<NYT_TEXT>BETHESDA, Md. — The Obama administration warned Americans on Thursday to be ready for an aggressive return of the swine flu virus in the fall, announcing plans to begin vaccinations in October and offering states and hospitals money to help them prepare.


“The potential for a significant outbreak in the fall is looming,” President Obama said via telephone link from Italy to the White House’s H1N1 Influenza Preparedness Summit, held at the National Institutes of Health.

With good planning, he added, “we may end up averting a crisis. That’s our fervent hope.”

The summit meeting, jointly led by the secretary of Health and Human Services, Kathleen Sebelius; the secretary of Homeland Security, Janet Napolitano; and the secretary of Education, Arne Duncan, gathered health and school officials from across the country and took questions by video link from the governors of several states — most of whom wanted to know who would pay for preparations like the vaccination drive.

Vaccinations will begin in October only if tests scheduled to begin in August prove that it is safe and effective. Even then, officials expect only tens of millions of doses to be ready, so they will have to decide who gets vaccinated first. The most likely candidates, Ms. Sebelius said, are school children, health care workers, pregnant women and people with asthma or other conditions that make the flu more risky.

While health officials were careful to warn that there is no evidence that the flu has mutated into a more dangerous form, they noted that it seriously disrupted some cities, including New York, in the late spring and could do worse as the fall flu season begins.

“This flu is not over,” said Dr. Thomas Frieden, the new head of the Centers for Disease Control and Prevention, describing its continuing spread in more than 50 summer camps, the large numbers of cases seen in Chile, Argentina and Australia, which are now at the beginning of their flu season, and the initial detections of three Tamiflu-resistant cases.

At the flu’s peak in May, Mr. Duncan noted, there were 726 schools closed across the United States. Decisions on closing schools will be made locally, he said. Officials from New York and Texas described the difficulties they had in deciding which schools to shut down and how hard it was to explain why they picked those they did.
Both schools and businesses need to prepare for the possibility of several weeks of high absenteeism, Ms. Napolitano said. She also reminded governors that not only the obvious services, like hospitals and schools, would be affected.

“As a former governor, I can say: make sure your payroll continues,” she said. “Whoever processes your checks, make sure they have a backup.”

Ms. Sebelius outlined actions the federal government is taking. It will offer $260 million in “preparedness grants” to states and cities for the vaccination drive and $90 million to hospitals preparing for surges of cases. (Congress has already appropriated $1 billion for vaccine ingredients and up to $7.5 billion more for testing, buying and distributing vaccine if health officials decide it is safe and effective.)

The Health and Human Services Department is also remaking its Web site, www.flu.gov, to be the central repository of information for everyone: parents, school officials, doctors. And it will hold a contest, asking Americans to film their own public service announcements about flu.

“This is a YouTube challenge to everyone,” Ms. Sebelius said. “The best will be aired nationwide.”

Health officials said that they were aware of fears that a Tamiflu-resistant strain of the virus is already spreading silently in the United States, but that they had not seen evidence that it is a threat.

The worry stems from a single case found in a teenage girl who flew to Hong Kong from San Francisco on June 12. According to Hong Kong media reports, she was found to have a fever during a routine airport screening and was hospitalized as a precaution and tested.

She was never dangerously ill, was not treated with Tamiflu and recovered. But the sequence of her virus, released by Hong Kong authorities last week, showed that it had a mutation, known as H274Y on the neuraminidase gene, making it resistant to Tamiflu, a neuraminidase inhibitor.

The fact that she had a resistant strain without being treated suggests that she caught an already resistant virus from someone else, presumably in Northern California, said Henry L. Niman, who runs a Web site tracking flu mutations (recombinomics.com). Because sequencing in the United States is done on only the tiny fraction of flu cases serious enough to be hospitalized, Tamiflu-resistant virus could be circulating without being detected.

Dr. Frieden said the disease control centers had begun intensifying its monitoring in Northern California, “and it does not appear to be widespread.”
Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Disease, said there was little reason to worry unless it becomes the dominant strain.
“But certainly you want to keep your eye on it,” he added.

Different strains of virus “compete” with each other each year, and the drug-resistant strains do not always win. But a Tamiflu-resistant strain of seasonal H1N1 flu utterly crushed its rivals during the last American flu season, rising to 99 percent of sequenced samples.

Also, Tamiflu-resistant strains can sometimes be successfully treated with Relenza, another neuraminidase inhibitor, with older flu drugs like rimantadine, or even with larger Tamiflu doses.

