As many as 46 people have died of encephalitis in a week [approx. 1-7 Sep 2014], and 7 in the past 24 hours [6-7 Sep 2014]. BRD Medical College statistics reveal that 991 encephalitis cases have been reported since January this year [2014], among which 291 have died and 184 are undergoing treatment in the encephalitis ward of the medical college. The mortality rate this year is the highest in 3 years. At present, the rate is 29.4 per cent. In the year 2011, only 18 per cent of encephalitis patients died; in 2012, the mortality rate was 20.9 per cent; whereas in 2013, it was 29.3 per cent. The increase in the last 3 years raises question on the claims made by the government.

The government is not following Holiya village model formula where the mortality rate came to zero. The chief campaigner of Encephalitis Eradication Movement (EEM) said: "In order to eradicate encephalitis, we have been demanding implementation of 10-point programme of NEEP in the country but till 2009. To show effectiveness of programme, we adopted a village of Kushinagar district [Uttar Pradesh state] which was badly hit by encephalitis with 5 deaths and 4 handicapped encephalitis victims and no one vaccinated till 2009. From 1 Jan 2010, we adopted Holiya village and implemented 10-point programme of NEEP, with a resolution that we'll make the village encephalitis free by 31 Dec 2010."

"Every year, more than 500 people die of encephalitis, and I'm sure this year [2014] will not be an exception. The government is spending so much money on vaccination program of JE [Japanese encephalitis] which is only 4 per cent left and not focusing on prevention of enteroviral encephalitis," he said.

The Holiya village formula is based on 100 per cent prevention programme, where pigs were kept out of the residential area and drinking water and hand washing water was disinfected by keeping it in plastic bottles under the sun for 6 hours, proper sanitation arrangements, use of mosquito net and arrangement of vehicles for taking the patients to hospital. Early vaccination programme is also the part of the Holiya village formula.

[No data are presented in the above report on the proposition of the 991 encephalitis cases in the area due to Japanese encephalitis virus (JEV) infections or from other causes. The Holiya village EEM mentioned encephalitis cases due to enteroviruses in contaminated water. Previous reports from that area also associated encephalitis cases with contaminated water. However, revisiting the comments made by Dr T Jacob John, he mentioned that cases in this area early in the year may be due to a Reye-like syndrome. In a 24 Jun 2014 comment (ProMED-mail archive no. 20140625.2563561) concerning the Reye-like syndrome, he stated that "the disease is undoubtedly hypoglycemic encephalopathy (with blood glucose levels below 40 in about half the children in whom testing was done), and lower than 80 in 80-85 per cent. The lesson was to give 10 percent dextrose infusion as soon as possible after drawing blood for glucose measurement. Pediatricians who did this tell me that children given 10 percent dextrose within a short period of onset tend to recover within a few hours and are well to go home the next day." In another comment, he noted on 3 Aug 2014 (see ProMED-mail archive no. 20140804.2656983), "I understand that West Nile virus is also present in that region, and some AES [acute encephalitis syndrome] may indeed be West Nile encephalitis." He indicated that JEV cases occur later in the year during the typical JEV transmission season. The above report provides no information about the JEV vaccination status of the population in the affected areas, nor the number of cases laboratory confirmed as JEV infections.


Comment - If there is one thing that I have learned about AES outbreaks in India, it is not to make generalizations! The first highlighted comment referred specifically to the area around Muzaffarpur in Bihar where malnourished children who eat less than fully ripe lychees may experience Reye's syndrome type symptoms that can be mistaken as encephalitis. There were very few JE positive samples this year. The outbreak peak was coincident with lychee harvesting season in June/July.
The second comment refers to West Bengal. Around 20% of the West Bengal cases have tested positive for JE this year and in Assam, around 40% were JE positive. Perhaps WNV was present to a small degree, but I have not seen any test results confirming that this year.
Uttar Pradesh, where the fatalities in the article occurred, may have an entirely different cause or a combination of several. The link to Muzaffarpur is that children are dying with similar symptoms, but 2 to 3 months later. They too are often poor and malnourished. Reye's syndrome is a possibility that, to my knowledge, has not been adequately investigated around Gorakhpur. But there must be some trigger other than lychees in this case. In the area around Saharanpur, UP, cassia beans played the role as trigger. In different locals, a different substance may play that role. The scanty testing regime has only picked up a handful of JE positives from the cases this year, but the program has been shown to be woefully inadequate and incompetent.
The theory of enteroviral infection from dirty water has been circulating for years without any real proof. As I mentioned earlier in the thread a CDC team is due to arrive on the 15th for even more investigation. - Ro

