    Main developments in past 24 hours

    • Study comparing laboratory sampling within two surveillance systems in England published;
    • Sweden reported the first fatal case related to pandemic (H1N1) influenza;
    • 285 new cases reported in the EU and EFTA countries, reaching a total of 46 301 cases.


    This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 31 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

    Disclaimer:

    Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


    Scientific Updates

    A study comparing the self-sampling scheme within the NHS direct telephone hotline and the HPA laboratory surveillance scheme was published today in the British Medical Journal (BMJ). The authors conclude that ?self-sampling? among people contacting NHS direct provided a complementary system to traditional laboratory surveillance and data from both systems were comparable.     Epidemiologic update

    All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. A total of 46 301 confirmed cases, including 105 deaths, have been reported since April 2009 (Table 1). In the past 24 hours, 285 new cases from the EU and EFTA countries were confirmed. The first fatal case was reported from Sweden.

    Currently, the majority of cases have been reported from Germany. The latest available updates on hospital admissions per Member State are presented in Table 2.


    Changing the format of the daily update

    As the pandemic (H1N1) 2009 evolves, new types of data become increasingly important, whereas other may become less interesting. ECDC has refreshed the format of the Situation Update to better accommodate current and anticipated information needs. The name of this report changed to "Daily Update" and some changes have been made in the data reported.

    Table 1 ? reported cases by the EU/EFTA countries remains as before. In addition, we are publishing a new Table 2 on the number of current and cumulative hospitalisations and admission to intensive care in EU/EFTA Member States. The source of the information is official statements or surveillance reports published by national health authorities. It should be noted that indications and thresholds for hospitalisations vary by country, so the figures quoted may not be directly comparable between countries.

    Assuming no changes in the national reporting system, trends of hospitalisations over time should give an accurate representation of the evolution of the pandemic. This might change if countries are faced with capacity issues within the healthcare system at a later stage of this evolving pandemic.

    Table 3 lists only reported deaths for countries outside the EU/EFTA area; we have discontinued reporting the case numbers for these countries. The main reason for this is that more and more countries have stopped the reporting of new cases, and these figures no longer give a representative picture of the situation.

    The two graphs now show number of reported deaths by week of report ? one graph for EU/EFTA, one for the entire world. The two maps depict cumulative deaths by country, one for EU/EFTA, one for the world.

      Scientific advance

      How best to use the pandemic vaccines when they become available?

      Pandemic vaccines will be in short supply when then are first licensed in the EU the autumn. In its scientific advances series ECDC today features a recent modelling paper (Medlock & Galvani A. Optimizing Influenza Vaccine Distribution Science 2009 August 20th http://www.sciencemag.org/cgi/rapidpdf/1175570v1.pdf) which argues for a particular strategy (only immunising young people). In a commentary ECDC makes the case for a more pluralistic approach focusing on a number of groups along the lines of what is recommended by World Health Organization (WHO). The commentary also has a useful Table listing the priority and target groups identified by expert groups advising the WHO, the United States Centers for Disease Control and Prevention as well as the statement of the European Union Health Security Committee.       Public health development

      Review of Influenza A(H1N1) 2009 infection confirmed in turkeys in Chile, South America

      Infection with the pandemic virus has recently been confirmed in two flocks of turkeys in Chile. ECDC describes what is known so far about the case and comments on its implications as a public health development.

      Epidemiologic update

      All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. A total of 46 635 confirmed cases, including 105 deaths, have been reported since April 2009 (Table 1). In the past 24 hours, 334 new cases from the EU and EFTA countries were confirmed.

      Currently, the majority of cases have been reported from Germany. The latest available updates on hospital admissions per Member State are presented in Table 2.


      Other updates

      Director of ECDC addressed the newly elected European Parliament?s ENVI committee

      Zsuzsanna Jakab, Director of ECDC, today addressed the inaugural meeting of the newly elected European Parliament's Committee on the Environment, Public Health and Food Safety. Mrs. Jakab gave ECDC's assessment of how the 2009 influenza pandemic will impact Europe. Her key message was that the pandemic will put significant pressure on health services this autumn and winter and that the EU and Member States need to be ready for this.

