Re: Latest Egypt Human Confirmed/Suspected Bird Flu Cases, Feb 8, 2009+

hat tip Fla Medic and Zeinobia -

Zeinobia confirmed to me that she found the information about these 2 new cases in a newspaper but there is no link. I do not know if they are suspected or confirmed H5N1.

Also, they are not the first bird flu cases in Cairo.

H5N1 Human infection reaches to Cairo

The first two human infection cases in Cairo were announced yesterday. Azza Hassen Hafiz "32 yrs old" and her 55 yrs old mother Ansaf Ibrahim Mustafa were infected by H5N1 from the domestice poultry they used to raise. They live in the Sahal area in Cairo. They were transferred to the hospital and were found H5N1 positive. Hopefully they will be cured.

CAIRO, March 4 (Xinhua) -- An Egyptian boy at the age of two years and eight months has contracted bird flu virus, bringing the number of human case of the avian influenza to 57 in the country, a Health Ministry spokesman said Wednesday.
The boy, Abdullah Nagy Amran, comes from the northern Egyptian governorate of Alexandria, some 220 km northwest of Cairo, the state MENA news agency quoted the spokesman Abdel Rahman Shahine as saying.
The boy showed symptoms after contacting with dead birds on Wednesday, and he is in a stable condition after being admitted to a hospital and given the antiviral drug Tamiflu, said Shahine.
This is the second human case of bird flu less than a week in Egypt, where millions of families raise poultry as a source of food and income.
On Sunday, Egyptian Health Ministry confirmed that two-year-old boy Youssef Abdel-Azim from Fayoum governorate, some 85 km south of Cairo, was infected with bird flu virus.
Egypt is the most affected country by the deadly avian influenza outside Asia. It reported its first H5N1 virus in dead poultry in February 2006 and the first human case in March of the same year.
According to the World Health Organization (WHO), some 409 people in 15 countries and regions have contracted the virus and 256 of them died of the disease as of March 2.

Re: Latest Egypt Human Confirmed/Suspected Bird Flu Cases, Feb 8, 2009+

CAIRO - Editor Masrawy - on Wednesday confirmed the incidence of human disease, the seventh session of avian influenza in a child at the age of two years and 8 months from the Alexandria governorate, Ameriya.

And Dr. Abdel Rahman Shahin said the official spokesman for the Ministry of Health and Population on Wednesday evening that the onset of symptoms on the Child Abdullah Naji Umran was entered Tuesday on the impact of Alexandria, a hospital admitted suffering from high fever and cough after being exposed to birds suspected of being infected with bird flu, adding that once the suspicion of being infected with the disease were given the drug Tamiflu, and a description of his sick Palmstqrp.

On the other hand, managed a team of Egyptian scientists in a vaccine for the prevention of bird flu in order to stop imports from abroad, scientists have won the patent and is manufactured locally.

And Dr. Hani Al-Nazer, head of the Egyptian National Center for Research, the vaccine was being imported from abroad at great cost and that is what led scholars to place the search for a local alternative, the same manufacturing and quality.

Nazer said that the problems facing the scientific research in Egypt in the forefront of the budget allocated to it with a one per cent, adding that there were distinct Egyptian scientists in many fields.

The children and women are more vulnerable to the disease, (not true - Fla1) because of the increasing number of people infected with bird flu in Egypt to occur on the path of migratory birds and the spread in poultry houses, which explains that the most of those who died from the disease of women and children.

Experts fear the virus will mutate to become easily transmissible between humans, threatening pandemic that could kill millions of people in different parts of the world.

Vaccine Resistant H5N1 Cases in Egypt Raise Concerns

Recombinomics Commentary 13:26
March 5, 2009

Mr. Yusuf Mohamed Said Abdel Azim of the second village management Yousef province of Fayoum, Friend, developed symptoms on Wednesday and entered the hospital Saturday

Abdullah Nagy Amran, comes from the northern Egyptian governorate of Alexandria, some 220 km northwest of Cairo, the state MENA news agency quoted the spokesman Abdel Rahman Shahine as saying.
The boy showed symptoms after contacting with dead birds on Wednesday, and he is in a stable condition

The above comments describe the two most recent H5N1 confirmed cases in Egypt. Although widely separated in space (see updated map), the two cases are close in time and were likely infected by the vaccine resistant strain of H5N1.

