Hong Kong: Revised clinical management guidelines for human swine influenza (7/20/09)

The following is issued on behalf of the Hospital Authority:

The Hospital Authority (HA) will revise the clinical management guidelines for patients in relation to human swine influenza (HSI) to target patients with high risk factors following the emergence of more severe cases and an increasing number of HSI cases in the community.

"The HA expert group which met last weekend recommended more active treatment for influenza-like illness (ILI) patients who develop features of pneumonia or signs of deterioration even when the test result of human swine influenza is not available or negative initially," the spokesman for the HA said today (July 20).

"The expert group advised that, in addition to chronic diseases and immuno-compromised state, pregnancy, current smokers, obesity, babies and children below six years old and persistent symptoms beyond 48 hours are also risks for developing severe symptoms.

''Pneumonia patients with such risk factors will be given antiviral treatment at the earliest possibility."

For out-patients attending HA's Designated Flu Clinics (DFCs) who have been confirmed with infection of HSI, the clinic will inform patients and prescribe antiviral drugs.

"As antiviral drugs may have more serious side-effects on pregnant women and children below the age of one, doctors will discuss with patients or family and prescribe medication only with their consent.

''To minimise the side-effect on pregnant woman, Relenza spray will normally be recommended," the spokesman added.

"Furthermore, laboratory tests also will be conducted for all community-acquired pneumonia (CAP) patients.

Repeat investigations will be conducted for CAP in-patients if the initial test shows negative results.

The enhanced laboratory tests will facilitate early confirmation of HSI patients and hence, commencement of antiviral treatment to prevent severe complications."

The HA today also announced that the operational arrangements of its eight Designated Flu Clinics will be slightly adjusted in August.

"The DFCs will continue to provide service from 9am to 5pm, seven days a week. However, to address the needs of chronic disease patients, the eight designated clinics will resume service for follow-up consultation for the chronic patients beginning on August 17," the spokesman said.

"Proper segregation will be put in place to prevent cross infection between the two types of patients. There will be two designated consultation rooms for the management of ILI patients in each DFC. "As the summer flu peak season will be here for some months, we have to ensure early treatment for ILI patients without compromising the ongoing clinical service for other patients with chronic conditions," the HA spokesman explained.

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority (HA) spokesman provided the following updates today (July 20):

The Designated Flu Clinics today (as at 5pm) provide treatment to a total of 707 patients.

The HA spokesman reminded the public that the eight DFCs have ceased the provision of general outpatient services.

Patients with other illnesses are advised to seek medical treatment at other general outpatient clinics in the district or private practitioners.

Chronic patients who have been pre-scheduled for follow up at the eight DFCs should proceed to their corresponding clinics with drug refill service according to their date of original appointment and bring along the appointment slip and Identity Card.

As at 2.30pm today, there were 76 newly confirmed cases of human swine influenza in the past 24 hours.

This brings to 1,886 the total number of confirmed human swine influenza cases in Hong Kong.

Currently, a total of 41 confirmed cases are staying in public hospitals for treatment.

Among of them, 36 cases are in stable condition (including two patients of Tuen Mun Hospital, a 34-year-old man and a 53-year-old woman, who were earlier in serious condition), and the other five in critical condition.

Furthermore, the confirmed cases announced today included a male nurse of Kwai Chung Hospital. He is isolated at home and under Tamiflu treatment, with stable condition.

(about the guidelines for school closure)

Secretary for Food and Health:
As the Controller of the Centre for Health Protection has stated, the criteria of 10% is a guideline.

It is a soft guideline.

We will have to apply it to individual cases, whether the 10% occurred in one day or progressively within two or three weeks has a very big difference.

And also in a big school with over a thousand students, the 10% is actually over a hundred people.

For a small school or kindergarten, they perhaps only have less than a hundred students, so 10% is just a few children.

This is a very different comparison.

So what we intend is that it is just a guideline. We will look at individual cases to decide on whether individual institutions need to be closed.

As far as the management of the schools or kindergartens is concerned, I would urge that they should discuss with the Centre for Health Protection before they make any decision on closing the school.

(about publicising human swine flu figures)

Secretary for Food and Health:
The main reason is we still want to monitor the progress of this epidemic in Hong Kong. The WHO has decided not to publish those figures because many countries are unable to produce the figures on a regular basis, particularly on a daily basis. If you look at the figures from WHO, very few places like Hong Kong can actually give daily statistics to WHO. But as far as Hong Kong is concerned, we feel this is important that we monitor the system. But we have also decided that in due course when we are more concerned about the number of serious cases, the cases that are hospitalised and cases of fatality. The time will come that we will switch over to this, but at the moment, we feel that since the peak of the epidemic is still going on, it is important for us to have a daily figure.

