Re: Suspected Bird Flu Outbreak in North Sumatra (Air Batu, & Environs) Indonesia

Is there any epdimiological data that anyone is aware of that covers cluster incidences of Dengue and DHF i.e data from which we can compare incidences in groups (i.e same street or same family) of these cases against historic data? It would be useful to assess if this is 'unusual', or within the bounds of what would normally be expected.

Polio is not the only disease associated with acute flaccid paralysis (AFP):

Enterovirus 71 has been associated with AFP, although apparently AFP is more likely to be unilateral, enterovirus infection may also be associated with rash "Results" of the "beginning diagnosis, casualties suspect the fever chikungu him because of being found red spots." see:

AJNR Am J Neuroradiol 22:200–205, January 2001
Acute Flaccid Paralysis in Infants and Young Children
with Enterovirus 71 Infection: MR Imaging Findings and
Clinical Correlates

Cheng-Yu Chen, Ying-Chao Chang, Chao-Ching Huang, Chun-Chung Lui, Kwo-Wei Lee, and Song-Chei Huang

BACKGROUND AND PURPOSE: Enterovirus 71 (EV71) infection is now considered an important cause of childhood acute flaccid paralysis. The purpose of our study was to determine whether EV71-infection–related acute flaccid paralysis in infants and young children has characteristic
MR imaging patterns.
METHODS: Seven infants and young children with acute paralysis of the upper or lower extremities and positive EV71 cultures underwent spinal MR studies during an outbreak of hand-foot-and-mouth disease in Taiwan in 1998.
RESULTS: Acute paralysis was observed in one upper extremity in two patients, in one lower extremity in three patients, and in both lower extremities in two patients. None of the patients had sensory impairment or bulbar palsy. MR studies showed unilateral or bilateral hyperintense
lesions in the anterior horn regions of the cord on T2-weighted images in six patients. No abnormal signal was present in one patient. Two of three patients who received intravenous injections of contrast material had ventral root enhancement on T1-weighted images. One of them also had enhancement of the unilateral anterior horn cells. At clinical follow-up, both
patients with bilateral anterior horn abnormalities had residual motor weakness, whereas only one of the five patients with unilateral involvement had residual weakness.
CONCLUSION: EV71 radiculomyelitis tends to be unilateral and to specifically involve both the anterior horn cells of the cord and the ventral roots. MR imaging allows early detection of spinal cord and root lesions.

Guillain Barre Syndrome, another cause of AFP, may occur after a generalized inflammatory event such as a respiratory or gastrointestinal infection.

