Comment: Bihar has reported 207 total AES deaths (153 in Muzaffarpur & 54 in other districts), Assam has increased the number of JE deaths to 142 (Aug 9) and AES to 139 (July 20) for a total of 281. There have also been reports of 2 deaths each in Jharkhand, Arunachal Pradesh, Manipur and Nagaland and 1 in Uttarakhand. Combined, this brings the number of fatalities so far this year to 1,280 (+369 in the past week). Uttar Pradesh: All 24 JE fatalities are reported from Bijnor district. Hojai: Japanese Encephalitis (JE) claimed one Abhisekh Tiwari, a resident of Halodhiati, under Bokolia Police Station of Karbi Anglong district on Sunday. Tiwari was a school teacher of Dulu Pathar LP School in Hojai district; he was 35 .He had been undergoing treatment in Gauhati Medical College & Hospital for last 45 days after he was detected with Japanese Encephalitis.

Pig blood samples test JE positive

ITANAGAR, Aug 12: About 15 percent of the blood samples collected from pigs in five districts have tested positive for Japanese encephalitis (JE).
Twenty-eight out of the 179 blood samples collected from pigs in East Siang, Upper Subansiri, West Siang, Lohit and Longding have tested positive for JE, the animal husbandry, veterinary & dairy development department said in a press statement.
The department conducted the survey recently to detect the possible presence of an antibody against the JE virus in the pig population of the five districts from where JE cases in humans were reported.

The Japanese Encephalitis death toll in Assam rose to 146 with one more case reported from western Assam?s Dhubri district according to National Health Mission (NHM) bulletin in Guwahati. A total of four positive JE cases were reported in the state on August 16th, 2019. A total of 614 positive JE cases have been reported so far according to the official data. On August 14th, 2019, a death case was reported from Barak Valley?s Cachar district.

News reports for the last few weeks have been omitting the full picture by not reporting on AES cases & deaths in Assam. To July 20, JE confirmed cases made up only 28% of the total AES (459 JE from 1,637 total AES) caseload and constituted only 45% of the fatalities (104 JE from 231 total AES). Since then, 155 additional confirmed cases of JE have been reported with 42 fatalities. Statistically, an additional 398 AES cases with 51 fatalities may have occurred in the same period. My estimate of the true AES toll in the state is around 2,190 cases with 324 fatalities. - Ro

Some think lychees are being blamed unfairly for an illness that breaks out among children in the north India state of Bihar each year.
Each summer in Muzaffarpur, in the north Indian state of Bihar, children suddenly wake up vomiting and delirious or they start having seizures. Some slip into a coma, and some die within hours of being wheeled into a hospital.

This phenomenon has been recurring for more than two decades. It generally begins in April and ends in June, when temperatures peak in this part of the country, soaring as high as 44 ?C. This year, the outbreak claimed the lives of 116 children in the state.

Some doctors thought it might be heat stroke. Others initially linked the recurring epidemic to acute encephalitis, an inflammation of the brain that can be caused in a variety of ways, including by bacterial or viral infection. They call it acute encephalitis syndrome (AES). But the situation in Bihar has been particularly puzzling: the symptoms come on too fast for encephalitis, and they’re accompanied by a drop in blood sugar.

So the search has continued. And within the past decade, researchers have identified a different culprit—a controversial one—lurking in fruit orchards.

Bad baby fan's fans

On one side there is hot summer, the mercury of the season has started reaching 40 degrees. While people are usually disturbed by the humidity and heat, in the district hospital's toddler ward, the patients of the patients are having to get fan with their hands. Of the 6 posts here, two fans have been opened for about a month as they are defective. Haven't stopped yet. The fans who are moving are so slow that their wind is not reaching the children. As a result, the patients are forced to fan themselves.

Comment: Assam has reported 147 JE confirmed deaths from 618 confirmed cases to Aug 20. The data above suggests thousands of additional AES cases which have resulted in another 173 deaths. Only 46% of the encephalitis deaths are confirmed JE.

