A 40?year?old woman died of H1N1 influenza at Jehangir hospital on Tuesday. Kalpana Balasaheb Jadhav, a resident of Khanapur in Satara district, showed symptoms of fever, diarrhoea and vomiting since October 3 and was admitted to the hospital on October 5.

This is the third Swine Flu casualty this month. Earlier, Vaishnavi Vaibhav Bhaskar (18), a resident of Junnar and Suman Govind Kalve (78), of Dhankawdi had succumbed on October 1 and 3 respectively at the Sahyadri Munot hospital.

There have been 426 swine flu deaths in Pune district since August 2009.

Tamiflu not working, docs want to import shots


With temperatures set to dip, and a very real possibility of a swine flu resurgence staring at its face, the state government?s H1N1 advisory committee has sent a recommendation to the centre to import Paromovir, an injectable drug for swine flu.



An audit by the committee suggested that Tamiflu, the widely used curative oral drug has failed to arrest death in high-risk patients and those who received delayed treatment.

The committee?s observations stated that majority of deaths in Maharashtra including Mumbai have been a result of co-morbid conditions in patients or delay in treatment.

?Such patients have not responded to Tamiflu and have eventually succumbed,? said infectious disease specialist Dr Om Shrivastav who is part of the advisory committee.

According to Shrivastav, the injectable drug may help reduce the number of deaths in the state, which is still higher than other parts of the country. The state has recorded 4677 cases of swine flu and 518 deaths since April 1 this year.

The state?s recommendation to the centre mentioned that importing the H1N1 injection may help in saving many lives.

Even as the swine flu pandemic was declared over by the World Health Organisation (WHO), the state fears the virus is very much in the air and may hit back during winter.

In fact, what has been worrying halth experts is the recent rise in swine-flu deaths in the state, especially in rural Maharashtra.

The recommendation also says that doctors have observed that the flu was resistant to Tamiflu in many cases.

?The high risk groups such as patients with existing ailments like pneumonia, asthma, respiratory disorders etc, did not show much improvement with Tamiflu.

Also, patients who were put on treatment late did not respond to Tamiflu,? said Dr Pravin Shingare, joint director, Directorate of Medical Education and Research (DMER) adding that only the injectable drug will work in such patients.

Paromovir is a drug from the same group as Tamiflu. However, it is considered to be an aggressive cure for high-risk patients.

At present, Japan and Europe are using Paromovir and they have given positive reports about the drug?s effectiveness in reducing the death rate.

According to Shrivastav, India does not have a curative injectable drug for swine flu yet. ?We only have the oral drug and a nasal spray in the market. Except for the vaccines that are only meant for preventive purposes, there are no curative injections,? he said.

Sources said the health ministry is studying the recommendation at the moment and is examining the drug?s feasibility in the Indian market.

14 govt hospitals to get 36 ventilators for H1N1 patients

The government has ordered 36 ventilators for 14 government hospitals across the state to prepare for a possible swine flu (H1N1) wave in winter. According to experts, virus outbreaks are common during monsoon and winter. ?The total dynamic of the influenza viruses is such that their
increased transmission occurs during the period when the general climatic temperature is low or humid,? said Dr Abhay Choudhary, director of Parel?s Haffkine Institute. ?In countries like India, increased transmission of this virus is seen post-monsoon and in winter.?

The 14 government hospitals presently have 500 ventilators, which are used for all patients. ?To fortify this existing stock, a contingency fund of Rs 4.5 crore was approved,? said Milind Mhaiskar, secretary, medical education and drugs development.

A state-level committee was appointed to decide about the requirements of the medical equipments, said Dr Pravin Shengare, joint director, Directorate of Medical Education and Research. The committee recommended that every medical college hospital should have at least three specific types of ventilators.

According to experts, the number of patients detected with the virus has gone down after September. ?In October, four swine flu cases were identified in the city and 70 across the state,? said Dr Daksha Shah, head of BMC?s epidemiology cell.

However, it is difficult to predict the situation this winter, said an officer.

Doctors ready to face second wave of swine flu



NAGPUR: With the second wave of swine flu expected as the winter progresses, doctors have suggested extreme caution to avoid a spread of the virus. Doctors claim they are well-equipped to tackle any eventuality, but they all agree that preventive actions need to be taken by everyone.

Quoting WHO's adviser Dr Keiji Fukada, Dr Sanjay Marathe revealed that this time around the pandemic might take a more deadly form, affecting 30% more people than earlier.

Differing a bit from these views, at an IMA conference, physician Dr Pradeep Mishra said that the virus is more prevalent in colder climes and not tropical countries like India. "Many deaths are recorded by road accidents and tuberculosis. Our priorities should differ from other countries," he added.

Dr Pallavi Bhargava, a consultant in infectious diseases and travel medicine from Pune, has seen many cases of the dreaded disease last year. She was in the city to share her experiences at the national conference of paediatricians. She pointed out that while last November there had been a flood of swine flu cases, there have been no cases in the month so far.

Many doctors also believe that even if the flu does break out, it would not be as hazardous this time, due to the phenomenon of herd immunity. This occurs when a large part of the population has become resistant to a disease, having either been vaccinated or survived the disease earlier. This way, the rest of the populace remains protected despite not being vaccinated. Dr Bhargava suggests a wait-and-watch approach for now. "I would only advise to go in for vaccination on a mass scale if the transmission occurs," she said.

Dr Kuldeep Sukhdeve of Orange City Hospital and Research Centre, one of the few city-based hospitals authorised by the government to treat the disease, couldn't agree more. "Yet, the more vulnerable sections of the population, especially schoolchildren and senior citizens should be vaccinated," he said.

In addition, Dr Sukhdeve, made it clear that there should be no panic or hasty decisions, which might be very expensive. As Dr Mishra put it, "It's only a virus not a monster. Taking all necessary basic precautions is all we need to worry about."