      Outbreak study in China documents droplet, not airborne transmission

      A study documenting a pandemic influenza outbreak among tour group members in China was published as a expedited report in the US CDC journal Emerging Infectious Diseases.       Pandemic influenza vaccine in China progresses in regulatory evaluation

      Media report suggest that the Pandemic influenza vaccine produced by Chinese producer Sinovac biotech has passed the expert evaluation in the State Food Safety Authority (SFDA) in China and expects to obtain a production license by the end of this week. This is a split influenza vaccine without adjuvant to be used as a single dose (15ug/0.5ml/dose)


      Rates of ILI and common colds increasing in Russia

      Rates of common colds and influenza have exceeded epidemic threshold levels in several regions of Russia according to the Russian National Influenza Center quoted by the Russian Itar-TASS news agency. The national rates are still under thresholds.

        Main developments in past 24 hours

        • Weekly review of selected non-EU countries pandemic situation included;
        • Norway reported the first fatal case related to pandemic (H1N1) influenza;
        • 1 193 new cases reported in the EU and EFTA countries, reaching a total of 47 828 cases.


        This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 03 September 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

        Disclaimer:

        Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


        Weekly review of selected non-EU countries

        Australia (pop. 20.7 million)

        In the national report for September 3rd the total number of deaths associated with the pandemic influenza (H1N1) 2009 is now 160. There are still relatively large numbers of cases hospitalised, currently 383 people of which due to (H1N1) 2009 and 75 (19.6%) of them are in Intensive Care Units. The total number of hospitalisations in Australia since the pandemic began is 4533. All of these figures indicate a decreasing trend for last few weeks. In most but not all of the jurisdictions of Australia the indications from clinical surveillance is that the peak of the first wave has been passed.

        New Zealand (pop. 4.2 million)

        The latest report (September 2nd ) indicates that the circulation of influenza in the community continues to decline. The ILI consultation rate from a sentinel surveillance system has been decreasing since week 29, but are still at higher levels than during the past two years. The number of deaths from Pandemic Influenza (H1N1) 2009 is now 17. These are deaths where swine flu was a primary cause. There are a number of other deaths that are being investigated by the Coroner's office. As of August 26th only 7 people are reported to be in hospital due to the influenza (H1N1) 2009 or its complications. The pressure on ICU?s is now so low, that national authorities have stopped monitoring patient numbers. In general, schools are reporting usual levels of absence for this time of year.

        The majority of isolates received through the sentinel surveillance system have been influenza A(H1N1)v from week since week 27. Last week 8 strains out of 12 collected through the sentinel system were of A(H1N1)v lineage.

        Brazil (pop. 192 million)

        Since the beginning of the pandemic until week 34, 36 542 Severe Acute Respiratory Infection (SARI) cases were notified in Brazil, of which 6 592 were subsequently laboratory-confirmed as influenza A (H1N1)v infections. The number of confirmed cases as well as ILI cases appears to have been decreasing for the last two weeks. Of the confirmed SARI cases, 657 (10%) have died. According to the health ministry, this is the highest number for a country in the world. However, Brazil has a large population and the reported mortality due to SARI cases with confirmed influenza A (H1N1)v is currently 0,34/100.000 inhabitants. Out of the 6 592 SARI cases linked to influenza A (H1N1)v, 54.2% presented with at least one of risk factor (including pregnancy), which does not significantly differ from what is observed for seasonal influenza in the country. Among SARI cases in fertile age 23.1% were pregnant, which is larger than the proportion of pregnant women among SARI cases due to seasonal influenza (16.8%). The number of reported cases and consultation rates from sentinel surveillance have been decreasing for the last 4 weeks.

        Out of 14 687 PCR tests conducted up to week 34, 68,5% tested positive for influenza A (H1N1)v. This proportion has not changed much in the last four weeks, although the number of samples being collected is decreasing rapidly.

        South Africa (pop. 48.6 million)

        As of September 1st, South Africa has confirmed 5 841 cases of pandemic (H1N1) 09, with 753 new cases being confirmed since the previous update. The number of deaths related to the pandemic is 27. It appears that the number of confirmed cases and deaths are still increasing.

