Ebola Update July 7, 2014

For the 6th July 2014, a total of 15 samples were received by the Laboratory: Kailahun (7), Kenema (7) and Bo (1).

Six (6) of the 15 samples were laboratory confirmed positive for Ebola, five (5) from Kailahun and one (1) from Bo. Two of the remaining samples are negative while results for 7 samples are pending.

The 2 laboratory pending results yesterday is still under processing in the laboratory

The cumulative number of Laboratory confirmed cases for Ebola is 275 whiles confirmed death is 88

Ninety-two(92) cases are currently admitted at the treatment centers, 56 in Kenema and 36 in Kailahun
The cumulative number of discharged cases is 34

Line listing of contacts and monitoring of case contacts is on-going in districts with confirmed cases

African Minerals Mining Company donated twenty-one million, five hundred thousand Leones (21,500,000) to the Ministry of Health and Sanitation to assist with the fight against Ebola disease in the country. The fund is to target interventions in schools across the country. Medical Research Center (MRC) loaned an ambulance to the Ministry of Health and Sanitation to assist with the fight against the Ebola Viral Disease in Kailahun district

MOHS Trains Ebola fight ahead of the Lakka Treatment Center operations



Freetown, July 3, 014 (SLENA) - Orientation for the training of another batch of health workers ahead of the Lakka Ebola Treatment Center in the Western Area ended on Thursday July 3,2014 at the Ministry of Health and Sanitation Conference Hall with 67 personnel committed to the task.

Trainees include Community Health Officers, Mortuary staff, State Enrolled Community Health Nurses, Burial Experts and Ambulance Drivers.


Spare heading the training the World Health Organization Expert, Head of Virology and Emerging Infections Department Associate Professor, Tulane School of Public Health and Tropical Medicine, USA, Dr. Daniel Bausch said the training is in two phases, one in Kenema and the other in Freetown effective Friday July 4, 2014.

Dr. Bausch who has a wide range of experience on the disease also helped in setting up the Lassa Fever Unit in Kenema. The training, he reiterated would help equip trainees to professionally handle case management in various health related clinical and public health concerns.

He lauded the government for its timely intervention, and encouraged citizens to dispel all false and negative rumors surrounding the Ebola outbreak.

Director of Environmental Health and Sanitation, Dr. Ansumana Sillah, and Training Facilitator and Manager, National TB/Leprosy Control Programme, Dr. Alie Wurie, underscored the importance of the training and encouraged participants to demonstrate patriotism and commitment in the discharge of their specific duties.

Dr. Wurie and Sillah described the event as a national call for action by all Sierra Leoneans in the fight against the spread of the Ebola viral disease.

Disease update

New cases and deaths attributable to Ebola virus disease (EVD) continue to be reported by the Ministries of Health in the three West African countries of Guinea, Liberia, and Sierra Leone. Between 3 and 6 July 2014, 50 new cases of EVD, including 25 deaths, were reported from the three countries as follows: Guinea, 0 new cases and 2 deaths; Liberia, 16 new cases with 9 deaths; and Sierra Leone 34 new cases and 14 deaths. These numbers include laboratory-confirmed, probable, and suspect cases and deaths of EVD.

As of 6 July 2014, the cumulative number of cases attributed to EVD in the three countries stands at 844, including 518 deaths. The distribution and classification of the cases are as follows: Guinea, 408 cases (294 confirmed, 96 probable, and 18 suspected) and 307 deaths (195 confirmed, 96 probable, and 16 suspected); Liberia, 131 cases (63 confirmed, 30 probable, and 38 suspected) and 84 deaths (41 confirmed, 28 probable, and 15 suspected); and Sierra Leone, 305 cases (269 confirmed, 34 probable, and 2 suspected) and 127 deaths (114 confirmed, 11 probable, and 2 suspected).

•For the 8th July 2014, a total of 38 samples were received by the Laboratory: Thirty (30) from Kailahun and eight (8) from Kenema. Seventeen (17) of the 38 samples have been confirmed positive for Ebola, 14 from Kailahun and 3 from Kenema. Nineteen (19) of the remaining samples are negative while 2 Laboratory results are pending.

