Bird flu cases down, but pandemic looms

Indonesia must continue preparing for a bird flu pandemic, despite reports suggesting declines in cases of bird-to-human and bird-to-bird infections, health officials said Tuesday.

Erna Tresnaningsih, the Health Ministry's director for animal-vector diseases, said the confirmed number of humans infected by the H5N1 virus in Indonesia this year was 20 as of August, with 17 fatalities.

This is a decline, she claimed, from 55 cases with 45 fatalities in 2006 and 42 cases with 37 fatalities in 2007.

Indonesia has recorded a total of 137 confirmed bird-to-human infections, 112 of them fatal, since July 2005,
when H5N1 was first found to attack humans.

Erna added 12 of the country's 33 provinces had recorded infections in humans, but seven of the provinces saw no new cases in the past year, leaving incidents of bird flu infections concentrated in West Sumatra, Banten, Jakarta, West Java and Central Java.

"Praise be to God, with good partnership we've been able to push the figures," she told a forum on boosting businesses' preparedness for a possible bird flu pandemic.

Muhammad Azhar, the Agriculture Ministry's avian influenza control unit coordinator, reported a declining trend in the number of poultry infected by the H5N1 virus.

He said poultry in 31 provinces had been infected by H5N1, with only Gorontalo and North Maluku free from the virus, but added that nine of the provinces had not reported any new cases in the past six months.

"Areas still at risk are those on Java Island, because it is the main producer of both pedigree and non-pedigree chickens," Azhar said.

Despite the declining number of infections, Indonesia has to be alert to the possibility of a bird flu pandemic, which, once it breaks out, could infect up to 66 million people, the Health Ministry warned.

Emil Agustiono, deputy to the coordinating minister for people's welfare, said a bird flu pandemic was unpredictable, but could happen through either the breeding of the H5N1 avian influenza virus with the H3N2/H1N1 human influenza virus, or through the mutation of the H5N1 virus in the human body.

Both processes could produce a new type of deadly, transmittable human-to-human influenza virus, which could then lead to a new influenza pandemic, he said.

Subhash Salunke, the World Health Organization's (WHO) representative to Indonesia, warned that once the pandemic struck, Indonesia would see an overwhelming of health care services, the collapse of critical public services, workforce absenteeism, public fear and panic, and the closure of borders, travel and trade.

He said Indonesia could suffer from economic losses of US$1.4 billion if the pandemic lasted six months, or up to $5.8 billion if it lasted a year.

Salunke emphasized the need for preparedness and intervention in the case of a pandemic, so as to minimize its severity, adding that if a new flu pandemic did strike, it could cause 160-300 million deaths globally.

Alan Boulton, the International Labor Organization's (ILO) Indonesia country director, said if employers wanted their businesses to keep running during such a pandemic, they would have to start preparing their workplaces and developing business continuity plans involving their employees.

137 BIRD FLU CASES IN RI UP TO AUG 2008

Jakarta 23/9/2008 (Kominfo-Newsroom) – Total of avian flu cases in Indonesia up to Aug. 2008 comes to 137, with 112 fatalities.

Avian Influenza (AI) cases are found in 12 of 33 provinces. And sufferers are 51% male and 49% female, according to Erna Tresnaningsih, Director of Control of Diseases caused by Animals, Health Ministry, here Tuesday (23/8).

Highest incidence of AI is in Tangerang. Intensive efforst aimed at surmounting the disease are pooled there. This includes training of officers to manage AI and procurement of medical instruments and equipment and medicines.

High rate of fatalities, she said, is due to shortage of AI knowledge among residents.

Also, most AI patients go to private hospitals in which AI medicine is not available.

AI medicines are only available in state hospitals or Puskesmas (public health centers).

Tamiflu is not available at private hospitals and dispensaries. The government is afraid they might be exploited by irresponsible business circles.

The phrase "Animal Welfare" is only an eufemism to pace on people when they passed the law.

It can't be any "Welfare" when the holder of the welfare would be killed for a meal.
But obviously when such butchering is a real food chain fact, than it must be enforced by a strict law to not allow some "humans" to enjoy in animal torturing and killing, normatived in such "Welfare" laws.

As always seen in history, on the heavy pain of humans and animals, their torturers were never be sensitive, nor they gave them an fast death.
Obviously if the situation was oposite, where they would be the ones to burning in pain, they would never more burned alive any chicken, or other species.