(about time spent on dealing with human swine flu and mental health services)

Secretary for Food and Health:
Not really. We have a very balanced strategy regarding various health issues in Hong Kong. Of course, we need to address the crisis of human swine flu because it is a pandemic and it is also something that will affect all societies. As I have mentioned earlier, it has already affected 135 countries. And in some European countries, they are still feeling the impact right now. The United Kingdom has also emphasised that they expect this will last for much longer than they first expected. So these are things that we need to be concerned. As far as mental health is concerned, it is always a continuous issue that will affect our health services. We understand that there is an increasing demand for services. We have already used the resources in this aspect every year. Of course, we would like to focus on some of the areas that we feel that can be improved further in the coming year.

(about when the pandemic will be over)

Secretary for Food and Health:
This is a very difficult question. First of all, nobody knows when this pandemic will end. Most of the pandemic would slowly dwindle away, with the increase of the herd immunity of the whole community. For human swine flu, I think it will take really a long time. That is why we think it is important for us to look at various protective mechanisms that we can introduce to our community. Getting appropriate vaccine is one of them. To ensure that we adopt a very high vigilance against infectious disease is another. We also make sure that we have the treatment required for this virus at the moment. With the stockpiling of Tamiflu and Relenza, I think we are well equipped for that purpose. But for the prediction of this pandemic, I think we have to rely on the experts' opinions from various sources. So far, nobody can predict exactly when it will end.

I think since we have made a lot of preparation, I am quite confident that we may be able to get vaccine. But it is important to note that it is not just a vaccine, we have to make sure that the vaccine is qualified for all the safety and quality standards that we have imposed. If most of the vaccine companies can produce vaccines that are of such high quality and safety, then I am sure that we will be able to get some.

Hong Kong: CHP investigating a case of Legionnaires' Disease (7/21/09)

The Centre for Health Protection (CHP) of the Department of Health is today (July 21) investigating a case of Legionnaires' Disease involving a 70-year-old man.

The patient, living in Yuen Long, developed cough and diarrhoea on July 14. He was admitted to Tuen Mun Hospital on July 16 and is now in stable condition. The CHP's investigation showed that the patient had no travel history before the onset of symptoms. His family members are asymptomatic.

This is the 16th Legionnaires' Disease case reported this year. 13 cases were reported in 2008, 11 in 2007 and 16 in 2006.

To prevent the disease, people are reminded to observe the following:

  • An individual may reduce his/her susceptibility to the disease by avoiding smoking and excessive drinking, and maintaining good health.
  • People handling garden soils, composts and potting mixes should take the following precautions:
    • Gently water gardens and composts using low pressure.
    • Open composted potting mixes slowly and make sure the opening is directed away from the face.
    • Wet the soil to reduce dust when potting plants.
    • Avoid working in poorly ventilated places such as enclosed greenhouses.
    • Wash hands carefully afterwards.
  • Proper design, operation and maintenance of man-made water systems. The newly confirmed serious case announced today was a 50-year-old male patient of Tuen Mun Hospital.

He is a smoker with chronic disease. He attended the Designated Flu Clinic on July 20 due to fever and flu symptoms. The patient was admitted to the isolation ward and is under treatment with Tamiflu and antibiotics. The hospital will continue to closely monitor the patient's condition.

Furthermore, the confirmed cases announced today included a female nurse of United Christian Hospital and a male clinical service assistant of Queen Elizabeth Hospital.

Both are isolated at home and their conditions are stable.

Only the nurse is under Tamiflu treatment.

Hong Kong confirms 127 new cases of A/H1N1 influenza

HONG KONG, July 22 (Xinhua) -- A spokesman for Hong Kong Department of Health said there had been 127 newly confirmed cases of A/H1N1 influenza in the 24 hours to 2:30 p.m. (0630 GMT) Wednesday in Hong Kong.

The new cases involve 64 males and 63 females, aged between nine months and 67 years, the spokesman said.

This brings the total number of A/H1N1 influenza cases to 2,091 in Hong Kong, said the spokesman.

The patient was confirmed human swine influenza last night. He is staying in the isolated ward and under medication treatment of Tamiflu. The hospital will continue to closely monitor his condition.

Furthermore, the confirmed cases announced today included a female community nurse of Our Lady of Maryknoll Hospital. She is under Tamiflu treatment and isolation at home, with stable condition.

