Continues to recruit patients into Purvanchal Inseflaitis. BRD Medical College in the last twenty-four hours, five new patients were admitted. Nehru, 95 patients are being treated in hospital.
Those who were recruited two of Gorakhpur and Deoria and Siddharth are a patient. Since January this year, the last one at BRD Medical College in 2156, of which 588 were killed in patient recruitment.

The struggle of encephalitis patients and their families in eastern Uttar Pradesh.

Encephalitis is a curse for the eastern part of Uttar Pradesh. During the last decade, it proved fatal for hundreds of children in the districts of Gorakhpur, Kushinagar, Siddharthanagar and Maharajganj. The situation has become even more critical because of lack of medical aid required to cope, as well as negligence of hygiene in affected areas.
This year, from January to November 21, 569 patients died of encephalitis. In 2011, 627 died and 619 in 2013. In the period of 2005 to 2013, 4,775 encephalitis patients died. This is from the records of BRD Medical College at Gorakhpur, where patients from the neighbouring districts of Kushinagar, Siddharthanagar and Deoria are referred. Patients from adjoining districts of Bihar and Nepal come here too.

Oddly, the National Vector Borne Disease Control Program has not included any cases from Odisha in its tally over the past 12 years. Another sure sign that incidence of Encephalitis is under reported. - Ro

Outbreak of Japanese Encephalitis in Odisha?s Malkangiri district has taken the life of 11 children in the last 20 days.

Sources in the district Health department said the deceased were in the age group of 1 to 3 years.

The viral disease causes sudden vomiting and fever and the patient dies within 24 to 48 hours of being afflicted witb the disease.

According to reports, Potrel village of Korukonda block lalone has recorded the death of eight children.

More than 80 children have died in the last seven years in the village. Twenty eight of them died in 2012 and 15 in 2013. More than 100 children have died in the district as a whole in the last three years.

10 children die of encephalitis in Malkangiri in four weeks

: Ten children have died of encephalitis in Malkangiri district of Odisha in the last four weeks.

Most of them were aged two years or less. As per physicians, superstitious belief of tribals is a major hindrance to saving children from the killer disease.

As per unofficial sources, the number of deaths of children due to encephalitis may be higher than the official death toll.

Uncertain cause

Although there is panic in the region that deaths of children may be due to Japanese encephalitis, Chief District Medical Officer (CDMO) of Malkangiri district U. Mishra said that as per initial findings, these deaths may be due to viral encephalitis.

It may be noted that in 2012, over 30 children were suspected to have died of encephalitis in Malkangiri district.

55 DISTS YET TO ACT ON ENCEPHALITIS: REPORT

Over 1,500 deaths due to deadly Japanese Encephalitis (JE)/Acute Encephalitis Syndrome (AES), it seems, is yet to wake up the authorities in 55 endemic districts in the disease-affected States of Bihar, Uttar Pradesh, West Bengal and Assam in ensuring safe water and basic medical facilities to the locals.

However, Tamil Nadu, where five districts are affected with the deadly disease, has fared better in implementing the national action plan initiated by the Centre last year wherein targets have been set up for the States to check the deadly disease over the years.

In a recent review by the Health Ministry, which is the nodal agency in implementing the plan, it was found that just half of the total 5,940 identified habitats in Bihar, Uttar Pradesh, West Bengal and Assam have been ensured access to safe drinking water. Similarly, of 14,875 targeted hand pumps, just 7,511 have been installed in the last two years in the disease affected States.

State wise, Uttar Pradesh has performed worst. Out of 3,378 habitation, 1,364 (40 per cent) have been provided safe water while out of target 7,902, 2,422 (30 per cent) hand pumps have been installed.

A mystery disease continues to create mayhem in backward Malkangiri district of Odisha. While 11 children have already fallen victim to it as per the report of the Chief District Medical officer (CDMO), unofficial sources, however, have put the toll at 20.

