Second Suspect H5N1 Cluster in Jakarta
Recombinomics Commentary
January 11, 2007

The 37-year-old woman, who tested positive for the H5N1 strain of avian influenza two days ago, remains in critical condition at Persahabatan Hospital in Jakarta, Mukhtar Ikhsan, a doctor at the hospital, said today.

Her 42-year-old husband and their 18-year-old son have symptoms of fever and respiratory infection, and are being tested for H5N1.

``They're showing symptoms similar to bird flu and we must remain vigilant,'' Ikhsan said in a telephone interview. Tests for H5N1 are also being run on two other people receiving treatment at the hospital, he said.

The above comments describe another suspect H5N1 bird flu cluster in the Jakarta area. Recently there were two confirmed cases and relatives and contacts of both cases have been hospitalized with symptoms.

The first case (14M) has died, and initial tests on hospitalized relatives and neighbors have been negative. The above patient (37F) is in the same hospital and her relatives are being tested. As indicated above, two other people are being treated with Tamiflu and being tested.

This explosion of confirmed and suspect cases is not limited to the Tangerang region to the west of Jakarta. A large cluster of 20 suspect cases has been reported in Bogor, south of Jakarta, where there have been prior confirmed cases. Another suspect cluster has also been reported in southern Sumatra.

Dead ducks linked to the recent H5N1 fatality have also tested positive for H5N1. The amount of wild bird testing in Indonesia has been limited, and matches between human and bird H5N1 sequences have been poor. The only bird isolate from Java that has the novel H5N1 cleavage site, RESRRKKR, has been from a duck in Indramayu, isolated in January, 2006.

More details on the sequences from patients and ducks would be useful.

Update from Bloomberg:

Bird Flu May Have Infected Relatives of H5N1 Patient (Update1)
By Karima Anjani
Jan. 11 (Bloomberg) -- Two family members of a woman infected with bird flu in Indonesia were hospitalized with symptoms of the virus, said a doctor treating the patients. The disease spread to fowl in a fifth province of southern Vietnam.

Infections in birds and people are increasing in Asia, where the virus was first identified a decade ago. South Korea reported its first avian outbreaks since March 2004 last month, and today reported an infected person who showed no symptoms. Disease trackers are looking for signs that H5N1 is changing to become more adept at infecting humans, which could start a pandemic.

``People were beginning to tell us that we were crying wolf and that this virus had gone away and there was no problem,'' Peter Cordingley, a Manila-based spokesman for the World Health Organization, said in an interview. ``Our message has stayed the same, which is that just because it's not in the headlines, it doesn't mean it's not in the environment.''

The H5N1 strain is known to have infected 264 people in 10 countries since 2003, killing 157 of them, the Geneva-based WHO said yesterday. Indonesia has reported 58 fatalities, more than any other country. Vietnam, which reported that the virus killed 30 chickens in Vinh Long province, hasn't had any human cases since 2005. South Korea hasn't had any cases confirmed by WHO.

Indonesian Cluster

Health officials monitor suspected clusters of cases closely because they may signal the virus is becoming more adept at infecting humans, not just birds.

``They're showing symptoms similar to bird flu and we must remain vigilant,'' Ikhsan, the Indonesian doctor, said in a telephone interview. Tests for H5N1 are also being run on two other people receiving treatment at the hospital, he said.

The Southeast Asian nation attracted international attention in May when seven members of a family from the island of Sumatra contracted the H5N1 virus, six of them fatally. The cases represented the largest reported cluster of infections and the first laboratory-proven instance of human-to-human transmission.

The latest confirmed case in Indonesia may have been caused by contact with a sick bird. The woman slaughtered a chicken she bought from a local market, I Nyoman Kandun, the ministry's director general of disease control and environment, said Jan. 9.

JAKARTA, Indonesia, Jan 12 (Reuters) - A 37-year-old Indonesian woman has died of bird flu, taking the country's human death toll to 59, a hospital official said on Friday.

