Investigation of Meningitis and Stroke after Spinal Injections

Virginia Department of Health is working with the Centers for Disease Control and Prevention and other states on a multi-state investigation of meningitis and strokes in patients who received spinal injections. While the investigation into the source of the infection is not yet known, all infected patients received preservative-free methylprednisolone acetate among three lots which have been voluntarily recalled by the New England Compounding Center, in Framingham, Massachusetts. This product of concern was distributed to 23 states.

Virginia Department of Health (VDH) is working closely with the only two medical facilities known to have received and used this product from this manufacturer. Both facilities have contacted their patients who received this medication via any route of injection between July 2012 and September 2012. Insight Imaging in Roanoke contacted more than 600 patients and New River Valley Surgery Center in Christiansburg contacted less than 30 patients.

The manufacturer has recalled the implicated lots of the medication and the product is no longer being used.

As of October 4, 2012, CDC is reporting 35 cases from 6 states with a clinical presentation consistent with fungal infection. Tragically, these numbers include 5 deaths. In Virginia, 4 cases, including 1 fatal case, have been identified. Fungus has been identified in specimens obtained from 5 patients thus far. Additional suspect cases are being investigated.

Patients who had spinal injection procedures at these two facilities are being asked if they developed any new symptoms since their spinal injection. Patients are being advised to seek immediate evaluation and let a physician know about this investigation and the concern for possible infection. Symptoms of concern are new or worsening headache, fever, localized weakness, falls, difficulty walking, slurred speech or other symptoms that may suggest a stroke.

"...As of 11 a.m. today, the number of people in Tennessee identified with meningitis, abscess or stroke associated with this outbreak is 29. The number of deaths is unchanged at three.

...Following the CDC/FDA joint press conference yesterday, we issued another alert through the Tennessee Health Alert Network, or T-HAN, advising Tennessee clinicians to NOT USE ANY materials from the New England Compounding Center. While we have identified every facility that has the lots of the methylprednisolone acetate of concern in this investigation, to protect the safety of Tennesseans, we are directing clinicians to not use ANY material from NECC until further notice. We are working on identifying those facilities that received other NECC products.

Without certainty as to their other products? integrity, we believe it is important these products not be used. We know the CDC and FDA are investigating other NECC products and we believe that is an important effort. "



While I'm not suggesting that products from any other company are in question, the following article might explain why clinics in Tennessee seem to have gotten such a large percentage of the contaminated products.

"Corporation records filed with the Massachusetts secretary of state show Barry J. Cadden and Gregory Conigliaro, who head New England Compounding Center, also are listed as the top officials of Ameridose LLC.

Ameridose has a contract with HealthTrust Purchasing Group ? based in Brentwood and partially owned by Nashville-based hospital chain HCA ? to provide member hospitals with sterile compounded drugs and repackaging services.

Similar products were offered by New England Compounding, which voluntarily surrendered its license this week amid state and federal investigations of the meningitis outbreak.

HealthTrust in June said its members included more than 1,400 acute-care hospitals and thousands of ambulatory surgery centers."

According to a report from WLFI, another patient from the OSMC Surgery Center in Elkhart, IN, has tested positive, raising the count in IN to 3 confirmed cases.

Idaho Public Notified of Meningitis Risk: Two Idaho Facilities Received Recalled Epidural Steroid Injections

The Idaho Department of Health and Welfare and Public Health Districts are working closely with the Centers for Disease Control and Prevention and two Idaho medical facilities in a 23-state investigation of non-contagious fungal meningitis in patients who received epidural steroid injections, commonly used to control back pain. The injections have been recalled by the facility that produced them. It is believed the solutions injected may have had fungal contamination.

No illnesses have been reported in Idaho patients who have received the injection. Nationally, 47 people have been diagnosed with infection, with five deaths reported. All infections occurred in seven states located east of the Mississippi River.

