Bacterial disease is scrub [typhus]

This is a disease caused by bacteria. This bacteria occurs in the saliva of a particular type of insect. This [larva] lives in the body of large [mammals], rats or squirrels. It is also called [chigger]. When this [mite larva] bites small children, then bacteria through saliva get into the blood of children.

It's symptom

In this there is a high fever in children. The fever lasts for seven to 14 days. As the pain in the head and muscles goes away, the rash turns out on the body. In some cases the patient also comes in shock. As the disease progresses, it also affects the functioning of the vital organ in the patient's body. It can also cause death of the patient.

The main cause of encephalitis in the purvanchal is not scrub [typhus]. This theory was introduced to confuse the public and the government. Exact treatment of scrub [typhus] is present. If the claims of the researchers presenting this theory have diminished, then the number of deaths decreases.

A much-awaited study on the presence of Scrub Typhus in AES patients in 2018 is out. An exclusive report by Hindi daily Hindustan says that only five out of 235 AES patients have been found to have Scrub Typhus. This is merely 2 per cent positivity, which is in sharp contrast to the expected 65 per cent positivity.

The Hindustan report gives these details: A total of 245 encephalitis patients have been admitted to BRD hospital since January, out of which 80 have died. Samples of blood and cerebrospinal fluid of 235 patients were sent to the Gorakhpur unit of NIV. The report has found that 230 patients did now show presence of Scrub Typhus. The five patients who did, belong to Gorakhpur, Deoria and Kushinagar districts.

But now that the NIV findings have almost discredited the Scrub Typhus theory - at least as of now - critics are questioning why the government gave unnecessary antibiotics to patients at all. Dr R N Singh, a former BRD doctor and long-time campaigner against encephalitis, told Swarajya that he had always objected to the Scrub Typhus study because "it just didn't make sense".

"I was a part of the Indian Medical Association meet when the study was first presented. My argument was that Scrub Typhus mainly occurs in cool weather, so an epidemic isn’t quite possible in this region. But at that time, my arguments were ignored," Dr Singh told Swarajya over the phone.

Going by decades of experience, Dr Singh suspects Enterovirus to be the cause in 75 per cent of cases and has always emphasised on better sanitation as the solution. It is to be noted that AES is not a diagnosis, it is merely a clinical term for a wide spectrum of diseases that manifest as brain inflammation. Simply put, AES is a huge category of diseases and can be caused by any of the three dozen bugs including various viruses, bacteria and fungi.

The NIV report, however, doesn't settle the debate on Scrub Typhus, as per Dr Pushkar Anand, additional director at BRD. He told Swarajya over the phone, "The NIV report is not 100 per cent authentic. There are loopholes."

Dr Anand didn't elaborate on the loopholes saying it's "controversial", but told Swarajya that the hospital will continue to give antibiotics for Scrub Typhus. "There is still no consensus," he said, politely declining to speak further on the matter.

Lots of people skeptical about the scrub typhus hypothesis. But there was strong evidence. How do you say dramatic things like "falls flat on its face" based on one news report which *supposedly* cites NIV data. @SwarajyaMag

The District Medical Office issued a clarification later in the day, though earlier reports had said that Siyan, son of Harshad of Puthuppady, was the third victim of shigella infection in the State. Quoting lab reports of Siyan’s body fluid samples, the authorities said he had died due to acute encephalitis syndrome.

The 11 contractual workers of the PMR department take care of people with encephalitis, and other disabled kids. The department was established on 28th February, 2011, and 37 posts were created. But only 17 of these posts were filled, and only three of the people employed were doctors. After non-payment of salaries, three doctors and three others have left their jobs. These appointments were done on a contractual basis. Now, four therapists (a physiotherapist, an occupational therapist, a speech therapist, and a clinical psychologist), one steno, one prosthetic worker, and five fourth class employees are associated with the department.

Speaking to Newsclick, a therapist associated with the department, who chose to remain anonymous, said, "Since March 2018, we are getting our salaries, but we have not yet received the salaries from the year 2015, up to February, 2018. The central government had set up the PMR department in 2010 under a memorandum of understanding (MoU) with the state government, in which it was agreed that the department will deal with handicapped Encephalitis survivors and others disabled kids as well. In the beginning, all was going smoothly because the central government had funded the department but after one year (2012), the fund ceased."