This strain emerged just over a year ago and quickly became dominated in poultry, including vaccinated flocks. The last human case reported last season was infected with the resistant strain and almost all poultry isolates from the late spring and summer were the vaccine resistant strain.

Although NAMRU-3 has not released any poultry or human sequences this season, two of the human isolates were included in a WHO HA phylogenetic tree of vaccine candidates indicate the human isolates had evolved away from last seasons sequences, but were still derived from the vaccine resistant strain.

These data indicate the H5N1 has defeated the extensive vaccine program in Egypt and now was dominant in the poultry and human population. The two cases above were confirmed within two days of each other and media reports suggest there is a familial cluster in Cairo.

This spike in human cases that are likely infected with vaccine resistant H5N1 is cause for concern.

Re: Latest Egypt Human Confirmed/Suspected Bird Flu Cases, Feb 8, 2009+

I think the differences between Egypt and Indonesia remain in the night and day category. In Indonesia, there is an obvious effort to limit confirmed cases. Announcements are made weeks or months after confirmation, and almost all cases are fatal. Media coverage is strong, and almost all confirmed cases are described in detail well before confirmation is announced, and in many cases the media describes lab confirmation, but cases are still not announced.

In Egypt, it is just the opposite. I think the number of cases identified in the media prior to confirmation remains close to zero (I think one was in the media prior to confirmation). However , dozens are hundreds of suspect cases are announced. The most dramatic flurry of suspect cases was a little over a year ago, when dozens of cases were announced daily, but none (one?) was confirmed. Similarly, almost none of the confirmed cases are noted in the media prior to confirmation. Similarly, Egypt has a record or reporting recovered cases. In fact in the spring of 2007, something like 16/17 confirmed cases recovered.

The Egyptian Chronicles recently cited two cases in Cairo, which were supposed to have been stated as confirmed. However, there has not yet been an announcement of the confirmations, which is unusual for Egypt, where confirmations are announced locally and in WHO updates shortly (24-48 h) after confirmation.

Thus, the situation in Egypt may change, but at this point the media reports of suspect cases do not precede confirmations.

Re: Latest Egypt Human Confirmed/Suspected Bird Flu Cases, Feb 8, 2009+

I think a few comments are in order regarding media reports of suspect H5N1 cases, which stemmed in part from the recent report of two confirmed cases in Cairo, which have now been officially declared as false.

While I considered the report as suspect, because of the prior track record of media reports from Egypt, as well as a lack or detail or verifiable source (by name or link), I didn't consider the information as being markedly different than other reports (other than the note that the two cases were CONFIRMED). Most media reports from Egypt only go as far as citing the cases as suspect, which are not confirmed or mentioned further.

I still think there is a place for suspect cases in Egypt, but think threads of suspect and confirmed cases should be separate until there is some evidence that the suspect cases are likely to be confirmed (such as a physician citing some prelimary lab results or some compelling clinical or epidemiological data).

The head of the Directorate of Health Affairs in Cairo, Dr. Abdul Qadir Nepal reports on the emergence of cases of bird flu in the Egyptian capital.

Abdul Kadir said in a press statement that all suspected cases of the disease found in two hospitals, laboratory results proved it was not the disease, except for a sample of a child from the province (Fayoum) at the age of two years he moved to Cairo.

He is receiving the injury cases of suspected bird flu cases infected by the Cairo hospital equipment and medical possibilities, noting that transmission of the cases led to believe the existence of the error cases in Cairo. (End)

H5N1 human infection reaches Cairo
----------------------------------
The 1st 2 human infection cases in Cairo were announced yesterday
[Tue 4 Mar 2009].

A 32-year-old woman and her 55-year-old mother were infected by H5N1
[avian influenza virus] from the domestic poultry they used to raise.
They live in the Sahal area in Cairo. They were transferred to
hospital and were found to be H5N1 positive. Hopefully they will be
cured.
