3 cases of Monkeypox were reported on Wednesday August 10 in a family staying in a campsite in a town in the Var. This is an intra-family contamination prior to the arrival of these people on the campsite. They were isolated and left the scene. The state of health of this family does not inspire concern.

According to the investigations carried out by the Provence-Alpes-Côte d'Azur Regional Health Agency, the family had no close and prolonged contact during their stay of a few days in the campsite (from August 6 to 11). with other holidaymakers or with the campsite staff.

The family used the campsite facilities (toilets, showers), however, outside the family home, the risk of transmission via surfaces is very low. The usual hygiene measures such as regular cleaning of the premises and washing of hands can reduce/limit the risk of contamination.

This situation does not therefore justify the isolation or vaccination of the staff and residents of the campsite.

As of August 16, 2022 at 12:00 p.m., 2,749 confirmed cases have been identified in France.

The distribution of cases by region of residence (or by reporting region when the region of residence is unknown) is presented in Figure 1. The Ile-de-France region concentrates the largest number of cases (1,642 or 60%), followed by Occitania (244 cases) and Auvergne-Rhône-Alpes (213 cases). Ten cases reside abroad.

For 747 cases (27%) the region of residence was not known and was replaced by the reporting region.

The distribution of cases by symptom onset date (when known) is shown in Figure 2. The case symptom onset date ranges from May 7, 2022 to August 10, 2022. reporting deadlines, the data for the last few weeks are not consolidated.

The reports received do not always mention the date of onset of the symptoms. As an alternative to this information, the distribution of cases according to their date of reporting is presented in Figure 3.

The reporting low observed in week 28 (July 11 to 17) can be explained by the public holiday (July 14).

All the cases identified to date are male adults, except 29 female adults (i.e. 1.1% of women) and 4 children under 15 years old. Adult cases have a median age of 36; 25% of adult cases are under 30 years old and 25% are 43 to 77 years old.

Characteristics of cases that may have been the subject of an investigation

Since the characteristics of confirmed cases in men have been stable for several weeks, epidemiological investigations will now focus on female, pediatric and atypical cases. Therefore, information on symptomatology and case profiles will no longer be updated. The information presented below is based on investigations carried out up to 08/08/2022.

Among the cases investigated, 75% presented a genito-anal rash, 70% an eruption on another part of the body, 76% a fever and 72% lymphadenopathy. Among the cases investigated, 74 are immunocompromised (5.1% of the cases having responded); 385 are HIV positive (i.e. 25% of cases knowing their HIV status). Among non-HIV carriers, 701 are on pre-exposure prophylaxis or “PrEP”* (i.e. 64% of non-HIV carriers who answered the question).

To date, in France, 95% of cases for which sexual orientation is given have occurred in men who have sex with men (MSM).

Among the cases for which information is available, 71% declare having had at least 2 sexual partners in the 3 weeks before the onset of symptoms.

Most of the cases questioned declare that they cannot identify the person who allegedly infected them; 24% are secondary cases, ie they report having been in contact with a
case of monkeypox in the three weeks preceding the onset of symptoms.

Epidemiological surveillance that adapts to changes in knowledge and diagnostic practices

Surveillance was initially based in France and internationally on the confirmation of cases by biological examination (PCR). As the epidemic progressed and knowledge of the symptomatology of cases was accumulated, clinicians no longer systematically resorted to laboratory diagnosis for clinically suggestive cases for which a risky exposure or contact with a case confirmed (by PCR) was documented.

In order to better interpret epidemiological trends, the Agency is adapting surveillance to extend it to all cases and no longer be limited to biologically confirmed cases. This change makes it possible to better describe the reality on the ground, quantify the impact of the epidemic, and take into account changes in diagnostic practices by clinicians.

As a reminder, Monkeypox virus infections (whether the cases are biologically confirmed or clinically suggestive with risky exposure) must be the subject of a mandatory declaration (Orthopoxvirus sheet including smallpox) allowing the feedback of information to the Regional Health Agency, then Public Health France as part of surveillance.

To date, only biologically confirmed cases must be transmitted at European level .

Information and prevention actions

Given what has been observed in Europe on the disease, targeted communication was quickly implemented towards MSM people. The sexosafe.fr site , dedicated to the sexuality of MSM people, is regularly updated with a summary of knowledge on the subject and preventive measures. Poster, radio and digital campaigns provide information to people in addition to actions in the field. Since June 17, the digital campaign has generated nearly 689,463 clicks on the banners and more than 599,065 visits to the Sexosafe site.

All information on vaccination is updated every week on the page intended for the general public and that intended for health professionals (accessible without login) of the vaccination-info-service site .
Prevention actions are continuously adapted to the evolution of the situation and the state of knowledge.

To facilitate access to information for vulnerable people in precarious situations, a pictorial tool, translated into 6 languages, developed with professionals working with these people is available on Sante Publique France.
The MOBCO newsletter n°7 compiled with actors in the field brings together a series of questions/answers on the subject of Monkeypox/monkey pox. It is aimed at professionals or volunteers in contact with people in precarious situations.
In France, long-term monitoring of monkeypox through the mandatory notification system is reinforced and information and alert messages are sent to health professionals. Exchanges are also continuing with other European countries, the WHO and the ECDC.

Monkeypox info service: a listening device to answer questions about monkeypox

Since Wednesday, July 13, a listening device has been open to answer questions raised by monkeypox. Subsidized by Public Health France and carried by SIS Association, the “Monkeypox info service” telephone line is accessible every day from 8 a.m. to 11 p.m., on the toll-free number 0 801 90 80 69 (free, anonymous and confidential call and services). As of August 23, 2022 at 6:00 p.m., 3,421 biologically confirmed cases have been identified in France.

As part of its regular approach to checking the quality of the indicators, the Agency has identified 350 cases not yet entered in the databases since the last report published on the agency's website on Thursday 18/08. The total number of confirmed cases published today therefore includes catch-up data which should not be interpreted as an exceptional increase in the number of cases. As of 09/01/2022 at 12 p.m., 3,646 confirmed cases have been identified in France. As of October 25, 2022 at 12:00 p.m., 4,094 confirmed cases of infection with the Monkeypox virus have been identified in France, i.e. 10 additional cases since the assessment of October 18.

The distribution of confirmed cases by region of residence (or by reporting region when the region of residence is unknown) is presented in Figure 1. The Ile-de-France region concentrates the largest number of cases (2,496, i.e. 60, 9%), followed by Occitania (312 cases), Provence-Alpes-Côte d'Azur (284 cases) and Auvergne-Rhône-Alpes (268 cases); 22 cases reside abroad.

The vast majority of confirmed adult cases identified to date are male and 107 cases over the age of 15 (2.6%) are female. The proportion of female cases increased until W36 when it reached 13.6% (16 cases out of 118) then decreased until W39 (with 7.5% of confirmed cases, i.e. 3 cases out of 40). Since week 39, the number of infected women has remained stable (around 3 cases per week). Twelve (0.29%) children under the age of 15 have been reported as confirmed cases since May 2022. Adult confirmed cases have a median age of 36; 25% of adult cases are under 29 years old and 25% are 43 to 81 years old. Of the confirmed cases for which information is available, 91 (2.2%) were hospitalized due to infection with Monkeypox virus; this proportion remains stable over time.

No deaths have been reported to date.

The distribution of cases by symptom onset date (when known) is shown in Figure 2. However, we must remain cautious because improving knowledge of the disease can reduce the need for treatment by the best-informed populations. Several other countries, particularly in Europe, are seeing similar trends in the number of new cases reported over the past 3 months.