“It does worry me, though we know enough to expect this,” said Dr. Anne Moscona, a flu treatment expert at Cornell University’s medical school. “It strengthens the argument for more Relenza in the stockpile,” she added, referring to the national stockpile of supplies for an influenza pandemic.

Dr. Niman has been right on so much. When he forecasts something, I listen carefully. When he was concerned about the "porcine bride" in 2006 I asked if she had moved on since her lone wolf groom, H5N1, seemed to have deserted her at the altar. Well, shoot, she just landed on us with all her significant weight!

The Obama Administration sent a strong message to the nation today that it is time to start planning and preparing for the fall flu season and the ongoing H1N1 flu outbreak and that the federal government is prepared to commit resources, training, and new tools to help state and local governments and America?s families get ready.


White House Homeland Security Advisor John Brennan, Secretary of Health and Human Services Kathleen Sebelius, Secretary of Homeland Security Janet Napolitano, Secretary of Education Arne Duncan joined with delegations from 54 states, tribes and territories today at the H1N1 Influenza Preparedness Summit at the National Institutes of Health in Bethesda, Md., to kick-off the government?s nation-wide fall flu preparedness efforts.

?The President and the administration are actively engaged in mitigating the effects of the H1N1 flu virus and developing a national response framework and action plan that builds on the efforts and lessons learned from this spring?s initial onset to prepare for the possibility of a more serious fall outbreak of the virus,? said White House Homeland Security Advisor Brennan in his address to summit participants.

?Over the course of coming weeks and months, we will move aggressively to prepare the nation for the possibility of a more severe outbreak of the H1N1 virus,? said HHS Secretary Sebelius. ?We ask the American people to become actively engaged with their own preparation and prevention. It?s a responsibility we all share.?

?The federal government is working together with its federal, state, local and tribal partners to develop a nation-wide plan to combat the H1N1 flu that incorporates the lessons we learned this spring,? said Homeland Security Secretary Napolitano. ?The H1N1 Summit will allow us to continue this aggressive preparation for all possible H1N1 virus outbreak scenarios to ensure that we are doing everything possible to keep our country safe and healthy.?

?Effectively dealing with a potential H1N1 outbreak requires all of us -- parents, educators, health providers, and local, state and federal governments -- working together on our emergency management plan,? said Education Secretary Duncan. ?Today?s Flu Summit is an important step in that direction. Our primary goals at the Department of Education are the health and well being of students, faculty and staff, and ensuring that, in the event of any school closures, the learning process will continue. ?

Maryland Governor Martin O?Malley moderated a Governors panel with participation via videolink from Governor Jim Douglas of Vermont, Governor Jim Doyle of Wisconsin, Governor Mark Parkinson of Kansas, Governor John Baldacci of Maine and Governor Jodi Rell of Connecticut.

?When responding to a national pandemic or a national recession, the basic principles of smart government remain the same -- to increase efficiency, openness, and transparency in everything we do. Today?s summit illustrates our collective commitment to that goal,? said Maryland Governor Martin O?Malley. ?The experience in the spring taught us that while earlier pandemic flu planning efforts were effective, there are also areas for improvement. Effective response requires accurate and timely information that is as close to real time as possible. We share the commitment of the Obama Administration to constantly monitor, evaluate and improve these processes as we continue to lead the world in emergency preparedness.?

Throughout the one-day summit, Administration officials laid out specific ways that states and local governments could start their planning and preparation efforts and announced new programs and resources to help state and local governments, the medical community and every day America prepare for H1N1 and the fall flu season.

First, HHS will make available preparedness grants worth a total of $350 million. These grants were funded by Congress in the latest supplemental appropriations bill and they will give state and local public health offices and health care systems valuable resources to step up their preparedness efforts.

Second, the federal government will centralize communications about H1N1 and seasonal flu on the federal government?s new Web site www.flu.gov. This one-stop comprehensive site brings together flu-related information from across HHS and other federal agencies. The expanded site builds on the pandemic planning information long presented on www.pandemicflu.gov, and incorporates information about the novel H1N1 flu as well as the seasonal flu. Finally, HHS is launching a new PSA campaign contest to encourage more Americans to get involved in the nation?s flu preparedness efforts by making a 15-second or 30-second PSA. Officials at the summit stressed the idea of ?shared responsibility? when it comes to combating the flu and the goal of the new HHS PSA campaign contest is to tap into the nation?s creativity to help educate Americans about how to plan for and prevent the spread of H1NI influenza. HHS will evaluate submissions and will present the best PSAs back to the public so everyone can vote on their favorite submission. The winning PSA will receive $2,500 in cash and will appear on national television. Contest details as well more information about the larger effort to plan and prepare for the flu season are available at www.flu.gov.

