EVD transmission is persistent in Guinea, although case incidence is stable or declining in several districts. A total of 74 new confirmed and probable cases were reported nationally during the week to 16 November (figure 1). However, data are missing for one day (15 November). The vast majority of new cases were reported from the south-east of the country, near Liberia?s northern border. Here, the neighbouring districts of Macenta (17 new confirmed and probable cases), N?Zerekore (21 new confirmed and probable cases), and Kerouane (12 new confirmed and probable cases) accounted for 67% of all new cases reported in the country during the past week (figure 4).

In the west of the country, the capital, Conakry, reported a single new confirmed case in the week to 16 November (figure 1). Sustained efforts will be required to maintain this decrease. Transmission in the district of Coyah, adjacent to Conakry, is persistent, with 6 new confirmed and probable cases reported in each of the past 2 weeks. Faranah, in the centre of the country and on the border with the Sierra Leonean district of Koinadugu, reported 4 confirmed and 2 probable cases during the past week. The district of Siguiri, which borders Mali, reported 2 new confirmed cases.

Case incidence continues to decline in the outbreak?s epicentre Gueckedou, which reported no new cases during the week to 16 November. Only 1 new case has been reported from Gueckedou during the past 21 days. It is unclear to what extent this reported decline may be attributable to the opening of new treatment facilities in neighbouring districts, and an associated reduction in the number of patients with EVD who seek care in Gueckedou. Of a total of 34 districts in Guinea, 10 are yet to report a case of EVD.

A total of 148 new confirmed cases were reported nationally during the week to 23 November (figure 1), compared with 81 cases the week before. The south-eastern districts of Macenta (26 new confirmed cases), N?Zerekore (29 new confirmed cases), and Kerouane (8 new confirmed cases) accounted for 43% of all new cases reported in the country during the past week (figure 4). However, the district at the outbreak?s epicentre, Gueckedou, which neighbours Macenta, reported only 2 new confirmed cases in the week to 23 November, and has reported no more than 3 confirmed cases in any one of the past 6 weeks. Similar to the districts of Kenema in Sierra Leone and Lofa in Liberia, Gueckedou, is one of several districts in the outbreak to date to have successfully brought down a very high case incidence to a level low enough to end local chains of transmission.

The neighbouring districts of Coyah (10 confirmed cases), Dubreka (6 confirmed cases), and Kindia (11 confirmed cases) all reported an increase in the number of new reported cases compared with each of the previous 2 weeks.

In the centre of the country, the district of Dabola reported its first confirmed case for 3 weeks, while the neighbouring district of Faranah, on the border with Sierra Leone, reported 16 new confirmed cases in the week to 23 November: more than the combined total of confirmed cases for the previous 6 weeks. The district of Siguiri, which borders Mali, reported 3 new confirmed cases, and has now reported between 1 and 3 cases for each of the past 6 weeks.

Of a total of 34 districts in Guinea, 10 are yet to report a case of EVD.

                          GUINEA
A total of 77 new confirmed cases were reported nationally during the week to 30 November (figure 1), compared
with 148 cases in the week before. The national trend in Guinea since early October has been slightly increasing,
with between 75 and 148 confirmed cases reported in each of the past 6 weeks, though this picture of relative
stability masks important changes in the pattern of EVD transmission within the country. The previous 3 weeks
saw a large number of new cases in the eastern districts of N’Zerekore (6 new confirmed cases in the week to 30
November; 29 cases in the previous week), Macenta (15 new confirmed cases; 26 in the previous week), and
Kankan (7 new confirmed cases; 7 in the previous week). The persistent transmission in Kankan, and the
surrounding areas of Kerouane, Kouroussa and Kissidougo (figure 4), is of particular concern, because the local
populations are likely to seek treatment in the north, and in neighbouring Mali in particular, rather than at
existing facilities in the nearby south-eastern districts of Gueckedou (1 new confirmed cases in the week to 30
November) and Macenta. The first case imported to Mali travelled from a city in the northern district of Siguiri,
which borders Mali, and where there has been persistent transmission since early November (2 new confirmed
cases this week; between 1 and 3 cases for the past 7 weeks). The lack of established EVD treatment and isolation
facilities in this northern, Sahelian zone of the country, combined with a higher than usual degree of resistance
among local communities to safe burial practices, make this area vulnerable to an increase in cases.

In the centre of the country, the district of Faranah, which borders the north Sierra Leonean district of Koinadugu,
has reported an average of 8 cases per week for each of the past 4 weeks. In the west of the country, the capital,
Conakry, reported 14 new confirmed cases in the week to 30 November (figure 1). Together with the
neighbouring district of Coyah (15 new confirmed cases in the week to 30 November), Conakry has now reported
an increase in the number of new cases during each of the past 2 weeks. Although 10 districts are yet to report a
case of EVD, there has been a geographical expansion in transmission: as at 1 October, 9 districts had reported an
infection during the past 7 days; as at 1 December 14 districts had reported an infection during the past 7 days).
