The spokesperson for Princess Margaret Hospital (PMH) today (February 13) made the following announcement regarding a newly confirmed critical human swine influenza case:

A 53-year-old male patient with diabetes and end-stage renal failure attended the Accident & Emergency Department of PMH on February 11 due to fever and pneumonia syndrome. He was admitted to the Intensive Care Unit on February 12. The test for human swine influenza was confirmed positive today. The patient was prescribed with Tamiflu and antibiotics. He is now in critical condition and requires ventilation support.

The hospital is closely monitoring his condition.

A 24-year-old man with past good health developed flu symptoms and was admitted to the isolation ward via Accident & Emergency Department of QEH on February 12. His condition deteriorated and was transferred to the Intensive Care Unit on the same day and required ventilation support. Positive result of human swine influenza was confirmed on February 13. He was given Tamiflu and antibiotics. The patient is now in critical condition.

The hospital will continue to closely monitor her condition. The case has been reported to the Hospital Authority Head Office and the Centre for Health Protection.

The Centre for Health Protection (CHP) of the Department of Health today (February 17) received report about a 75-year-old woman with history of human swine influenza (HSI) vaccination who was admitted to hospital for lower limb weakness.

The patient developed bilateral lower limb weakness on February 8 and was admitted to Queen Elizabeth Hospital on February 9. She is now in critical condition in Intensive Care Unit.

The patient received HSI vaccination on December 31 last year at a public outpatient clinic, more than five weeks before the onset of symptoms.

A CHP spokesman said both nerve conduction study on the patient and CSF findings were suggestive of Guillain-Barre Syndrome (GBS).

According to the Hospital Authority, 40-60 GBS cases were recorded each year between 2000 and 2009, with anything from zero to 10 cases per month, regardless of vaccination history.

"Literature review shows that the majority of GBS cases that are temporarily associated with seasonal influenza vaccination occur between the second and the third week, with a median latency of 13 days," the spokesman said.

"The CHP will closely monitor the clinical progress and further laboratory results of this patient. Information about the case will be reviewed by the Expert Group in due course for determination of diagnosis and etiology."

According to the World Health Organization, global experience so far showed that HSI vaccination is not associated with increased incidence of GBS.

The spokesperson of Princess Margaret Hospital (PMH) today (February 17) announced a fatal case of human swine influenza (HSI):

A 51-year-old male patient with history of diabetes mellitus and lung disease was admitted to Princess Margaret Hospital via Accident & Emergency Department on December 30 last year because of flu symptoms. Positive result of human swine influenza was confirmed on the same day and he was prescribed with Tamiflu and antibiotics. His condition deteriorated on January 4 and he was transferred to the Intensive Care Unit. He finally succumbed this morning (February 17). The hospital would like to express its condolences to the patient's family.

The case has been reported to the Hospital Authority Head Office and the Centre for Health Protection.

Of this number, 174 were administered at HA and DH clinics while the eHealth System received 77 and 495 vaccination reimbursement claims from private doctors and visiting medical officers to residential care homes respectively. It is expected that some additional vaccinations given will be recorded in the eHealth System later. The administered doses comprised 83 for community-living elderly aged 65 or above, 58 for persons under 65 years with chronic illnesses, four for a pregnant woman, 28 for healthcare workers, 99 for children aged between six months and less than six years, and 474 for residents of residential care homes.

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority (HA) today (February 18) provided the following wrap up of the past week:

In the past week (February 11-17), a total of 482 patients with flu-like symptoms attended the DFCs. Among all confirmed human swine influenza patients, 64 are now staying in public hospitals for treatment, with 57 in stable condition, four in serious and three in critical condition.

Furthermore, the newly confirmed cases in the past week included eight staff members of public hospitals (one male and seven female, comprising three nurses, one allied health professional and four supporting staff).

So far 1,345 HA staff members have been confirmed with human swine influenza.

Among them, 1,339 have already recovered and returned to work.

The HA spokesman reminded members of the public that the eight DFCs run a half-day service from 9am to 1pm during weekends and public holidays, while full-day service (9am to 5pm) is maintained from Mondays to Fridays.

The Hospital Authority (HA) today (February 22) announced two fatal cases of human swine influenza (HSI):

A 77-year-old man suffering from end-stage lung cancer, heart disease and hypertension attended the Accident and Emergency Department of Prince of Wales Hospital on February 20 due to cough and fever. He was admitted into the isolation ward and prescribed with antibiotics. Patient's condition was further deteriorated and succumbed yesterday (February 21). Test result was positive to HSI. The hospital would like to express its sincere condolences to his family.

Furthermore, a one-year-old boy with congenital heart disease developed flu symptoms on December 1, 2009 and attended the Accident and Emergency Department of Kwong Wah Hospital on December 3. He was admitted to Paediatric general ward and later admitted to Paediatric Intensive Care Unit on December 11. Positive result of HSI was confirmed on December 12. The boy was prescribed with Tamiflu and antibiotics and was on ventilation support. The boy was transferred to the Paediatric Intensive Care Unit, Queen Elizabeth Hospital for further treatment on his underlying diseases on February 8, 2010. His condition was further deteriorated and he finally succumbed on February 19. The hospital would like to express condolences to the patient's family.

The cases have been reported to the HA Head Office and the Centre for Health Protection.

Positive result of HSI was confirmed on February 17 and he was transferred to the isolation ward for treatment and was prescribed with Tamiflu and antibiotics. The patient's condition continued to deteriorate and he succumbed today (February 23). The hospital would like to express condolences to the patient's family.

The case has been reported to the Hospital Authority Head Office and the Centre for Health Protection.

Members of the Expert Group on Serious Adverse Events with History of Human Swine Influenza (HSI) vaccination will meet at the Centre for Health Protection (CHP) of the Department of Health (DH) on February 25 (Thursday) to discuss recent cases of neurological and pregnancy-related conditions.

The CHP of the DH has established a surveillance system to monitor serious adverse events since the launch of the HSI vaccination programme.

The meeting will discuss a case of an intrauterine death involving a 28-year-old pregnant woman reported to CHP on February 12 and a case of 75-year-old woman with suspected Guillain Barre Syndrome (GBS) following HSI vaccination reported to CHP on February 17.

The meeting will also review two new cases with neurological symptoms reported to CHP today (February 23).

The first case involved a 34 year-old man who presented with headache on February 20, followed by double vision the following day. He was admitted to hospital on February 21. He received HSI vaccination on December 21, 2009. Investigations are being carried out to ascertain the diagnosis. He is now in stable condition.

The second case involved an 80-year-old man with history of hypertension, ischaemic heart disease, renal impairment and asthma. He received HSI vaccination on December 29, 2009. He presented with sudden onset of limb paralysis and loss of voice on February 5 and was admitted to hospital on the same day. He is being investigated for transverse myelitis and other neurological conditions. The patient is now listed in critical condition; his vital signs are stable.

The spokesman reiterated that a baseline number of such neurological conditions and intrauterine deaths occur in Hong Kong every year. Some of the events are expected to occur coincidentally following HSI vaccination.

He stressed that, H1N1 virus, changing the future of the epidemic will lead to more serious or that there is 2 or 2 wave outbreak is still unknown.
