Gorakhpur: A 10-bed Critical Care Unit has been prepared to save the lives of extremely serious patients of Insepholitis in BRD Medical College. Built at a cost of Rs 3.5 crore, this unit has high level facilities from ICU. Chief Minister Yogi Adityanath will inaugurate this.

Although the ICU facility is already available for serious patients of INSaphlitis in BRD Medical College, despite lack of some facilities, despite the efforts of lakhs, doctors could not save the lives of delicate patients. Now this problem will be solved.

Dr. Rajiv Mishra, Principal, BRD Medical College, expressed hope that the preparation of CCU will help prevent at least 25 per cent of the deaths. In this high level unit, no object or baggage can be taken without sterilized. Even doctors will have to change their clothes. Just like in the OT.

At least 60 children have died over five days at a government hospital in northern India that suffered oxygen shortages, officials said Saturday amid fears the toll could rise.

Authorities said they have launched an inquiry but denied reports that a lack of oxygen had caused the deaths at the Baba Raghav Das Hospital in Gorakhpur district in Uttar Pradesh state, which is ruled by Prime Minister Narendra Modi's Bharatiya Janata Party.

Indian media said 30 children died on Thursday and Friday after oxygen supply was disrupted in wards housing the sick, allegedly because the suppliers' bills were not paid.

Comment: The supplier warned on July 30 that service could be interrupted because as the hospital was in arrears to the tune of 57 million rupees ($91,000US). See post 87 in this thread. If it is true that the hospital allowed the oxygen service to be interrupted at this time of the year resulting in child deaths, this should be investigated and prosecuted as a crime. Year after year governments and other authorities promise that conditions are improving, but still every year hundreds of children die from a treatable condition due to lack of proper care. The death of more than 60 children in the span of a few days in a major referral hospital in Uttar Pradesh has jolted the conscience of the nation. This was an entirely preventable tragedy. It will take an independent inquiry to establish why children perished at the BRD Medical College in Gorakhpur between August 7 and 11. Such an inquiry should examine whether and to what extent the disruption of oxygen supply to those who were extremely sick was a cause for the deaths. That the two events were not entirely unrelated seemed to be indirectly confirmed with the frantic requisitioning of emergency oxygen supplies and the State government suspending the principal of the college.

Breeding of innocents in the Department of Pediatrics of BRD Medical College continues. During the last 24 hours, nine more patients died during treatment in different wards of the department. Five newborns are included in the deaths, while two innocents have died from Incephalitis.

The two patients who died from Incephalitis include Afsana Khatoon (16 years) daughter of Western Champaran Nuthuni Mia and Anuj (5 years) son of Hata Kushinagar, Rammilan. In medical college this year, 147 patients have died from Inphalitis. During the last 24 hours, nine new patients of Insefitis were admitted in Medical College. These include three patients from Deoria and Siddharthnagar, Kushinagar, Santakbir Nagar and Gorakhpur each. So far this year, 563 infections have been admitted for treatment. 70 patients are still undergoing treatment in Insefalitis Ward.

Gorakhpur: 15 deaths in 24 hours in BRD Medical College's Pediatric Disease and Medicine Ward These include 13 innocents. The dead include six patients of Incephalitis. These include Palak (2) of Salar Deoria, Sujit (3) of Gulariha, Sawal (3) of Ghogali in Maharajganj, Raju Sahni (17) of CampierGanj, Acta of Siddhartha Nagar (4) and Rabita (25) of Gauri Bazaar of Deoria ) Are included. During this 13 new patients were admitted. These include Deoria's three, three in Gorakhpur, four in Kushinagar, Maharajganj, Bihar and one in each of the slum areas. There are 114 patients in ICU, 181 children are being treated in PICU.

Monsoon fevers are de rigueur in Odisha. While swine flu and dengue are spreading in the urban areas, the tribal hinterland has been beset with outbreaks of encephalitis since 2012. Though no Japanese encephalitis (JE) mortalities have been registered as yet in the official records, 473 acute encephalitis syndrome (AES) cases have been diagnosed so far. Odisha stands 5th in AES mortality after Uttar Pradesh, Assam, West Bengal and Bihar.

