Japanese Encephalitis claimed life of one more child in Odisha’s Malkangiri today taking the death toll in the vector-borne disease to 89 in the district.

The deceased was identified as Surjya Madhi (2) of Mudakhala village under Korukonda block in the district.

According to reports, the child succumbed while undergoing treatment in the ICU of the District Headquarters Hospital.

Malkangiri: While the death toll in Japanese Encephalitis in Malkangiri district has gone up to 90 with the death of three more children at the district headquarters hospital today, a 12-member team including seven experts of the state Health department and the Centre has arrived in the district today to take stock of the situation.

Soon after its arrival, the team went to Daniguda, Badali, Palakonda and Doriguda villages in four groups for a ground survey.

?We have come to these affected villages to find out the causes of the outbreak of the vector-borne disease. We are interacting with the ASHA workers, Anganwadi workers, ANMs and the teachers. Since this is a preliminary survey, right now we cannot say about the exact cause of the outbreak of the disease. After the preliminary survey, we will have a scientific analysis,? Madan Mohan Pradhan, Joint Director, National Vector Borne Disease Control Programme (NVBDCP) told media persons here.

Meanwhile, the Encephalitis death toll in the district went up to 90 with the death of three children at the district headquarters hospital. While two girl children? Debika Pujari (2) of Chitapari village under Korukonda block and Shalini Khara (2) of Kandel village under Kudumulgumma block? died earlier in the day, a two-year-old boy Adama Mutaki of Motu village under Padia block died in the afternoon.

Bhubaneswar: With the death of three more girl child due to Japanese Encephalitis (JE) in Malkangiri district on Sunday, the toll has reached to 93. Meanwhile, disturbed with the rising death and failure of state government to curb the spread of the vector-borne disease the tribals have called Malkangiri bandh on November 8.

The deceased has been identified as Sushila Khara (3) a girl child of Tulibeda village under Korukonda block, Sushila Burudi (2) of Mathili and Basanti Jani (2) of Kumutiguda in Malkangiri district. So far the death toll has reached 93 and is still counting.

Notably, three children succumbed to JE on Saturday. So, far 290 cases of JE have been reported at district headquarters hospital out of which 140 have been tested positive , said sources. -

BHUBANESWAR: It may sound strange, but the government has not done any diagnostic test to ascertain the cause of death of at least 18 children, who died in Malkangiri district headquarters hospital since September 9, official records show.
From among the 76 deaths due to acute encephalitis syndrome (AES), laboratory tests confirmed Japanese encephalitis (JE) in 28, there was no test on 18. The tests (blood and/or cerebrospinal fluid) could not nail the virus in other cases. The average age of children who died is 3.2 years. Of the 282 children, who were admitted to the Malkangiri hospital with suspected AES syndrome so far, no laboratory test was done on 75 children while 135 were found JE positive.

Koraput Japanese Encephalitis toll increases to 11

KORAPUT/MALKANGIRI: The toll in Japanese Encephalitis has shot up to 11 in the last three months in the district with the death of a three-year-old girl, Gayatri Dalai of Kauriaguda village under Boipariguda block. She was undergoing treatment in the District Headquarters Hospital (DHH) where she died on Friday morning.

The number of children suspected to be suffering from the disease has also been on the rise with four more admitted to the DHH on Saturday. Of them, a 12-year-child Tufaan Charka of Ramgiri village under Boipariguda block was re-admitted. Tufaan was earlier admitted on October 28 with fever and discharged from DHH on November 1. The three others are Ajaya Pujari (4) and Bijaya Pujari (4) of Chotaguda village and Buduli Pangi (5) of Matikhalguda village under Boipariguda block.

At least 12 more children are undergoing treatment in the DHH for the last two weeks. As many as 65 children are in the DHH since September and of them, 50 are suffering from JE and Acute Encephalitis Syndrome (AES).

As many 32 JE affected children are under going treatment at the district headquarter hospital Malkangiri, five kids have been kept in the ICU of the hospital, sources said. On the other hand, the residents of the town those who have lost their confidence in the hospital performed yajnas at various temples praying to protect their children from the killer disease JE.

Bhubaneswar, Nov. 8: Laxmi Agali, 18 months, today died in Malkangiri taking the toll in the district due to Japanese encephalitis to 104 in 61 days.

