Two persons, including an elderly woman, died of suspected dengue in the City on Tuesday. Dilip (23), a resident of Kereguddadahalli died due to suspected dengue viral disease at BGS Hospital here on Tuesday.

The victim suffered from high fever for two days following which he was hospitalised. He failed to respond to treatment and died on Tuesday.

In yet another case that came to light late, 59-year-old Champa Prahlad, a resident of Subramanyanagar in the City, died of suspected dengue fever.

Dengue fever seems to have taken a grip on Kerala yet again, with the commencement of the South-West monsoon.

Unless vector control and other preventive measures are intensified on a war footing, the State could be in for an explosive situation as in 2013, when some 25,000 suspected cases of dengue and over 180 suspected dengue deaths were reported. The number of cases serologically confirmed by the public health system alone had stood at 7,938 in 2013.

The number of dengue cases reported in the City is far more than what is placed on record by the Bruhat Bengaluru Mahanagara Palike (BBMP) and the Department of Health and Family Welfare. For, they have only been putting on paper cases tested positive only by the National Institute of Virology (NIV), ignoring those reported from private hospitals. Even as the City has seen three suspected dengue deaths, there has been no official confirmation in this regard. This, even as the families have laboratory reports confirming that it is a positive case of dengue. For instance, last Thursday, 59-year-old Champa Prahlad, a resident of Subramanyanagar, died of dengue. However, even after the family claims to have approached the Palike with the reports, the officials refused, reasoning that it was from a private laboratory. Prahlad's reports are from a laboratory accredited by the National Accreditation Board for Testing and Calibration Laboratories. Speaking to Deccan Herald, one of the top officials from the BBMP confessed that the number of cases in the official statistics would only indicate a part of the actual number of cases in the City.

Southwest Monsoon Causes Rise in Number of Dengue Cases in Kerala, India

Dengue cases are reported throughout the year in Kerala, the incidence peaks twice every year, during the two monsoon seasons. With the onset of monsoon in the state, dengue cases have been on rise.

This year, the number of officially suspected dengue fever cases from January 1 to May 31 this year was 1,793.

The total number of suspected dengue cases as on June 20 has increased to 4,062, with 11 suspected dengue deaths and 12 confirmed cases of dengue deaths.

A 90-year-old Chembur resident died of dengue on Sunday, becoming the first person to succumb to the infection in Mumbai this monsoon.



According to doctors, Pandurang Bhosale was brought to the hospital with fever and low platelet count.

?Despite transfusion, his platelet count dropped from 1 lakh units to 23,000 during the treatment. His test reports confirmed he had contracted dengue,? said Dr Dipen Deole, medical superintendent, Surana Hospital, Chembur.

The municipal epidemiology cell has, so far this month, recorded 28 cases of dengue and 354 cases of malaria. The Kasturba hospital at Chinchpokli has got around 70 patients of malaria and dengue.

Dengue is known in India since 1940s, but the disease is very limited in its spread. Dengue is becoming
rampant in many states of southern India. As of now, no specific treatments (therapies) or vaccines are
available against the disease. The people of Tamil Nadu and Puducherry, where the dengue incidences
are increasing, perceive this spurt as due to poor rainfall and power supply. We attempted to verify this
perception. We also sought to find out whether these factors are in concert with the monthly surveillance
reports, used to predict yearly dengue cases by formulating a prediction model. The results showed that a
combination of rainfall and power supply had major effect on the spread of dengue. The prediction model,
incorporating rainfall and power supply data for four seasons, could not predict the dengue outbreaks accurately.
Due to poor rainfall and power supply, people resort to storing water when power is available, and
this prolonged open storage of water provides habitat for the vector to breed. The present study emphasizes
the need to strengthen the surveillance system for timely and effective execution of vector-control programme.
It also highlights the need to improve awareness among the public about the vector?s ecology. The
strategy against such vector-borne diseases has to focus more on environmental health rather than only
on disease management that is patient-centred, largely curative and chemical/medicine intensive.

