Consultant Neurology found that the patient was having hallucinations was talking on irrelevant topics. He was also having episodes of unconsciousness and seizures (Fits). Subhash also had rigidity in the neck and decreased movement in the left side of the body. The CT scan showed spots in the brain but it was not conclusive. The pattern of brain involvement in the MRI scan looked similar to Herpes virus encephalitis, which is a common type of viral infection that causes brain fever and is responsible for high mortality.

The neuro care team at Paras Institute of Neurosciences, Patna confirmed the diagnosis with a lumbar puncture which confirmed herpes virus infection. He was started on anti-viral drug (Acyclovir). After 7 days of ICU care, anti-viral medication, specialized uninterrupted care, the patient starting showing signs of improvement.

After a stay of 2 weeks the patient started to eat food orally and was fit for discharge. According to Dr Abhishek, Consultant Neurology, Paras Patna, ?Encephalitis is a major issue in Bihar. Herpes as well as non-herpes encephalitis are common here. The signs and symptoms usually get misdiagnosed as brain stroke, status epileptics/seizure (fits) disorder, simple fever or unconsciousness due to other infection resulting in delayed treatment; which can further complicate the situation. We want to spread awareness in the public that if they have fever, headache with consciousness in the form of irrelevant talk, hallucination or unconsciousness with or without seizure they should immediately come to a super specialty hospital, where all facilities are available.?

He adds, ?Paras Patna has specialized clinical protocols to investigate and treat Encephalitis and is supported with the best radiology and pathology department. It is our endeavor to raise awareness about this fatal disease and prevent mortality with early detection and timely treatment.?

Kamrup (M) has reported 9 positive cases of Japanese Encephalitis (JE) while the state has witnessed 127 numbers of cases with 38 deaths. Upper Assam has been the most vulnerable.

22 districts in the state have reported positive cases of JE and districts like Nagaon, Sivasagar and Dibrugarh are more vulnerable. ?This is the season that paddy grows in the fields. Therefore the mosquitoes increase in this season and there is no system to make the areas mosquito free. Fogging is done in many places and Impregnation Treated Bed Net (ITBN) has also been distributed to the people so that the cases do not increase. The people should also be aware about keeping the surroundings clean and hygienic. Unhealthy and unhygienic environment is the reason for the increase of mosquitoes,? said B.C. Bhagabati, State Surveillance Officer, IDSP, National Health Mission while speaking to G Plus.

Comment: Assam reported 83 cases of Japanese Encephalitis with 21 fatalities to June 30. This is a significant increase for the past 11 days. Assam also reported an additional 385 cases & 41 deaths which were either untested or tested negative for JE to June 30. Historically in Assam confirmed JE deaths reflect 30-50% of total Encephalitis deaths.
