A study on patients with the Covid-19 coronavirus allows a better understanding of the profile of the patients as well as the aggravating factors of the disease.

As the weeks go by, knowledge of the Covid-19 coronavirus that has hit China since December 2019 is becoming clearer. Several research teams are trying to better understand how the virus behaves and what its characteristics are. New work, conducted by the State Key Laboratory of Respiratory Disease in Guangzhoun, in partnership with several hospitals in the city of Wuhan, where the epidemic has exploded, has just been shared on Medrxiv, a site for scientific publications whose articles have not been reread by a scientific committee (as is the case in conventional journals.) However, this work sheds light on the symptoms and aggravating factors around the coronavirus.

Coronavirus patients, weakened by other pathologies

Carried out on a sample of 1099 patients in whom the presence of the virus has been confirmed by DNA analysis, they allow a better understanding of the profile of the patients treated. Those in critical condition present several factors which seem to worsen their pathology, such as hypertension in 23.7% of cases, then diabetes, which concerns 16.2% of the most serious cases.

Other factors could come into play, such as smoking. Of course, 85.6% of infected patients are non-smokers. But there are more severe cases among smokers (16.9%), while smokers are slightly less (11.8%) with a less severe form of the disease. Earlier this year, a team of Chinese researchers published a study on the profile of infected people in the specialized journal The Lancet. The study involved only 99 patients, but in about half of the cases (50 patients), coronavirus infection occurred in people who already had an underlying pathology, such as cardiovascular disease (40) and diabetes (12).
Median age of coronavirus patients set at 47

The median age of patients in this study is 47 years, compared to 55.5 years mentioned in the previous study published in the Lancet. The proportion of women and men is also slightly different. Women represent 41.8% of patients, while the study published by The Lancet included around 23%. Very different figures which can be explained by the fact that the previous study published in the Lancet only concerned a sample of 99 people, compared to 1099 for the study published in Medrxiv. The latter fixes the median incubation period is 3 days. It is established in a time ranging from 2 to 14 days.

The new study published in Medrxiv also confirms the symptoms that had already been described in previous official publications. This new work describes three major symptoms of COVID-19 coronavirus: body temperature above 38 degrees (87.9% of patients), coughing fits (67.7%), and severe fatigue (38.1%) . However, "this study shows that fever was only present in 43.8% of patients with the coronavirus at the time of their management, while it rose to 87.9% during hospitalization."

The exams also show leukopenia, an abnormal drop in the number of white blood cells in 82.1% of patients and a lung condition detectable on CT in only 76.4% of patients. "Many patients present without symptoms of fever but with abnormalities on the pulmonary scanner. Very few of them have diarrhea," conclude the researchers.
Fever and cough in the majority of patients

The older Lancet study mentioned that all patients admitted to hospital had pneumonia. Most of them (74 out of 99 patients) had both lungs infected. The majority (82 patients) had fever, cough (81 patients) and a third were short of breath (31 patients). Five severely ill patients co-infected with bacteria (1 person) and fungi (4).

On the treatment side, among the 1099 patients studied, 57.5% receive antibiotics intravenously, 35.8% have antiviral treatment by oral route and 18.6% of corticosteroids. In all, severe cases represent 18.17% of the patients observed in this work. Out of 1099 patients, 55 were able to return home, cured. Of the 99 patients seen in The Lancet, most received antivirals - 75 patients, combined with antibiotics (70) and oxygen therapy (75), which involves supplementing the patient with oxygen.

The report shows that as of February 11, 72,314 cases were reported. Among them, 44672 cases were confirmed (61.8%).

An analysis of 1688 confirmed cases with severe severity was performed in 1080 cases (64.0%) in Wuhan, 394 cases (23.3%) in Hubei other than Wuhan, and 30 provinces in the country except Hubei (Districts and cities) 214 cases (12.7%).

The severe illness rate of medical staff was 17.7% in Wuhan, 10.4% in Hubei (except Wuhan), and 7.0% in the country (except Hubei). According to different time periods, the severe illness rate of Wuhan medical staff gradually decreased from 38.9% at its peak to 12.7% in early February.

Of the 1,688 confirmed cases by medical staff, most cases were mild patients (85.4%), and the mortality was lower than other cases. The main reason is related to age. Medical staff are all working staff, generally under 60 years old, and deaths mainly occur in patients over 60 years old.


The report mentions that, as of now, there is no evidence that a "super communicator" incident has occurred in any of the medical institutions serving patients with the new coronavirus pneumonia.

The report reminds that serious medical staff infections have occurred in some areas of Wuhan and Hubei, but the specific causes of medical staff infections and protection failures have yet to be investigated further. (Finish)
