She is on the life-support system and her health condition is critical.

It's noticeable the pregnancy status of the patient, since she's in critical condition; pregnancy seems to be linked to a poor illness outcome.

طالب أحمد إسماعيل أبوطالب، زوج الحالة رقم ٦٤ سعدية أحمد عبداللطيف حامد ?٣٣ سنة?، التى أعلنت وزارة الصحة عن إصابتها بمرض أنفلونزا الطيور، بنقلها من مستشفى حميات كفر الشيخ إلى مستشفى حميات العباسية، لعدم وجود الرعاية الكافية بمستشفى كفر الشيخ - على حد قوله.

وطالب بتدخل وزير الصحة لتحقيق رغبته فى هذا المطلب، لضمان عناية أكثر بزوجته، وقال إن هناك نقصاً شديداً فى أسطوانات الأكسجين بالمستشفى، لأن قوته ١٣ أسطوانة فقط، ويتم استخدام من ٦ إلى ٨ أسطوانات يومياً، وأضاف: ?كانت الأسطوانات قد انتهت، أمس الأول، واضطررت لتأجير عربة نصف نقل على حسابى الخاص والتوجه إلى مستشفى كفر الشيخ العام لتبديل أسطوانة أكسجين?.

وأضاف أن حالة زوجته كانت قد تحسنت ووصلت نسبة الأكسجين بالدم ٤٦٪، إلا أنها انخفضت مرة أخرى إلى ٣٥٪ بسبب نقص الأكسجين، مما يضر بصحتها، معرباً عن قلقه من نقص الإمكانات فى المستشفى، وعدم تشغيل جهاز فحص نسبة غازات الدم حتى الآن رغم إحضاره منذ أربعة أيام فقط.

وطالب بتدخل وزير الصحة لعدم وجود إخصائى عناية مركزة بمستشفى حميات كفر الشيخ، وأن جهاز فحص الدم معطل والفنى لم يستطع تشغيله، وأن المريضة مهددة بالوفاة لعدم وجود الرعاية بالمستشفى.

وفى الغربية، احتجز مستشفى حميات طنطا، أمس، حالتين جديدتين للاشتباه فى إصابتهما بمرض أنفلونزا الطيور.

وقال الدكتور شريف حمودة، وكيل وزارة الصحة بالمحافظة، إن الحالتين لسيدة تبلغ من العمر ?٢٣ عاماً? وتدعى بشرى أحمد موسى، ونجلتها سلمى محمد ?عام ونصف العام?، بعد شعورهما بضيق فى التنفس وإحمرار الوجه وارتفاع شديد فى درجة الحرارة،

وتم احتجازهما داخل المكان المخصص لمثل هذه الحالات فى المستشفى، وتم أخذ عينات دم ومسحة حلق منهما، وإرسالها للمعامل المركزية بوزارة الصحة لبيان مدى إصابتهما بالمرض من عدمها.

A 25-year-old Egyptian woman has contracted the highly pathogenic bird flu virus after coming into contact with infected birds, the latest case in a growing spate of infections in Egypt, state media said on Friday.

The woman, from El-Marg on Cairo's outskirts, was in a critical condition on a ventilator after falling ill with a fever on April 11.

The above comments describe the latest confirmed H5N1 case in Egypt. Earlier cases had been predominantly (10/11) toddlers (age 1 ? to 2 ?) with mild H5N1 (only one of the 10 had been characterized as ?critical?) and all have recovered, as did the first adult (38F).

Recently however, the cases have been critical (6M, 33F, 25F) and these cases have had a larger gap between disease onset day and the start of oseltamivir treatment. The delay in the first such case (6F) was due in part to a denial if contact with dead or dying poultry. In Egypt, patients with bird flu symptoms and stated contact with dead or dying poultry leads to the immediate start of osletamivir treatment, which is generally prior to test result.

The above comments that the patient had contact with infected birds suggest that the birds have been H5N1 confirmed, but there has been no confirmed birds reported in the area of the most recent case (see updated map). The closest H5N1 confirmation was the 6M case, raising concerns of a more efficiently transmitted H5N1.

There is concern that the mild toddler cases is linked to a 3 BP deletion that was in the H5N1 in a Qena cluster in 2007, when there was another series of mild cases (16/17 survived, and 16/17 were children between ages of 3-10). However, some of the older cases infected with H5N1 with the deletion died or survived after being in critical condition.

Therefore, the release of sequences from the 2009 sequences would be useful. The first two sequences were included in a WHO phylogenetic tree, which suggested that those two mild toddler cases had the deletion.

Identification of the ?infected birds? in northeastern Cairo would also be useful.

I wonder why the hospital waited so long to start in with the Tamiflu....
The 6M was also delayed and also treated at the same hospital. I suspect the delay was linked to a denial of poultry contact.
This is a teaching hospital in Cairo and must be aware of the policy in Egypt to treat suspect cases immediately.

Note that 33F developed symptoms April 7, raising concerns that the toddler is her child and she infected the toddler. No mention of toddler in WHO update, but WHP update on Beheiura cluster failed to state that they were cousins and next door neighbors

17 April 2009 -- </SPAN>The Ministry of Health of Egypt has reported a new confirmed human case of avian influenza . The case is a 33 year old female from Kellin district, Kfr El Sheikh Governorate. Her symptoms began on 7 April and she was hospitalized at Kfr El Sheikh Fever Hospital on 15 April where she was started on oseltamivir the same day (15 April). She is in a critical condition.
Infection with H5N1 avian influenza was confirmed by the Egyptian Central Public Health Laboratory on 15 April.
Investigations into the source of her infection indicate a history of close contact with dead and sick poultry prior to becoming ill.
Of the 64 cases confirmed to date in Egypt, 23 have been fatal.

Another H5N1 Human Infection case in less than 72 hours

This is not good.

The ministry of health has announced today that year and half Manar Ibrahim Zidan is No.66 in the H5N1 human infection roll in Egypt. This is so fast , it is less than 48 hours from the announcement of Case No.65 !!

Baby Manar is from a village in Kafr El-Sheikh ,Delta, there were several case from Kafr El-Sheikh this year, I recall case No.64 Mrs. Sadia is from Kafr El-Sheikh too . I hope someone will make a map like the one posted last year with the infections locations and compared it with last year?s map. I think Al-Fayoum,Bahaira and Kafr El-Sheikh are the top governorates with the highest H5N1 infection rate.

It seems that we are going to back to babies with H5N1 after all. Anyhow Manar has entered the hospital on the 15th of April and she was given Tamflu and thank God her condition is stable .