The two Idaho facilities that received shipments of the recalled injections are the Walter Knox Memorial Hospital in Emmett and Pain Specialists of Idaho in Idaho Falls. Both facilities were notified by public health officials of the danger and are cooperating with the public health investigation and are working with their doctors to notify patients who may have received an injection with the recalled drugs.

Fungal meningitis is not transmitted from person to person. All infected patients did receive an injection with preservative-free methylprednisolone acetate prepared by New England Compounding Center in Framingham, Mass., but investigators continue to examine all possibilities as the source for the outbreak.

Anyone who has received this type of injection from either of the Idaho facilities since July 1st should seek evaluation from their health care provider immediately if they experience symptoms. Symptoms may include a new or worsening headache, dizziness, fever, nausea, and sensitivity to light. A number of people who became ill also had symptoms of stroke, such as weakness or difficulty with speech.

Patients who received the injections can also contact either of the two facilities at:

Healthcare providers who evaluate symptomatic patients who have had exposure to the recalled product have been asked to contact their local public health district or the Idaho Division of Public Health to report cases.

Ind. reports 3 meningitis cases tied to steroid

Two patients treated at a northern Indiana health clinic have tested positive for fungal meningitis after receiving a recalled back pain medication that's been linked to a deadly nationwide outbreak, the clinic's chief executive said Friday.

OSMC Surgery Center CEO Don Hammond said two of the Elkhart clinic's patients have been hospitalized with the rare illness.

Indiana State Department of Health spokesman Ken Severson said three Indiana patients have contracted the illness after receiving spinal injections, revising an earlier report that had downgraded the number from three to two. He said a case thought to involve an out-of-state patient actually involved an Indiana resident.

Six Indiana health facilities received batches of the medication that was recently recalled by a Massachusetts manufacturer. The other clinics are in Evansville, Fort Wayne, South Bend, Terre Haute and Columbus.

Hammond said about 400 people had received the tainted injections at the clinic, which is about 15 miles east of South Bend.

Any patients known or suspected to have been injected with products in these three lots should be evaluated immediately for infection and subsequently monitored or treated as necessary.

DOH and DBPR strongly endorse FDA?s recommendation that all practitioners, pharmacies and clinics discontinue use of all NECC products until and unless CDC and FDA can make determinations indicating that the products are safe.

Federal health officials have expanded the recall list of potentially contaminated injectable medications suspected in a multi-state meningitis outbreak and the death of five people. More and more cases are being reported, with 47 people having contracted meningitis in seven states.

The Food and Drug Administration released a list of 34 different drugs this morning that included steroids, epidural anesthetics used to block pain during childbirth, and surgery and saline solutions that are compounded to make different medications. All of the products on the recall list were made by the New England Compound Center (NECC) in Framingham, Mass., and are administered by injection into the spine. ..

..The Centers for Disease Control and FDA have asked the clinics, doctors offices and hospitals in 23 states that have received shipments of medications from NECC to stop using them immediately. Anyone who has received epidural injections since July 1 should watch for symptoms of meningitis. Health officials say the most common symptoms are worsening headache, fever, stiff neck, trouble walking or falling and progressing back pain. Many of these patients have suffered strokes.

NASHVILLE, Tenn., Oct 5 (Reuters) - The number of people stricken with a rare form of meningitis linked to steroid injections rose to 50 in seven U.S. states, authorities said on Friday, in a widening outbreak that has killed at least five people.

Michigan said it had confirmed six cases of fungal meningitis, the seventh state to report people falling ill after receiving the injections, mainly for back pain.

Other states with cases are Tennessee, Virginia, Florida, Maryland, North Carolina and Indiana.

The U.S. Centers for Disease Control reported 47 cases of meningitis on Friday, while Michigan reported an additional two cases and North Carolina one not included in the CDC count. That brought the national total to 50, compared with 35 on Thursday...

ATLANTA -- No infections have been reported here yet, but 189 Georgia patients received steroid injections in Macon that may have been contaminated with a deadly fungal meningitis.

