Chile 250 influenza H1N1 cases, two people are still serious
Saturday May 30 2009 19:20 GYT

SANTIAGO (Reuters) - Chile on Saturday reported 26 new confirmed cases of the feared H1N1 flu, which reached a total of 250 infected, including children and adults, two of whom remain in serious condition.

In his daily report, the Health Ministry said that two more people have no serious deterioration in his condition.

"In the past few hours, people are both stable in severity, with assisted ventilation and have not evolved negatively," said Secretary of State's report.

Of total cases, first appeared in a city of Arica in the extreme north of Chile, while in the Chilean capital amounting to over 65 and the remainder in other regions of the country, with a high proportion of children contagion.

SANTIAGO .- A rose seven serious cases of human influenza, according to the latest report issued by the Public Health Institute, which amounted to 369 new infections at country level, confirming that confirm the additional nine days were affected.

Of those who presented clinical severity, one comes from the city of Chill?n (Regi?n del B?o B?o) and six other Puerto Montt (Lake District).

The health authority said that in Chile 98 percent of the cases have been mild, and has presented tables 1.9 and 0.2 serious deceased (Fernando Maldonado Vera, 37, whose death occurred on Tuesday in the south).

In the morning, President Michelle Bachelet, accompanied by Minister of Health, ?lvaro Erazo, led a workshop with the highest authorities of the portfolio and the Committee of Experts that advises decision makers to manage the country of influenza A (H1N1).

Among the items discussed at the meeting was the analysis of the evolution of the virus, measures to strengthen the levels of care (mainly in primary care), with higher staffing, training and delivery of clinical guidelines, and outreach the course and development that is taking the epidemic in Chile.

Subsequently a meeting was held at the Ministry of Health, which experts called intensivists to define actions for the treatment of severe cases that may arise.

It was decided to send to Puerto Montt for a group of experts formed by Elena Pedroni (PAHO expert Argentina), Maritza Garcia (Department of Epidemiology Minsal), Veronica Esp?nola (Head of the Department of Infection Intrahospitalarias Minsal) and Mario Calvo (infectologists from Universidad Austral in Valdivia) and Arancibia Francisco (president of the Chilean Society of Intensive Care Medicine), with the aim of analyzing in detail what is happening in the city and look at some additional measures that can be implemented, given the high number of infection by the disease.

Thursday begins the work in the field of five experts in the Lake District that aims to find out the reasons why in this area were six of the seven cases of severe influenza AH1N1 and has been the only death.

Mario Calvo Arellano, infectologists of the Institute of Medicine of the University of Southern, is one that integrates the group and explained that one of the assumptions that drive has to do with chance.

"All human beings are different and there are people who react to the same virus with a very severe disease and others with the same virus can cause a mild disease," he said.

Another possibility is the cold of the region that helps "the poor quality of the defenses of the upper airway," and that "the organism and produce infection arrives, this is a situation that facilitates the spread of infection is much easier .

"The possibility that one of those cases resulting in an infected severe box is much higher," he said.

Another way to understand the speed is the way we live as people usually share indoors due to rain.

Meanwhile, Debbie Guerra, president of the College of academic anthropologists and public health, said that one important point is the perception of people about the emergency care system in the region because it depends on the decision and the time that served with a specialist.

"The other element is this notion that 'I am not going to happen,' and in that sense, we have practices that are not necessarily prevent disease, not to attend to medical services, suggests that influenza is a common and that we will self, "he said.

Until Sunday, the five experts working in Puerto Montt assessing, among other things, the management of seriously ill patients in order to avoid more deaths.

Six of the seriously ill from flu are AH1N1 of Puerto Montt
Minsal sends medical team to examine cases. Complex patients are hospitalized.
by A.M. Morales and J.R. Maldonado - 04/06/2009 - 07:09 Of the seven patients for severe influenza AH1N1 in the country, six are of Puerto Montt. This region is the second most infected people, after the Metropolitan. So much so that even the director's ill health service Reloncav?, Eugenia Schnake.

Is this health situation which prompted the government to send a team of experts to investigate the causes of the outbreak, which is composed of the adviser of PAHO Elena Pedroni.

The issue was discussed yesterday at La Moneda, in a meeting headed by President Michelle Bachelet, health authorities and the group of expert advisers on this issue. This, following the death of Fernando Vera (37), the first fatal victim of avian AH1N1 in the country.

