REGIONAL EPIDEMIOLOGICAL POINT GUYANA

DENGUE January 18, 2024

Summary of the epidemiological situation

Over the past two weeks, the circulation of the dengue virus was increasing sharply and
a high level. Epidemiological indicators were on the rise in town and in emergency rooms,
particularly on the Island of Cayenne and the Savanes where viral circulation continued to intensify.

The indicators reached levels higher than those observed during the peak of the previous
epidemic in 2020. A more moderate increase is observed on the western coast and after several
relatively calm weeks, the number of consultations in CDPS on the Maroni like the Oyapock
was on the rise.

Serotypes DEN-2 and DEN-3 co-circulate in Guyana. The DEN-2 serotype has become the majority on
the Island of Cayenne and is also progressing in the Savanes sector.

As a reminder, in order to be able to best assess the epidemiological situation, it is important to
prescribe biological testing for dengue fever for any case clinically suggestive of dengue fever, preferably by RT-PCR.
...
Epidemiological situation in S01-02 - data as of January 14, 2024

The estimated number of cases clinically suggestive of
dengue fever seen in general medicine consultation was
clearly increasing over the last two weeks: 1321 cases
were estimated in S01-02 compared to
779 in S51-52 (Figure 1).

The number of biologically confirmed cases of dengue
was at a high level and increasing sharply with 860
biological confirmations identified in S01-02 (in progress
consolidation for S02) compared to 343 in S51-52
(Figure 1).

DEN-3 and DEN-2 serotypes co-circulate with
respectively 57% of DEN-3 and 43% DEN-2 identified
these last two weeks, mainly in the
Cayenne Island sector.

At the same time, a sharp increase in transitions to
emergencies was also observed with 308 passages
for suspected dengue fever in S01-02 (vs. 146 in S51-52).
Thus, the last two weeks have been marked by
a sharp acceleration in the circulation of the virus
dengue fever in Guyana.

It mainly concerned the Island of Cayenne but the
traffic was also at a high level in the other
sectors. In particular, we note an increase
consultations for dengue-like syndrome in
Maroni and Oyapock health centers (notably in
St Georges) during the last week S02.

Fight against the dengue epidemic: three weeks of spraying in Saint-Georges de l'Oyapock

Dengue fever is rampant throughout Guyana. Measures have been taken by the Territorial Collectivity of Guyana to fight against this epidemic. Additional rotations of the spreader nicknamed “baygonneuse” are planned from February 5, particularly in Saint-Georges.

Catherine Lama • Published on February 4, 2024 at 10:12 a.m.

Cases of dengue fever caused by the bites of the Aedes aegyti mosquito are increasing throughout Guyana. On both banks of the Oyapock, populations are particularly affected. Also the CTG is increasing its means of combating and preventing the epidemic, between February 5 and 25, several rotations of the “car-mounted spreader” will be carried out in Saint-Georges and its surroundings.

This chemical spreading will be done in the early morning between 4 a.m. and 6:30 a.m.

The product distributed may cause health problems, it is therefore recommended to avoid exposure during spreading.
...
Four cases of death due to dengue

In its latest bulletin, the Regional Health Agency specifies: “Since the start of the epidemic, 5,015 confirmed cases of dengue have been recorded, including 2,211 this year. Since the start of 2023, 151 hospitalizations have been recorded at the CHC, including 76 in the first four weeks of 2024. Across Guyana, at least 253 patients have been hospitalized, ten of whom were admitted to intensive care. Four people who tested positive for dengue died , one of whom was directly linked to dengue. »

Guyana facing its largest dengue epidemic in 20 years

In January 2024, 800 new cases were reported on average per week, according to health authorities.

Guyana is facing its largest dengue epidemic in around twenty years: it began in mid-2023 and has accelerated since the beginning of January 2024 with 800 new cases declared on average per week, according to the health authorities. This virus transmitted by Aedes mosquitoes, against which no vaccine is recommended by the High Authority for Health (HAS), can cause hemorrhages or shock syndromes in the most serious cases.

According to Public Health France, 5,800 confirmed cases of dengue fever have been recorded in the Amazon department of 300,000 inhabitants since the start of 2023, including 2,996 already in 2024. In tropical and intertropical zones, such as Guyana , epidemics recur every three to five years and generally last 12 to 18 months. Viral waves are more or less intense.

A monitoring unit associating State services and the Territorial Collectivity of Guyana (CTG) was activated on February 6 with a view to measures "to slow down this dynamic as much as possible" , announced the prefect Antoine Poussier.

Dengue: “Guyana is facing an exceptional epidemic”

Alexandre Marsat February 26, 2024

With 800 new cases per week, Guyana is experiencing its most serious dengue epidemic in several decades. Professor Charles Cazanave, infectious disease doctor at Bordeaux University Hospital, explains the reasons for such an epidemic. He also explains the forms of the virus and tells us if dengue fever can arrive in mainland France.

...
Charles Cazanave: It is an epidemic which began in the summer of 2023 and which is quite unprecedented compared to previous ones. Since the start of 2024, there has been an increase favored by the rainy season which began in December, beneficial to the proliferation of larval breeding sites. This is ideal for mosquitoes of the Aedes genus (which includes the tiger mosquito).

This year, French Guiana has been facing an exceptional dengue epidemic for several decades with 6,600 confirmed cases (per test) and 12,900 cases (evocative) for a population of only 300,000 inhabitants. Public Health France reports 4,000 confirmed cases between January 1 and February 15 of the 6,600 cases recorded since last summer. So we are at an epidemiological peak.

It is likely that mosquito breeding site treatments were overtaken by a very marked rainy season. We will have to see if we also have a different type of dengue fever with a greater spread of the virus.

Furthermore, arboviruses (viruses transmitted by arthropods including mosquitoes) tend to increase with warming and the accumulation of mosquitoes.

Should we expect dengue epidemics in mainland France?

Charles Cazanave: This is an element that we monitor very carefully because the answer is yes. To speak of an epidemic, the disease must spread. This is possible with the reservoir (man) and Aedes mosquitoes .

For the moment in mainland France, we have mainly had imported cases so this is not part of the epidemic: people are returning with the disease from a region where dengue fever is circulating. Last year, there were 2,000 imported cases in mainland France, mainly in Île de France, Auvergne Rhône-Alpes, Occitanie and Nouvelle-Aquitaine.

As infectious diseases specialists, we are then hyper-vigilant because if a tiger mosquito bites an infected person, the mosquito can then transmit dengue fever to another. Last year, we identified 43 indigenous cases: people who contracted the dengue virus in mainland France.

Originally, it is a disease restricted to tropical areas but with global warming, it is moving north.

What are the symptoms of dengue and how to cure the virus?

Charles Cazanave: Dengue is a generally benign disease but there can be serious forms. In Guyana since the start of summer 2023, there have been 300 people hospitalized in the 3 Guyanese hospitals, including 12 in intensive care and 4 deaths (3 fragile patients). Who says viral disease, says no antibiotics... The incubation is very short: once bitten, the symptoms appear 3 to 7 days later. In the majority of cases, there will be no symptoms.

For symptomatic forms, we see a flu syndrome with fever, headache, and diffuse pain. There may be damage to the conjunctiva with red eyes and a skin rash where the skin turns red. The most serious form (1% of cases) is dengue hemorrhagic fever with multiple hemorrhages, particularly gastrointestinal.

In the vast majority of cases, there is no treatment so you have to wait.
On the other hand, as a preventive treatment, it is necessary to fight against mosquitoes by eliminating all potential larval breeding grounds by emptying all containers containing water.