The present study also reveals a few other interesting observations. First, the outbreaks coincided with the
northeast monsoon with 1?2 times more number of dengue cases than during the southwest monsoon. Deficit in
rainfall and power supply paved the way for severe outbreak in 2012. Such an outbreak did not happen in 2011
and 2010, because the deficit in one of the determinant variables was counterbalanced by the surplus in the other
determinant variable.

Thirdly, the present prediction model showed significant correlation with
NVBDCP dengue cases, but not with IDSP dengue cases. This suggests that reliability of a prediction model in the
case of contagious diseases, such as dengue, depends on accuracy and timely updation of the data by the surveillance
system. It is discernible that IDSP does not have actual figures of dengue cases. Incidentally, highly populated
districts such as Chennai, Coimbatore, Tiruppur and Vellore (where dengue is prevalent) and some coastal districts
did not report any dengue cases in these three years.
This would be possibly due to underreporting of the infections for the high dependency of patients on private
health care, lack of adequate testing facilities, mix-up of the symptoms of dengue and other seasonal viral fevers
and perhaps intentional attempts to underreport the cases.
The flaws in the surveillance and reporting system could be a possible, but crucial, reason for the failure of this
prediction model. Thus, possibly this model emphasizes the need for accurate IDSP alert reporting through better
collection, collation, compilation and validation of data.

Background

Dengue and chikungunya are rapidly expanding viruses transmitted by mosquitoes of the genus Aedes. Few epidemiological studies have examined the extent of transmission of these infections in South India despite an increase in the number of reported cases, and a high suitability for transmission.

Methods and findings

We conducted a household-based seroprevalence survey among 1010 individuals aged 5-40 years living in fifty randomly selected spatial locations in Chennai, Tamil Nadu. Participants were asked to provide a venous blood sample and to complete a brief questionnaire with basic demographic and daily activity information. Previous exposure to dengue and chikungunya was determined using IgG indirect ELISA (Panbio) and IgG ELISA (Novatec), respectively. We used this data to estimate key transmission parameters (force of infection and basic reproductive number) and to explore factors associated with seropositivity. While only 1% of participants reported history of dengue and 20% of chikungunya, we found that 93% (95%CI 89-95%) of participants were seropositive to dengue virus, and 44% (95%CI 37-50%) to chikungunya. Age-specific seroprevalence was consistent with long-tem, endemic circulation of dengue and suggestive of epidemic chikungunya transmission. Seropositivity to dengue and chikungunya were significantly correlated, even after adjusting for individual and household factors. We estimate that 23% of the susceptible population gets infected by dengue each year, corresponding to approximately 228,000 infections. This transmission intensity is significantly higher than that estimated in known hyperendemic settings in Southeast Asia and the Americas.

Conclusions

These results provide unprecedented insight into the very high transmission potential of dengue and chikungunya in Chennai and underscore the need for enhanced surveillance and control methods.

Author Summary

Despite a recent increase in the number of cases, little data exist on the extent of dengue and chikungunya transmission in Indian cities. We conducted a household-based serosurvey conducted in randomly selected spatial locations across the metropolis of Chennai. We tested samples for evidence of previous infection by dengue and chikungunya viruses and used this data to estimate key transmission parameters (force of infection and basic reproductive number) and to explore factors associated with seropositivity. BENGALURU: Healthcare infrastructure is creaking in the city in the face of an unprecedented spiral in the number of dengue cases. Prominent hospitals are shutting their doors on fresh cases, saying they are brimming with patients.

Areas within BBMP limits in the past 15 days have seen 140 positive cases, a 40% jump in incidence till now this year. Till July 15, there have been 496 cases of dengue. The number was 356 on June 30. Adding to this, 118 cases of dengue positive cases have been reported from Bengaluru Urban areas this year.