?The disease is seen among children aged between one and fifteen. It starts with fever and causes vomiting, joint pain and muscle aches. The patients usually die within a day or two,? said Ghenu Madhi from Patrel village.

Dehat, News Reporter: Japanese Inseflaitis prevention campaign from December 18, children aged up to 15 year olds immunized. The campaign has set a target of 5.87 million children's vaccinations.
In the past few years, many people in the district bizarre disease have died from the outbreak. The teams of experts from Delhi and Lucknow district was surveyed and was expected to be affected by the Japanese Inseflaitis. After drinking water and sanitation in the first phase, the government has directed the vaccination campaign. Order to begin the vaccination campaign is December 18. The 1 587 391 children up to 15 years has been the target of immunization. District Immunization Officer Dalmahendra Jtaria teams have made 125 to the campaign said. Each team 2 ANM, Aangnbadi, hope and teacher or placed Shicshamitr. All government and non-government schools and ANM centers run vaccination campaign. 25 are deployed to monitor the program supervisor.

Bhubaneswar, Dec 15: This is not dengue, malaria or jaundice. The Health department has found traces of a new epidemic in the tribal district of Malkangiri where 14 children were reportedly afflicted recently. It is spreading its tentacles slowly.
The name, cause and source of the disease remains a mystery for health officials, veterinary experts and researchers as they have found that there is no record of such ailment anywhere in the world.
The initial observations suggested that the disease appears among children below five years of age. The symptoms are unusual. The patient initially vomits and becomes weaker and dies within 48 hours. At least 14 children out of 15 who showed symptoms of the disease have died and the doctors have succeeded in saving only one life.
?We suspect the cause of the deaths could be Japanese encephalitis, but after various tests, we concluded that this is not the cause. However, we are treating the patients considering it as a variant of encephalitis, an acute inflammation of the brain,? said Kailash Chandra Dash, director, Public Health.

Bhubaneswar, Dec 16: The health department Tuesday sent the blood samples of people afflicted with the mysterious disease broke out in Malkangiri district to Pune for further examination. Health department sources said about 14 lives in the tribal dominated area were perished so far and the mystery behind the disease would be revealed in two days.
Regional Medical Research Centre (RMRC) director Shantanu Kumar Kar told Orissa POST that they have collected about 35 blood samples of which one tested dengue positive while another infected with herpes simplex. The remaining 33 samples have also been tested but have turned out to be negative.

BJP memebr from Uttar Pradesh Jagdambika Pal today made a strong plea for launching an effective control programmee to check the spread of encephalitis which was claiming lives of large number of children living in eastern district of his state, especially Gorakhpur.
Raising the issue during the Zero Hour, Pal said today, the whole country was standing by Pakistan, mourning the death of 132 children in terror attack in Peshawar, but it is not to be forgotten that in January this year, 603 children died under one roof in Gorakhpur Medical College. He said so far, doctors had not been able to find the cause and the way of treating the victims of the deadly virus, so there was need for special attention to be given to research on the disease.[/TD]

BHUBANESWAR: An unknown fever, which claimed lives of 12 children in a remote village of Malkangiri district last month, has baffled medical experts as they are seeking to know the source of the fatal virus.
The cerebro-spinal fluid and blood samples collected by Regional Medical Research Centre, an ICMR arm, have not provided any clue so far.
Tests at the National Institute of Virology (NIV), Pune too have not been able to reveal what caused the sudden deaths. ?Tests for Japanese Encephalitis (JE), Chandipura and Enterovirus have tested negative. Since JE was reported in Malkangiri two years back, we thought it might be the reason following a recurrence but it is not so this time,? Director, RMRC Dr SK Kar told this paper.

The samples are now being tested for Nipah Virus (NiV) which not only causes asymptomatic infection but also lead to acute respiratory syndrome and fatal encephalitis. In West Bengal?s Siliguri, NiV was reported in 2001 when it had claimed at least 45 lives. ?We are waiting for the results on NiV which is expected in a couple of days,? Kar said.