Laxmi succumbed to the disease while undergoing treatment at the intensive care unit of the district headquarters hospital. At present, 35 children are undergoing treatment for the disease at the hospital.

Protesting against the state government's failure to check the unabated death of the children, the Zilla Adivasi Maha Sangha of Malkangiri district today observed a 12-hour strike. All shops downed their shutters and vehicles stayed off the roads voluntarily to show their solidarity with the families of the tribal people, who have lost their children over the past two months. The association demanded compensation of Rs 10 lakh to each of the families that had lost a child. It also demanded that the situation in Malkangiri district be declared a national emergency.

Malkangiri, Nov 14: 108 is now the total number of children who are dead due to Japanese Encephalitis in Malkangiri. The death toll increased when three more children named Ramesh Bisoi of Kataguda village under Padia block, and Muna Sodhi and Radhika Madakami of Korukonda blosk died on Monday at the district headquarters hospital while undergoing treatment.

It?s stated that the District Administration will conduct a door-to-door survey for three days starting from Nov 20 to prevent the spread of the disease. Also children between the age of 1 and 15 will be provided with vaccination from December first week. All these initiatives were decided at a meeting held yesterday under the chairmanship of District Collector.

Since January this year, BRD Medical College in 1776 have been admitted, of which 453 patients have been killed.

Death toll of tribal children due to Japanese Encephalitis and (JE) and Acute Encephalitis Syndrome (AES) rose 108 with four more deaths during the past 24 hours.

Meanwhile, experts have started to suspect that consumption of beans of Cassia occidentalis, locally known as ?bada chakunda?, may be reason behind continuing encephalitis deaths in the area. Official sources put the death toll at 91 till Sunday. Out of them 30 children were identified to be infected by JE virus while reason behind other AES deaths has not been ascertained.

Recently, the medical experts have blamed consumption of beans of ?bada chakunda? by tribal children. In this remote tribal district children usually consume beans of Cassia occidentalis plant which are found abundantly here. It is believed that these beans purify blood and kill worms in children.

Past studies have ascertained that consumption of these beans can cause severe acute childhood illnesses, AES with severe brain dysfunction other than JE. It can also lead to death. Experts of Central and State governments are also investigating this aspect.

The death toll due to Japanese Encephalitis disease, which spread like wild fire in Odisha's Malkangiri district, reached 115 with three more children succumbing to the disease on Monday while getting treatment in the district head quarter hospital (DHH). However, the official sources put the toll at 96 in the last 68 days.

At present 18 Japanese Encephalitis (JE) affected children are under going treatment at the DHH. 14 are in the JE special ward and 4 of them in ICU. According to the hospital sources, 325 kids have under gone treatment for JE at the DHH while 214 children have been discharged so far.

Comment: I'm not sold on the Cassia theory. Consumption of Cassia was shown to cause encephalopathy in Saharanpur, Uttar Pradesh. The big difference in UP was that many of the cases had been vaccinated for JE and few if any JE positives were found. This is not the case in Malkangiri where 65% of children tested were positive for JE. The ambiguity arises because many of the cases were not tested and a third were not positive. This does not mean that those cases are not JE. My questions: - why is this outbreak affecting mostly kids? In Assam where confirmed JE outbreaks are an annual occurrence, adult cases occur as a significant portion of the cases. In outbreaks that affect mostly kids, causes other than JE have been proposed, often linked to food toxicity such as in Muzaffarpur, Bihar and the Cassia cases in Saharanpur, and frequently very few JE positives are found. This outbreak in Odisha & neighboring Chhattisgarh seems to be predominantly JE. Are there other factors involved that are exacerbating this outbreak and causing such large child mortality. How much does malnutrition, diet, access to healthcare & clean water, and other environmental factors play into this tragedy? - Ro

One government hospital with nine doctors for 600,000 people ? and nearly 100 children dead
The non-existent public healthcare system in Malkangiri in Odisha is struggling with an encephalitis outbreak.

Not enough doctors
Underlying the deaths of children in Malkangiri is a weak and ineffective public healthcare system, plagued by the lack of hospitals, and the absence of doctors.