        The recommended national strategy is laboratory confirmation only for moderate or severe cases, however the national authorities believe it is likely that most cases are still being captured by the surveillance system.

        South Africa has a large HIV-positive population, but thus far no information is available on severity of illness or changes in transmission patterns among this important group.

        Epidemiologic update

        All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009 a total of 47 828 confirmed cases have been reported (Table 1). Out of the 111 reported deaths, one new fatal case is from Norway, four from France (two New Caledonia and two The Reunion) and one from Malta. In the past 24 hours, 1 193 new cases were confirmed from the EU and EFTA countries.

        The latest available updates on hospital admissions per Member State are presented in Table 2.

          Weekly EISN surveillance bulletin published

          Sweden is currently the only European country reporting widespread influenza activity at medium level with an increasing trend. In Ireland and the UK (England and Northern Ireland), influenza activity continues to decline, while all other countries are reporting low local activity at most.           Epidemiologic update

Out of the 120 reported deaths, one new fatal case is from Italy, one from Greece, two from Spain and five cases from UK. In the past 24 hours, 395 new cases were confirmed from the EU and EFTA countries.

          Other updates

          Media sources quote regional health officials in Sweden confirming the second fatal case in the country.

          Media sources report that China has approved the 1-dose Sinovac pandemic influenza vaccine.


          Study on first 36 pediatric pandemic influenza fatalities in US published

          The US CDC Morbidity and Mortality Weekly Report (MMWR) published a case series of the first 36 deaths related to pandemic influenza among children and adolescents during April-August 2009. Results indicate that 67% of the fatal cases had a high risk medical condition, almost all (92%) with underlying medical conditions. Among the previously healthy children a high proportion had a bacterial co-infection contributing to their deaths.

          US CDC pathologist describes lung tissue damage in fatal cases

          A Canadian media source quotes leading US CDC pathologist, Dr Sherif Zaki, saying that in a study of 70 fatal pandemic influenza cases, lung tissue damage resembles that of influenza H5N1 rather than the damage caused by seasonal influenza strains and is consistent of acute respiratory distress syndrome (ARDS). However in about a third of the patients studied, bacterial co-infection was also detected.


          US Institute of Medicine recommendation on respiratory protection for healthcare

          The US Institute of Medicine published recommendations for Respiratory Protection for Healthcare Workers in the Workplace Against Novel H1N1 Influenza A. The main recommendation is that healthcare workers in close contact with pandemic influenza or influenza-like-illness patients should use fit-tested N95 respirators.           Intra-venous zanamivir used successfully in one severely ill patient

          A study published in The Lancet today documents successful use of un-licensed intravenous zanamivir in a 22-year old, severely ill and immune suppressed patient. Full article available at: http://www.thelancet.com/


          Novartis says pilot study suggest single dose of vaccine offers protection

          Vaccine producer Novartis has released a statement on the results of their first (pilot) clinical trial showing 80% protection induced by a single dose of the MF95 adjuvanted pandemic influenza vaccine. http://www.novartis.com/newsroom/media-releases /en/2009/1339223.shtml


          Pandemic influenza strains identified in pigs in Singapore

          Media reports quote Singapore's Agri-Food and Veterinary Authority (AVA) saying that A(H1N1)v has been detected in 12 pigs imported from Indonesia's Bulan island (Pulau Bulan). Four of these pigs have been slaughtered and sold, but assurances were given by the agency that there is no risk for people consuming pork. Protective measures and enhanced surveillance has been put in place in the abattoir ion question.

            Main developments in past 24 hours

            • 46 new cases reported in the EU and EFTA countries, reaching a total of 48 269 cases.


            This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 05 September 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

            Disclaimer:

            Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


            Epidemiologic update

            All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009 a total of 48 269 confirmed cases have been reported (Table 1). In the past 24 hours, 46 new cases were confirmed from the EU and EFTA countries.

            The latest available updates on hospital admissions per Member State are presented in Table 2.

            The latest available updates on new and on confirmed fatal Pandemic (H1N1) cases outside of the EU/EFTA area by country are presented in Table 3.


            Other updates

            The UK government has published updated planning assumptions for the current A(H1N1) Influenza Pandemic.

