Beds doubled in hospitals

Nearly 500 beds now reserved for H1N1 patients in city to cope with swine flu spike

TO cope with the rising number of swine flu cases in the city, the state health department has increased the number of hospital beds that are to be reserved for H1N1 patients.

The move came after it became impossible to accommodate the H1N1 patients in the 250-odd beds across isolation wards reserved for them.

"Over the past few days, the average number of patients admitted for swine flu has been around 325, while some 250 beds had been reserved for them. That is why we started getting reports that in some private hospitals, swine flu patients were being admitted to general wards with other patients, which posed the risk of
infection spreading,"
said a senior official from health department. "It was then decided to increase the bed capacity from 290 to 497."

MiD DAY had first reported about the shortage of beds across hospitals in the city and mixing of swine flu patients with people suffering from other ailments in non-H1N1 wards.

Dr C A Kante, assistant director of health services, confirmed that 200 additional beds had been reserved for H1N1 cases.

"The number of cases is quite high and after looking at the trend of fresh infections and hospitalisations, it
was recommended that extra beds be reserved for swine flu in whichever hospitals it was possible to do so,"
said Kante.

Tamiflu debate
With the swine flu epidemic having reached its "third peak", doctors are debating whether they should revert to last year's practice of giving Tamiflu as prophylactic treatment to all those who come in contact with swine flu patients.

"As of now, we wait for people who are in touch with H1N1 patients to show symptoms and then give Tamiflu.

But if we give Tamiflu to all those in contact with swine flu patients, then the rate of infection can come down," said Dr Sharad Agharkhedkar, president of Indian Medical Association, Pune chapter. "One H1N1 patient today affects 1.5 new people. By following last year's regime, the rate of infection spread can be brought down to 1.2 people."

But some doctors disagree. Dr Aarti Kinikar, head of paediatric department at Sassoon said prophylactic treatment didn't work last year.

"If a person doesn't have H1N1 and is given Tamiflu, then the drug might not give desired results if that person later on actually gets swine flu," said Kinikar. "Besides, the World HealthOrganisation also recommends treatment only after symptoms develop."

Four H1N1 deaths in two days


PUNE: With four more people succumbing to influenza A H1N1 in the city, the swine flu death toll has climbed up to 351 since August 2009.

State health officials said that three deaths were reported on August 18, while one was reported on August 19.

According to the officials, Machindra Baban Dhokle (55) of Pradhikaran Nigdi died at the Lokmanya hospital. He had developed symptoms like fever, cough and breathlessness since August 5. Ronit Dyaneshwar Nikam (27), of Pimple Nilakh died at the Deenanath Mangeshkar hospital and Ramabai Baghu Bavdhane (50), of Ganegaon in Shirur taluka died at the Sassoon hospital. On August 19, Waman Kashinath Wadkar (72) of Balajinagar died at the Bharati hospital. She had developed symptoms like fever and severe breathlessness.

Meanwhile, 27 people tested positive for the H1N1 virus on August 18. The number of people who have contracted the infection since this April has reached 1,519. Meanwhile, the condition of 20 H1N1 patients continued to be critical at various city hospitals and they have been put on ventilator support. At present, there around 145 patients being treated for H1N1 at various city hospitals. Over 4,982 people visited different screening centres on August 19, the officials added.

1st report appears to be a report on the death of 27 yo Santosh Shinde 'step' at YCM hospital Pune. 2nd report is a little clearer. 4th report is Santosh again. Details: Sick since Aug 7, Adm YCM Aug 10, (I think tested Aug 11, positive Aug 13, ventilator Aug 20, then died.)
3rd report seems to be the death of 26 yo Shivaji Cedare from Sogras. I don't remember coming across that town before.
4th report seems to be the death of a woman, Goandiatil ML Patla, 46 at Wockhard hospital Nagpur Aug 21.

PUNE: The H1N1 influenza claimed one more life in the city, taking the death toll up to 352.

According to state health officials, Rohini Ram Rudi (37), a resident of Katraj, died due to swine flu at Sassoon General Hospital on August 20. Rudi had developed cough and fever from August 13, and had tested positive of swine flu on August 20.

Her condition further deteriorated, following which she developed acute respiratory distress syndrome (ARDS) along with bilateral pneumonia.

H1N1 claims 11 lives in 19 days in city

NAGPUR: Although 11 people have died of H1N1 infection in city hospitals in last 19 days, doctors insist there is no cause to worry. They point out that these were sporadic cases spread over a huge area. Eight of these deaths have taken place in GMCH and all but one patients came from adjoining towns. The total deaths since April 1 this year are now 14. "There is nothing to panic as the infection is distributed over a large population in region. It cannot even be called a second wave," Dr R Powar, GMCH dean told TOI.

He said that the number of patients as well as deaths due to other diseases is more than swine flu. The 11 deaths in city since August 5 include eight in GMCH, two in Chandak Hospital and one from the Orange City Hospital and Research Institute. Of these, only three patients were from city (two in Chandak hospital and one in GMCH).

