I'm not convinced there's that much out of the ordinary going on here. Juarez apparently only confirmed 20 H1N1 cases in all of 2009 in a city of 1.5 million. I think the low case counts are an artifact of the testing regime; you have to be seriously ill to get tested for H1N1 (and by the time you get tested, the virus is gone, so it comes back negative). I suspect there have been thousands of H1N1 infections in Juarez over the past few weeks.

We only have one instance where the deaths clustered (the police officers). The other deaths appear sporadic, and include at least a couple pregnant women, the group hit hardest in 2009. And at least one article mentions a case (Liz Aguilera) that was discarded without testing on the grounds that the illness was mild.

What concerns me most is that before they identified pH1N1, they were reporting many cases of atypical pneumonia. Either they didn't bother testing for flu or the virus is different enough that they couldn't subtype the samples for some reason.

There are several Influenza A not subtypable

That is very similar to the pattern that we've seen in many U.S. states where they have been hit really hard by Flu B and/or H3N2, with few deaths and now pH1N1 is starting to show up, and although %ILI and %positive specimens have decreased significantly overall, severe cases and deaths have started to increase. The main difference is that it looks like there has been very little ph1N1 circulating in Mexico until the last few weeks.

So, it doesn't look like there is anything different going on in Mexico, but I still fear that a mutated form of pH1N1 will create a late spring/early summer wave just like 2009. Furthermore, although some states have had some pH1N1 co-circulating with seasonal flu since late 2010, it's possible that the circulating strain pre-dated the mutations identified in samples from December/January in the UK that appeared to allow for vaccine escape. So, even those states could experience another wave of mutated pH1N1.
