At least 1,060 doses of human swine influenza (HSI) vaccine were administered at Hospital Authority (HA), Department of Health (DH) and private clinics to target groups during the 24 hours ending at 1pm today (January 28).

Of this number, 798 were administered at HA and DH clinics while the eHealth System received 262 vaccination reimbursement claims from private doctors. It is expected that some additional vaccinations already given by private doctors will be recorded in the eHealth System later. The administered doses comprised 316 jabs for community-living elderly aged 65 or above; 193 for persons under 65 years with chronic illnesses; two for pregnant women; 32 for healthcare workers; 516 for children aged between six months and less than six years; and one for pig farmers and slaughterhouse workers.

This brings to at least 151,481 the cumulative total of HSI jabs administered to target groups since the start of the vaccination programme on December 21, 2009.

The following is issued on behalf of the Hospital Authority:

A spokesperson of Princess Margaret Hospital (PMH) issued the following announcement today (January 29):

Since January 22, two female patients (aged 76 and 77) and one female staff member of a Medicine and Geriatrics Ward in PMH have presented with flu symptoms.Viral tests were arranged for them and all were subsequently confirmed with Human Swine Influenza.

The two patients are being treated under isolation and prescribed with Tamiflu.

Both are in stable condition.

The staff member is in stable condition after treatment and on sick leave. Admissions to the ward has been suspended and infection control measures have already been stepped up. Patients' relatives have been informed about the situation.

All other patients and the staff in the ward are under close monitoring.

The cases have been reported to the Hospital Authority Head Office and the Centre for Health Protection for follow up.

Secretary for Food and Health:
The most simple explanation is if you look at all the mortalities in the last few weeks, all of them actually did not have any vaccinations. And this is already a good proof. Of course, flu vaccination is not new as such because it has been in the market for many years. It has been proven to be useful to prevent outbreaks and also serious cases, particularly personal protection for flu. Swine flu is one form of flu and you have to understand that H1N1 is also a form of influenza, just like H1, H3 as well as flu B. This type of manufacturing of vaccine is the same as other vaccines but coming from different viral prototype. So the manufacturing process and also the effect is all the same. I do not think it is right to say they have no effect. So far, it has also been proven that all the cases admitted into hospital, severe cases, and also those who unfortunately died, also did not have the vaccine. So it has actually proved that it works.

As I said earlier, the reason why we have the vaccine is to ensure that we have sufficient supply for all the patients who are in need. Obviously, we have sufficient supply for all the five at-risk groups in Hong Kong, plus some extra for people who are willing to take the vaccine. I am not worried that it is not taken entirely because I think, if just in case, the peak of the influenza is getting worse, I am sure more people will be interested to take it. It is important that we have it in reserve and can be able to use it in the peak if necessary. It is like insurance. It is like something you put there so that in case of need, you can use it.

The definition of pandemic is a new influenza infectious epidemic that spreads from continent to continent. As far as today is concerned, there are over 160 countries that have already actually reported the presence of the new influenza. So I think by all definitions this is a pandemic.

Another 4,986 doses of human swine influenza (HSI) vaccine administered to target groups have been recorded by the Department of Health (DH) as at 1pm today (February 1).

Of this number, 849 were administered at Hospital Authority and DH clinics between 1pm on January 29 and 1pm today while the eHealth System received 685 vaccination reimbursement claims from private doctors during the same period. In addition, 3,452 HSI claim reimbursements were received from outreach teams to residential care homes between January 18 and 1pm on February 1. It is expected that some additional vaccinations given will be recorded in the eHealth System later. The administered doses comprised 362 jabs for community-living elderly aged 65 or above; 220 for persons under 65 years with chronic illnesses; five for pregnant woman; 187 for healthcare workers; 901 for children aged between six months and less than six years; one for pig farmers and slaughterhouse workers, and 3,310 for residents of residential care homes.

This brings to at least 157,440 the cumulative total of HSI jabs administered to target groups since the start of the vaccination programme on December 21, 2009.

Regarding the 91-year-old man admitted with lower limb weakness to Princess Margaret Hospital on January 12, subsequent investigation including examination of cerebrospinal fluid and nerve conduction tests did not support the diagnosis of Guillain-Barre Syndrome. He was also found to have pneumonia which could explain his symptoms. The man has recovered and was discharged on January 30.