            The assumptions provide a common agreed basis for planning across all public and private sector organisations. These planning assumptions are based on analysis and modelling of data from both inside and outside the UK. They supersede the previous planning assumptions (dated 16 July). Modifications reflect the latest evidence on the severity of the current A(H1N1) strain. While considerable uncertainties remain, this evidence is sufficient to exclude some of the most severe possibilities included before.

            CDC Guidance on Helping Child Care and Early Childhood Programs Respond to Influenza during the 2009?2010 Influenza Season

            This document provides guidance to help decrease the spread of influenza (flu) among children in early childhood programs and among early childhood providers during the 2009?2010 flu season. The guidance expands upon earlier guidance documents by providing a menu of tools that health officials, Head Start, and other early childhood and child care providers can choose from based on conditions in their area. It recommends actions to take now, during the 2009?2010 flu season, suggests strategies to consider if CDC determines that the flu is becoming more severe, and provides a checklist for decision-making at the local level. Explanations for the recommended strategies and suggestions on how to use them are included in the Technical Report. Based on the severity of 2009 H1N1 flu-related illness thus far, this guidance recommends that children and early childhood providers with flu-like illness remain home until 24 hours after resolution of fever without the use of fever-reducing medications.

                Main developments in past 24 hours

                • Media highlights for last week included;

                This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 07 September 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

                Disclaimer:

                Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


                Media highlights 31 August ? 4 September

                The media last week has stayed with the previous week?s main issue; vaccines. In particular, several papers focused on vaccines and their availability and approval in certain countries. Many papers raised the uncertainty surrounding whether one or two doses would be the normal vaccination strategy, and how this would be communicated to healthcare professionals and the public.

                It was also widely reported that President Obama is urging Americans to get the A(H1N1)v vaccination, with many Americans reported to be anxious around the future development of the virus. Many papers also highlighted that industry and certain public health bodies have faith in the efficacy and safety of vaccines.

                The media have also reported widely on the projected impact of A(H1N1)v, with the southern hemisphere showing signs of a gradual decrease in reported numbers as they moved out of their winter. The UK forecast that infection rates will not be as high as originally thought. In addition, many papers have highlighted how prepared countries are, i.e. what strategies are being implemented, ahead of the likely next wave this autumn.


                Epidemiologic update

                All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009 a total of 48 668 confirmed cases have been reported (Table 1). In the past 24 hours, 399 new cases were reported from the EU and EFTA countries.

                The latest available updates on hospital admissions per Member State are presented in Table 2.

                The latest available updates on new and on confirmed fatal Pandemic (H1N1) cases outside of the EU/EFTA area by country are presented in Table 3.

                  Scientific advance

                  Analysis of first paediatric deaths in US reviewed

                  In the Morbidity and Mortality Weekly Report of 4 September 2009, the United States Centers for Disease Control and Prevention (CDC) presented data from its routine influenza-associated paediatric mortality reporting system from the period of April to August 2009. These results based on the first 36 childhood deaths due to the pandemic indicate the vulnerability of children with certain high risk medical conditions (especially neurodevelopmental disorders) and the importance of bacterial coinfections.

                  Other updates

                  Increasing domestic transmission in China

                  A media report quoting the Chinese minister of health says there has been sudden upsurge of new cases of pandemic influenza last week, with 95% of infections being contracted domestically. The pandemic influenza vaccination campaign will start by vaccinating the attendees of the celebrations of the 60th anniversary of the founding of the People?s Republic of China in Beijing, involving more than 200 000 people.

                    Main developments in past 24 hours

                    This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 09 September 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

                    Disclaimer:

                    Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


                    Public Health Development

                    US ACIP recommendations for seasonal vaccine updated

                    The USA Advisory Committee on Immunisation Practices has updated its annual recommendations on the use of vaccine for the prevention and control of seasonal influenza for the 2009-2010 season. The only significant change this year has been to extend recommending vaccination to all children aged 6 months to 18 years. Approximately 85% of the US population is recommended annual flu vaccination though actual coverage is considerably less. A comment is made concerning the use of seasonal vaccine in Europe this season.