The woman, from Serpong town in western Java, had been diagnosed as positive for the virus earlier this week.

"The woman died yesterday at 7.35 p.m. (1235 GMT) because of organs malfunction which is caused by (bird flu) virus H5N1," Dr. Muchtar Ichsan, chief of bird flu management at Persahabatan Hospital in Jakarta, told Reuters.

Four others are receiving treatment for bird flu symptoms at the hospital, although the virus has yet to be confirmed, he added.

Ichsan said on Thursday afternoon the woman's husband and son, who had also developed bird flu symptoms, had been hospitalised at Persahabatan.

Two other women have been treated since yesterday for the same symptoms and their conditions are not good, he said.

"Their test results will be out within two (or) three more days," Ichsan said.

Indonesia leads the world in bird flu deaths, and has had 76 confirmed cases.

Officials say efforts to contain the virus are having some success and the number of provinces where it is present in fowl, the usual source of human infection, has fallen sharply.

Health ministry official Joko Suyono told Reuters earlier in the week that the woman who has since died had bought a live chicken and slaughtered it at her house, but it was unclear whether this was the cause of the infection.

The Resident's willingness opposed Bird Flu
Jan 12, 2007

"Mpok burungnya kite ambil sekarang ye." The voice of two managements RT 08/RW 10, the Kalideres District, Jakarta West, startled his resident named Wati, that was sitting in the house terrace with his children. The two officials were undertaking the extermination program of the poultry after the bird flu attack in this region claimed casualties. Randy Setiadi (14 years), this RT resident positive died because of bird flu on Wednesday (10/1).

With this incident, spontaneous the daughter Wati immediately answered, "Ambil aja." For a moment Wati fell silent then he said, "Eh, that would his pet birds Engkong. Later if Engkong angry gimana?" Could not be second his child replied, Wati has answered personally his question, ''Udah ambil aja deh. Biar nanti dibilangin anak-anak,'' tutur Wati.

Pascakematian Randy, apparently the local resident was indeed shadowed by the concern about the spreading of this virus. Then did not surprise, when the regional government's local equipment ask for was done by depopulation (the extermination), the moving together resident helped him. Since Wednesday early afternoon (10/1), equipment of RT/RW was helped by the local young man, has moved personally. They visited the resident's house that had kept the poultry, both that took the form of the chicken, the duck, the duck, and birds. Various poultry kinds were taken during immediately was destroyed.

Haunted by the concern was attacked by bird flu, the resident submitted when his chicken was taken. Although in fact they experienced the upper material loss was destroyed by him kept they. "The birds in fact has been bargained by Rp 500 thousand. But, okay then than later was the illness," said Wati. However, said Wati, if being given by compensation, he will feel happier.

On the resident's willingness, depopulation of the poultry in RW 10 could go smooth. The number of poultries during yesterday took place the burning reached 722 tails. The details are, the duck 236 tails, birds 149 tails, and the chicken discussed 337 tails. After taking place the extermination, the condition for the local environment was then seen changed. If beforehand, many wandering chickens in the roads and the page of the resident, since yesterday were not seen by one then the chicken in the region. "While our 3 months forbade the resident to maintain the poultry," said the Representative of the Guardian of the Jakarta City West, Murdhani.

The resident's willingness RW 10 indeed must get acungan the thumb. They wanted to suffer a loss in the interest of together. But, this willingness should did not make the Regional Government local closed the eyes towards the resident's loss. The head Sudin West Jakarta Livestock Breeding, Chaidir Divine Guidance, said his side will give compensation, of Rp 10 thousand per the tail for all the poultry kinds. At this time, the West Jakarta Municipal Government was compiling the mechanism of giving of his compensation.

The concern would the bird flu outbreak was not only monopolised by the resident RT 8/RW 10. The resident RT 1/RW 7 local districts also began to be worried after finding seven chickens belonging to the resident in this location died suddenly. They then at once buried and burnt the chicken coop that died suddenly that. Rukidah, this RW resident, claimed worried if the bird flu virus will attack. "Moreover in sono (RW 10) has had the incident that until died', said Rukidah. He asked that the case of the death of the sudden chicken was followed up.