The spokesperson of North District Hospital (NDH) today (February 2) announced a newly confirmed critical case of human swine influenza (HSI):

A 58-year-old male patient with diabetes mellitus and hypertension developed fever and cough in Shenzhen on January 10. He was admitted to a local hospital on January 16 and prescribed Tamiflu. Positive result for HSI was confirmed on January 17. The patient has been put on ventilator support since January 24. Since his conditionhas deteriorated, he was transferred to NDH on January 27 and admitted to the intensive care unit for further treatment. The patient is now in critical condition.

The hospital will continue to closely monitor his condition.

Of this number, 715 were administered at HA and DH clinics while the eHealth System received 302 and 775 vaccination reimbursement claims from private doctors and visiting medical officers to residential care homes respectively. It is expected that some additional vaccinations given will be recorded in the eHealth System later. The administered doses comprised 253 for community-living elderly aged 65 or above; 192 for persons under 65 years with chronic illnesses; six for pregnant women; 211 for healthcare workers; 385 for children aged between six months and less than six years; and 745 for residents of residential care homes.

This brings to at least 159,232 the cumulative total of HSI jabs administered to target groups since the start of the vaccination programme on December 21, 2009.

Meanwhile, in response to media enquiries, a DH spokesman said that regarding a 46-year-old man with a history of HSI vaccination who died on January 26 at Princess Margaret Hospital, a post-mortem examination had found evidence of acute myocardial infarction. Available evidence does not indicate any causal relationship between the HSI vaccination and the case, the spokesman added.

What is very interesting about Hong Kong is that they are actually stockpiling on vaccine and keeping it in reserve, to the opposite of several countries cancelling their orders or trying to sell their stock.

The spokesperson of Queen Mary Hospital (QMH) made the following announcement regarding a newly confirmed critical human swine influenza case:

A 20-month-old baby girl developed flu symptoms and was admitted to the paediatric ward via Accident & Emergency Department of QMH on January 28. Positive result of human swine influenza was confirmed onJanuary 29. She was given Tamiflu and antibiotics. The condition of the patient improved and was discharged on February 1. The baby girl developed shortness of breath and attended the Accident and Emergency Department of QMH today (February 3). She was admitted to the Intensive Care Unit and required ventilation support. The patient is now in critical condition.

The hospital will continue to closely monitor her condition.

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority (HA) today (February 4) provided the following wrap up of the past week:

In the past week (January 28 - February 3), a total of 987 patients with flu-like symptoms attended the DFCs. Among all confirmed human swine influenza patients, 67 are now staying in public hospitals for treatment, with 49 in stable condition, five in serious condition and 13 in critical condition.

Furthermore, the newly confirmed cases in the past week included 10 staff members of public hospitals (two male and eight female, comprising five nurses, one allied health professional, three supporting staff and one administrative/clerical staff).

So far 1,323 HA staff members have been confirmed with human swine influenza.

Among them, 1,320 have recovered and returned to work.

The HA spokesman reminded members of the public that the eight DFCs run a half-day service from 9am to 1pm during weekends and public holidays, while full-day service (9am to 5pm) is maintained from Mondays to Fridays.

Of this number, 448 were administered at HA and DH clinics while the eHealth System received 252 and 944 vaccination reimbursement claims from private doctors and visiting medical officers to residential care homes respectively. It is expected that some additional vaccinations given will be recorded in the eHealth System later. The administered doses comprised 191 for community-living elderly aged 65 or above; 156 for persons under 65 years with chronic illnesses; one for a pregnant woman; 103 for healthcare workers; 276 for children aged between six months and less than six years; and 917 for residents of residential care homes.

The cumulative total of HSI jabs administered to target groups since the start of the vaccination programme on December 21, 2009, is now at least 162,758.

Meanwhile, the DH's Centre for Health Protection has received further information on the 67-year-old woman with history of HSI vaccination who was admitted to Kwong Wah Hospital (KWH) on January 15, complaining of limb weakness. A CHP spokesman, while reaffirming that the woman was not suffering from Guillain-Barre Syndrome, said updated information from the hospital suggested that the patient might be suffering from encephalitis.