RI shuts down U.S. medical laboratory

The U.S. Naval Medical Research Unit number 2 (NAMRU-2) laboratory in
Jakarta will be temporarily shut down because it offers "little
benefit" to Indonesia, Health Minister Siti Fadilah Supari said
Thursday [25 Sep 2008].


"They are not allowed to conduct any activities anymore. The term is
suspension, but there are no activities at all now," she was quoted
as saying by Reuters.

Siti, Indonesia's senior-most medical official, said she staunchly
opposed the medical lab and called for its closure, arguing that it
did not benefit national interests and even compromised national security.

Siti said the lab refused to share all of its findings with the host country.

Indonesia and the U.S. ceased negotiations on the future of the lab
in June [2006] after officials and legislators proved unable to agree
whether the lab, active since 1970, should continue its operations in
Indonesia.

The Indonesian Foreign Ministry said the temporary closure of the lab
was not contentious as negotiations between the 2 sides were ongoing.

"As long as we are still negotiating a new umbrella agreement for
NAMRU, the termination of activities is no problem," Foreign Ministry
spokesman Teuku Faizasyah told The Jakarta Post by phone from New York.

The halt in activities does not mean the lab will be closed
permanently as the contract with Washington to operate the lab has
not yet terminated, another Indonesian Foreign Ministry official said.

"We still have a contract. Although it was terminated in 2000, then
President Abdurahman Wahid revived it," ministry director general for
relations with America and Europe Retno P. Marsudi said.

The U.S. and Indonesia have yet to resume talks since the June [2008]
negotiations breakdown, she said.

The negotiations were complicated by an international dispute over
how to share crucial bird flu samples.

U.S. Health and Human Services:tiphat: Secretary Michael Leavitt:tiphat: has said the
quarrel over Indonesia's refusal to share its samples of the H5N1
avian influenza virus with the rest of the world has spilled into the
NAMRU talks.

Siti earlier forbid samples of the virus to be shipped outside of
Indonesia, citing that rich nations and pharmaceutical companies
would benefit by using the samples to develop vaccines that poor
countries would not be able to afford.

Many international observers have said NAMRU is central to
Indonesia's early efforts to track mutations of H5N1, which is feared
may become transmissible among humans with the potential for it to
sweep the world in months and kill millions.

Indonesia has suffered 112 deaths from the virus, making it the
country with the highest death toll.

Indonesia has halted the activities of a U.S. naval medical lab in
Jakarta following a dispute over the terms of a contract, the health
minister said on Thursday [25 Sep 2008].

The U.S. Naval Medical Research Unit No. 2 [NAMRU-2] has been key to
efforts to track bird flu in Indonesia, the country with the most
human deaths from the H5N1 virus.

But a memorandum of understanding allowing the lab to operate in
Jakarta expired 2 years ago and was not renewed, as a dispute arose
over Indonesia's sharing of samples of H5N1 with the rest of the world.

"They are not allowed to do any activities anymore," Health Minister
Siti Fadillah Supari told Reuters by phone. "The term is suspension,
but there are no activities at all now."


Earlier in April [2008], she said the lab was not very beneficial to
Indonesia because it refused to share all its findings with the host country.

Retno Marsudi, Indonesian directorate general for America and Europe
relations, said the contract with Washington to operate the lab had
not been terminated.

A U.S. embassy spokesman in Jakarta declined to comment.:coffee:

Indonesia ordered hospitals and labs in the country to stop supplying
bird flu samples to the American lab early this year [2008] and now
only reports bird flu cases in humans to the public every 6 months, a
move some scientists say could lead to delays in containing outbreaks
of the disease.

Jakarta has also refused to share bird flu samples, saying it wants
guarantees from richer nations and drug makers that poor countries
would get access to affordable vaccines developed from their samples.

International health experts say it is vital to have access to
samples of the constantly mutating H5N1 virus, which they fear could
change into a form easily transmissible among humans and sweep the
world in months, killing millions of people.

NAMRU-2 is collaborating in other countries in Southeast Asia,
including Viet Nam, Laos, Singapore, the Philippines, Thailand, and
Cambodia, and a satellite laboratory was opened in Cambodia to
conduct regional infectious disease outbreak research and diagnostic
