"Silent" bird flu carriers could be spreading the disease among the local fowl population, Hong Kong's health chief York Chow Yat-ngok warned yesterday.

"Are there poultry out there that are carriers of the disease but do not show symptoms? This point is cause for concern since, if this is correct, it will be the most difficult issue for us to handle," Chow said.

"We are now seeing people suffering from bird flu being linked to contact with birds who are sick even though there has been no outbreak.

"We are concerned about whether there have been some small changes in the way poultry reacts to the flu virus."

Chow aired his fears at a Heung Yee Kuk function after the mainland confirmed another case of human bird flu infection.


There have been eight human cases of H5N1 infection in the mainland since December, five of whom have died.

The most recent case involves a 21-year-old woman in Hunan who was taken sick on January 23. The woman, who has a history of exposure to sick poultry, is in stable condition, according to the mainland's health ministry.

Hong Kong University microbiologist Ho Pak-leung warned: "The virus is at our front door - very likely in Guangdong [at this moment]. Something unusual is happening."

But he said there is no evidence to suggest birds can carry the H5N1 virus without showing symptoms of the disease, and that the chances of "silent" carriers are minimal.

"If we look at the eight human cases of bird flu in the mainland, the majority were the result of exposure to sick or dying birds," he said. "There was contact, and it wasn't simply exposure to live animals that displayed no symptoms.
"We have not found evidence as yet that animals can survive [bird flu] without showing symptoms."


Nevertheless, he said the eight cases are of concern "as this has been the highest occurrence within a narrow timeframe since the first mainland case was documented in 2003."

Regarding the lack of an outbreak among poultry before human infection, Ho said a likely explanation is that most animals have already been immunized, but that vaccines are not always 100 percent effective.

He believes measures should be stepped up to prevent cross-border infection and poultry smuggling by boat, especially with many travelers returning from the mainland after the Lunar New Year holiday.

"Prevention is more important than surveillance," he said.


He also encouraged the government to speed up vaccination research with the mainland.

Health chief Chow also made reference to the discovery last Thursday of two geese and one duck found dead on Sha Lo Wan beach on Lantau.

All were confirmed to have been infected with H5N1.

He said investigations are centered on whether the birds had been dumped there by individuals who owned poultry or whether they had died in the mainland and been washed ashore.

Chow also requested mainland officials to investigate whether there have been any bird flu outbreaks along the Pearl River Delta.


There are no poultry farms or live poultry within a three-kilometer radius of Sha Lo Wan.

Islands district councillor Chau Chuen-heung said that, within the vicinity of Sha Lo Wan, it is unlikely that villagers are keeping backyard poultry - a once common practice that has since been banned.

"One would be able to hear noises such as rooster calls if anyone kept poultry, but you don't hear that anymore here," she said.

Fears of a bird flu outbreak in Hong Kong rose yesterday after nine more dead birds were found on Lantau.

The carcasses of a goose, five chickens, one duck and two birds that were not identified bring to 12 the number of dead birds found on the island since Thursday. The first three tested positive for H5N1.

The Agriculture, Fisheries and Conservation Department is investigating whether the birds may have come from nearby villages or farms.

Department assistant director Liu Kwei-kin said inspections will cover Lantau and then the rest of the territory.

Liu said Hong Kong will collaborate with mainland officials to determine whether the birds came from over the border.

"We will also conduct tests on all carcasses to see whether they are carrying the avian flu virus."

Villagers said there have been other occasions when dead poultry washed ashore. Sha Lo Wan village head Lee Chi-fung said residents believe the dead birds came from the Pearl River Delta.

"We will need to be more careful when walking on the beaches," a female resident said.

The HKU department of microbiology said it is working at sequencing the carcasses to determine whether the birds had succumbed to avian flu, and hopes to complete the process today.

Eighteen people who were exposed to the carcasses in Sha Lo Wan are under medical surveillance by the Center for Health Protection.

The mainland has reported eight human cases of H5N1 infection since December. Five of the victims died.

?There?s no doubt of an outbreak of bird flu in China, though the government hasn?t admitted it,? Lo said in a telephone interview today. ?Inefficient communication between the Hong Kong and mainland authorities is an ongoing problem. Hong Kong has not been well-informed by the mainland.?