                    Epidemiologic update

                    All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009, a total of 49 543 confirmed cases, including 121 deaths, have been reported (Table 1). In the past 24 hours, 431 new cases were reported from the EU and EFTA countries.

                    The latest available updates on hospital admissions per Member State are presented in Table 2.

                    The latest available updates on new and on confirmed fatal pandemic (H1N1) cases outside of the EU/EFTA area by country are presented in Table 3.


                    Other updates

                    US recommendations for antiviral use updated

                    The US CDC updated the Interim Recommendations for the Use of Antiviral Medications in the Treatment and Prevention of Influenza for the 2009-2010 Season.                     Risk-Benefit analysis of pandemic vaccination published in Sweden

                    The Medical Products Agency of Sweden published a benefit-risk analysis of pandemic influenza vaccination of children and pregnant women with the Pandemrix vaccine. Pandemic influenza vaccine is recommended in Sweden for children 6 months to 3 years of age who fall into risk groups and to all children and adolescents between 3 years and 17 years of age. It is also recommended for all pregnant women. The recommendations are backed by the National Board of Health.

                        Weekly overview of selected non-EU countries

                        Chile (pop 16.6 million)

                        The number of pandemic influenza cases reported weekly to the national level has been decreasing since week 28. Until week 34, the number of confirmed cases was 12 190, out of which 1 504 presented with Severe Acute Respiratory Illness (SARI). The majority of SARI cases were infants (69.1%) or children between 1-4 years of age (15.2%). The majority (63.8%) of the 125 laboratory-confirmed fatal cases had an underlying illness. The majority of influenza strains identified until week 34 were A(H1N1)v and less than 0.5% were identified as seasonal H1 or H3 strains. Very few influenza viruses were identified through surveillance activities during week 34.

                        Brazil (pop. 192 million)

                        Since the beginning of the pandemic until week 34, 36 542 Severe Acute Respiratory Illness (SARI) cases were notified in Brazil, of which 6 592 were subsequently laboratory-confirmed as influenza A (H1N1)v infections. The number of confirmed cases as well as ILI cases appears to have been decreasing for the last two weeks. Of the confirmed SARI cases, 657 (10%) have died. According to the health ministry, this is the highest number for a country in the world. However, Brazil has a large population and the reported mortality due to SARI cases with confirmed influenza A (H1N1)v is currently 0,34/100.000 inhabitants. Out of the 6 592 SARI cases linked to influenza A (H1N1)v, 54.2% presented with at least one of risk factor (including pregnancy), which does not significantly differ from what is observed for seasonal influenza in the country. Among SARI cases in fertile age 23.1% were pregnant, which is larger than the proportion of pregnant women among SARI cases due to seasonal influenza (16.8%). The number of reported cases and consultation rates from sentinel surveillance has been decreasing for the last 4 weeks. Out of 14 687 PCR tests conducted up to week 34, 68,5% tested positive for influenza A (H1N1)v. This proportion has not changed much in the last four weeks, although the number of samples being collected is decreasing rapidly.

                        Australia (pop. 20.7 million)

                        In the national report for September 10th the total number of fatal cases associated with the pandemic influenza (H1N1) 2009 is now 165. There are still 335 cases hospitalised due to (H1N1) 2009 and 64 (19.1%) of them are in Intensive Care Units. The total number of hospitalisations in Australia since the pandemic began is 4 610. All of these figures indicate a decreasing trend for last few weeks. In most but not all of the jurisdictions of Australia the indications from clinical surveillance is that the peak of the first wave has been passed. Twelve percent of the fatal cases and 6.6% of the hospitalised cases are indigenous Australians. According recent censuses, around 2.5% of the Australian population consider themselves indigenous.

                        New Zealand (pop. 4.2 million)

                        The latest report (September 9th) indicates that the circulation of influenza in the community continues to decline. The ILI consultation rate from a sentinel surveillance system has been decreasing since week 29, but is still at a higher level than during the past two years. The number of deaths from Pandemic Influenza (H1N1) 2009 is now 17. These are deaths where swine flu was a primary cause. There are a number of other deaths that are being investigated by the Coroner's office. As of September 9th, only one patient is reported to be in hospital due to influenza (H1N1) 2009 or its complications. The pressure on ICU?s is now so low that national authorities have stopped monitoring patient numbers. In general, schools are reporting usual levels of absence for this time of year. The majority of isolates received through the sentinel surveillance system have been influenza A(H1N1)v from week since week 27.