17 new cases in a day might suggest otherwise. At least the mortality rate is staying relatively low - I saw a report which indicated that MSF clinics had a 40% mortality rate from 200 cases, which is a drastic improvement on the usual situation with Ebola Zaire.

Seven health workers infected with Ebola at the hospital have died, including Kamara?s sister, also a hospital staff member.

Health workers, frequently in close contact with the infectious virus, have been particularly badly hit. They work long hours. Some are volunteers.

Recently, those working in the Ebola ward went on strike in a protest over pay: they say the 100,000 leone (US$23) per month bonus they were offered is inadequate in view of the risks they face. The government later said the bonus will be paid per week rather than monthly.

Ebola outbreak closes United Methodist health center

The United Methodist Health Center in Manjama, near Bo City, was quarantined July 4 following the deaths of two Ebola-infected children who were found at the center.

"The health center will remain closed until the period of quarantining is over and until the District Health Management Team gives clearance before we can resume our normal operations," said Beatrice Gbanga, United Methodist health coordinator for the Sierra Leone Annual Conference.

The two children reportedly were brought to the center from the Kailahun District by their grandmother after their parents died of Ebola. The Kailahun District has recorded the highest deaths since the disease entered Sierra Leone in late May.

One of the children, only known as Fatmata, died on the evening of June 30. The children were discovered by Sierra Leone Ministry of Health and Sanitation officials following a tip based on the severity of their conditions.

Blood samples taken from nine members of staff and relatives have been confirmed negative, Gbanga said.

Ebola surveillance officers are looking in the community for anyone who had contact with the infected children, including anyone who has shared a handshake, exposure to fluids from an infected person, direct bodily contact such as sleep with the infected person or other bodily contact with an infected person either dead or alive.

"Considering all I have outlined, about 22 people had contact with the children while they stayed at the Manjama Health Center," Saffa said.

"The only way the United Methodist Church can help us is to liaise with us to continue to monitor the cases. The problem we have with the disease is that people often hide their relatives when they become infected. That is how these children were stolen from Kailahun when their parents died of Ebola," Saffa said.

"We are very concerned about this unfortunate situation in our health facility and the Manjama Community and will do our level best to minimize further infection of people," Gbanga said.

"I am afraid that we are very likely to have more cases here in the Bo area following this development,? Saffa said, "because once one is in regular contact with an Ebola patient for over one week, there is a high tendency that one would catch the disease."

Beyond medical care, control of the epidemic requires the deployment of a large number of teams for the training of health personnel measures of infection control for monitoring and case finding for the establishment of a network for the epidemiological surveillance and to promote public health messages.

Due to limited human resources, MSF focused on the care of patients and community awareness with nearly 150 national and international staff mobilized in Sierra Leone.

MSF teams are afraid of hidden patients

Ebola Update July 11, 2014

? For the 10th July 2014, a total of 14 samples were received by the Laboratory: Kailahun (5), Kenema (8) and Western Area (1). Seven (7) of the 14 samples have been confirmed positive for Ebola, 5 from Kailahun, 1 from Kenema and 1from Western Area. The remaining 7 samples are all negative.
? Three of the 4 samples that were under processing yesterday are now Ebola positive while 1 is negative
? The cumulative number of Laboratory confirmed cases are 315 and 99 confirmed deaths
? Seventy-six(76) cases are currently admitted at the two Ebola treatment centers, 48 in Kenema and 28 Kailahun
? The cumulative number of discharged cases is 51
? NB: The Western Area confirmed case came from Kenema to seek medical care in Freetown

• For the 11th July 2014, a total of 23 samples were received by the Laboratory: Kailahun (9), Kenema (11), Bo (1) and Western Area (2). Ten (10) of the 23 samples are Laboratory confirmed positive for Ebola from Kailahun (7) and Kenema (3). Six of the remaining samples are negative while 7 results are pending.

• The cumulative number of Laboratory confirmed cases are 325 and cumulative number of confirmed deaths is 100

• The cumulative number of discharged cases is 55

Ebola Spreads to Sierra Leone Capital of Freetown as Deaths Rise

The worst outbreak of Ebola moved to Sierra Leone?s capital of Freetown where an Egyptian was found with the city?s first confirmed case of the disease.