So frightened him ketularan bird flu, according to a resident RT 02/RW 10 that did not want to be acknowledged as his name, and so have several neighbours who evacuated his child to the other area. "My neighbour his name of Yuyun and his child ngungsi to Tangerang. He was frightened tertular because of his child be hit by hot," said the resident.

The concern would bird flu also kept the potential for the interregional conflict. A resident RW 7, Agus, said his neighbour who did not have the poultry blamed his neighbour who maintained the poultry. "They ngedumel behind, kayaknye nyalahin that had the chicken," said Agus. Whereas his neighbour who had the chicken, said Agus, just very calm, because of being not yet proven his kept animal was affected by bird flu. The head of Sudin West Jakarta, Ariyani Murti, asked that the resident stayed calm in responding to bird flu. He explained, was not yet Indonesia the spread of bird flu antarmanusia [H2H].

Bird flu was contagious if someone direct contact with the poultry that was infected by this illness virus. Therefore, Ariyani asked the resident to maintain the cleanliness of the kept poultry and his pen. The resident's Kalideres concern about bird flu was not yet matched by the understanding on this illness. Although worried was attacked by the bird flu virus, they continued to carry out direct contact with the poultry. Proven by them themselves who gathered the poultry to be destroyed. In fact, the Sub-district Head Kalideres, Ruslan, has asked so that his resident not direct contact with the poultry.

The case of the death resulting from bird flu in Jakarta this equipped Indonesia as the country that worst was attacked by bird flu. One of the factors that caused Indonesia to experience the most serious attack, said the Team of the Bird Flu Media of RSU Dr Sutomo, the University School Of Medicine of Airlangga Surabaya, Laksmi Wulandari, was the low level of the awareness of the community. The other factor was the limitations of the fund and the not firmer government.

The Chairman's words of the Regional Working Group the National Commission the Control of Bird Flu and Preparedness of the influenza Pandemic, Heru Setijanto, to the end of November 2006 the bird flu attack in Indonesia killed 57 people from 74 cases that happened. He added this illness was contagious in 30 provinces in Indonesia.

Four Suspected Bird Flu Sufferers Hospitalized In E Jakarta
Jan 12, 2007 at 08:50 AM

Jakata ( Berita ): Persahabatan Hospital in East Jakarta on Thursday admitted four suspected bird flu sufferers after previously treating seven other people for the same reason two of whom had tested positive for the virus.

The four newly admitted patients were identified as Mrs Am (22), D (42) and A (18) from Tangerang, Banten province, and Z (27) from South Jakarta.

Of the seven patients treated earlier, five were declared free from the virus and allowed to return home. Of the remaining two others, one was in critical condition and one died on Wednesday (Jan 10).

Muchtar Ikhsan, chief of Persahabatan Hospital's Bird Flu Handling Team, said the four suspected bird flu sufferers were sent to the hospital after showing symptoms associated with the disease.

They were now being intensively treated in an isolated ward in the hospital, he said.

Meanwhile, a patient who had tested positive for the virus, identified as R (37), was still in critical condition and undergoing intensivetratment in an isolated room at the hospital. In fact, R's condition was worsening as the patient was also suffering from severe pneumonia. "The pneumonia is impeding oxygen supply to all parts of the patient's body," Muchtar said. [This is Riyah, who has since died.]

B [R?] had complained about having dizzy spells, breathing difficulty and weakened consciousness and was therefore taken to the hospital.

Ikhsan said on Thursday morning officers of the WHO (World Health Organization) came to the hospital to collect data on bird flu cases the hospital had handled.

Bird Flu Kills Indonesian, May Have Returned to Japan (Update4)

By Karima Anjani and Kanoko Matsuyama

Jan. 12 (Bloomberg) -- Bird flu killed a 37-year-old woman in Indonesia and may have infected hundreds of poultry in Japan as the lethal virus resurfaces across Asia.