July 9 (Bloomberg) -- Swine flu resistant to Roche Holding AG?s antiviral drug Tamiflu has been detected in at least three world regions, raising concern the pandemic virus is developing immunity to the medicine, health officials said.
The drug-resistant cases ?need to be watched,? though recommended use of Tamiflu hasn?t changed, said Thomas Frieden, director of the U.S. Centers for Disease Control and Prevention in Atlanta. Frieden spoke at a swine flu meeting in Washington organized by U.S. health officials to plan for outbreaks.
U.S. Health and Human Services Secretary Kathleen Sebelius said the government is making $350 million available to states and hospitals to prepare for pandemic flu. The H1N1 virus is infecting a younger population than seasonal flu and causing potentially fatal complications in otherwise healthy people ages 30 to 50, pregnant women and those with asthma, diabetes and obesity, the World Health Organization said.
?The possibility of an anti-viral resistant strain emerging is a serious consideration,? Sebelius said at the conference. ?To date, anti-viral medications have proven effective against the disease in most cases.?
Sebelius said applications for the money are due in the next two weeks, and the funds will be distributed by the end of this month.
Vaccines
Vaccines offer the best protection against the flu, and the U.S. will begin human tests of an immunization in the first week of August, said Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases in Bethesda, Maryland. If health officials decide to go ahead with vaccinations, they will likely begin in early October, he said.
?It?s impossible to predict right now, here in the northern hemisphere, whether this will dominate in the sense of crowding out the standard seasonal circulating flus,? Fauci said today in a telephone interview. ?We know that it is still hanging around, we?re still seeing outbreaks in summer camps, but we don?t know what?s going to take over in the fall and winter season when you have the optimal spread of influenza.?
Last week, Hong Kong?s health department said Tamiflu- resistant swine flu was found in a 16-year-old U.S. girl who hadn?t taken the pill. It was the first known instance of resistance in a swine flu patient not treated with Tamiflu, stockpiled by governments worldwide to fight pandemic influenza. The teen?s flu was diagnosed after she flew from San Francisco to Hong Kong. Cases of Tamiflu-resistant flu have also been reported in Denmark and Japan.
Resistance Tests
Swine flu has infected more than 1 million people in the U.S., though most haven?t been tested for the illness, according to the CDC. About 94,500 cases have been confirmed by laboratory tests worldwide, the World Health Organization reported on its Web site. About 1,000 virus samples have been tested for drug- resistance, and only the three samples tested resistant to Tamiflu, the WHO said.
Roche?s Tamiflu, a pill, and GlaxoSmithKline Plc?s flu drug Relenza, an inhaled powder, reduce the severity and the duration of influenza symptoms by 24 to 30 hours if treatment is started within the first two days of illness, according to the companies.
Tamiflu was unable to fight most seasonal flu strains that circulated in the U.S. earlier this year, according to the CDC. In December, the agency recommended that U.S. patients take Relenza or a combination of Tamiflu and an older drug called rimantadine to fight seasonal infections.
Stockpiling Tamiflu
Fears of a global influenza outbreak prompted governments, mostly in developed countries, to store millions of courses of Tamiflu that could be used to prevent infection in health-care workers and others performing critical job functions.
That demand spurred Tamiflu sales of 2.6 billion Swiss francs ($2.4 billion) in 2006 and 2.1 billion francs in 2007. Last year, sales dipped to 609 million francs as government stockpiles filled.
Asthma is ?the leading single underlying diagnosis with H1N1, present in a third of hospitalizations? and patients with morbid obesity have higher rates of asthma, the CDC?s Frieden said today.
State and local officials are ?critical links? to fighting a pandemic, President Barack Obama said in a telephone address to the meeting from Italy. He said health agencies at every level need to plan for vaccine programs and disruptions to schools and workplaces in the U.S. fall, when illness rates are expected to rise.
?The potential for a significant outbreak in the fall is looming,? Obama said. ?We want to make sure that we are not promoting panic, but we are promoting vigilance and preparation.?

There are VERY few who are paying attention and are not concernd about the fall flu season in the northern hemisphere (those who are not sure if the 2009 pandemic is a pandemic notwithstanding).

Yes, Dr. Niman, this fall will be quite an intersting experience to say the least.