The unidentified Egyptian national had traveled from Kenema, the largest city in the nation?s Eastern Province, and checked into a clinic east of Freetown, Sidie Yahya Tunis, director of Information, Communication and Technology at the Ministry of Health and Sanitation, said by phone today.

The person was moved back to the Ebola center in Kenema, he said.

?The Ebola disease usually spreads to other places when suspected or confirmed cases in one community move to another, they abandon treatment centers to stay with relatives or they seek treatment outside the Ebola centers,? Tunis said.

• For the 12th July 2014, a total of 36 samples were received by the Laboratory: Kailahun (24), Kenema (6), Bo (2), Port Loko (2) and Western Area (2).

• Fourteen (14) of the 36 samples were Laboratory confirmed positive for Ebola. 8 from Kailahun, 3from Kenema;1 from Bo;1from Port Loko and 1from Western Area

• Fourteen of the remaining samples are negative while 8 results are pending.

• Four of the 7 Laboratory pending results yesterday are now negative while 3 are still under processing in the Laboratory

• The cumulative number of Laboratory confirmed cases are 339 and confirmed death is 106

• Eighty-one(81) patients are currently admitted at the Kenema and Kailahun Isolation Units

Sorry, my original post skipped the first update. So that's at least one secondary case from the region of the capital (Western Area) Unusually, they do not specify the location of the positive cases in the second update. Could more be in the capital?

Ebola virus disease, West Africa ? update 14 July 2014

Epidemiology and surveillance

The World Health Organization (WHO) continues to closely monitor the evolving Ebola virus disease (EVD) outbreak in Guinea, Liberia, and Sierra Leone. The current epidemic trend in Sierra Leone and Liberia remains serious, with high numbers of new cases and deaths being reported. Between 8 ? 12 July 2014, 79 new cases, and 65 deaths were reported from Sierra Leone, and Liberia.

In Sierra Leone, 49 new cases and 52 deaths have been reported, while in Liberia, 30 new cases and 13 deaths were reported. These include suspect, probable and laboratory confirmed cases. This trend indicates that a high level of transmission of the Ebola Virus continues to take place in the community.

The epidemic situation in Guinea is being closely observed, with 6 new cases and 3 deaths reported between 8 ? 12 July 2014. The respective Ministries of Health are working with WHO and partners to step up outbreak containment measures.

Disease update

New cases and deaths attributable to EVD continue to be reported by the Ministries of Health in the three West African countries of Guinea, Liberia, and Sierra Leone. Between 8 ? 12 July, 2014, 85 new cases of EVD, including 68 new deaths, were reported from the three countries as follows: Guinea, 6 new cases and 3 deaths; Liberia, 30 new cases with 13 deaths; and Sierra Leone 49 new cases and 52 deaths. These numbers include laboratory-confirmed, probable, and suspect cases and deaths of EVD.

As of 12 July 2014, the cumulative number of cases attributed to EVD in the three countries stands at 964, including 603 deaths. The distribution and classification of the cases are as follows: Guinea, 406 cases (297 confirmed, 92 probable, and 17 suspected) and 304 deaths (198 confirmed, 92 probable, and 14 suspected); Liberia, 172 cases (70 confirmed, 41 probable, and 61 suspected) and 105 deaths (48 confirmed, 33 probable, and 24 suspected); and Sierra Leone, 386 cases (339 confirmed, 37 probable, and 10 suspected) and 194 deaths (151 confirmed, 38 probable, and 5 suspected).

? Sixty (60) patients have to date survived the disease, discharged and now back with their families in their various communities. These sixty survivors reported to health facilities early for treatment when they started exhibiting signs and symptoms of Ebola

Health Ministry Expresses Concern over the Unethical and Illegal Treatment of suspects of Ebola at home and Pharmacies by some Health Workers

Freetown, Jul. 10, 014 (MOHS) - It has come to the notice of the Ministry of Health and Sanitation that some health workers and Pharmacists are unethically and illegally treating patients at home and their clinics or pharmacies.
Considering the spate at which some nurses and Pharmacists admits and treat potential suspects from Ebola Treatment Centres and Observation Units at home, clinics and pharmacies, the Ministry would like the public to know that it is a criminal offence, and an action that puts the entire country at risk.