The OCHRI patient belonged to Jabalpur. The only cause of concern is presence of three pregnant women among casualties. They were from Kalmeshwar, Saoner and Narsala in Hudekeshwar. The two deaths at GMCH in last week included a 35-year-old pregnant woman, Girija Pandit, from Hudkeshwar and a 46-year-old man Madanlal Patle from Tiroda. They died last Friday but GMCH declared them H1N1 positive deaths only on Monday.

Swine flu claims five more lives in city, toll up to 362

PUNE: The H1N1 influenza claimed five more lives, four on Sunday and one a day before, taking the toll in the city up to 362. Of the dead, three are from rural areas.

According to district information officer Dattu Sitaram Talape (55), a resident of village Kharpur in Pune district, died due to swine flu at the Yashwantrao Chavan memorial hospital on Saturday. Talape had the onset of the illness from August 16. Varsha Vishnu Kokate (27) of village Karjule Harya in Ahmednagar district, fell victim to the H1N1 virus at the Noble hospital on August 22. Kokate had fever, throat pain and mild breathlessness from August 11.

Tanaji Vaman Kshirsagar (40) of Chintamaninagar in Hadapsar died on August 22. He had tested positive for the H1N1 virus on August 17. Dattatray Chandrashekhar Ambige (21) of village Manjari in the Pune district died at the Sassoon general hospital on August 22. He had tested positive on August 11. Dilshad Anwar Sheikh (26) of Gavalinagar, Bhosari, also succumbed to the infection at Sassoon Hospital on August 22. He had tested positive on August 22.

Meanwhile, 16 fresh cases of swine flu were detected on Sunday. With this, the number of positive cases has gone up to 1,590 since April this year. Condition of 22 patients continue to be critical. All of them have been put on ventilator support at various hospitals in the district, said officials.

A total of 2,504 people were given Tamiflu in the district on Sunday. As many as 266 patients are being treated at various hospitals. Of them, 130 are positive while 181 are suspected to have caught the infection, said officials.
The toll since since April this year has gone up to 151.

Miscreants snatch chain


NAGPUR: Despite availability of both nasal and intra-muscular vaccine for swine flu in the market, common people as well as doctors remain confused about it. This is because of absence of clear cut guidelines or directives from the state health authorities.

While state government is officially recommending the vaccine only for paramedical and medical staff in constant touch with swine flu patients, they are not against common people taking it as preventive measure.

Dr Ravishankar Sathe, assistant director of state public health department from Mumbai, says they are not recommending the vaccine for common people as there is no such instruction from the central government.

The union government has begun to supply the intramuscular vaccine for only health staff a few days back. Though the drug controller has cleared the nasal spray vaccine prepared by Serum Institute, Pune, central government has not yet issued any guidelines recommending it to common people.

However, both vaccines-Penenza, the intramuscular one, and Nasovac, the nasal spray one-are not available with the government. They are available in certain medical stores like the Panchsheel Medical stores and the Bombay Medical Store in city. Some other stores near GMCH and on Wardha Road had also procured it but withdrew it because of close expiry date.

Dr Rajesh Atal, physician at OCHRI, is in favour of medical staff as well as general people taking the vaccine.

Are there confirmed totals for India as a whole? As the recent beggars colony deaths indicate, there may be many more that were never attributed. However, a comparison of total trends during this wave and last might provide an indication as to whether the H1N1 situation is deteriorating this year vs last?

I think that comparing waves within each state can give the most data.

West Bengal and Kerala seem to have gone through two complete waves.

West Bengal reported approximately the same number of cases in each wave. Both waves peaked at 44 cases per week. First time around 0 deaths, second time 4 deaths.

Kerala's first and second waves each reported approximately 1400 confirmed cases. The first wave lasted around 14 weeks, the second about nine weeks. The number of cases per week in the second wave was higher, and there were approximately twice as many fatalities, 81 vs 37.


Maharashtra, Tamil Nadu and Andhra Pradesh have held steady last week and may be at the peak of the second wave.

So far in AP the wave has been less intense, and resulted in 28 fatalities vs 49 for wave 1.
In TN, the number of confirmed cases has peaked at around a third of wave 1, with no fatalities as opposed to an official 7 from wave 1. Disclaimer: I have a strong feeling that TN underreports H1N1 fatalities.
From Aug 4 - Oct 30 2009 Maharashtra recorded about 3600 cases and about 180 fatalities. From Nov 1 2009 to June 27 2010, fatalities and cases continued at a high background level with around 270 fatalities and 2761 cases. In the past 8 weeks, 2086 cases and 234 fatalities constitute this wave so far.

Karnataka, Rajasthan, Gujarat, Delhi, Madhya Pradesh, Haryana and Orissa are all still on the growth side of this wave. Of these States, Karnataka and Orissa are registering higher numbers of cases so far when compared to the first wave, and a higher intensity of fatalities. The others are too early to tell.