                        South Africa (pop. 48.6 million)

                        As of September 8th, South Africa has confirmed 7 606 cases of pandemic (H1N1) 09, with 1 735 new cases being confirmed since the previous update. The number of deaths related to the pandemic is 31. It appears that the number of confirmed cases and deaths are still increasing. The recommended national strategy is laboratory confirmation only for moderate or severe cases, however the national authorities believe it is likely that most cases are still being captured by the surveillance system. South Africa has a large HIV-positive population, but thus far no information is available on severity of illness or changes in transmission patterns among this important group.

                          Detection of Influenza A(H1N1)v virus by real-time RT-PCR

                          This article describes the development and validation of a set of real-time RT-PCR assays that can identify the influenza A(H1N1)v strain and discriminate it from other influenza A.

                          The first assay is directed against the influenza A(H1N1)v haemagglutinin gene. The lower limit of detection of the assay was 384 copies of viral RNA per ml of viral transport medium (95% confidence interval: 273-876 RNA copies/ml). Specificity was 100% as assessed on a panel of reference samples including seasonal human influenza A virus H1N1 and H3N2, highly pathogenic avian influenza A virus H5N1 and porcine influenza A virus H1N1, H1N2 and H3N2 samples. The other assay is an adaptation of a WHO-recommended assay for the detection of influenza A in general, targeting the matrix gene and modified to work under the same reaction conditions as the influenza A(H1N1)v virus-specific test. Both assays were equally sensitive. Clinical applicability of both assays was demonstrated by screening of almost 2,000 suspected influenza (H1N1)v specimens, which included samples from the first cases of pandemic H1N1 influenza imported to Germany. Measuring influenza A(H1N1)v virus concentrations in 144 laboratory-confirmed samples yielded a median of 4.6 log RNA copies/ml.

                          Response after one dose of a monovalent influenza A (H1N1) 2009 Vaccine ? preliminary report

                          Preliminary data on immunogenicity and safety after one dose of a monovalent unadjuvanted influenza A (H1N1) 2009 vaccine provided to 240 subjects in a randomized, observer-blind, parallel-group clinical trial conducted in Australia is presented today in New England Journal of Medicine. The vaccine used in the trial was a monovalent, unadjuvanted, inactivated, split-virus vaccine produced by CSL Biotherapies (Parkville, Australia). The seed virus was prepared from the reasortant vaccine virus NYMC X-179A derived from the isolate of influenza A/California/7/2009 (H1N1) virus. 120 subjects were provided 15 μg hemagglutinin and 120 subjects 30 μg hemagglutinin. The immunogenicity end points were the proportion of subjects with antibody titers of 1:40 or more on hemagglutination inhibition assay, the proportion of subjects with either seroconversion or a significant increase in antibody titer, and the factor increase in the geometric mean titer. The safety end-points were reports of local and systemic adverse events using a 7-day diary card. Three weeks post-vaccination 96.7% in the 15-μg group and 93.3% in the 30-μg group had developed an antibody titer of >1:40. Seroconversion or a significant increase in titer on hemagglutination-inhibition assay was observed in 74.2% of subjects with no difference between the two groups. A significant increase was also observed in geometric mean titers, with no difference between the groups. Of interest is that individuals older than 50 years had lower factor increase in the geometric mean titers than individuals younger than 50 years. In addition, as many as 31.7% of the vaccinated individuals had antibody titers of 1:40 at baseline, before vaccination. Solicited local adverse events were reported by 46,3% of subjects with injection-site tenderness in 36.7%of subjects and pain in 21.7% of subjects. Solicited adverse events were reported by 45% of subjects, with headache, malaise and myalgia being most commonly reported. The authors conclude that a robust immune response develop after one dose of a monovalent unadjuvanted influenza A (H1N1) 2009 vaccine. The authors list a number of factors that could have contributed to the observed titers in both age groups at baseline, such as previous 2009 H1N1 infection in the study subjects despite stringent exclusion criteria, cross-reactive antibodies, possibly induced by seasonal flu vaccination. Finally, the authors conclude that the efficacy and effectiveness of this vaccine used in mass vaccination campaigns will come first in studies performed post-licensure.

