Since 2023, the number of imported cases of dengue fever reported in France has reached unprecedented figures, more than 60% of which return from the French West Indies, where an epidemic has been ongoing since mid-2023. Between January 1 and April 19, 2024, 1,679 cases of imported dengue fever were notified to Public Health France compared to 131 over the same period in 2023. Over this period, 82% of cases returned from the French West Indies.

How is the dengue virus currently circulating in France?

The number of imported cases of dengue fever in France reported by mandatory declaration has increased since mid-2023 to reach unprecedented figures in the first months of 2024. Between January 1 and April 19, 2024, and while the data were not stabilized for the last few weeks, 1,679 cases of imported dengue fever were notified compared to 131 over the same period in 2023 (figure 2).

The majority of cases (82%) returned from the French Antilles, where an epidemic has been ongoing since mid-2023 .

This situation comes as the Pan American Health Organization (PAHO) has warned of a sharp increase in dengue cases in the Americas, compared to previous years. Between weeks 2024-01 and 2024-12, the cumulative incidence rate in the Americas region was 451 cases p. 100,000 inhabitants, i.e. 3 times more than the same period in 2023 and 5 times more than the average number of cases over this period over the last 5 years (figure 3).

How to avoid being infected by the tiger mosquito?

An infected person can infect a mosquito when the dengue virus is present in their blood (so-called viremic phase, lasting approximately one week). During the 15 days following return from areas where dengue cases have been reported, it is recommended:
  • to consult a doctor in case of fever and to tell him which regions you are returning from;

Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to a virus circulation area, having become locally contaminated), vector control measures are reinforced with the implementation of two adulticide mosquito control treatments in a wider area ( 300m). Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to a virus circulation area, having become locally contaminated), vector control measures are reinforced with the implementation of two adulticide mosquito control treatments in a wider area ( 300m). Active case finding is also implemented around outbreaks of indigenous cases, to determine the extent of local transmission and adjust the scope of vector control treatment if necessary. This active research is carried out jointly by Public Health France and the regional health agencies concerned. It is based on raising awareness among health professionals in the sectors concerned and the general population, and on door-to-door surveys in the vicinity of known cases to identify possible other cases. Cases of dengue fever are on the rise in mainland France, with 601 cases of the illness recorded in the last six weeks, new figures show.

The cases - recorded between May 1 and June 18, 2024 - were all imported, the latest epidemiological report by France’s public health authority (Santé Publique France) states.

The same report notes that two cases of chikungunya and one case of zika virus were also recorded in France at this time.

Around 60% of the dengue fever cases relate to people returning from the overseas French departments of Guadeloupe and Martinique, areas which have tropical climates more susceptible to tropical diseases such as dengue. No ‘native’ cases have been recorded in mainland France this year.​..

Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to an area where the virus is circulating, having been contaminated locally), vector control measures are reinforced by carrying out two adulticide mosquito control treatments in a wider perimeter (300 m). An active case search is also implemented around the outbreaks of indigenous cases, to determine the extent of local transmission and adjust the scope of the vector control treatment if necessary. This active search is carried out jointly by Santé publique France and the regional health agencies concerned. It is based on raising awareness among health professionals in the sectors concerned and the general population, and on door-to-door surveys in the vicinity of known cases to identify possible other cases. Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to an area where the virus is circulating, having been contaminated locally), vector control measures are reinforced by carrying out two adulticide mosquito control treatments in a wider perimeter (300 m). An active case search is also implemented around the outbreaks of indigenous cases, to determine the extent of local transmission and adjust the scope of the vector control treatment if necessary. A case of indigenous dengue fever was detected this Friday in the Alpes-Maritimes. It concerns a resident of La Colle-sur-Loup (Alpes-Maritimes). It is the first detected in the PACA region and the second in France. Mosquito control actions must begin the following day.

After a first case detected in Hérault at the beginning of July, a second indigenous case of dengue fever was recorded this Friday in the Alpes-Maritimes, according to the Regional Health Agency (ARS) of the Provence-Alpes-Côte d'Azur region.

The infected resident lives in La Colle-sur-Loup, he is a man aged between 20 and 30. According to our information, he is fine.

"We speak of an indigenous case when a person has contracted the disease in metropolitan France and has not travelled to a contaminated area in the 15 days preceding the onset of symptoms ," the ARS specifies in its press release.

"To prevent the spread of the disease, mosquito control is being carried out by the Interdepartmental Agreement for the Control of Mosquitoes on the Mediterranean Coast (EID-Med) to the east of the municipality (public roads, private gardens) ," continues the ARS. The aim is to eliminate potentially infected adult mosquitoes to prevent them from biting other people in the neighbourhood and to eliminate larval breeding sites.

"This mosquito control action will be supplemented by a door-to-door survey carried out by the ARS and Public Health France in the affected area to identify people who may have symptoms ," she continues. "Finally, hospitals, private doctors and medical analysis laboratories in the area are also being mobilised to care for and screen people who may have symptoms of dengue fever."