The government is taking up immediate and long term measures to control the spread of Japanese Encephalitis in the state, stated health and family welfare minister L Jayantakumar in response to a calling attention motion raised by MLAs Kh Joykisan and Surjakumar Okram on the sudden outbreak of Japanese Encephalitis in the state during the concluding day of the 8th session of the 11th Manipur Legislative on Tuesday.
Jayantakumar informed the House that 47 Japanese Encephalitis cases were reported in 2016, 195 in 2017, 85 in 2018 and 232 in 2019 with the reported death of 11 people in 2016 and 2017, three in 2018 and five this year. He stated that spot verification, environment evaluation and counselling sessions have been conducted in each district under National Vector Borne Disease Control Programme.

LUCKNOW: Deaths due to encephalitis in Uttar Pradesh have come down by 65 per cent in the past two years due to sincere efforts by the government, Chief Minister Yogi Adityanath said on Wednesday.
"I started the battle against encephalitis in 1998. At that time there was an acute shortage of resources at Baba Raghav Das (BRD) Medical College in Gorakhpur but today the college has all the facilities.

"Due to sincere efforts of the state government, the death toll has now been reduced by 65 per cent during last two years," the chief minister said at an event.

The Adityanath government had drawn flak over the death of scores of children at the BRD hospital in August 2017, months after it came to power. Gorakhpur, which the chief minister represented for nearly 20 years in Lok Sabha, has been for long the epicentre of Encephalitis outbreak in the state.

According to the state government, more than 5,400 cases of Acute Encephalitis Syndrome (AES) and Japanese Encephalitis (JE) were reported in the state in 2017, leading to 748 deaths.

Uttar Pradesh has managed to tackle the scourge of Acute Encephalitis Syndrome (AES) and Japanese Encephalitis (JE) that has earlier killed hundreds of children in the districts of east UP and Terai region, officials said.

In 2016, AES afflicted 3,911 children and 641 out of them died; the next year 4,724 children were in the grip of the disease and 655 died; in 2018, 3,077 children were afflicted with AES and 248 died. In 2019 till August, a lesser number --890 ? of children were afflicted with AES and 30 died. The number of JE cases too has also come down from 442 in 2016 to 55 in 2019, the officials said.

Comment: The latest NHM report to Aug 24 says that 5,498 tests for Japanese Encephalitis have been performed in UP this year. This suggests far more than 890 patients exhibiting Acute Encephalitis symptoms. - Ro

The Gorakhpur division comprises four districts ? Adityanath's bastion Gorakhpur along with Kushinagar, Deoria and Maharajganj. Though there cannot be an excuse for even one life lost, the figures (in Table 1) tell a heartwarming story.

As figures show, in 2017, a total of 3692 encephalitis patients were admitted in these facilities, of which 556 died. Of these, 366 were found to be positive for Japanese encephalitis (JE) - a biological cousin of encephalitis that is trigger 10 percent of encephalitis cases - out of which 80 died.
In 2018, a total of 1660 patients were admitted, of which 186 died. Of these, 190 were found to be positive and 21 died.
In 2019 (till August 22), a total of 389 patients were admitted, of which 35 have died. Of these, 34 have been were found to be positive and five have died.

Comment - Some number analysis:
Comparing the figures for 2017 to statewide NVBDCP numbers.[SUP]1[/SUP] 3,692 of 4,724 cases (78%) were admitted to facilities in Gorakhpur region. 556 of 654 deaths (85%) occurred there. Gorakhpur CFR 15.1%. Statewide CFR 13.8%. Percent positive for JE - Gorakhpur: 9.91%; Statewide: 14.7%
For 2018: 1,660 of 3,018 cases (55%) were admitted to facilities in Gorakhpur region. 186 of 230 deaths (81%) occurred there. Gorakhpur CFR 11.2%. Statewide CFR 7.6%. Percent positive for JE - Gorakhpur: not available; Statewide: 10.5%.
For 2019 comparing to figures in the post immediately above as the NVBDCP website hasn't updated since June 30. 389 of 890 cases (44%) were admitted to facilities in Gorakhpur region. 35 of 30 deaths (117%!) occurred there. Gorakhpur CFR 9.0%. Statewide CFR 3.4%. Percent positive for JE - Gorakhpur: 8.7%; Statewide: 6.2%