The potential scope of the meningitis outbreak that has killed at least five people widened Thursday as health officials warned that hundreds, perhaps thousands, of patients who got steroid back injections in 23 states could be at risk.

Maryland could see new cases of meningitis for a month or more as health officials continue to search for new and past cases of the disease, which has sickened 50 people in several states who received injections of a tainted steroid.

In the cases health officials are already aware of, the incubation period for the infections has been between two days and four weeks, but it's possible the period could be even longer for some, said Dr. Lucy Wilson, chief of Maryland's infection prevention and outbreak response program. New England Compounding Center (NECC) Potentially Contaminated Medication: Fungal Meningitis Outbreak

AUDIENCE: Surgery, Anesthesia, Neurology, Healthcare Professionals

ISSUE: FDA has observed fungal contamination by direct microscopic examination of foreign matter taken from a sealed vial of methylprednisolone acetate collected from New England Compounding Center (NECC). FDA is in the process of conducting additional microbial testing to confirm the exact species of the fungus.

Investigation into the exact source of the outbreak is still ongoing, but the outbreak is associated with a potentially contaminated medication. That product is preservative-free methylprednisolone acetate (80mg/ml), an injectable steroid produced and distributed by New England Compounding Center (NECC) in Framingham, Massachusetts. CDC?s interim data show that all infected patients received injection with this product.

BACKGROUND: FDA was been working closely with CDC, several state health departments, and the Massachusetts Board of Pharmacy to investigate the scope and cause of the outbreak of fungal meningitis. FDA inspectors in the New England District Office, in cooperation with the Massachusetts Board of Registration in Pharmacy have been conducting an inspection of the New England Compounding Center. The firm voluntarily ceased all operations and surrendered its license to the Massachusetts Board of Registration in Pharmacy on October 3, 2012.

RECOMMENDATION: Out of an abundance of caution, FDA is taking the additional step of recommending that health care professionals and consumers not use any product that was produced by NECC at this time. In addition, FDA requests that health care professionals retain and secure all remaining products purchased from NECC until FDA provides further instructions regarding the disposition of these products.

Although the investigation into the source of the outbreak is still ongoing, if you have purchased a product from NECC, FDA is advising not to use it at this time. This includes all products compounded and distributed at NECC; not just the ones that have been recalled. Please see the CDC website for additional information.

Yes, from what I'm reading, that number is correct for, for people in VA who received injections from the suspect lots that were originally recalled. At this time, there's no telling the numbers of people who received doses of the other items on the 2nd, much lengthier recall list.

Two clinics known to have received tainted injections were:

Insight Imaging in Roanoke, Va.
New River Valley Surgery Center in Christiansburg, Va.

More than 600 patients at Insight received the injections, but fewer than 30 patients at the New River Valley facility did."

BOSTON ? October 5, 2012 ? The Massachusetts Department of Public Health (DPH), the U.S. Centers for Disease Control and Prevention (CDC) and Food and Drug Administration (FDA) continue to collaborate on the ongoing investigation into an outbreak of Aspergillus (fungal) meningitis in Florida, Indiana, Maryland, Michigan, North Carolina, Tennessee and Virginia.

There are four products common to all of these cases.

One of the medications, Methylprednisolone Acetate, a steroid, was prepared at the New England Compounding Center, Inc., (NECC) of Framingham, Mass., which is being investigated as a source of the outbreak.

All medications that are similar to Methylprednisolone Acetate have been recalled.

Authorities are advising health care providers to stop use of all products prepared by NECC.

The Massachusetts Department of Public Health, through its Board of Registration in Pharmacy, has state regulatory authority over NECC.

At the request of DPH, NECC has agreed to voluntarily surrender their license to operate until this investigation is complete.

Under the Board?s regulations, Massachusetts pharmacists and pharmacies are only permitted to dispense and compound medication pursuant to a prescription from a registered practitioner for an individual patient.

All matters concerning NECC drug compounding and distribution remain under investigation by federal authorities and DPH at this time.