For a district spread over 5,971 sq kms, home to 660,000 people, Malkangiri has just one large government hospital with only nine doctors. As the only tertiary care hospital in the district, it is responsible for handling complicated cases that are referred to it from the primary and community health centres located in the villages.

The district hospital has 125 beds and 44 sanctioned posts for doctors of which 35 are vacant. Six posts meant for child specialists and intensivists trained in critical care have remained vacant since they were created in 2012.

Of the 103 sanctioned posts of doctors in the district, only 37 have been filled. Not just doctors, the district does not have enough nurses, the workers who form the backbone of the healthcare system. Only 43 of the 122 sanctioned positions for nurses are filled.

The district hospital in Malkangiri had just one paediatrician when the outbreak started in September. ?We immediately alerted the superiors and got some doctors to our hospital and started a paediatric intensive care unit,? said Dr KK Sarkar, joint director of health for Malkangiri.

Since then, a batch of six to seven doctors from other districts has been stationed in Malkangiri at any given time. Every two weeks, a new batch of doctors comes in.

The hospital had to borrow ventilators from a medical college to treat children with the infection. ?We are purchasing some monitors and ventilators,? said Dr US Mishra, the chief district medical officer of Malkangiri.

Dr Mohammad Hussain joined the community health centre in Kalimela in October. ?I was supposed to join here last year but I didn?t want to. It is a red zone,? he said, referring to the Maoist presence in the district. In the last week of October, 24 alleged Maoists were killed in a gunfight with the security forces. Government employees, including health workers, are reluctant to venture into the villages, even those that are far away from the arena of conflict. ?Government officers are always a target,? said Hussain.

Local politicians have claimed that more than 150 children have died in the district since September but government records put the death toll at 93.

Activists point out that only cases that have come to the district hospital have been recorded. There is little information coming from the villages across the reservoir, and even in the rest of the district, several cases may have escaped the attention of the health authorities.

?Many children don?t come to the hospital,? said Ashok Patnaik, a right to information activist from Malkangiri. ?Their deaths are not being taken into account.?

Yesterday, in a Press Release in Malkangiri, the District Collector and other local government officials made a statement that everything is under control and there is no need to panic. Since, five children have died. One of them is Shivani Podiami from Potrel Village. Shivani would've turned 7 in another two months.

Pooja Podanaik, aged 3, who's photo we shared a while back, has been shifted to the ICU after her breathing worsened. She's in a very critical condition. Her parents Rabindra Podanaik and Nabina Podanaik, from Potrel, very young themselves are distraught and hope their daughter returns from the ICU - dubbed as the deathbed here in Malkangiri Govt Hospital. Just a few hours back their neighbors daughter, Shivani Podiami, aged 7, was brought out dead from the very same ICU.

Potrel mourns the death of another child
Consisting of 90 families, Potrel is the first village in Malkangiri where children started dying with symptoms similar to what point to Japanese Encephalitis. 38 children died in 2011, following which every year, there have been more than a dozen deaths. Why then have none of the children been vaccinated yet in Potrel? Could Shivani Podiami's death have been averted? Had death of her adivasi brothers and sisters from her village been important enough for the administration five years back to swing into action, would she still be alive?

As you'll hear a senior activist from Malkangiri explain, these deaths are not sudden deaths. Adivasi children have been dying since '11, numbers reaching upto 1500 every year. Why haven't the children from the area been vaccinated yet then? The children belong to Porja, Koya, Halva, Bhumia, Matia, Durua, Gaduva, Kotia, Kondadura, (Adivasi) Didai, Bonda (PTG) Dom (Dalit), Rana, Goud (OBC) communities, all of them from socially and economically backward communities.
Sonu Anugu, a local adivasi activist, in a moment of desperation says "No, we don't matter, people can say what they want but lives of our children doesn't matter, they wouldn't be dying by the bulk if this wasn't true. They have accepted it as their fate and so have we. What else do we do anyway?". We know why the Government and the National media is silent, they are busy pretending to convene midnight meetings about demonetization while children continue to die in Malkangiri. But all our Ambedkarite leaders talking about Dalit Adivasi Bahujan Minority unity, today is Birsa Munda Jayanthi. Birsa's progeny, an entire generation is being wiped out. Where is the rage? When will the government to its knees like they've in the past few months? Is this call for unity we keep making in all our speeches and writing just rhetoric? The situation is beyond horrifying, we need to come together now.