Indonesia rules out H5N1 case cluster in Sumatra

Lisa Schnirring Staff Writer

Aug 13, 2008 (CIDRAP News) – In an online statement posted yesterday, the Indonesian government said 12 villagers from North Sumatra who were hospitalized for suspected avian influenza symptoms had tested negative for the disease, dampening speculation about a possible case cluster.
The statement from the health ministry's avian flu committee, known as KOMNAS FBPI, was dated Aug 9, but appeared on the group's Web site yesterday. It said 12 villagers from Air Batu village tested negative for the H5N1 virus. All were being treated at Kisaran Hospital, except for a 7-year-old girl and an 8-year-old boy who were at Adam Malik Hospital in Medan, the provincial capital, where they were reported to be in stable condition.
The ministry's statement did not mention three deaths from suspicious symptoms that have been mentioned along with accounts of up to 13 sick patients in recent media reports. Yesterday, however, Chandra Syafei, an official from North Sumatra's health office, acknowledged that three people had died and said his office was on "extraordinary occurrence" alert status, according to a Jakarta Post report.
"The three people died following the discovery of dead poultry, but we don't know whether or not it [avian influenza] was the cause," Chandra told the Post. He said his office had not received autopsy reports from the health ministry.
The suspected cases were identified by provincial and local health officials between Aug 5 and 7, and the patients were given oseltamivir when they were hospitalized, the health ministry's statement said. Also, it said the ministry was conducting surveillance in the area for more patients with suspicious symptoms.
The ministry's message, its first since Jun 19, said the tests were conducted by its Health Research and Development Center. Official news out of Indonesia about H5N1 developments has been scarce since early June, when Health Minister Siti Fadilah Supari said the country would stop reporting human cases as they occur and instead announce them at longer intervals, perhaps as long as 6 months.
Syafei said several health workers, including a team from the World Health Organization (WHO), were in Asahan to investigate the illnesses and monitor developments, according to the Post report.
He said animal health officials had destroyed 276 infected birds, and an official from nearby Labuhan Ratu regency said local veterinary workers culled 1,126 birds after hundreds of recent poultry deaths, the Post reported.
The health ministry statement said tests conducted by North Sumatra's livestock office on 15 poultry that died suddenly on Jul 28 were positive for avian influenza. (The statement did not specify the subtype.)
Meanwhile, flu blogs and message boards have been following developments in what some fear could represent an H5N1 case cluster, an event that suggests the possibility of human-to-human transmission and an increased risk of a pandemic. For example, members of Flu Trackers and Flu Wiki have been translating foreign-language media reports for clues and developments, though the translations are often difficult to interpret and can be unreliable.
ProMED-mail, the Internet based reporting system of the International Society for Infectious Diseases, has also been posting media reports on the suspected case cluster and has appealed for more information on the cases and deaths. On Aug 10, after noting conflicting reports about the cases, ProMED said it would no longer report suspected avian flu cases in Indonesia until they have been confirmed by the nation's health ministry. But a protest from a reader prompted the service to say yesterday it would reconsider the decision. :tiphat: http://www.cidrap.umn.edu/cidrap/content/influenza/avianflu/news/aug1308indonesia.html

Indonesia rules out H5N1 case cluster in Sumatra

The guardian of Doctor's City, the Health was ignored
Wednesday, 13 August 2008

The case of dengue fever dengue fever (DBD) that spread in the Tanjungbalai City until claiming four fatalities and infected 21 other was gazed at strangely. Because, although the guardian of his city a person dokter, the problem of the health was still being ignored in the area. The member the Commission of B DPRD North Sumatra Abdul Justice Siagian said, the DBD incident in Tanjungbalai should be able to not happen. That was very startling for the North Sumatran community because the Guardian of the Tanjungbalai Sutrisno Hadi City was a person dokter. The incident that result in fell casualties totalling 25 people caused the impression that the problem of the health in this city did not receive attention.

??Ini pulled at us. Indeed the problem of the health in Tanjungbalai did not yet receive attention from the local government. Necessarily, this incident could be anticipated because of the guardian of his city of a doctor. It is best to, the related agency descended immediately overcame the problem of DBD ini,? flat Justice from the Area of the Election (Dapil) Tanjungbalai. He asked for the government of the city (pemko) immediately handled the patient DBD seriously. If being felt was needed by the handling was more again maximal, this patient could be reconciled to the central Public Hospital (RSUP) H Adam Malik,Medan. The impression was not serious him Pemko Tanjungbalai handled this problem was seen from the report lack officially on the DBD case to the Kesehatan Service (Dinkes) North Sumatra. In fact, at this time this illness was spreading and alarming the resident.

Moreover, already dozens of residents became casualties and four including dying. ??Up to currently Dinkes North Sumatra does not yet receive the report on the DBD patient from the Health Service Tanjungbalai. Ha the telephone and short message service (the SMS) ,? flat the Head of the Prevention Sub- Service, the Eradication of the Illness, and environmental Sanitation (P2PL) the Health Service of the North Sumatran Province of Surya Dharma yesterday. From the oral report and the short message itu, diketahui had 25 residents of the Subdistrict Datuk the Port that underwent the maintenance in the Public Hospital (RSU) Dr HT Mansyur,Kota Tanjungbalai. He also received the news about the death of four casualties. The four of them died on Friday (1/8) and last Thursday (7/8). ??Keempat the resident who died that because already dengue fever shock syndrome (DSS) that was the end stage of DBD. So, the condition the four of them has experienced shock when being brought to RS,?papar him.