The disease had taken the form of an epidemic in a few tribal pockets of Malkangiri in the last week of November and had killed at least 14 children under the age of five in just a week.
After ground-level health workers failed to detect the cause of the disease, the Regional Medical Research Center (RMRC) intervene and conducted various tests to check for indications of Japanese encephalitis, dengue, herpes simplex and other deadly viruses, but found nothing.
Recently, the state health department collected a few blood and spinal fluid samples and sent it to the National Institute of Virology in Pune for further investigation of the source of the disease. However, all the tests done by the apex laboratory were found to be negative, including those for Chandipura, West Nile, and Nipa viruses.

NEW DELHI: The Centre on Monday admitted that efforts to control encephalitis had not been able to contain the high death toll even as BJP members in the Lok Sabha accused the state government of indifference.

In his calling attention motion in the Lok Sabha, BJP MP Yogi Adityanath pointed out that more children have died of encephalitis in a year than in the terrorist attack on the Peshawar school recently. About 1,495 people, mostly children, have died so far this year.

States told to use funds meant to curb encephalitis: Nadda

The central government will ask of all five states - Uttar Pradesh, Bihar, Assam, West Bengal and Tamil Nadu - where Japanese encephalitis cases are reported to utilize funds meant for the disease's prevention, union Health Minister J.P. Nadda told parliament Monday.

Replying to a calling attention motion in the Lok Sabha on the deaths of children due to the dreaded disease, Nadda singled out contaminated ground water and lack of general immunization as the causes for the disease.

"It is a matter of grave concern. The issue is of implementation of the funds at the grassroots level which lies with the state governments," he said.

The health minister said that the matter would be taken up at the inter-ministerial meeting to be held later this month.

Karnataka Garbage dumps breed Japanese encephalitis

Four children have been admitted to Indira Gandhi Institute of Child Health (IGICH) with Japanese encephalitis in the last one week. Doctors are blaming the incidence of the disease on garbage dumps piling up across the city.
Two of the children are in the Intensive Care Unit (ICU) — one is on ventilator, while the other was taken off ventilator on Monday morning. One patient recovered and was discharged two days ago. Private hospitals are yet to report positive cases.

Statement by Shri Jagat Prakash Nadda, Minister of Health & Family Welfare on 22nd December, 2014 in Lok Sabha in response to the call attention motion regarding the situation arising out of spread of “Encephalitis’’ in Uttar Pradesh and other parts of the country and steps taken by the Government in this regard

Hon’ble Members will recall that my predecessor in office and esteemed colleague had on 4th August, 2014 apprised the Hon’ble Members regarding the situation arising out of spread of Encephalitis in Eastern Uttar Pradesh and other parts of the country and steps being taken by the Government for the same. That we are again discussing this topic today is an indicator of the significance of this important issue impacting large sections of our population especially in the eastern parts of the country.

Though many Hon’ble Members, I am sure, are already aware, it would be useful to apprise this august House regarding certain basic facts of Encephalitis also known as brain fever. Encephalitis is inflammation of the brain which can be caused due to various pathogens including virus, bacteria and protozoa. While Japanese Encephalitis is a Vector Borne Disease transmitted through Culex group of mosquitos, Encephalitis can also be caused by entero-viruses which are water borne. Recently in Muzaffarpur and Malda, cases reported for Encephalitis were neither due to Japanese Encephalitis nor due to entero-viruses. Normally, Encephalitis is affecting children below 15 years of age. However, in the last few years, epidemiological data has revealed that many adults are also being affected and cases of morbidity and mortality, particular for JE, have been observed in adults in Assam and recently in the districts of North Bengal.