More lies from the Malkangiri District Collector
The District Collector in his statement which started with 'Everything is under control' also mentioned that Anganwadis in every village is equipped to identify symptoms and offer free services in case of need to affected children. Almost all villages do not have a Primary Health Care center and the ones DC spoke to said they go to the Anganwadi when its open if their children are sick for basic medication. The state of the Anganwadi in Potrel looks quite the opposite of what the statement of the District Collector suggested. Looking at the state of this Anganwadi, one is one left to wonder how this Anganwadi, in a village where Japanese Encephalitis struck first, is equipped to handle anything at all.

Q: But majority kids have been dying because of Malaria since 2011? Are you sure? What about these new statements about AES, JE by the Collector then? And if the deaths are caused by Malaria, like you say, what measures have been taken?

Conclusion: Atleast, we know one thing for sure. No vaccinations since 2011.

Children are first admitted to a common ward at the Malkangiri Govt Hospital from where if their fever doesn't decrease or of they've incessant vomiting, they are shifted to the Japanese Encephalitis Ward.This doesn't mean they are getting any different treatment, there isn't any such treatment one is aware of, as far as one understands. From here, if their condition worsens, they are shifted to the ICU. All the beds in the JE Ward are occupied. the three beds which are empty were emptied yesterday evening, the children being shifted to the ICU. All three of them died later in the night.

Madkam Bire's son, Bheema Madkam, aged 7 has been in the ICU for seven days now. After admission into the ICU, he has largely been unresponsive, his mother says. He hasn't been responding to his mother, though he has been conscious every time she enters the ICU, she says. After this interview, Babula Khora who's son is also admitted inside and Madkam Bhire mentioned that all these medicines and injections that are beng pumped into the children are giving the children side effects. Just like how Madkam Bheema seems to be going deaf, Babula shared that this has happened with other children before too. One child accoriding to him, who was discharged the previous day, has completely turned deaf. "We see them taking out buckets and buckets of syringes, what are they pumping into our children", he asks.
He also mentioned that Kasturi Hantal, aged 5, who died the previous night at the ICU is survived by a younger brother who was brought along with her for admission. But her grandparents took him away as soon as soon as the doctors left. He stayed out of the hospital and has recovered. She didn't. Babula seemed to suggest that those who are admitted at the hospital had more chances of dying than those who refused to bring their children to the hospital.
The question one needs to be asking is when the District Administration has themselves made it clear either directly or indirectly that nobody knows decisively what is killing these children, what treatment are they being given then? Even if the children survive, though hardly anyone who makes it to the hospital seems to be coming out alive, what is the expertise of Doctors who are administering medication and overseeing treatment? Are they equipped to handle a disease which still has a cloud of confusion around it? When will the experts from AIIMS and other centers of excellence be wheeled in? After how many more deaths? All those loans which have been waived off by SBI to defaulters including Mallya post-demonetization, is this what was discussed in the midnight emergency meetings? How about discussing flying in medical experts or shifting children to a better facility immediately?

Rama Beti and Debe Beti from Kothaguda, Kalimala Block admitted their son five days back. Today morning, at around 9 AM, Debe was informed that his son was declared brain dead.
Shivani Podiami, Kasturi Hantal amd Seema Gui, all below the age of 7 were also declared brain dead today.

And Modi's government instead of declaring a medical emergency, have been using these deaths for political gain. It was almost 11 PM on 29.10,2016 when Irme Padiami and Sudarshan Padiami realized that the fevers their children seemed to have since the day before wasn't a normal fever. Susila Paddiami and Ramesh Padiami, 2 and 4 respectively hadn't eaten a morsel for two days now. Their temperature just kept going up and at a point it seemed like their bodies were on fire. When Sudarshan tried calling 108, he realised his phone had network issues. In Daniguda, if you are lucky, your phone will have network during the day. After 8 PM, it was almost impossible to venture out to a spot where your phone would connect. Inspite of desperate attempts, Sudarshan wasn't able to reach 108. Finally, at day break, he went out till the main road, made a call to 108 and within a couple of hours, his children were admitted at the Malkangiri Govt Hospital. In two days, both of them were dead. Though both the cases were diagnosed as Japanese Encephalitis, the authorities at the Hospital didn't know how to treat them, nor was their case escalated to higher authorities or to a better facility. Like many other adivasi children from the neighboring villages of Daniguda, both Susila and Ramesh lost their lives due to the State's historical apathy towards adivasi lives, even before they could hit the age of 5. The official figure of deaths attributed to Japanese Encephalitis since October '16 is at 112 now. The unofficial figure, local activists say, has crossed 300.