A lot changed in 2018[SUP]2[/SUP] - Mass vaccination for Japanese Encephalitis and a shift to improved local clinics rather than an arduous journey to BRD in Gorakhpur was expected to give more timely treatment with improved outcomes. From 2014 on, several research groups were studying the role of Scrub Typhus in AES cases in India. One such study showed that AES cases in Uttar Pradesh had a 61% positivity for ST.[SUP]3[/SUP] This led to new treatment protocols and again improved outcomes were expected. On the face of it, the numbers being reported for AES in 2018 and 2019 indicate success.

While I am hopeful, there are aspects of the situation in UP that make me worried that the AES situation is being used as a political football. BRD hospital in Gorakhpur was forbidden from releasing daily encephalitis case numbers and deaths.[SUP]4[/SUP] Officially released figures in 2018 began to behave oddly. Two media outlets reported 80+ deaths at BRD through July, but by August 31, this number was being reported as 63 with only 6 deaths occurring in August.[SUP]5[/SUP] Reporters found that patients from out of state were no longer being included in official figures leading to numbers that were artificially 35% lower than those of previous years.[SUP]6,11[/SUP] Official figures reduced the total number of deaths reported statewide from 118 on July 31 to 113 by August 31.[SUP]7[/SUP] Reports emerged of widespread disease data manipulation, for example in Bareilly where, of 202 fever deaths, only 28 were recorded by the district.[SUP]8[/SUP] In my analysis: Up to June 29th 112 fatalities from 1,171 cases (9.6%) were reported, 11 fatalities from 1,517 cases (0.73%) were tallied from July to October, then 107 deaths from 392 cases (27.3%) from October to the end of the year. It seems to me that some manipulation of those figures is apparent.

One doctor was detained for examining patients classified as "fever" and declaring them to be AES cases.[SUP]9[/SUP] Other reports suggest that Doctors were ordered not to report cases of AES, but to classify them as something else.[SUP]10,11[/SUP] An opposition party organized a walkout from the State Assembly in protest.[SUP]12[/SUP] News reports continued to find higher numbers of deaths than were being officially reported.[SUP]13[/SUP]

This year, the figures are being tightly controlled and only released by political figures. Compare the dearth of information from UP in this thread this year to the wealth of local reporting that was available as recently as 2017. Such a lack of transparency leaves many questions. Are ST cases being included in the AES figures this year? If not, then that would reduce the numbers significantly. Are out of state patients included? What is the mechanism of reporting from the local PHC's? How many of the cases had been vaccinated? Where are the patients being treated? How are they being treated? What are the outcomes with regard to mental and physical disability in survivors? So long as the data is only available through political sources, it cannot be trusted. I call for openness & transparency. Let the sunlight in!

CCpur, August 30 2019: Japanese Encephalitis and Dengue cases in Churachandpur is inching towards the hundred mark, barely three months after the first case was detected in the district, according to data available to the press.

The JE virus was first diagnosed on May 10 with the latest accumulative figure recorded at 84 as of yesterday.

Sharp decline in Acute Encephalitis Syndrome, Japanese Encephalitis deaths: UP govt


Lucknow, Sep 1 The Uttar Pradesh government on Sunday said there has been a "sharp decline" in the number of cases and deaths due to Acute Encephalitis Syndrome (AES) and Japanese Encephalitis (JE) this year.
In a statement issued here, Devesh Chaturvedi, principal secretary, Medical, Health & Family Welfare Department said, "As many as 34 deaths have been caused due to AES this year (till August 27), while 890 patients of the disease have been admitted. As far as Japanese Encephalitis is concerned, only 4 deaths have been reported this year (till August 27), while 55 patients of the disease have been admitted."
