China woman infected by bird flu, recovering
Sat Jan 31, 2009 7:18pm IST

SHANGHAI (Reuters) - A 21-year-old woman has caught the H5N1 strain of bird flu in central China's Hunan Province but is recovering in hospital, the official Xinhua news agency said on Saturday.

The farmer was found sick on Jan. 23 after coming into contact with birds that died of the disease, Xinhua quoted provincial health authorities as saying.

People who had close contact with the patient are under observation, but none of them has fallen ill so far. China has reported the case to the World Health Organization, Xinhua added.

On Monday, China said a young man had died of the H5N1 strain of bird flu in the southwestern region of Guangxi, the country's fifth reported death from the disease this month.

H5N1 remains largely a virus among birds, but experts fear it could mutate into a form that is easily transmitted by humans, which could kill millions of people worldwide.

Since the virus resurfaced in Asia in 2003 it has infected about 400 people worldwide and killed over 250 of them, according to WHO data released earlier this month.

<!-- end .related-media -->BEIJING (AFP) – A 21-year-old woman has been diagnosed with the H5N1 form of bird flu in central China, state media reported Saturday.
The woman, a farmer from Hunan Province was hospitalised on January 26, three days after falling ill, the state-run Xinhua news agency said, adding that her condition had since improved.
The report said China's Centre for Disease Control and Prevention had confirmed it was the H5N1 form of the disease, which can be fatal to humans, and had reported the case to the World Health Organisation.
Five people are reported to have died from bird flu this year in China, a country considered to be one of the nations most at risk from bird flu epidemics because of the large poultry population that lives in close proximity to humans.
At least two other people in China fell ill with avian influenza in January, including a two-year-old girl who contracted the disease in Hunan.
Health authorities in Hunan ended a week-long quarantine of 157 people who had close contact with the toddler and a teenage boy who died in the province, saying none had developed any flu-like symptoms, Xinhua said Tuesday.
So far, 25 people have died from avian influenza in China since the disease re-emerged in 2003, according to World Health Organisation figures.

Hunan reports bird flu case
Created: 2009-2-1

A 21-YEAR-OLD female farmer was confirmed to be infected with bird flu in central China's Hunan Province, the provincial health bureau said yesterday.

The farmer, surnamed Shu, fell ill on January 23 in Xupu County and was hospitalized at the county's People's Hospital on Monday, said a report from the bureau.

Shu was transferred to another hospital in Changsha, the provincial capital, on Thursday. So far her condition has been stable and she is said to be recovering.

According to the results of a test conducted on Friday by the National Center for Disease Control and Prevention, the patient tested positive for the H5N1 strain of avian influenza.

An investigation found that Shu had been in contact with fowl that died of the disease before becoming sick.

Hunan has reacted swiftly to curb the spread of the virus. Those who had close contact with the patient are under observation, but none has fallen ill so far.

The health ministry said it had reported the bird flu case to the World Health Organization and informed the health authorities of Hong Kong and Macao special administrative regions as well as relevant countries.

Another Confirmed H5N1 Case in Hunan China Raises Concerns
Recombinomics Commentary 13:53
January 31, 2009

female, 21 years old, farmers, who lives in Hunan Province溆浦县. Patients January 23, 2009 in the incidence溆浦县whose condition had worsened in the January 26 People's Hospital溆浦县treatment, January 29 transferred to Changsha Xiangya Hospital, Central South University hospital. At present, the basic stability of patients, clinical symptoms improved significantly and are actively in treatment.

results for the birds H5N1 influenza virus nucleic acid positive. Epidemiological survey found that patients had come into contact with dead poultry

The above translation from the Ministry of Health website describes the eighth H5N1 confirmed case in the past several weeks (see updated map). Unlike the earlier cases, this victim is not in critical condition, but the treatment location is near two earlier Hunan cases. One (2F) was H5N1 confirmed and recovering. Her mother died with bird flu symptoms, when her daughter developed symptoms, confirming that the mother infected her daughter, as indicated by matching symptoms as described by the husband/father of the victims. However, no sample was tested from the mother, so she is not listed as a confirmed case by the WHO, and in fact is ignored in the WHO statement that all contacts linked to the confirmed case were "healthy". This denial of H2H transmission is repeated by comments in ProMED as well as subsequent statements by WHO, which have no credibility or scientific basis.

The above translation also notes positive H5N1 poultry linked to the latest case, which is contradicted by statements associated with the earlier Hunan cases which noted that no H5N1 positive poultry was found in Hunan or Shanxi. The absence of H5N1 in poultry is supported by the absence of any recent OIE H5N1 reports from China, other than the two vaccine resistant December outbreaks in Jiangsu.

Thus, China has not filed any OIE reports on H5N1 positive poultry linked to the eight confirmed cases, including the most recent case. The clustering of three cases in Hunan, and the spate of confirmed cases throughout the country signal a more efficient transmission of H5N1.

Sequence data has not been released for any of the confirmed cases. The poultry outbreak in Jiangsu is said to be related to earlier outbreaks in Shanxi, which was clade 7. However, specifics on the isolate from the case in Beijing, A/Beijing/1/2009 have not been released. It was said to be similar to H5N1 isolates in China collected between 2005-2008, but that description would apply to clade 2.3.4, which has been linked to the majority of prior human infections in China, but the description would also apply to clade 7, which was isolated in Hunan in 2006, as well as multiple northern locations in China, which had multiple poultry outbreaks in 2005/2006 which were not controlled by the poultry vaccines.

A poor match between vaccines directed against clade 2 and clade 7 is expected. WHO has selected a clade 7 vaccine target from a 2008 isolate in Vietnam, and WHO has acknowledged that the cross reactivity between clade 1 and 2 vaccine with clade 7 is poor, but WHO has withheld the sequence of the vaccine target, which is indefensible.

Release of the sequences from the human and poultry outbreaks in China, as well as outbreaks in Vietnam, including the clade 7 vaccine target are long overdue.

Is China Covering up a New Bird Flu Epidemic?

Posted by: Frederik Balfour on February 03

Is China covering up a new outbreak of bird flu? Certainly there seems to be very strong evidence it is. There have already been eight reported cases of humans contracting the potential deadly H5N1 virus, from which five people have died this year. And despite the fact that Hong Kong officials have been finding dead birds infected with the virus washing up onto its shores in recent days from the mainland, China has not made any official statement concerning an outbreak among birds. At least one Hong Kong health advisor to the government, Lo Wing-lok, says the government “just isn’t admitting” to the problem.
If this is true, both Chinese health officials and the state media must share the blame. Surely after so many human infections people must be asking questions of how the caught the virus, as human to human transmission is highly unlikely. But China has a horrible track record of squelching bad news at the cost of public safety, usually with the complicity of the local media. Back in 2003 Guangdong provincials covered up the SARS epidemic for 22 weeks before informing neighboring Hong Kong. By that time it was too late, and nearly 300 people died of SARS in Hong Kong, as did hundreds more worldwide. You would have thought that China had learned its lesson back then.
But as last summer’s melamine milk scandal showed, China is still unwilling to inform the public of health hazards in a responsible and timely manner, with tragic consequences. Had the problem come to light sooner, the lives of four infants who died from poison milk might have been saved, and while hundreds of thousands of other kids might have avoided contracting kidney disease.
How many more people must die before Beijing takes the muzzle off its health officials and reporters?