The Ministry also wishes to re-emphasize that harboring a suspected or positive patient with the Ebola Viral Disease is a grave offence under the Public Health Ordinance Act of the Laws of Sierra Leone liable to both fines and imprisonment.
The Ministry wish the general public to know that any person who obstructs or interferes with the performance of the medical team in any of the Ebola Observation and Treatment Centres would be guilty of an offence and liable to punishment by Section 34-37 of the Public Health Ordinance Act of Sierra Leone No. 23 of 1960 under Part IV of Prevention, Notification and Treatment of disease with specific reference to the duty of head of family to give notice of certain diseases and notification to Health Authorities.

It is a serious crime that some persons infected with Ebola are being shielded by their relatives which creates a potential risk of the Ebola disease among family members and the entire nation.

In light of the prevailing situation, members of the public are advised to report any suspected Ebola case to the appropriate authority and nearest health facility for timely intervention.

The Ministry of Health and Sanitation also wishes to remind the general public that all deaths must be certified by the appropriate Health Authority before burial, and wish to re-emphasize that the treatment for Ebola is FREE in all Government health facilities.

I was expecting the Ebola epidemic to be quite gruesome and unlike anything I had seen before. But I really didn't expect its magnitude ? this outbreak is enormous. In Sierra Leone, it killed off a lot of health workers before MSF even arrived. Not surprisingly, medical staff were reluctant to work with us at first. They'd never come across Ebola before ? but at least they had previous experience of people suffering and people dying. But for the non-medical staff, like the hygienists ? our hospital cleaners ? it's been a new and disturbing experience, and a large part of my work involves helping them with counselling and support.

They are the ones who mop up the vomit, the stools and the blood. And when there's a death, they are the ones who retrieve the body from the isolation ward, put it in the morgue and disinfect it. In the final stages of the disease, the viral load increases massively, which means the dead bodies are extremely contagious and very dangerous to deal with.

Dealing with the dead bodies is disturbing: the hygienists experience feelings of sadness and fear, but also disgust. When Ebola patients die, there's bleeding, vomiting and diarrhoea. The cleaners tell me they have flashbacks of the things they have seen and of things they have smelled. Even wearing a mask, you can't shut out all the smells.

Traditionally, in Sierra Leone, dead bodies are taken care of by the tribal elders. A lot of the hygienists feel they are too young to be dealing with the dead, so worry they're being disrespectful of their culture's traditions. We make sure that on each shift there is always one man and one woman, so that when someone dies, there will always be someone of the same sex to tend to them.

Feeling The Heat, Burning The Suits: Reporting On Ebola From Sierra Leone

NPR's Jason Beaubien is in Sierra Leone, covering the Ebola outbreak that began in March in Guinea and has spread to neighboring countries. When we spoke Thursday, he had just toured the treatment center built by Doctors Without Borders in the town of Kailahun. With 64 beds, it's the largest Ebola isolation ward ever built. Currently there are 31 patients.

Can you describe the treatment center?

It's basically a compound with a series of different tents. There are tents where people get suited up to go in. Another tent seems to be for storage, and one of the tents contains a lab. Then there's a double fence about 3 1/2 feet high, made of orange plastic mesh. They designed the fence so people can see where the patients are, so it wouldn't seem as if the patients are completely walled off.

Why a double fence?

So no one can get within 6 feet of someone who has Ebola. In case a patient from the isolation area reaches out or vomits, [Doctors Without Borders] wants to make sure there won't be any accidental contamination.

How do the doctors record information on the patients?

Doctors go into the isolation area completely suited up, do their rounds and write down what's happening with patients. Then they stand next to the fence and shout out to people on the other side of the fence [information about each patient]. Say, for patient 105, the doctor says, "diarrhea, vomiting." Then the doctor's notes [made inside the isolation area] are burned.

Where do they burn the notes?

They have a big pit in the back.

What else do they burn?

They burn everything. They say nothing comes out of isolation ? although obviously they're taking blood samples out. People come out. They strip off their protective gear, the Tyvek suits they put over their entire body and shoes.

When they get suited up, you do not see a bit of skin as they're going in. There are multiple layers of gloves, goggles, a mask. As they're coming out all that stuff gets stripped off. Some things like rubber aprons and boots are disinfected and reused. But they burn the whole suit. A suit costs about 60 euros [about $80], and they use 70 or 80 suits a day.