                          Trial of Influenza A (H1N1) 2009 monovalent MF59-adjuvanted vaccine ? preliminary report

                          Preliminary data on immunogenicity and safety from another randomized, phase 1, clinical trial using a monovalent adjuvanted influenza A(H1N1) 2009 vaccine is also presented in the same issue of New England Journal of Medicine. This study was conducted in the United Kingdom. The vaccine used in the trial was a monovalent (influenza A(H1N1) 2009 vaccine adjuvanted with MF59 using several different doses; 3.75 μg, 7.5 μg and 15 μg of hemagglutinin produced by Novartis Vaccines and Diagnostics (Siena, Italy), . Nonadjuvanted vaccine was provided to control subjects. In total 175 adults, 18 to 50 years of age were included in the study. Results from an interim analysis of the immune responses to the 7.5 μg dose of MF59-adjuvanted vaccine, provided in one or two doses, in sera collected 14 and 21 days post-immunization (data for a total of 100 subjects) are now presented. The immunogenicity end points were the proportion of subjects with antibody titers of 1:40 or more on hemagglutination inhibition assay and 1:10 or more on microneutralization assay. The safety end-points were reports of local and systemic adverse events using a 7-day diary card. The rates of seroconversion after one dose of vaccine by heamagglutination inhibition assay was 76% and by microneutralization assay 92%. After two doses these increased to 88-92% and 92-96%, respectively. Pain at the injection site was noted in 70% and muscle aches in 42% of subjects. Two subjects reported fever with a temperature of 38C or higher after the first dose. The frequency or severity of adverse events did not increase after the second dose. The authors conclude that oil-in-water emulsion adjuvants have short-term reactogenicity equivalent to that of inactivated seasonal vaccines and the monovalent influenza A (H1N1) 2009 MF59-adjuvanted vaccine generates antibody responses likely to be associated with protection within 14 days after a single dose is administered. Their head-to-head evaluation with nonadjuvanted formulations will assess the effect of adding MF59 adjuvant on the speed andmagnitude of antibody responses.

                          Epidemiologic update

                          All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009, a total of 50 470 confirmed cases and 133 deaths have been reported (Table 1). In the past 24 hours, 455 new cases were reported from the EU and EFTA countries.

                          The latest available updates on hospital admissions per Member State are presented in Table 2.

                          The latest available updates on new and on confirmed fatal pandemic (H1N1) cases outside of the EU/EFTA area by country are presented in Table 3.

                            This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 12 September 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

                            Disclaimer:

                            Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


                            Epidemiologic update

                            All 27 EU and 4 EFTA countries are reporting cases of pandemic (H1N1) 2009 influenza. Since April 2009, a total of 50 470 confirmed cases and 135 deaths have been reported (Table 1). In the past 24 hours, no new cases were reported from the EU and EFTA countries. Out of the 135 reported deaths, two new fatal cases are reported from Spain.

                            The latest available updates on hospital admissions per Member State are presented in Table 2.

                            The latest available updates on new and on confirmed fatal pandemic (H1N1) cases outside of the EU/EFTA area by country are presented in Table 3.

                                Media highlights 7 ? 11 September

                                This week much of the media focus has again been around the issue of vaccines. In particular many stories have surrounded the availability, delivery and safety of vaccines in particular countries. Numerous reports have focused on whether vaccines will actually work amid some reports of tests suggesting resistance to vaccines. In addition several reports have again considered whether a single or double dose will be required as no concrete information has been made available.

                                In the US many reports have focused on an earlier flu season than usual amid concern that many people will be hospitalized this winter. Likewise in Europe the media has picked up on several countries where an increase in cases has been observed, notably Spain and Sweden.

                                Reports from the UK however suggest that the government feels it is well placed to deal with any future threats. Many reports have also focused on the preparedness of countries, in particular with questions surrounding the involvement and vaccination of health care providers and how schools and child care facilities will manage during a possible increase in A(H1N1)v numbers.
