The National Committee for Avian Influenza Control and Pandemic Preparedness says it has found the way forward in the fight to control avian influenza.

The committee plans to compile all recommendations given by experts, especially those on vaccination, on ways to battle the disease.

"We have decided that vaccination is still needed and will become part of our roadmap in handling the disease. But we will focus more on bio-security. People tend to think that after the vaccination, all problems are solved. We need to familiarize people more on bio-security," said the committee's chief Bayu Krisnamurthi. Bayu was speaking at the end of a four-day seminar and meeting of experts on avian influenza on Thursday.

He said vaccinations should be integrated into all poultry sectors. Previously, he said, the larger poultry sectors vaccinated their chickens themselves, while the government only focused on backyard farming, the smallest poultry sector. "We will use two approaches to integrate vaccination: area and supply-chain approaches. If an area that is infected with bird flu has several poultry sectors, they will be handled together," he said. The supply-chain approach addresses infected chickens being shifted from one sector to another.

Bayu said three variations of the H5N1 virus can be found in Indonesia: the South Sulawesi, Bali and Sumatra strains. He added that the three types are also found on Java.

"The most effective way to prevent the disease is to integrate the vaccination with a bio-security program. The vaccine used should be compatible with the virus (strain) found in the field, because there is no universal vaccine for avian influenza," Bayu said.

The vaccinations will be carried out only on sick chickens and under veterinary supervision. Other poultry will be targeted further into the vaccination process, he said.

Bayu said a specific law on animal health required composing to respond to obstacles in the field. "The Association of Indonesian Veterinarians has drafted a bill on animal health. We have presented the bill to the House of Representatives. It will become a benchmark on avian influenza prevention for all ministries. The bill will also (handle) backyard farming," Bayu said.

Meanwhile, the secretary at the Directorate General of Livestock at the Agriculture Ministry, Tjeppy D. Soedjana, said a new law is needed to replace the existing 1967 law, which he said is incapable of responding to current developments in animal health. "The House will start the (bill's) deliberation next Monday. We hope that it will be finished by the end of this year," Tjeppy said. The bill stipulates that the community has a responsibility to report the outbreak of any animal diseases to authorities. It also calls for the formation of a veterinary authority at the regency and municipality levels. "Besides bird flu, the bill also regulates other animal diseases such as anthrax," Tjeppy said. Called MPAT TEMPEST EXPRESS-12 Staff Planning Workshop, the activity focuses on a Government of Indonesia-led response to a simulation natural disaster in western Java affecting Jakarta.

The primary goals of the event are to better understand planning of major civilian-led humanitarian assistance and disaster relief operations; improve civil-military cooperation among the various organizations involved in disaster response; build on Government of Indonesia and TNI disaster response experience and procedures; educate key planners on basic principles, tasks, and roles and responsibilities of responders to disasters; plan for support by, and the transition (departure) of, international military support in disaster relief operations; and increase familiarity with international military crisis action planning.

The lead response organizations in the workshop are BAKORNAS PB, Indonesia?s lead emergency management agency, and the SATKORLAK Jakarta, Jakarta emergency response organization. These organizations are supported by TNI Headquarters (national level) and the KODAM (Jakarta military area command) with significant ministerial, agency, Red Cross-Red Crescent Movement and local nongovernmental support.

Foreign governments are supporting the event with planning expertise provided by military officers from nations participating in the MPAT Program. International military support is being provided by liaison officers from 22 nations. The international military officers will offer and help coordinate the delivery of international military assistance that might be provided in a major natural disaster. The event is also supported by several UN agencies/funds/programs and international and nongovernmental organizations.

Nike, when I went there, I clicked on TE-12 (left side of screen). After that, I went over to Scenerio Documents - Situation Update 12 June 07. While the Spanish flu virus possibly might be compared to a one-headed monster, it may be possible that highly virulent varieties of H5N1 virus might better be compared to a three-headed one - because there is evidence of at least three independent virulence factors connected with three different genes.

It is highly unlikely that all of the high-virulence alleles will simultaneously mutate and disappear if and when the haemagglutinin gene changes so as to make the haemagglutinin molecule better adapted for the human-type (alpha-2,6-linked) receptor (which is a necessary prerequisite in order that a pandemic with H5N1 virus may start).

It is more probable that evolutionary adaptation of the haemagglutinin of H5N1 viruses to the human-type receptor will happen without any simultaneous change in those other genetic properties that now are important for explaining the exceptionally high virulence of certain strains of avian-adapted H5N1 influenza virus.

The change of the haemagglutinin molecule from avian adaptation to human adaptation must be expected to act as an additional virulence factor because it will enhance the total number of cells that can be infected (per host organism), increase the total rate of virus replication and potentiate the effects of the other virulence factors already present. The monster will then have four heads, not three, and case fatality rates must be expected to become even higher than they have been until now, perhaps reaching as high as 98-99% (at least in poor countries with less than optimal nutrition).

Another bird flu myth. The current CFR in Indonesia stands at 80%. All of the 100 infected Indonesia received the best possible local care, including isolation, respirators, and antivirals. Still the CFR is 80%. I have seen no references that suggest the 80 individuals who died in Indonesia were in poor health or had poor nutrition. Nor have I seen any information that ARDS, as it related to flu victims, is affected by "less than optimal nutrition". It is my understanding that it is the healthy individuals that produce stronger cytokine storms.

Believing that having a healthy diet will protect oneself from a novel pandemic virus is misplaced faith.