Eight people have been infected with the H5N1 strain of avian influenza in China this year, the most in a single month since 2003 when the lethal virus was first detected in humans, according to the World Health Organization. Three out of 12 dead birds found on Hong Kong?s Lantau Island tested positive for the strain, stoking concerns the virus is circulating widely among birds in southern Guangdong province which borders Hong Kong.

China hasn?t reported any cases of H5N1 among birds since December. The human cases show the virus must be circulating among birds, said Vincent Martin, a senior technical advisor on avian flu for the Food and Agriculture Organization of the United Nations.

?There must be some cases which have not been reported,? Martin said in a telephone interview today. ?It?s not normal that we don?t receive any confirmation or any reports of outbreaks in poultry.?

?We are concerned about the possibility of the virus being present in birds in Guangdong, given the sheer numbers of chickens and ducks there and their proximity to humans,? Peter Cordingley, a spokesman for the World Health Organization, said in an e-mailed response to questions from Bloomberg News. Still, human infections in China so far this year are ?within expectations,? Cordingley said.

Human Pandemic

International health officials have been monitoring H5N1 for more than a decade for signs it could mutate into a form that is easily spread between humans. A flu pandemic of avian or other origin could kill 71 million people worldwide and lead to a ?major global recession? costing more than $3 trillion, according to a worst-case scenario outlined by the World Bank in October.

H5N1 has infected at least 404 people in 15 countries since 2003, killing 63 percent of them, according to the Web site of the Geneva-based WHO. Of 38 confirmed cases in China so far, 25 have been fatal.

Avian Influenza Viruses (H5 or other subtypes) in Chinese mainland poultry are probably widespread since the early stage of current panzootics (started in 2003).

So I wonder why do several ''experts'' continue to ''suspect'' poultry outbreaks in the same areas.

AIV circulation is almost certain - because virus spillover onto nearby countries and regions cannot be explained in full solely with poultry movement, for trade or for smuggling.

I cannot understand how can someone express surprise after collections of several carcasses of birds at Hong Kong.

Perhpas they think to be afloat in Easter Island...

I don't want to be sarcastic when people are at risk but some degree of intellectual honesty is needed even for HK (and elsewhere) ''experts''...

Three of 12 birds found on the Hong Kong island of Lantau in the past five days have tested positive for H5N1.

Villagers on the island said dead birds have been washing ashore in recent days and they believe they come from China's Pearl River Delta, which flows out into the South China Sea surrounding Hong Kong.

Hong Kong: Nine more dead birds collected on Lantau today (2/3/2009) [AFCD]

A spokesman for the Agriculture, Fisheries and Conservation Department (AFCD) said that nine more dead birds were collected in Sham Wat Wan and San Shek Wan, Lantau today (February 3), adding that preliminary tests for the H5 virus are being arranged.


The carcasses found today included one duck, six chickens and two birds of as yet identified species.

Of them, a chicken was collected in Sham Wat Wan and the rest were found in San Shek Wan.

Preliminary results available today showed that the dead Cattle Egret and one of the chickens collected in Sha Lo Wan yesterday (February 2) tested negative for the H5 virus.

As the other bird carcasses were highly decomposed when found, more time is required to confirm the test results.

AFCD staff inspected 28 villages in North Lantau today.

This is my 4th winter covering bird flu, and just about every winter we hear rumors about `something' going on in China. Often these reports come from Boxun, or some other dissident news outlet, and these reports are almost never confirmed by the Chinese.

Whether they signified anything real or concrete . . . well, that's hard to say.

The 2002-2003 SARS outbreak was effectively hidden for months as that virus burned its way through mainland Chinese provinces, and filled hospital wards with the sick and dying. There were thousands of cases before the rest of the world came to realize there was a problem.

The Chinese government can be very effective when it comes to containing or suppressing news.

At least in the short run.

And so it is again we find ourselves in a winter season, and the rumors and stories (mostly unverified) are coming again out of China.

Frankly, we know only what the Chinese government is willing to admit.

And for now they admit that 8 people contracted the H5N1 virus during the month of January. They deny outbreaks of the virus in poultry, while at the same time, blame infected birds as the source of these infections.
Meanwhile, Hong Kong, which sits adjacent Guangdong Province, has recently discovered dead birds on their beaches (ducks, geese, & chickens) - some of which have tested positive for the H5N1 virus.