According to him, Dinkes local still collect data on and examined the other patient who underwent the maintenance in RSU Dr HT Mansyur.Tim from the Department of the Health (the Department of Health) also will investigate this DBD origin. ??Sekarang this Dinkes Tanjungbalai still investigated from where his source (the DBD spreading).

The Department of Health then tomorrow (today) would there (Tanjungbalai) to help investigation epidomologis. Jadi, dari this investigation, we will know this DBD source so that his link could cut off,?papar him. The sun added, related 21 residents of the Subdistrict Datuk Bandar, Kota Tanjungbalai, that became casualties of the mosquitoes sting aides aegypti that, the number of DBD patients in the Kerang City increased every month. ??Every month, we received the report had DBD casualties in Tanjungbalai totalling five casualties, but now the amount is 25 was accompanied by the death resulting from the illness ini,? he said. He menambahkan, hal this also was caused the Tanjungbalai City that was classified as the small city with population density so as with the existence of this DBD plague would fast attacked the community.

Increased again, the change in the weather during ini. Namun, this was not free from the Dinkes role, the awareness of the community to maintain the health, the cleanliness himself, and the environment also really must be paid attention to. ??Dinkes has undertaken the program to the community, as giving counselling. DSS described above is dengue hemorhhagic fever, which is commonly confused with H5N1 because both lead to bleeding.

Pemko Tanjung Balai Dinilai Lamban Tangani Wabah DBD

Tanjug Balai, (the Analysis)
<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" /><o:p></o:p>
Pemko Tanjung Balai was it was thought slow in handling the case of Dangue Dengue Fever (DBD), until causing casualties to reach one hundred souls with the spreading period during eight months. This was in accordance with the data received in RSU from January to August 2008 fell casualties 95 people with the mortality rate reached 6 people.
<o:p></o:p>
?Waktu that was quite long for 8 months and the number almost one hundred have been enough to prove, Pemko Tanjung Balai was not serious and slow in responding to this DBD case and this clear caused the community's doubt towards the government capacity in protecting, protecting and pacifying the resident kota,? revealed DPK Indonesian Justice and Unity Party Chairman (PKPI) Tangjung Balai Suresh Kumar St, MSi when being met in his office, on Wednesday (13/8)
<o:p></o:p>
According to Suresh, very complete government equipment and the fund that were available in each related service apparently did not give the benefit to the community. Moreover, very ironic when this plague attacked cruelly the machinery only could use the excuse of not yet having the budget for his control. This became the main cause why this plague as being difficult to be stopped now casualties that all of them the resident could not continue to improve.
<o:p></o:p>
?Kami saw this DBD control must be begun from the return of the task of the subject and the function (Tupoksi) all the related agency and empowered the existence of the head of the environment to carry out the clean movement of the environment with the resident as well as check the nest of the swiftlet nest that mushroomed in the core kota,? he said.
<o:p></o:p>
Already eight DBD plague months attacked the Balai Cape, but the action of the government machinery was still being waiting for the ball or was waiting for the existence of the resident who reported to be sick. However the action konkrit to menhentikan him was even tended to point at each other whoever was more authorised.
<o:p></o:p>
?Bila the activity on Saturday clean considered to be able to not ask the resident to clean the environment, all that because that was present the officials in the future kepling that knew his resident in order to descend from the door to the door cleaned his environment and when being kepling that did not want to descend better be replaced, because of them has mendaptkan the stipend from pemko and was appointed to carry out the direct service to warga,? he said.
<o:p></o:p>
Moreover, said Suresh the health service to casualties DBD must be carried out in a first-rate manner. When RSU Tengku Mansyur could not accommodate the number of casualties DBD, already during him the health service prepared the alert command post command post all over Puskesman and the auxiliary Community Health Centre in the Subdistrict. ?Keterbatasan that was had by RSU could not be made the reason for making the patient DBD must jostle in the room of the maintenance, because still many community health centres in each subdistrict that more close to the resident but was needed by the increase in the number of medical staff who was ready alert in the postal post itu? he said.