The woman's death late yesterday brings to 59 the number of fatalities from the H5N1 avian influenza virus in Indonesia, Runizar Ruesin, a Health Ministry official, said in a mobile phone text message today. Japan's farm ministry suspects the virus killed poultry on a farm on the southern island of Kyushu.

Diseased birds increase the opportunities for human infection and provide chances for H5N1 to mutate into a form more dangerous to people. Millions could die if H5N1 mutates and begins spreading easily between people, sparking a lethal pandemic. South Korea, Vietnam and Nigeria had fresh poultry outbreaks last month, while China and Egypt found human cases.

``It makes biologic sense that where there is more virus activity, there is a greater likelihood of some type of genetic change,'' Michael Osterholm, director of the University of Minnesota's Center for Infectious Disease Research and Policy in Minneapolis, said in a telephone interview today.

The H5N1 strain is known to have infected 264 people in 10 countries since 2003, killing 158 of them, the World Health Organization said yesterday. Indonesia recorded the most deaths.

New Indonesian Case

A 22-year-old man from Tangerang, in the Banten province on the west of Java island, tested positive for H5N1, I Nyoman Kandun, the ministry's director general of disease control and environment, said today in a mobile phone text message.

The man is from the same province as the woman who died yesterday.
She developed symptoms on Jan. 1 and was admitted to Jakarta's Persahabatan Hospital five days later. Her 42-year-old husband and their 18-year-old son are suffering from symptoms of fever and respiratory infection, and are being tested for avian flu, Mukhtar Ikhsan, a doctor at the hospital, said yesterday.

Test results are pending for the other family members and, if confirmed, would represent the latest cluster of H5N1 cases in the southeast Asian nation, said Ruesin, who is head of the Health Ministry's avian flu information center.

Initial investigation suggested diseased poultry as the possible source of infection, the Geneva-based WHO said Jan. 9 in a statement on its Web site.

Japan's Agriculture, Forestry and Fisheries Ministry said it will isolate fowl on the farm in the southern prefecture of Miyazaki, where the 750 bird-deaths were reported. It will ask other poultry farmers within a 10-kilometer (6-mile) radius not to move their produce until the investigation is finished. The farm has 12,000 poultry, the ministry said yesterday.

Test Results

Results of tests for the H5N1 strain may be announced as early as tomorrow, ministry officials told reporters in Tokyo today.

``This case is very likely to be highly pathogenic bird flu'' virus, said Takashi Himeda, a manager of with the ministry's animal sanitation department. ``The possible infection route may be migrant birds, but we don't know yet.''

The ministry is sending two avian-flu specialists to help implement control measures and the cabinet office will meet today to coordinate an investigation with other ministries. It is advising meat distributors and restaurants on poultry-meat safety. Local governments will be asked to contact farms throughout Japan to check for signs of infection.

Trade Halted

South Korea temporarily halted imports of Japanese poultry, the country's agriculture ministry said in a statement today. A ban will be imposed if a highly pathogenic avian flu virus is confirmed, the ministry said.

Japan's first H5N1 outbreak was reported in a commercial poultry flock in Yamaguchi prefecture on the southwestern edge of the main island of Honshu three years ago. Outbreaks were also found in poultry in Oita and Kyoto prefectures, the last of which occurred in March 2004.

Japan is the world's second-largest importer of chicken meat, according to data kept by the U.S. Department of Agriculture. It bought 443,000 metric tons of the white, worth 102 billion yen ($846 million) in 2005, according to Japan's farm ministry.

Avian-flu outbreaks ``will remain to be the biggest factor'' affecting world poultry demand and supply in 2007, the USDA said in a report last year. ``Any major outbreaks in both exporting and importing countries will likely disrupt the trade causing uncertainties'' for the Japanese meat trade, it said, adding that Brazil is a key supplier to Japan.