The sudden upsurge in admitted human cases, the discovery of dead (normally domesticated) birds on the beaches of Lantau Island, combined with rumors and stories emerging from China, has got Hong Kong authorities worried.

What is different this winter over winter's past is that we are seeing open questioning of the situation in China by mainstream media sources. Not just dissent voices.

And authorities in Hong Kong are publicly questioning what is going on in mainland China.
BusinessWeek, Bloomberg, and today, The Telegraph have all run articles in the past couple of days questioning whether `something' is going on, and is being hidden, in China.

Meanwhile, the newshounds on the flu forums are on the job, looking for any clues as to what is going on.

If anything is seriously amiss in China (and despite the speculation, we don't know that yet), I wouldn't be surprised if they are the first to get wind of it.

China is suspected of covering up an outbreak of bird flu among poultry, according to experts.

The accusations came after at least 21 dead birds, including ducks and chickens, washed up on beaches in Hong Kong. At least three of them have tested positive for the H5N1 avian flu virus.

The Hong Kong government has said that there are no bird farms near where the corpses washed ashore and that it is liaising with the Chinese authorities to determine if they came from the Pearl River delta region.

There have been eight cases of bird flu infecting humans in January alone, five of which have been fatal. The cases have been distributed widely throughout China and there is a growing fear that contaminated poultry has entered the food chain.

There have been unconfirmed rumours that infected chickens in Jiangsu province were bleached with hydrogen peroxide and sold. There was a mass cull of 300,000 birds in December after an outbreak.

Lo Wing-Lok, a bird-flu expert in Hong Kong, warned that "something very terrible" could be happening in China. "There's no doubt there has been an outbreak, but the government has not admitted it," said Mr Lo.

The United Nations Food and Agriculture Organisation also said that there "must" have been outbreak among chicken, duck or geese flocks.

"There must have been some virus circulation or possibly some outbreaks lately," said Dr Vincent Martin, an FAO senior technical adviser on bird flu. "There are more cases than last year, including in places where the disease was not reported before," he said, but added that the FAO had not been told about any further problems since the Jiangsu incident.

Since bird flu rarely passes from human to human, the victims in January must have been infected by eating contaminated meat, he noted.

"There must be some suspicions of the disease reported to the ministry of agriculture, and some of those suspicions might have come up positive," he said. "We are waiting to receive some reports describing the overall epidemiological situation and the results of investigations."

The World Health Organisation declined to comment on whether or not there had been a cover-up by the Chinese government. China was widely criticised for suppressing and distorting the true extent of the SARS outbreak in 2003, when officials in Guangdong covered up the epidemic for almost six months before informing Hong Kong.

It is very difficult to postulate where the three carcasses came from. But from the various investigations we have done, it is most likely drifting down from the Pearl River in the last week or two. Some of the new carcasses we have found are still under investigation. So far we have not found any extra H5 affected carcasses.
But as you know that we are doing wild bird and dead bird surveillance all the time. We test about 40 to 80 dead birds all over the territory every day. These include both those we have discovered in North Lantau and other areas. If we found any positive test results, we will announce right away. Every year we found some 10 to 20 such cases. As long as they are limited to wild birds, the risk to Hong Kong people is not that high because it has not been reported that wild birds can transmit the disease directly to human beings. It is usually through infection of poultry, and then the sick poultry would affect human beings.

I don't think so because the whole region is still under the threat of avian flu, particularly during the whole winter. It is usually after May that the whole region starts to warm up, we will then see a decline of these incidents.

Let me talk about the virus samples we have collected so far. In the three carcasses which we have isolated the virus, the preliminary discovery is that it is related to a similar type of virus that happened in the southern part of China in the last two years, which is clade 2.3.2.
But it is slightly different from what we have discovered last year in our market and the outbreak in our local farm in December, which is clade 2.3.4.
But these are quite common clades of avian flu in the southern part of China and Hong Kong. It is certain that there is no significant mutation of the virus.
That is actually what we have discovered so far.
On the eight human infections of avian flu in the Mainland, we do not have the full report yet. But according to the preliminary information that is given to us, it is similar to the various virus that was isolated in northern China in the last one or two years.
So there is no significant epigenic mutation.
As far as the nature of the virus is concerned, we are confident that at this moment there is no evidence of human to human transmission and no significant epigenic change.