Japanese Encephalitis case: 9-yr-old's kin struggling to pay mounting bills

Ashrita Iyengar (9) has been hospitalised for more than a month; her cost of treatment of Japanese Encephalitis has already accumulated to Rs 6.70 lakh (about $11,000)

It has been a month since nine-year-old Ashrita Iyengar was hospitalised, suffering from symptoms of high fever, nausea, headaches and brain inflammation, and a fortnight since she was diagnosed with Japanese Encephalitis (JE), the city’s first patient in two years.

Despite the treatment by doctors at the Hinduja Hospital, the Kalina-resident continues to be on ventilator support. JE, which affects the central nervous system, has left the patient’s limbs paralysed and as her treatment continues, her family is worried how they will manage to pay off the extensive hospital bills.
“She has been on ventilator support from the first week of her hospitalisation when she was admitted to the ICU on August 9. While right now she has been shifted to the general ward, she still needs oxygen support and is not able to even open her eyes,” said Nagesh Iyengar, her father. Doctors are currently trying to take her off ventilator support to encourage mobility.

Gorakhpur: The team of American specialists of CDC (center for disease control), on the second day of their visit to Gorakhpur, went to BRD Medical College and met the patients of ward no. 5, 9 and 12 and also checked their case sheet.

The team includes Dr James A Mercy, Dr Padmani and Dr Mayank Diwedi from CDC Atlanta, Dr Ravi Yadav and U Ravi from Bangalore. Dr Anil Mishra, GM, national health mission, state encephalitis officer Dr MK Gupta, district Malaria officer Dr AK Pandey and Dr Mahima Mittal were also in the team.

To the Editor: Kakkar et al. recently concluded that the low-quality surveillance data on acute encephalitis syndrome (AES)/Japanese encephalitis (JE) in Kushinagar District, India, provide little evidence to support development of prevention and control measures and to estimate the effect of interventions (1). Analysis of the surveillance data, however, does provide evidence supporting the effect of an ongoing intervention (i.e., JE vaccination).

In accordance with the surveillance protocol, cerebrospinal fluid and/or serum samples from AES patients admitted to the Baba Raghav Das Medical College (Gorakhpur, India) are tested for IgM against JE at the field laboratory of National Institute of Virology (NIV) at Gorakhpur (2). The samples are tested by using the ELISA developed by NIV Pune (Pune, India), which has a specificity of 85% (range 77%–95%) and sensitivity of 71% (range 71%–75%) (3). Patients with samples negative for JE are considered to have JE-negative AES.

We obtained the line-list of AES patients from the NIV laboratory at Gorakhpur for 2008–2012. Analysis of the surveillance data indicated that 251 (8.2%, range 4%–14.7%) of the 3,047 AES patients from Kushinagar were positive for JE IgM and therefore considered to be JE case-patients. JE incidence per 100,000 persons in the district declined from 2.3 cases in 2010 to 0.81 in 2011 to 0.58 in 2012 (Figure). The decline in JE incidence since 2010 could be a consequence of JE vaccination activities in Kushinagar. In 2010, a mass vaccination campaign with 1 dose of JE vaccine (SA 14–14–2 strain) was conducted among children 1–15 years of age. Subsequently, the vaccine was introduced into the childhood vaccination program as a 1-dose strategy in 2011 and a 2-dose strategy in 2013. Unfortunately, information about evaluated coverage of JE vaccine is not available from the district. On the other hand, the average annual incidence of JE-negative AES during the same period was 16 cases per 100,000 persons (95% CI 14.8–17.2), and this incidence has remained relatively stable since 2008.