He went on to add, "I, along with other workers, went to Lucknow to meet the Director General regarding our salary, and he sorted out this issue. We then received the salaries for two years (March, 2013-March, 2015). Till March 2015 we hadn?t received our salaries."

Comment: 12 total fatal cases have been reported in Kerala to July 25 in addition to 17 fatal cases due to Nipah, 6 fatal cases have been reported from Madhya Pradesh, 2 from Maharashtra, 1 from Gujarat and 1 fatal case from Tripura. I doubt that the figures from Uttar Pradesh are in any way up to date. The implication of 128 cases and 6 fatalities in the month of July is just highly unlikely. Last year there were 274 reported cases with 37 deaths from July 9-30. In 2016, there were 261 cases with 26 deaths from June 30 to July 29 etc. Media reporting has put the number of fatalities in the state at a minimum of 124 to July 24th. Whether the data is being manipulated or not for political purposes is moot, but it is worth noting that BRD hospital in Gorakhpur is no longer permitted to provide data to the media. Figures for West Bengal have not been updated. Thus the national toll is at a very minimum 292. - Ro

There is a shameful picture of discrimination from the girls in the Purvanchal. Two thirds of the girls have died in this year so far this year from encephalitis in the BRD Medical College of Gorakhpur. Even among malnourished children, daughters are 65 percent. According to medical science, daughter's disease resistance is higher than boys. Obviously, parents in the Purvanchal are ignoring daughters in catering and treatment. The medical college is going to start researching the cause of encephalitis due to excessive death of daughters.

NORTH LAKHIMPUR, Aug 8 - Japanese Encephalitis (JE) cases are on the rise in Lakhimpur with deaths of six persons since January this year. JE is a deadly disease characterized by inflammation of brain and high fever and is a type of acute encephalitis syndrome (ASE). The vector-borne disease is transmitted by culex mosquitoes.
According to National Vector Borne Disease Control Programme (NDGCT), 73 acute encephalitis cases have been reported in Lakhimpur district between Januarys to July this year, of which 43 have been found to be positive.

Updated Encephalitis figures from Assam, 874 cases of AES, 324 positive for JE with 66 fatalities confirmed JE. This is an increase of 101 confirmed JE cases & 14 JE deaths since July 31. The total number of AES cases has increased by 156 in that time. The article does not provide updated AES fatality information, but the number of additional AES fatalities has been running at about 50% of the confirmed JE figures. As a result, Assam has seen at a minimum 93 total AES fatalities and likely the number is approaching 100. - Ro

Encephalitis claims 66 lives in Assam

Guwahati: Japanese encephalitis has claimed 66 lives in Assam so far while the total number of positive JE cases in the state stood at 308.

Data provided by the National Vector Borne Disease Control Programme, Assam, revealed that Sonitpur and Kamrup are the two districts where the number of deaths from JE has increased significantly than in the past year.

The highest number of casualties has been recorded in Dibrugarh (13) followed by Kamrup (10), Sivasagar (8) and Sonitpur (7) till Thursday.

The toll during the same period in 2017 was 10 in Dibrugarh, four in Kamrup, 14 in Sivasagar and two in Sonitpur.

When I talked with the senior resident accompanying me, he said, ?Actually, AES is like an umbrella and meningitis comes under it.? I asked, ?So, why are you not labelling it as AES just like you are doing with other encephalitis patients?? He did not say anything.

Comparing with data from earlier, the mortality rate in 2016 was 25.80 percent while in 2017 it was 26.98 percent. This year it is 32.65 percent.

I'm also seeing a lot of reports today giving the total number of fatalities in UP as 103. This is just plain wrong. The NVBDCP had already reported 112 deaths by June 29th (see post #44 in this thread). The July report increased this to 118, but I think that is a bit low. Either way around 70% of cases and 30% of fatalities in UP are not occurring at BRD, I think that this may result in a lag in reporting as many of these pathways are new. Coupled to this is the fact that BRD has been forbidden from reporting case figures to the press and the stories going around that some AES cases are being renamed as meningitis in order to manipulate the case and fatality figures.