According to the National Vector Borne Disease Control Programme, AES had taken 13 lives by the 1st week of August [2017]. Apart from AES, 8 persons have fallen victim to swine flu in Odisha in the last 2 weeks. More than 100 positive cases have been detected. Dengue is the other scourge in the state with 2 deaths and 367 positive cases.

Both AES and JE have been regular in Odisha since 2012. Last year, the state grabbed the national headlines when 115 children died of AES and 42 fell to JE. Most of the deaths were reported from Malkangiri district.

But the state government has been equivocating on the incidence of AES. It went to the extent of claiming that consumption of seeds of a plant called _Cassia occidentalis_, locally known as Bada Chakunda, caused death of a 3rd of the children who fell to encephalitis in Malkangiri district.

An expert committee suggested that JE could not be blamed for all the deaths because "encephalopathy" was the culprit. Encephalopathy, it said, is a non-infectious biochemical syndrome that can be aggravated by the consumption of _Cassia_ seeds. The fact is the Naveen Patnaik government remained aloof to the spread of AES, which is caused by a vector-borne virus hosted by pigs.

The Regional Medical Research Centre (RMRC), Bhubaneswar, carried out a study on the outbreak of AES in Malkangiri in 2015 but its findings were not paid heed to. From 38 deaths in 2012 to 115 in 2016, the AES surge was waiting to happen.

[As of the end of May [2017], there were 313 cases of AES with 10 deaths in Odisha and now the case count is 473 with 13 deaths in the 1st week of August [2017]. Odisha state is endemic for Japanese encephalitis (JE) virus, and cases occur there frequently. As with cases of encephalitis mentioned above and in other states in northeastern India, not all encephalitis cases are due to JE virus and are classified as acute encephalitis syndrome (AES) with undetermined etiology.

As noted in previous ProMED-mail comments, AES is a term used for multiple etiologies or often when no etiology has been determined. The encephalitis picture in northeastern India is complicated. Although JE virus etiology of some previously reported encephalitis cases has been confirmed by laboratory tests, many cases go undiagnosed, or JE virus infection is ruled out, and other etiologies, including Reye syndrome-like disease, possible enterovirus infection from polluted water, heat stroke, scrub typhus (_O. tsutsugamushi_) and lychee fruit consumption, have been proposed.

However, in November 2016 the Odisha state government formed the 6-member panel, led by eminent virologist Dr Jacob John from Vellore's Christian Medical College. In the interim report, Dr John said that anthraquinone, a toxin found in the plant [_Cassia occidentalis_], was found in the urine sample of 5 deceased children. "This indicates that the children (had) consumed the seeds of the plant," he said. Getting to an accurate diagnosis in these cases of AES will require active field surveillance with epidemiological studies supported by timely laboratory testing.

All the three doctors associated with the department have left it one after the other over non-payment of salaries.

The department is being run by 11 personnel, none of them doctors. They are four expert therapists, three technical hands, and four class four attendants. Even these 11 personnel have not been paid their salaries for the last 28 months.

?Towards the end of the central government?s term, it paid us outstanding (salaries) of 25 months in March 2015. By this time, the three doctors had already left. Now, we have outstanding (salaries) of 28 months,? said a staffer who did not wish to be named.

From April 2015 to July 2017, the total salary dues of the 11 staffers amounted to Rs 36,56, 800. ($58,500US)

NBT, Gorakhpur: In the BRD Medical College, there is a continuation of the death of innocents from encephalitis. In the last two days, four patients died in the Encephalitis Ward of the BRD. This figure of death is from 12 a.m. to 21st of August 19th, on the night of August 19. The dead include Kushinagar, Deoria, Basti and Bihar's one patient. According to BRD data, 157 offspring have died since January from encephalitis. At the same time 598 patients have been admitted.