The patient was transferred to Intensive Care Unit yesterday due to deterioration in condition, and medication treatment of Tamiflu and antibiotics was started. The patient now requires intubation and ventilatory support. The hospital will continue to closely monitor her condition.

Furthermore, the confirmed cases announced today included a female clerk of Pamela Youde Nethersole Eastern Hospital and a male staff at HA Head Office.

Both are under Tamiflu treatment and isolation at home, with stable condition.

Currently, a total of 43 confirmed cases are staying in public hospitals for treatment.

Among them, 36 cases are in stable condition (including a 50-year-old male patient of Tuen Mun Hospital, who was reported earlier in serious condition), one in serious and six in critical condition.

The newly confirmed cases announced today included a female nurse of Tuen Mun Hospital. She is now under isolation at home and in stable condition. She does not require Tamiflu treatment.

Hong Kong: 160 new cases of human swine influenza (7/25/09)

A spokesman for the Department of Health (DH) said there had been 160 newly confirmed cases of human swine influenza (Influenza A H1N1) in the 24 hours to 2.30pm today (July 25).

The new cases involve 77 males and 83 females, aged between one and 72.

This brings to 2,512 the total number of human swine influenza (HSI) cases in Hong Kong.

Meanwhile, DH is investigating a suspected HSI case involving an 81-year-old woman living in an elderly home in Yuen Long. The patient developed fever and cough on July 23 and was admitted to Tuen Mun Hospital the following day. Her condition is stable. The specimen taken from her was tested positive to HSI by Tuen Mun Hospital today.

Confirmation by the DH's Public Health Laboratory Centre is still pending.

DH's medical personnel had visited the institution and provided health advice, the spokesman said.

''So far, there is no influenza-like illness outbreak in the elderly home and tamiflu has been given to close contacts of the patient as antiviral chemoprophylaxis.

''The department will continue medical surveillance for staff and residents of the elderly home,'' he said.

Hong Kong: HA update on Designated Flu Clinic and admitted human swine influenza patients (7/25/09)

The following is issued on behalf of the Hospital Authority:

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority (HA) spokesman provided the following updates today (July 25):

The Designated Flu Clinics today (as at 5pm) provide treatment to a total of 547 patients.

The HA spokesman reminded the public that the eight DFCs have ceased the provision of general outpatient services.

Patients with other illnesses are advised to seek medical treatment at other general outpatient clinics in the district or private practitioners.

Chronic patients who have been pre-scheduled for follow up at the eight DFCs should proceed to their corresponding clinics with drug refill service according to their date of original appointment and bring along the appointment slip and Identity Card.

As at 2.30pm today, there were 160 newly confirmed cases of human swine influenza in the past 24 hours.

This brings to 2,512 the total number of confirmed human swine influenza cases in Hong Kong.

Currently, a total of 39 confirmed cases are staying in public hospitals for treatment.

Among them, 32 cases are in stable condition, one in serious and six in critical condition.

Furthermore, the newly confirmed cases announced today including a male doctor of Tuen Mun Hospital and a female Technical Service Assistant (TSA) of United Christian Hospital.

They are now under isolation at home and in stable condition.

The TSA requires Tamiflu treatment.

Hong Kong: United Christian Hospital fatal case of human swine influenza (7/27/09)

The following is issued on behalf of the Hospital Authority:

In response to the death case of human swine influenza of a Filipino female patient, the spokesperson of United Christian Hospital today (July 27) made the following update:

The Filipino woman attended Accident and Emergency Department of United Christian Hospital on July 7 night. She was directly admitted to Intensive Care Unit and with severe pneumonia. Admission Screening reviewed that no MRSA. The patient has been on Tamiflu and antibiotics treatment. She was in critical condition requiring mechanical ventilation. Relenza was tried but stopped because of patient intolerance. A life support system (Extra-corporeal membrane oxygenation system) was put on July 13. Her condition continued to deteriorate and she was certified dead at 17:47 today (July 27). The case has been referred to Coroner. We have informed her family, employers and the Consulate General of Philippines immediately. We would like to send our condolences to her family.

Hong Kong: Fatal case of human swine influenza recorded (7/27/09)

The Centre for Health Protection (CHP) of the Department of Health has received notification from the Hospital Authority that the 37-year-old woman Filipino patient who was confirmed to have infected with human swine influenza (Influenza A H1N1) on July 11 passed away today (July 27).

The woman arrived in Hong Kong on June 28 and developed fever, sore throat, running nose and diarrhoea on June 29. She was admitted to the Intensive Care Unit of United Christian Hospital (UCH) on July 7 in critical condition. The case has been referred to the Coroner by UCH for investigation. CHP will report the case to the World Health Organisation.