The total numbers of Encephalitis cases reported were 5167 in 2010, 8249 in 2011, 8344 in 2012, 7825 in 2013 and 9912 cases this year up to 17th December. For mortality the numbers were 679 in 2010, 1169 in 2011, 1256 in 2012, 1273 in 2013 and 1495 up to 17th December, this year. In terms of distribution of cases this year we find that the maximum reported numbers of 3291 cases were from Uttar Pradesh, followed by 2317 cases in West Bengal, 2194 cases in Assam and 866 cases in Bihar. If we look at the districts, the main districts affected in Uttar Pradesh are Khushinagar, Deoria, Maharajganj Gorakhpur and Sidharthnagar; in Bihar the district Muzaffarpur; in West Bengal, the districts Jalpaiguri, Bankura, Darjeeling, Coochbehar, Burdwan and Bankura and in Assam, the districts Sonitpur, Golaghat, Dhimaji, Shivsagar, Dibrugarh, Kamrup (Metro) and Tinsukia

The Government of India launched a National Programme for Prevention and Control of JE/AES in the end of 2012-13. This Programme envisages a multipronged strategy in 60 high priority districts in five high endemic States of Assam, Bihar, Uttar Pradesh, Tamil Nadu & West Bengal. The districts covered include 20 in Uttar Pradesh, 15 in Bihar, 10 in Assam, 10 in West Bengal and 5 in Tamil Nadu. The stakeholder Ministries include Ministry of Drinking Water and Sanitation with the task to provide safe drinking water and sanitation facility; Ministry of Social Justice & Empowerment with the task to provide rehabilitative services to disabled children; Ministry of Women & Child Development to provide nutrition to children in the affected districts; Ministry of Housing & Poverty Alleviation to provide drinking water facility in urban slums and Ministry of Human Resource Development (Education Department) to develop special curriculum for mentally and physically disabled children. The Ministry of Health and Family Welfare is the nodal Ministry with the task to vaccinate the children in the endemic districts, to improve the case management by establishing Pediatric ICUs at district hospitals, to establish Physical Medicine and Rehabilitation Departments and to strengthen public health measures including information-education-communication/behavior-change-communication for prevention and control of JE/AES.

I would like also to share with the Hon’ble Members the action taken by Government under the National Programme. For JE vaccination, out of 60 high priority districts, vaccination has been completed in 59 districts, the remaining one District (Kanpur Dehat) will also be covered during this financial year. Funds have been released for setting up of Pediatric ICUs in 27 districts. We are following up with the State Governments for setting up and complete operationalization of these Pediatric ICUs on priority. This requires civil work, procurement of equipment and recruitment of manpower. While the Physical & Medical Rehabilitation Department at BRD Medical College, Gorakhpur is already functional, the Units at K.G. Medical College, Lucknow; BHU, Varanasi; Bankura Medical College; North Bengal Medical College; Gaya Medical College and Patna Medical College require upgradation. Units are required to be set-up at Assam Medical College, Dibrugarh and at Guwahati Medical College in Assam. Out of total 10 proposed Physical Medicine and Rehabilitation Units, Rs. 25 crores has already been released for 5 Physical Medicine and Rehabilitation units in 2013-14 and for another 4 units release of Rs. 20 crores is under process. Vector control and surveillance activities are being supported under National Vector Borne Disease Control Programme. Surveillance is also being undertaken through the Integrated Disease Surveillance Project under the National Centre for Disease Control (NCDC).

For detection of non JE pathogens, Indian Council of Medical Research has already established a field unit of National Institute of Virology, at BRD Medical College, Gorakhpur. In addition, ICMR is conducting research-cum-intervention projects. NIV, Pune is supplying IgM ELISA kits to sentinel laboratories for the detection of JE cases. These kits are funded by the Directorate of National Vector Borne Disease Control Programme, Government of India.