Despite the press headlines Obama only made a short speech (from Italy) - this is all news from the Flu Summit yesterday and plenty of other people spoke at length. It was stated clearly that (wording from my memory) "CDC has multiple personnel on the ground in the southern hemisphere getting up close and personal with the virus to watch how it develops in real time."

Homeland Security said that they are preparing for a 1918 level epidemic and hoping it won't be that bad. Despite being pressed they could give no advice on which scenario was most likely.

Here's a quote from Secretary Napolitano (from prepared speech online at http://www.dhs.gov/ynews/releases/pr_1247229218508.shtm )

"So even though we cannot—and science cannot predict what kind of flu or flu epidemic we are going to experience, and how serious its lethality will be—whether we're in a 1918 situation or a 1968 situation—that remains to be seen."

One piece of general advice given out (my paraphrasing) was "don't be afraid to admit there are some things we just don't know yet and whatever you do, don't tell people just not to worry."

obviously presidents have additional skills over Secretaries.
They can predict these things.

I still think you are reading too much into selected quotes.The entire conference yesterday was about the "looming potential" as you (and Obama!) put it.

on the one side they say : "noone knows, noone can predict"

but then they make these conferences and statements
which apparantly _do_ include some such predictions.

They don't give numbers for probability estimates but imply
them by their wording, knowing and willingly anticipating that
people will interpret
them as increasing danger (rarer decreasing danger)


we've been seeing this contradiction since at least 2005 with birdflu
most famous example Rob Webster in March 2006.

Science had NO problem seeing the pandemic months ago, and no problem seeing a looming and growing pandemic in the fall.

The initial WHO definition of a pandemic was a novel serotype that was transmiting H2H in a sustained manner.

This definition was met as soon as it was clear that the widespead "mystery illness" in Mexico, which was killing previously healthy young adults, was caused by the same H1N1 isolated in southen California in late March / early April. By the time the confirmation was announced, there were multiple locations in the US and the death rate had skyrocketed in Mexico.

Thus, the swine H1N1 was clearly transmitting H2H in a susptained manner, meeting the original WHO definition.

WHO did a phase re-do that was based on politics, not science. Sustained transmission was moved up to phase 4 and phase 5 and 6 were based on further spread.

WHO declared the 2009 swine flu outbreak as phase 4 some time ago, which was quickly followed by phase 5. Phase 6 was delayed because countries outside of the Americas were largely just screeing airport travelers and contacts, but this was just more politics.

Thus, on the science side, the pandemic was identified months ago.

On the fall prediction side, 2009 looks exactly like 1918.

It was mild in the spring and is a swine H1N1 that has moved into a human population.

The target groups is largely children and relatively young adults. The vast majority of fatalties are under 65 and most are relatively healthy. The underlying conditions, like preganacy or asthma, are not life threatening, in marked contrast to seasonal flu, which is largely a fatal disease of the very old, who have serious life threatening chronic illness.

The pandemic H1N1 has persited long after the end of seasonal flu season in the northern hemisphere, and is crowding out seasonal flu in the southern hemisphere, where flu season is just beginning.

Fatalities are on the rise worldwide, but have accelerated dramatically in Argentina in the past couple of weeks, causing panic and a serious degradation of health care.

Thus, at this time 2009 is tracking with 1918, and there is little data supporting a deviation.

Virtually all paying attention, especially those looking at the pandemic scientifically, know that the fall will present major problems.

In summary what was said at yesterday's summit (as I saw it) - was that they (US agencies) are working on the assumption that this will be at least a 1918 type replay but retaining the ability to step back from that assumption should things not be so bad. If anyone else who watched it came away with a different impression then feel free to correct me.

The message for delegates to take away from the conference was definitely to prepare for a 1918 scenario but hope for better. Despite questions asking for probabilities (one from a Governor) the advice was none could be given at this time.

This begs the question: Would they prepare for something like that if they didn't believe there was a liklihood of it?

.......... Most people here either know nothing about it or think it's some govt conspiracy with the drug companies.............The only reason panflu was on local tv yesterday was because Obama spoke.

I'm hearing the same thing from many people - either they don't keep up on news and are in the dark, or they think it's a big con to increase big pharma profits. They are absolutely convinced that a 1918-level pandemic cannot happen today. :rolleyes:

There was a recent FT thread saying we needed a spokesman. I suggested the Queen of England, but I'll settle for Obama as a momentary spokesman bringing attention to the issue. :D
