18 days on ventilator, woman delivers baby

PUNE: A pregnant woman who was battling for life after suffering from swine flu delivered a pre-term baby through the normal procedure, a case which doctors at the hospital where she was admitted term as a near-miracle.

The 28-year-old woman from Nashik, Rupali Tathe, had developed the acute respiratory distress (ARDS) syndrome while suffering from swine flu and was put on a high-frequency ventilator at the Deenanath Mangeshkar Hospital (DMH). It was in this state of virtually no consciousness that she delivered the baby.

"ARDS is characterised by very low levels of oxygen in the blood due to extensive lung damage," the hospital's consultant intensivist and pulmonologist B Y Pawar, who managed the critical care of the patient, told TOI. "The conventional ventilatator proved insufficient to maintain an adequate level of oxygen in Rupali's blood, following which she was put on a high-frequency oscillatory ventilator (HFOV). It took 46 days for her to recover from ARDS. She was discharged on November 19. Both mother and child are safe."

Rupali showed typical symptoms of swine flu sneezing, running nose, sore throat, high grade fever, cough and difficulty in breathing. "She was treated at a private nursing home in Nashik. When her condition deteriorated, we shifted her to DMH," said her brother-in-law Rahul Londhe, a medical practitioner.

Pawar said Rupali was admitted to the hospital on September 22. "She was seven months pregnant at the time of admission. She delivered the baby boy on October 10, the 18th day of being on the ventilator. She was not aware of the labour as she had been sedated," he said.

"The patient had gestation of only 28 weeks at the time of delivery as against the normal 37 weeks. But we felt the delivery might actually help her poor respiratory condition," said gynaecologist Pratibha Kulkarni, who handled the delivery.

Kulkarni said attempts at inducing labour were not successful initially. "But she started getting labour pains after we administered inducing drugs through the intravenous route. She delivered the baby through the normal procedure. She was not fully conscious at the time of delivery," she added.

Paediatricians Arti Rajhans and Rashmi Gapchup said the baby weighed around 1,300 grams at the time of delivery. "The boy required neonatal intensive care for three weeks. He was then shifted to a regular ward for another two weeks."

Flu attacks Mumbai with a vengeance

MUMBAI: With the sudden drop in temperature, the city is reeling under a vicious attack of sniffles and sore throats, and general practitioners are busy fielding calls from citizens worried about whether this is a resurgence of the dreaded swine flu. The H1N1 strain has seen a resurgence in certain parts of the UK, and experts there have linked the comeback of the pandemic virus to the onset of winter. Back home, though, this does not seem to be the case?so far. According to state health officials, the H1N1 virus has claimed two lives in Maharashtra this month and only five people have tested positive. In November, two swine flu deaths were recorded and less than 10 positive cases were identified. Mumbai has not recorded any swine flu.

Dr Abhay Chaudhary, the director of Haffkine Research Institute at Parel, believes the culprit behind Mumbai's sniffles is the relatively milder B strain of influenza (see box). "We are studying samples sent to us by hospitals on a weekly basis, and but the H1N1 strain is absent. Influenza B could be the cause of some of these ailments," he said, adding that the weather, too, is conducive towards the transmission of air-borne viruses.

Common complaints among Mumbaikars queuing up at out-patient departments across city hospitals are sore throats, persistent coughs and body aches. "There is no doubt that a flu-like strain is in circulation. Bronchitis after an initial bout of influenza is more rampant," said Dr Om Srivastava, infectious disease consultant, Jaslok Hospital. Dr Jayanti Shastri, head of microbiology at Nair Hospital concurred and said the symptoms indicate upper respiratory tract infection.

Children are the worst-affected, and city paediatricians are flooded with patients complaining of breathlessness. "There has been a 25% rise in cases of respiratory ailments in the past seven days," said Dr Vijay Yewale, convenor of immunisation committee, Indian Academy of Paediatrics. "Though most of it is self-limiting, vaccination is a good option for children with pre-existing lung or cardiac problems." According to the BMC, around 200 people are being admitted every day for fever. This does not include out-patient treatment. The head of epidemiology cell of the BMC Dr Daksha Shah assured citizens that "winter ailments are under control."

Swine flu makes comeback, three dead this month

PUNE: The swine flu virus has made a comeback after a considerable gap, claiming three lives in Pune so far this month. Only one swine flu casualty was reported in November in the entire state. The National Institute of Virology (NIV) also confirmed a slight revival in the infection rate, which had come down to zero just a couple of weeks back.

Experts attribute the return of the virus to the cold weather. State epidemiologist Pradip Awate said, "The swine flu virus has become endemic in India - it has permanently settled here like the seasonal flu. Therefore, sporadic swine flu cases and casualties are likely to be seen due to seasonal variations."

The infection rate of the H1N1 influenza has shown a slight revival since the start of this month. "There has been a one or two per cent rise in the infection rate. We receive around 20 samples every day. One or two of these test positive for H1N1," said NIV deputy director Mandeep Chadha.

The infection rate had gone down to zero in November, the first time since June 2009. None of the swab samples received by NIV during that month tested positive. Incidentally, the infection rate was at an all-time high between mid-July and September this year.

State-wise, there were 78 fresh cases of infection in October and 13 casualties. In November, there were only four fresh cases and one death which occurred in Aurangabad.

One of the three swine flu victims in Pune so far this month was a 16-year-old boy from Nana Peth, Shubham Jadhav, who developed symptoms and initially took treatment at the PMC's Kamla Nehru Hospital and Naidu Hospital.

However, when his condition deteriorated, he was admitted to Sassoon Hospital and put on the ventilator. He died on Friday.

The two other casualties were a three-month-old boy from Gulbarga in Karnataka, who died at Sassoon, and Ahmednagar resident Kisan Gambhire (65), who died at Inlaks and Budhrani hospital.

According to the daily State reports there were 5 positive cases in November, and 4 in the first 13 days of December. The numbers are reproduced above earlier in this thread.
