The Russian Federation has reported one case this week, bringing the total number of cases to 12 WPV1. However, only five of these cases are believed to be local transmission. The balance of these cases could be attributed to neighbouring polio-infected countries following further investigation.
The cases in Chechnya and Dagestan represent a significant risk of further international spread. Countries of the Caucasus are encouraged to ensure all children under five are fully immunized against poliovirus.
Russia has planned SIAs in the North Caucasian Federal District (using tOPV) in September and October. The country will hold two SNIDs in October in the Southern Federal District and Volga, Northwest, Central, Ural, Siberian and Far East Federal districts, using tOPV.

Kazakhstan has recorded its first case of polio since 1995 - a wild poliovirus type 1 (WPV1) case close to the border with Uzbekistan. While Uzbekistan is yet to confirm any cases, the risk of transmission in Uzbekistan must be considered to be high.
The Republic of Dagestan has recorded its second case - the 13th case in the Russian Federation overall. The Dagestan and Chechnya cases constitute a threat of further international spread in the Caucasus.

Kazakhstan has reported its first case since 1995 this week: a seven-year-old boy living in Saryagach, South Kazakhstan - close to the border with Uzbekistan - suffered onset of paralysis from WPV1 on 12 August. This case has already been covered by one SIA, with 1,668,727 children immunized with tOPV from 6-10 September. An emergency response plan to the case is currently being developed.
The Republic of Dagestan has recorded its second case, representing the 13th case in the Russian Federation. This is Russia's most recent case, with onset of paralysis on 18 September. The Russian Federation has conducted sub-national catch-up immunization activities delivering OPV in 42 of 83 territories. In addition, all children under 16 in the North Caucasus and Southern Federal Districts will be immunized from 1-5 November and 29 November-3 December with tOPV.
Although Uzbekistan has not reported any wild poliovirus cases, stool samples from acute flaccid paralysis (AFP) cases are not currently being tested in a WHO-accredited laboratory, and the fact that both Turkmenistan and now Kazakhstan have reported cases close to the border with Uzbekistan means that ongoing wild poliovirus transmission in Uzbekistan can not be ruled out. Uzbekistan has conducted three national SIAs targeting all children under five and a mop-up activity targeting all people under 26 with mOPV1 (with reported high nation-wide coverage confirmed by independent monitoring). An SIA is planned from 25-31 October, targeting all children under 15 with mOPV1, and a surveillance review will take place from 1-5 November.
In Tajikistan, the total number of cases (all WPV1) remains 458. The most recent case had onset of paralysis on 4 July. As of 07 October, Tajikistan has conducted five national rounds of SIAs (two in children <6 years of age and three in children aged <16 years of age), with high coverage verified by independent monitoring and reflected in the sharp reduction in case numbers. A mop-up was conducted in 34 high-risk districts from 13-17 September, while a further national round will be held from 8-12 November, using tOPV.
In Turkmenistan, the total number of cases is three (all WPV1). The most recent case had onset of paralysis on 28 June, and all cases have been covered by three national immunization activities and one mop-up activity in the infected area. The most recent SIA ended on 29 September, targeting all children less than 15 years of age with mOPV1.