With the isolation of enteroviruses from JE-negative AES patients, waterborne transmission has been hypothesized, and the focus of intervention has shifted toward improving sanitation and water quality. However, enteroviruses were detected only in a small proportion of AES patients. Although the quality of AES surveillance needs to be improved, as Kakkar et al. suggested (1), further studies are needed to understand the etiology of JE-negative AES in the district and the risk factors for transmission. These studies might include systematically investigating patients and environmental samples for enteroviral and other etiologic agents.

Encephalitis virus in Kushinagar looking for a team of American scientists

Gorakhpur. Purvanchal Inseflaitis and her continuing to explore deaths occurred due to the CDC (Centers for Disease Control) in Atlanta, the American team of scientists is working on the third day of his campaign. Inseflaitis team BRD Medical College Kushinagar visiting with children suffering from the disease related details collected at ground level.

Kushinagar team of American experts arrived on Thursday. PHCs and CHCs village of Kushinagar affect the team on the field where the most number of patients in the BRD Medical College Inseflaitis come. The team was in some affected villages and interacted with the local people or the victims' families. The team took samples from many places.

National Institute of Virology and investigation team also took stock of the arrangements. Last Wednesday, the team reached the district hospital and encephalitis with Saisi talks on the issue.

Inseflaitis claimed six more lives

Inseflaitis recruitment of patients and deaths continues. BRD Medical College in the last twenty-four hours, while in the nineteen new patients were admitted and six others were killed. Nehru admitted 220 patients are being treated in hospital.
Those who were admitted to five Gorakhpur, Kushinagar six, three in Bihar and Deoria, Maharajganj, Sntkbirnagr, Ballia and Siddharth are in one patient. Medical College from January till the year 1235, of which 351 patients have been admitted have died.

Sources said since January this year, 1258 patients were admitted to the hospital for the treatment of encephalitis of which 359 died. (This figure is BRD only, the toll is at least 383 counting other Gorakhpur hospitals - Ro)

Sources said 23 new patients were admitted today in the hospital whereas 215 patients were still treated.

Of the total deaths, 39 were the natives of Bihar while one was from Nepal.

Inseflaitis suffering from BRD Medical College in the past 24 hours, killing eight people. While 23 new patients were admitted. The 215 people currently being treated in the ward concerned. From January 1258 to now die of this deadly disease has killed 359 people. These figures are just the medical college. So the suffering and the death toll is more than this.
Among patients who died in the past 24 hours Kiran (18) Kushinagar, Karim (5) Siddharth, Pushpa (18), Bihar, Asif (2) Suresh (3) Maharajganj, Sky (3) and Shivanshu (13), Gorakhpur, fragrance (9) include Sntkbir city.
Sntkbir patients recruited in the city, one of Gonda and Siddharth, Gorakhpur, 6, 7 Kushinagar, Maharajganj 3, 4, Deoria.

10 new cases of JE

JAMSHEDPUR: City Japanese Inseflaitis (JE) cases are growing rapidly. What MGM Medical College on Saturday investigated a total of 8 samples were confirmed JE in an 0. Three of these patients have been treated at TMH, there are seven Admit patient at Mercy Hospital. Fbr the total of JE cases in the district have arrived so far. Also killed were two JE patients. He also is running an awareness campaign among the people.

The three people who killed 46-year-old Israwati Sntkbirnagr them, fifteen-year-old Gorakhpur hum of the half-year are playful and Bihar. Those who were admitted to Gorakhpur and Kushinagar, one of three patients and Sntkbirnagr and Siddharth. In January this year the BRD Medical College in 1266, of which 361 have arrived to treat the patient deaths. (385 including other Gorakhpur hospitals - Ro)

Among the eight people were killed Anup four years of Gorakhpur, Deoria Ranoo twelve-year, half-year and five-year-old flame control, six-year-old Ravi Kishan Maharajganj, Kushinagar and thirteen-year-old lore and demolition of five years of Bihar There are ten-year-old Rubina. Those who were admitted to five Gorakhpur, Kushinagar four, three of Ballia, Deoria and Maharajganj two, Siddharth and Bihar including one patient. BRD Medical College from January of this year so far has recruited 1283 patients, 370 of whom were killed. (394 including other Gorakhpur hospitals - Ro)
