Today, Ms Zsuzsanna Jakab, WHO Regional Director for Europe, concluded her two-day visit to Dushanbe, Tajikistan, to support immunization activities launched to stop a polio outbreak that threatens children across the country.

During the first day of her visit, Ms Jakab met the Minister of Health, Dr Salimov; the Deputy Prime Minister, Mrs Kurbanova; and His Excellency Mr Emomali Rakhmon, President of the Republic of Tajikistan, to advocate and support the Government in controlling the spread of the highly infectious poliovirus.

"Tackling the current outbreak and maintaining the polio-free status of the country and the WHO European Region are WHO's and my highest priority," said Ms Jakab, addressing journalists at the conclusion of her meeting with the President. "With no cure for polio, we are calling for the full support of all sectors of society to ensure that every child under 6 years of age receives the polio vaccine during this and other immunization campaigns, until the country is polio free again." The mass media play a crucial role in ensuring parental demand for vaccination, she added.

Ms Jakab commended the Ministry of Health of Tajikistan for its transparent and proactive action in informing WHO/Europe through the WHO Country Office about the outbreak. "The immunization campaign starting today was organized within 10 days of the confirmation of the outbreak as polio related, which is a record time and a model for response," she said. Ms Jakab stressed the importance of political commitment and leadership, as well as collaboration with WHO, the United Nations Children's Fund (UNICEF) and other international partners, to strengthen immunization and surveillance in the country and to mobilize resources. A multidisciplinary team of experts from WHO has been supporting the epidemiological investigations and response activities since 16 April 2010.

On 4 May, Ms Jakab took part with the Minister of Health in launching the national polio immunization campaign. It aims to ensure that, within a month, health workers and volunteers conduct three rounds of immunization of almost 1.1 million children under 6 in health centres and house-to-house visits across the country. The Regional Director and the Minister vaccinated some children at the central polyclinic of Dushanbe, the Tajik capital.

"I was impressed to see so many children queuing for vaccination during my visit this morning," she said at a joint press conference following the launch. "The initial campaign in Dushanbe and surrounding districts that began on 1 May has already reached two thirds of the targeted children, which proves how effective the information distributed about the importance of vaccination and the safety of polio vaccine has been. We need to continue in this direction."

Ms Jakab concluded her visit by briefing the representatives of the international community about the outbreak and the action taken to stop its spread, as well as longer-term work on preparedness and strengthening the health system.

Alert - thank you for posting this data and keeping up to speed with the situation.

Please can I clarify a couple of points with you as you seem well versed in the aetiology of polio.

a) it increasingly appears that there may be a vaccine failure - i.e change of the virus away from the vaccine strain. If this is the case, do we have any indication of how transmissible this variant is / polio normally is, and what rate of spread could be anticipated amongst an immunoligically naive population?

b) what is the transmission method for polio - i.e what is the vector for human to human spread? Aerosol, body fluids or??

b) is this strain unusually pathogenic or within 'normal' parameters for polio?

A vaccine failure would be unheard of, as we have been using the same polio vaccines since the 1950s. It also might not be quite as catastrophic as one might think at first, as there are actually two polio vaccines - the oral one in use in much of the developing world, and an injected vaccine that is used in much of the developed world that has several advantages, but is more expensive. I have been waffling back and forth over the past week as to whether this looks like a vaccine failure; I'm not sure. If this really was a vaccine failure of that nature, I think I would be expecting more polio in Uttar Pradesh itself; Shiloh has posted an article indicating that UP has gone 7+ weeks without a single AFP case.

Polio is mainly spread through fecal-oral transmission - an infected person sheds the virus in their stool. It isn't really airborne. Nevertheless, we really can't quarantine out a vaccine failure in the way that we eliminated SARS, say, because for every paralytic case of polio, there are 50-1000 non-paralytic cases that could easily be mistaken for other gastrointestinal illness, or flu, or even be asymptomatic. The fact that the 178 or so AFP cases are spread over 24+ provinces does seem to indicate, IMO, that the non-paralytic cases are probably out there.

That's what I've learned from researching this so far. I hope this helps.

On May 2, the Federal Service for Supervision of Consumer Rights Protection and Human Welfare banned the import of Tajik dried fruit and nuts because of an outbreak of polio in southern Tajikistan:




Considering active trade relations, there is a risk of importing ?wild? poliovirus to the territory of our country through contaminated food products. Considering that, a particular danger is represented by dried fruits and nuts, used directly in food without processing.



The ban "applies not only to firms and companies that import Tajik dried fruits into Russia, but also to individual who carry the goods in their baggage and hand luggage," Ferghana.ru reports, noting the importance of the income to Tajik farmers.



Russian customs officials at land crossings and airports have been instructed to prevent import of the products.


Polio is transmitted via oral contact with stools and saliva.

The deputy minister of health of Uzbekistan denied any cases of
poliomyelitis in the country that were reported previously. He said that
since the beginning of this year [2010] there had been 23 AFP [acute
flaccid paralysis] cases in the country but none of them were linked to
polio after laboratory testing. He underlined that the local WHO office did
not disseminate information about polio cases in Uzbekistan. Concerning the
AFP cases, he said, all were etiologically different entities. He added
that WHO experts, who came to Uzbekistan recently, appreciated the
infectious diseases prevention activities in the country.

An outbreak of AFP caused by wild type polio was registered in the
Tajikistan territory neighboring with Uzbekistan. 12 out of 170 cases died.
There were several tens of confirmed polio cases [according to the most
recent WHO report, there were 32 cases confirmed (see Poliomyelitis -
worldwide (06): Tajikistan, WHO 20100502.1423]. Previously WHO reported
that there were several AFP cases in the territory of Uzbekistan, but it
added that the laboratory confirmation was not achieved.

[The newswire above theoretically provides the laboratory results on the
previously mentioned 3 cases of AFP identified in Uzbekistan that the media
had reported at the same time as reporting the outbreak of wild poliovirus
type 1 in Tajikistan (see Poliomyelitis - Worldwide (04): Tajikistan,

identification of polio cases. There are many causes of AFP, one of which
is infection with a poliovirus. An indicator of good AFP surveillance in a
country is a non-polio annual AFP rate for children less than 15 years of
age greater than or equal to one per 100 000 population. In 2008, the
non-polio AFP rate for Uzbekistan was reported as 1.2 per 100 000
