The Ministry of Health and Population of Egypt has announced a new human case of avian influenza A(H5N1) virus infection. The case is a 4 years old female from El-Edwa district, Menea governorate. She developed symptoms on 21 February and was hospitalized on 24 February. She is receiving treatment and is in a stable condition.
Investigations into the source of her infection indicate exposure to sick poultry in the week prior to onset of symptoms.
Of the 44 cases confirmed to date in Egypt, 19 have been fatal.

CAIRO, March 1 (Reuters) - A 25-year-old Egyptian woman tested positive for bird flu on Saturday, the 45th confirmed human case in the Arab world's most populous country, state news agency MENA said.

Ministry of Health spokesman Abdel Rahman Shaheen said the woman had been treated with the antiviral drug Tamiflu and had been transferred from Fayoum province to a hospital in Cairo, the agency said.

The woman was suffering from a high fever, both her lungs were inflamed, and she was being kept on a respirator and under observation, Shaheen said.

A 4-year-old girl tested positive for bird flu on Monday.

Four Egyptian women died from bird flu in December. Their deaths ended a 5-month pause in human cases in Egypt and brought to 19 the number of Egyptians who have died of the H5N1 bird flu virus since it emerged in Egypt in early 2006.

It is the third winter the virus has struck after lying low during Egypt's hot summers.

Around 5 million households in Egypt depend on poultry as a main source of food and income, and the government has said this makes it unlikely the disease can be eradicated despite a large-scale poultry vaccination programme.

Deaths from bird flu total more than 230 worldwide since 2003 and have been reported in several African and Asian countries. Egypt has been the worst-hit country outside of Asia.

Health experts fear the virus could mutate into a form that spreads easily from one person to another, possibly triggering a pandemic that could kill millions. (Writing by Aziz El-Kaissouni; editing by Keith Weir)

Ministry of Health and Population declare positive AI H5N1 human case in a girl 4 years old (case number 44) from El-Edwa district Minea governorate. The symptoms started February 21, 2008 and admitted to El-Edwa hospital February 24, 2008, the case confirmed today February 25, 2008 11:30 pm by Real Time PCR in Central Public Health Laboratory of Ministry of Health. She has a history of contact with sick poultry in backyard since one week. The case was referred to Manshyet El-Bakry General Hospital in Cairo February 25, 2008. The field investigation was started by Central team from Epidemiology and Surveillance Unit in Preventive Sector and Local Surveillance team to identify any other suspected in close contact and in the village, also to assist the Veterinary Staff in their field activities. The case started Tami Flu since suspected in February 24, 2008 and her general condition is moderate.
This brings up the total number of confirmed human cases of AI in Egypt to 44 with 19 deaths.

CAIRO, March 1 (Xinhua) -- Egypt on Saturday confirmed that a 25-year-old woman was infected with bird flu virus, which was the 45th human infection case in the populous Arab country, the official MENA news agency reported.
The woman had been transferred from a local hospital in Fayyum,85 km south to Cairo, to a hospital in the capital after having developed the symptom of high fever and been treated with the antiviral drug Tamiflu, Health Ministry spokesman Abdel Rahman Shaheen was quoted as saying.
This is the second human bird flu case detected in a week in Egypt after a four-year-old girl on Monday was confirmed being infected with bird flu virus in El Minya governorate, Upper Egypt, some 220 km south of Cairo.
The girl is now still under treatment at Manshiat al-Bakri hospital in Cairo with her condition in stable, according to MENA. The Egyptian government has exerted more efforts to prevent further spread of bird flu since the fatal virus caused four human deaths in less than a week in late December, 2007. Among a total of 45 human cases in Egypt, 19 deaths have been reported since the country detected the first H5N1 virus in dead poultry in February 2006 and the first human case in March of the same year.

The above comments describe the second confirmed H5N1 case in Egypt this week. Like the earlier case, this patient has also developed pneumonia. The recent case has pneumonia in both lungs and is on a respirator. This case is in Fayoum, which is adjacent to the location of the earlier case in Menya.

New cases in Egypt at this time of year are not unexpected, but the two most recent cases are more severe than infections in Egypt a year ago. More severe cases are not a surprise. Last season all cases with M230I were fatal. The recently released poultry sequences indicated M230I was still in circulation in Egypt, with and without V223I, the other receptor binding domain polymorphism in the Gharbiya cluster.

The recently released sequence data identifies an increase in the rate of H5N1 and increased genetic diversity and complexity, which includes non synonymous HA changes signaling vaccine escape.

The recent confirmed human cases follow H5N1 outbreaks in vaccinated flocks, raising additional concerns about the effectiveness of the current vaccine(s) in use.

Release of full sequences from the cases at the end of 2007 in Egypt would be useful.

Accelerated H5N1 Evolution in Egypt

The recently released H5N1 poultry sequences from Egypt demonstrate an accelerated rate of genetic change, coupled with a high frequency of non synonymous changes, which include positions which have changed to multiple amino acids. The dramatic changes signal vaccine escape.

These developments have been recent and appear to be associated with an expanded vaccine program. The dramatic increase in genetic diversity is cause for concern.

Egypt has had over 1000 outbreaks described in OIE reports and the two confirmed cases this week raise the number of reported cases to 45, the highest in any country outside of Asia. Moreover, the two cases this week developed pneumonia, in contrast to the cases reported a year ago.

The recent HA sequences have multiple positions which have changed to more than one amino acid. Position 144 has had three recent changes, R144K, R144I, and R144S. Similarly, position 158 has changed to two amino acids, D158N and D158G. This type of rapid change may signal escape from recent vaccines.

These data raise serious concern about the rate and direction of H5N1 evolution in Egypt. Release of the human H5N1 from cases at the end of 2007, as well as the two cases reported this week would be useful.