As a reminder, the symptoms of dengue are most often flu-like (fever, headache, body aches). They appear within three to 14 days (four to seven days on average) following the mosquito bite.
Nearly a thousand cases of imported dengue fever

With the Paris Olympics, and while the tiger mosquito is present in the vast majority of French departments, the authorities and epidemiologists predict an even greater circulation of the virus during the summer, due to the influx of many travelers.

While this is the second indigenous case of dengue fever in France, 934 cases have been imported since May 1, according to the latest bulletin from Public Health France. As a reminder, dengue fever is a viral disease transmitted by mosquitoes to humans, most often benign, which can develop, in about 1% of cases, into a more serious form causing bleeding in particular. Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to an area where the virus is circulating, having been contaminated locally), vector control measures are reinforced by carrying out two adulticide mosquito control treatments in a wider perimeter (300 m). An active case search is also implemented around the outbreaks of indigenous cases, to determine the extent of local transmission and adjust the scope of the vector control treatment if necessary. This active search is carried out jointly by Santé publique France and the regional health agencies concerned. It is based on raising awareness among health professionals in the sectors concerned and the general population, and on door-to-door surveys in the vicinity of known cases to identify possible other cases. Episodes of indigenous transmission of dengue, chikungunya and Zika

The detection of a case of dengue, chikungunya or Zika gives rise to epidemiological and entomological investigations to quickly trigger vector control measures and thus limit the risk of local transmission of the virus. When an indigenous case is identified (person who has not recently traveled to an area where the virus is circulating, having been contaminated locally), vector control measures are reinforced by carrying out two adulticide mosquito control treatments in a wider perimeter (300 m). An active case search is also implemented around the outbreaks of indigenous cases, to determine the extent of local transmission and adjust the scope of the vector control treatment if necessary. This active search is carried out jointly by Santé publique France and the regional health agencies concerned. It is based on raising awareness among health professionals in the sectors concerned and the general population, and on door-to-door surveys in the vicinity of known cases to identify possible other cases. Appropriate measures to secure products from the human body (blood, grafts, etc.) are also implemented.

Tiger mosquitoes infected with dengue virus detected for the first time in mainland France

Dengue fever – or tropical flu – is the main viral disease linked to mosquitoes and for which there is no specific treatment. Prevention is therefore essential. Although historically restricted to the intertropical zone, human infections by the dengue virus have been detected in mainland France since 2010. INRAE ​​scientists, in collaboration with the Claude Bernard Lyon 1 University, the EPHE-PSL and the MASCARA [SUP]1[/SUP] network , intervened on an outbreak of dengue cases detected in the Drôme in 2023. For the first time in mainland France, the researchers found mosquitoes infected with the virus.

This discovery is major because not only does it allow us to confirm the vector species (here the tiger mosquito), but it also offers an innovative way to trace the virus and monitor the epidemiology of the disease easily, quickly and at low cost, by analyzing the contents of mosquito traps in citizens living around detected cases of the disease.
The virus responsible for dengue fever belongs to the Orthoflavivirus genus, like the Zika virus or the West Nile virus . While human infections with the dengue virus have been detected in mainland France since 2010, data to identify the mosquito species involved and trace the virus are lacking. Supported by the local MASCARA network, which includes public health agencies and research laboratories, INRAE ​​researchers and their colleagues analyzed mosquitoes from private traps in a French urban neighborhood affected by a dengue outbreak in late summer 2023.

The outbreak, in the Drôme, included 3 cases, including 2 confirmed indigenous cases (by serology or virus detection), meaning that the patients had not traveled to an area where the disease is present in the last 15 days and had therefore been contaminated on French territory. A 3rd case, identified near the first two following the epidemiological investigation, was classified as imported. The individual had returned from a recent stay in the French Caribbean islands with symptoms compatible with dengue fever, but it had not been diagnosed.

Aedes albopictus mosquitoes (tiger mosquitoes) trapped within 100 m of the infected individuals' area of ​​residence contained the virus. Genetic data from the mosquitoes collected linked this viral strain to the strains responsible for the 2023-2024 dengue epidemic in the French Caribbean islands. This suggests that mosquitoes may have transmitted the virus from the imported case to the indigenous cases, causing the outbreak of infection.

This study indicates that the virus is circulating in metropolitan France via local populations of tiger mosquitoes. It shows the importance of implementing individual and collective control plans for this species [SUP]2[/SUP] , but also of raising awareness among health professionals about diseases transmitted by mosquitoes. Finally, it highlights the importance of entomo-virological surveillance, in particular through the use of private mosquito traps.


Since when have we observed indigenous cases of dengue fever in France?

In metropolitan France, autochthonous dengue infections, i.e. affecting people who had not travelled in the last 15 days to an area where the disease is present, were initially detected in Nice and remained limited to the south of the country until 2018. Since then, the epidemiological profile of dengue has evolved. In 2022 alone, 65 autochthonous cases were identified and this total exceeds the cumulative number of cases reported over the previous 10 years. In addition, the latitudinal limit of dengue clusters observed in metropolitan France has moved northwards, first towards the Auvergne-Rhône-Alpes region in 2019 and then towards the Île-de-France region in 2023.