Indonesian woman dies from bird flu, bringing death toll to 59
Jan 12, 2007

An Indonesian woman died from bird flu while a second was in critical condition on Friday, as the country worst hit by the virus battled new cases following a six-week lull, health officials said. A teenage boy was killed earlier this week.

All three cases occurred in the industrial town of Tangerang to the west of the capital, Jakarta.

The 37-year-old woman died late Thursday soon after being admitted to a Jakarta hospital.

The woman's husband and 18-year-old son are being treated at the same hospital for symptoms of the H5N1 virus, said Haris Sugiantoro, an official at the Health Ministry's bird flu information center.

Tests also confirmed a 22-year-old woman was also suffering from the virus, said Joko Suyono, another official at the center. Neighbors reported that chickens living near her house died suddenly and the woman was seen picking one of them up and scratching her nose soon after.

"She is now in critical condition in an intensive care unit of the hospital," Suyono said.

On Wednesday, a 14-year-old boy also from Tangerang died after being treated for four days at the same hospital, reports AP.

He was the country's first fatality in six weeks a pause that had led Indonesian officials to say they were making progress in the fight against the disease.

"Indonesia has been on the right track in handling the bird flu, but the danger still exists," Bayu Krisnamurthi, head of the country's bird flu commission, told reporters. "We hope the virus decreases more this year, but the war is continuing."

At least 59 people have now died from bird flu in Indonesia more than a third of the world's total since the virus began ravaging Asian poultry stocks in 2003.

The virus remains hard for humans to catch and kills a tiny percentage of the hundreds of thousands of people who die each year in developing countries from easily preventable diseases.

But international experts fear it may mutate into a form that could spread easily between humans and potentially kill millions around the world.

Indonesia woman tests positive for bird flu, in critical condition
Jan 12, 2007

(Kyodo) - A woman from Tangerang near Jakarta, where a 37-year-old woman died of bird flu Thursday, has been tested positive for the H5N1 strain of the disease, an official at the Health Ministry's Bird Flu Center said Friday.

The 22-year-old woman "is in critical condition," Joko Suyono told Kyodo News. He said she apparently did not have contact with chickens.

The death of the 37-year-old woman, which raised the toll from the disease in the country to 59, followed a case of a 14-year-old boy from West Jakarta, who died Wednesday. The boy's case was the first in three months.

Bayu Krisnamurthi, chairman of the National Commission for Bird Flu Control and Preparedness to Anticipate Influenza Pandemic, told a press conference the latest cases still have "similar patterns" to the previous ones.

"The problem is they came to hospital late, about eight days after the onset" of the disease, he said.

No other information about the 22-year-old woman was available.

In the case of the 14-year-old boy, he fell sick after some ducks near his house reportedly died, Krisnamurthi said, adding it is difficult to determine whether ducks were the source of his exposure.

"It is very rare for ducks to transmit the virus to humans. Usually, ducks only act as reservoir which transmits the virus to chickens and then chickens to humans," he said.

The husband of the woman who died Thursday reportedly slaughtered five chickens, "one of which was sick" on Dec. 30, while three of her other chickens died for unknown reasons.

Her 45-year-old husband and 18-year-old son are also in hospital after suffering from high fever and breathing difficulties. If both are tested positive of having bird flu, it would be the first cluster case this year.

Since bird flu began to hit Indonesia in mid-2003, up to 15 million infected poultry birds have died in 224 regencies in 30 of the country's 33 provinces.

Coordinating Minister for People's Welfare Aburizal Bakrie said 16 of the 30 provinces are considered to be free of bird flu, having had no cases in the last six months.

As of Tuesday, bird flu had infected 264 people in 10 countries since its reemergence in December 2003, with 158 of them having died, according to World Health Organization statistics.

Indonesia led with 58 fatalities, followed by Vietnam with 42 and Thailand with 17. The other affected countries are Azerbaijan, Cambodia, China, Djibouti, Egypt, Iraq and Turkey.