"Further investigation will be carried out to ascertain the cause of her illness," he said.

The woman is now in the intensive care unit in KWH. She received an HSI vaccination on December 23, 2009, and developed weakness on January 12, and fever, headache and vomiting on January 15.

The following is issued on behalf of the Hospital Authority:

A spokesperson of United Christian Hospital (UCH) today (February 9) announced a fatal case of human swine influenza (HSI):

A 73-year-old male patient with history of diabetes mellitus and multiple chronic diseases attended the Accident and Emergency Department of UCH on January 5 with flu-like symptoms. The patient was admitted to the medical ward and positive result of HSI was confirmed on the same day. He was prescribed with Tamiflu, Relenza and antibiotics. His condition improved and he was transferred to Kowloon Hospital on January 23 for rehabilitation. The patient developed symptoms of stroke and was transferred back to the medical ward of UCH last Friday (February 5). He passed away on Saturday night (February 6). The hospital would like to express condolences to the patient's family.

The case has been reported to the Hospital Authority Head Office and the Centre for Health Protection.

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority today (February 11) provided the following wrap-up of the past week:

In the past week (February 4-10), a total of 895 patients with flu-like symptoms attended the DFCs. Among all confirmed human swine influenza patients, 60 confirmed patients are now staying in public hospitals for treatment, with 50 in stable condition, six serious and four critical.

Newly confirmed cases in the past week included 14 staff members of public hospitals (three male and 11 female, including eight nurses, five supporting staff and one administrative/clerical staff).

So far there have been 1,337 Hospital Authority staff members confirmed as human swine influenza. Of them, 1,331 have already recovered and returned to work.

A Hospital Authority spokesman reminded people that the eight DFCs ran a half-day service from 9am to 1pm during weekends and public holidays, while full-day service (9am to 5pm) is maintained from Mondays to Fridays.

Of this number, 311 were administered at HA and DH clinics while the eHealth System received 133 and 498 vaccination reimbursement claims from private doctors and medical officers visiting residential care homes respectively. It is expected that some additional vaccinations given will be recorded in the eHealth System later. The administered doses comprised 104 for community-living elderly aged 65 or above, 126 for persons under 65 years with chronic illnesses, one for a pregnant woman, 31 for healthcare workers, 196 for children aged between six months and less than six years, and 484 for residents of residential care homes.

The cumulative total of HSI jabs administered to target groups since the start of the vaccination programme on December 21, 2009, is now at least 173,732.

The Centre for Health Protection (CHP) of the Department of Health, through the surveillance system established since the launch of the human swine influenza (HSI) vaccination programme, today (February 12) received a report about a case of intrauterine death (IUD) involving a 28-year-old pregnant woman.

The woman is now staying at Tuen Mun Hospital (TMH) in stable condition.

The CHP's investigation showed that the woman was at 39 weeks of gestation. She was followed up by a Maternal and Child Health Centre (MCHC) for this pregnancy. She had received HSI vaccination at the MCHC on December 21, 2009. The woman, with good past health, was admitted to TMH today for vaginal spotting. However, foetal heart beating was not detected after admission. Intrauterine death was subsequently confirmed.

Investigations for common causes of IUD, including infection, genetic and metabolic disorders, are ongoing.

A CHP spokesman noted that the HSI vaccination was given more than seven weeks ago. He said there is no medical evidence at this stage to suggest that the IUD was related to HSI vaccination.

In Hong Kong, about 150 to 220 stillbirths are recorded every year. A significant proportion, 15 to 70 percent, of the cases does not have identifiable causes. According to international and local experience, the rate of stillbirth is about 0.3 to 0.5 percent of total deliveries (including live births and stillbirths). According to the World Health Organisation, studies to date do not show harmful effects from the HSI vaccine with respect to pregnancy or a developing embryo or foetus.

Cases of IUD following an HSI vaccination have also been recorded and monitored in overseas countries.

So far no causal relationship has been found between HSI vaccination and IUD. The CHP will closely monitor the woman's clinical progress and results of further investigation.
