Two water samples collected from OT negative tested water supply. Two stool samples collected from the patients. Control measures were started soon after the information came from the affected area on 4.8.12.A total of 110 patients were admitted in the CHC Maurawan for treatment and most of them discharge meanwhile 18 patients were sent to the district hospital, Unnao.A team of doctors & paramedical staff is camping at the CHC Maurawan.No casuality has been detected. So that water gets polluted. In heavy rains the water over flowed and polluted water entered into the premises of houses. 31 blood smears collected, 3 were positive for Malaria parasite (P. vivax). Out of 3malaria positive 2 are same house, where the death occurred. District RRT investigated the out break. Fever surveillance done. No The RRT visited the affected area,taken control measures and thereafter investigated and found it as food poisoning. Investigation Closed Except one death, all other 4 cases have been saved, the situation is under surveillance and no further cases have been reported.

Survey in all affected area house to house visit done District Malaria Officer Team and Epidemic Combact Team.And create awareness in community to prevent dengue in area.Check out 30 deserts cooler and 70 water containers in house to house for Adease agypti breeding.In Rekha Shrivastava house there Adease agypti larvae found in flower pot. There are travel history of Pramod Parikar and S.K.Biswas.give advice to community for cleaning of desert cooler and all water containers in weekly interval. Acute Diarrhoeal Disesase Cases of watery diarrhoea and vomiting reported from Patori Tola Vill- Patori, sub-centre - Nawadih, PHC - Kawakole, Nawada 62 cases 2 fatalities 13/08/2012 2012-08-16 11:52:00 4000 Cases occured due to Envromental variation heavy rain fall after excessive heat then again hot climate. Age group of 2 to 10 years mostly affected. Male & females are almost equaly affected. Water sample is collected for further investigation. District RRT visited the affected area an investigated the outbreak. Active house to house survey conducted to find new cases. All cases treated with metronidazole. ORS packets and chlorine tablets distributed in the locality. People were educated to maintain proper sanitary conditions and regular hand washingafter defaecation. Investigation Open Under Control. No any new case is detected today.

Chikungunya Ayothiyapattnam, Achankuttapatti , Achankuttapatti , Achankuttapatti AD Street 27 0 05/08/2012 2012-08-16 13:30:31 1540 Date of onset of fever in the village on 04.08.12, affected people approched Achankuttapatti PHC for treatement. 27 blood smears were taken and examined, found negative for malaria. 5 Serum samples were taken and sent it to the ZET, Salem on 06.08.12 and All were found positive for Chikungunya. Intensified house to house fever surveillance done by field staff. Anti Larval and Anti audlt (fogging) activity is being carried out by field staff and VHSC member. Health Education about Mosquito breeding and safe drinking water practice done. Residual Chlorine level checked found NIL. Chlorination of OHT done. Investigation Open Fever outbreak may be due to high vector Aedes mosquito density . Weekly Anti larvel work without omission of breeding places may be reduced vector density.Neiboring villages of 1KM radius may be conduted fever surveillance by PHC Staff. Health education may play effetive roll to change the people water storing behavior . Affected area is under vigil surveillance by PHC Team.

The affected persons are small kids and one adult. Investigation open Cases of diarrhea with vomiting reported from village Gondwara, Block BelaBhela, CHC BelaBhela. Medical team investigated the cases. Active house to house search was done. The symptoms started due to contaminated drinking water. Health education given regarding consumption of freshly cooked food and clean drinking water during this season. Adequate amounts of antidiarrheals, ORS packets, and chlorine tablets were given to the affected persons. A strict vigil to be maintained in the village for the next seven days has been ordered from the district headquarter. Investigation Open Under control

Unhygenic practices were found among inhabitats. Two water samples one fom Source and another from user end taken and sent to PHL Barzulla Srinagar for analysis, results awaited a. Medical team from Shopian and team from DSU Pulwama provided standard case management to the patients. b. Sanitation drive started in the village through village health sanitation & nutrition committee. c. Inhabitants were advised to remove the open latrines which were observed on the banks of a canal flowing through the village. d. Acute Diarrhoeal Disease Bidhuna, Bidhuna, Bela, Jauhar accordingly 4 0 20/08/2012 2012-08-24 14:57:10 1400 Village is surrounding by the paddy fields. Water supply is being the over head tank there was a leakages pipe lines at few places, due to which whole water supply got contaminated and resulting in diarrhoea in the village. Two water samples collected from the village. ( India Marka hand pump) District RRT investigated the out break, 06 well disinfected 50ORS packets and 2000 chlorine tablet were distributed with the help of ASHA. Investigation Closed 04 India Marka hand pump situated in the village but it is not in working condition.

Water supply is being the over head tank there was a leakages pipe lines at few places, due to which whole water supply got contaminated and resulting in diarrhoea in the village. Two water samples collected from the village. ( India Marka hand pump) District RRT investigated the out break, 06 well disinfected 50ORS packets and 2000 chlorine tablet were distributed with the help of ASHA. Investigation Closed 04 India Marka hand pump situated in the village but it is not in working condition.

Food Poisoning Achalda, Achalda, Muhumdabad, Ghashi accordingly 4 cases 1 fatality 21/08/2012 2012-08-24 18:13:28 500 A villager name Shri Ramnarash and his four family members developed vomiting and diarrhoea after 4-6 hours of consuming contaminated food. Investigation Open Lack of knowledge and literacy Mushars had ingested unsafe drinking water with resultant diarrhoea and vomiting which was treated with ORS and zink sulphate and IV infusion. Rushers community need repeated councelling.