Nutrition determines survival chances
Seven out of ten children in Malkangiri are underweight, according to the 2014 Annual Health Survey report. The same report found that the district ranked third in the country among 100 districts that have the highest prevalence of underweight children under the age of five. About 33.4% of children in Malkangiri have low weight for height, which is classified as wasting and represents acute under nutrition caused by a lack of food. Wasting is predictor of mortality.

The district officials dismissed the possibility of malnutrition playing a role in the deaths of children who succumbed to Japanese encephalitis. ?Malnutrition was not established as a cause of death,? said Dr US Mishra, chief district medical officer at Malkangiri.

Another medical officer in the district, however, admitted that most children who died of Japanese encephalitis were malnourished. ?If I say this in the public, the government will not spare me,? he said, requesting anonymity. He added that many children would have been able to fight the Japanese encephalitis virus if they had been well nourished.

Toxic Beans Suspected Behind Odisha Deaths, Not Japanese Encephalitis

Toxic bean called Cassia Occidentalis is the suspected reason behind the outbreak.
Malkangiri, Odisha: Ninety six children have died in Odisha?s Malkangiri over a span of two months. While the cause of death is suspected to be Japanese Encephalitis, sources say that the technical experts, who are probing the outbreak, have some startling findings. The announcement for the same will be made on Friday.

Some experts, however, suspect that a toxic bean could be behind the deaths in Odisha as was the case in 2007?s Saharanpur deaths.

In 2007, at Saharanpur in Uttar Pradesh, 81 children died of Encephalitis-like symptoms. Later, a bean called Cassia Occidentalis was identified as the real culprit.

The Saharanpur expert team included Dr T Jacob John, professor emeritus of Christian Medical College, Vellore. He is also part of the Malkangiri study led by Madan Mohan Pradhan, joint director of National Vector Borne Disease Control Programme.

The outbreak has claimed lives of 96 children in Odisha?s Malkangiri.
While there will be more clarity to the cause on Friday, the current situation is not geared up to handle the current crisis.

Bhubaneswar: The central expert team constituted by government today claimed in its interim report that children deaths in Malkangiri are not only due to Japanese Encephalitis (JE), but also on account of Encephalopathy which is caused due to the consumption of ?Bana Chakunda? (Cassia Occidentalis).

Talking to media persons, the head of the team Jacob Jonn said ?Anthraquinone is the suspected toxin found in a plant called ?Bana Chakunda in Odia or Cassia occidentalis. We found toxin from the urine sample of a few tested children, which indicates the children consume the seeds of the plant?.

He added the urine sample of few kids of Malkangiri district was sent for examination to the Indian Institute of Toxicology Research (IITR), Lucknow. The report found the toxin in the urine of the children.

Comment: It seems that the outbreak is due to at least two causes showing once again that Encephalitis in India is complicated. JE vaccination alone will not solve the problem, but should be considered only part of a multi pronged approach. Removing the Cassia plants from the region will not help the underlying poverty, malnutrition & overall lack of healthcare in Malkangiri. - Ro

Gorakhpur:

Twenty-four hours in the Medical College of innocents were killed, two of encephalitis. The victims of Paniyra Irfan Maharajganj and Kushinagar's Padrauna Arpita. Both were 18 months of age.

Meanwhile, 12 new patients in the ward concerned college were recruited. 74 of these patients are still being treated here. 5 These Gorakhpur, Kushinagar 3, Sntkbir City 2, Maharajganj and Deoria 1-1 patients. So far this year, 1812 patients admitted to the medical college of 463 have died.

Comment: Uttar Pradesh has seen at least 48 deaths in facilities other than BRD raising the state toll to a minimum of 511 an increase of 10 in the past 5 days. - Ro