PUNE: The H1N1 influenza claimed three more lives, taking the death toll in the city to 365 since August 2009. Of the three dead, one is from Nashik.

According to district information officer, Navnath Baban Bankhele (36), a resident of village Manchar in Pune district, died due to the virus at Sahyadri Munot Hospital, Swargate at 9.50 pm on August 22. He had tested positive for the virus on August 8. Vijay Namdeo Damdhere (38), a resident of Warje Malwadi, died due to swine flu at 6.30 am at KEM hospital on August 23. He had tested positive on August 10. Asha Keshav Musale (28), a resident of Nashik, fell victim to the virus at Deenanath Mangeshkar Hospital at 2.20 pm on August 23. She had tested positive on August 6.

Meanwhile, 13 fresh cases of swine flu were detected on August 23. The condition of 25 patient continues to be critical.

High risk category more prone to succumbing to H1N1


PUNE: As much as 47 per cent of the people who succumbed to the H1N1 infection in the state since April this year were those falling in the high-risk category with either some kind of associated illness or low immunity level due to pregnancy, says a state government report.

"Of the 311 people who died due to the H1N1 influenza in Maharashtra since April, 149 had other associated illness like diabetes or hypertension. Among the dead, 54 were pregnant women," said health officials.

Devendra Shirole, former state president of the Indian Medical Association (IMA), said: "Underlying medical conditions make it more likely that a person will become severely ill. It confirms what we knew, that pre-existing medical conditions increase the risk substantially."

Pregnant women, people above 65 years, under-5s, people with chronic lung disease, chronic kidney disease, chronic heart disease neurological diseases (including motor neuron disease, multiple sclerosis and Parkinson's), hypertension, diabetes, history of asthma in the past three years, etc., fall among the high-risk category.

"People in the above groups should seek immediate medical attention if they display symptoms of the flu," said Sharad Agarkhedkar, president of the city chapter of IMA.

Gynecologists explained why the H1N1 virus is taking a heavy toll on pregnant women. "A woman's immune system changes during pregnancy to prevent an immune reaction against the foetus. That why the risk of infection is greater," says senior gynecologist Sanjay Gupte.

"The other reason is mechanical. As the womb gets larger, with the baby growing, there can be pressure on the airways and a restrictive lung disease. So it's harder to take a deep breath, and consequently, it is harder to fight off a lung infection, especially in the later stages of pregnancy," said Gupte. However, early initiation of medical treatment can make a lot of difference, Gupte added.

"Since throat swabs of severely-ill patients only are considered for test, we don't have the data of total number of positive cases in pregnant women in the state. Suspected cases are administered Tamiflu without going for the test," said a health official.

"Two lives are involved in pregnancy -- of the mother and the child. Pregnancy itself is a co-morbid condition because it is a sort of stress to the mother. Besides, any infection in pregnancy may lead to complication. Hence, the early initiation of treatment is significant," Gupte said.

Rural areas witness more deaths

PUNE: More than half of the H1N1 deaths reported in the state this year have been from rural areas. Of the 311 people who succumbed to the swine flu infection in Maharashtra since April this year, 162 were from the rural areas. Fresh cases of swine flu are being largely reported from rural areas which were left unscathed last year, say experts.

"Viruses always find newer areas to get entrenched in. Last year, cities bore the brunt. This time around, it is mainly the rural areas," said Arun Jamkar, dean of the Sassoon general hospital.

"In June, the mortality rate vis-?-vis the number of severely positive cases had gone down. However, with the rise in critically ill patients in July and August, more casualties were reported from rural parts of the state," said a health official.

"With the fall in temperature, there is a sudden rise in critical cases requiring artificial respiration. Besides, inordinate delay in administration of Tamiflu is also big cause of concern," Jamkar said.

As per the findings of a recent study at Sassoon hospital, very few people were found to have been given Tamiflu within a span of 48 hours of the onset of the illness. Only two of the total 28 people who succumbed to the H1N1 influenza at the isolation ICU at Sassoon since April, had received Tamiflu within 48 hours. The remaining 26 were administered Tamiflu when they had been well past the crucial period of receiving the drug, Jamkar said.

"Taking Tamiflu within the initial 48 hours is very important. This message needs to be disseminated to all citizens and medical practitioners," Jamkar said. Moreover, general awareness among people living in rural areas about signs and symptoms of the H1N1 influenza is very low, he added.

The country's preparedness to deal with a fresh wave of the swine flu pandemic was reviewed at a meeting presided over by the cabinet secretary in New Delhi recently. The meeting, which was attended by Union health ministry's top brass, including health secretary K Sujatha Rao and ICMR director-general V M Katoch, came to the conclusion that the virus had got entrenched in India and would soon replace the seasonal influenza strain to become the dominant virus causing fever, cough and cold. If not treated early with Tamilfu, people are more prone to die of it than the seasonal bug.