I would like to further elaborate on JE vaccination. This was started in 2006 and was scaled up in a phased manner over years. The strategy for JE vaccination is to conduct a one- time campaign (which targets all children from 1-15 years of age), after which JE vaccination is included as a part of routine immunization in that area. Initially, only one dose of JE vaccine was provided at the age of 16 to 24 months (with DPT/OPV booster). From April, 2013 onwards two doses for JE are scheduled under routine immunization, the first at 9 to12 months and second at 16 to 24 months of age. Out of 179 JE endemic districts in the country, 152 districts have been covered by vaccination from 2006 to 2014. Further, a catch up round to cover children missed out during the campaign and routine immunization rounds has been carried out on 22-23 June, 2014 for ten districts of Uttar Pradesh and eight districts of Bihar. Due to recent cases of JE in adults, this issue was discussed in the National Technical Advisory Group on Immunization. It was decided that we can take up vaccination for adults too in districts where such adult cases are being reported. The Assam Government has covered adults with JE vaccination in nine districts. This has been beneficial. It is crucial that the coverage of immunization remains high. Our reports show that routine immunization undertaken by the States may not have high coverage in all the target districts. States must, therefore, focus on this important aspect also.

On 5th of December this year, a coordination committee under the Chairmanship of Secretary (Health and Family Welfare) has reviewed the progress on the implementation of Programme. Ministry of Drinking water and Sanitation has initiated one week awareness campaign regarding the installation of IM-II hand pumps in the affected districts and till date 75% of 60 programme districts have been covered under this awareness programme. To monitor the programme in Uttar Pradesh, a sub-committee has been formed which will periodically review the progress. The Ministry of Women and Child Development has made provisions for take home rations under Integrated Child Development Services Scheme for the moderately mal-nourished children and also initiated a training programme for Anganwadi Workers. Disability Rehabilitation Centers have been established in 11 districts out of proposed 15 districts by Ministry of Social Justice and Empowerment. NCERT is developing special curriculum for children affected by disability due to JE/AES. The State of Assam has informed that additional 5 districts (Barpeta, Darang, Naogaon, Sonitpur and Udalgiri) have been identified to be taken up under adult JE vaccination. It is informed by the State of West Bengal that Critical Care Units are functional and doctors from the District Hospitals are undergoing one month hands on training at SSKM Hospital, Kolkata. Tamil Nadu has informed that Paediatric ICUs of district hospitals are already functional though they need upgradation as per the specifications suggested under the National Programme. The State further informed that the procurement of equipment needed for PMR Deptt of Madurai Medical College, is under process. In Tamil Nadu Medical Officers from the District Hospitals are undergoing training in critical care management at Institute of Child Health, Egmore.

Funds released under National Vector Borne Diseases Control Programme during 2014-15 to Assam were Rs. 9.16 crores, Bihar Rs. 28.57 crores, Tamil Nadu Rs. 15.61 crores, Uttar Pradesh Rs. 23.76 crores and West Bengal Rs.18 crores. These funds are to be used for prevention and control of Vector Borne Disease including implementation of the JE/AES activities. In addition, Rs 5.35 crores has been released under NHM flexi pool to support the contractual human resources at JE/AES wards at BRD Medical College, Gorakhpur. Similarly, in 2011-12 and 2012-13 too an amount of Rs. 3.05 crores was released under NHM to improve the manpower situation at BRD Medical College, Gorakhpur.

Hon’ble Members will appreciate that prevention and control of AES/JE requires concerted and coordinated action along with expeditious setting up of infrastructure being supported under the programme. This entails effort by all stakeholders including both Central and State Governments, local self-governments, medical fraternity and non-government organizations. Provision of clean drinking water is critical for prevention of AES. For JE, effective vaccination coverage and vector control measures are necessary. Vector control measures include source reduction, sanitation and improving practices to pig rearing. For quick response, doctors, whether from government or from private institutions, must assess cases of fever for neurological symptoms and refer such patients to higher health facility without delay. The programme for Prevention and Control of AES/JE is being regularly monitored and I would especially appeal to the State Governments to utilize the funds already released or being released to them. The Ministry of Health and Family Welfare will continue to closely monitor the situation. I will be very happy to receive guidance from the Hon’ble Member for any further action in the matter.