During the meeting, the experts said that for every confirmed case of human influenza, there are another 10 infected. Ie, the number of infected could skirt today the four thousand, as argued by Dr. Carlos Perez, infectologists of UC.

PUERTO MONTT
Dr. Jorge Jimenez de la Jara said that there is no clear explanation of why so many cases recorded Puerto Montt and why most of the major categories corresponding to the area. These are young people who have no link or common household or workplace. All developed a viral pneumonia. "Tables are simple in a complicated moment to another," he said.

"It works in various scenarios, ranging from models of living in Puerto Montt and Chiloe, which is made around the kitchen and where, therefore, is most contagious until hiperreacciones of people whose immune system reacts very strongly against a new virus, "said De la Jara.

The fear of the disease that caused many users come to the Hospital de Puerto Montt. Some of them complained about the attention.

CRITICAL PATIENTS
One of the six seriously ill patients is Pavez Ismael Gonzalez (40). The accountant is the company Aguas Claras salmon and was interned in the Cl?nica Los Andes May 28 last, with a severe respiratory failure that forced him to be connected to a mechanical ventilator. With its picture day worsened, it was decided to move it to the Hospital Cl?nico Universidad Cat?lica, where he is hospitalized since Monday.

The doctor Thomas Regueira, from the intensive, said yesterday that Pavez is in "extremely serious". In view of this is connected to an artificial lung and extracorporeal membrane oxygenation (ECMO). Has a severe lung failure, with involvement of both lungs, which keeps him with life-threatening.

Health Minister Alvaro Erazo said that following the emergence of several serious cases were given the instruction to all emergency services in the country (public and private) that any patient who arrives with severe flu symptoms have been immediate hospitalization.

He added that there is strengthening emergency care, through recruitment and training of clinical teams, in addition to the delivery of diagnostic and treatment guidelines. It was also recommended to health care the separation of suspected influenza patients AH1N1 the rest of the population in emergency consultations and use of masks.

Parliamentarians from different sectors criticized actions taken by the government to contain the outbreak. Mrs Karla Rubilar (RN) stated that the massive infection, the government should order the phase 6 of the epidemic and deal with more resources.

Spread of the virus A (H1N1) in Puerto Montt:
Fifteen health officials are infected with influenza

Epidemiologists Minsal be a week in the area looking for information to help understand aspects of the infection. It has been established that some are worse and others do not.

SOLEDAD NEIRA FAR?AS

At least ten of the Base Hospital in Puerto Montt and five Health Service Reloncav? (SSR), including its director, Eugenia Schnake, were confirmed with medical leave after their infection with influenza A (H1N1).

This forced to seek urgent replacements. "We are hiring more people to the urgency. Sapu and strengthen, with doctors and nurses, and bought two more mechanical fans, one for adults and children," confirmed the director (s) of the SSR, Monica Winckler.

The expert acknowledged that there is concern among health personnel. "They feel more exposed than the rest of the community people, which is true," he said.

They are not the only restless. Psychosis in the population reaches the point where people come to Sapu because their employers have been required to ensure that examinations are not with influenza.

Winkler said the community must understand that if you're not sick, should not go to health centers, including those chronic patients, who recommended to seek to send their drugs to prevent the spread and do not saturate.

Experts in the field

Yesterday came the team of epidemiologists announced by the Government which, together with experts from the region, consider the infection, but require at least a week to evacuate conclusions.

The SEREMI Health Bernardo Martorell, details to be sizing up the size, to characterize the behavior and set the type and speed of transmission of the virus, among others, to predict their potential effects, "because this could be happening is a phenomenon that after could move to other parts of Chile or the world. "

They must also identify cases, see if any more than other years and more in depth study to assess whether there are serious risk factors. "To understand better the transmission, to improve management in health services," he stressed Martorell.

Elena Pedroni, epidemiologist for the Pan American Health Organization (PAHO), said that studying the case of Puerto Montt can serve as a model of how to tackle the epidemic in other countries. And confirmed that after Puerto Montt to Ecuador and Paraguay will continue to monitor the issue further.

The specialist said that a major unknown is why some cases are worsening more and stressed that the epidemic in Chile until now nothing more than what has been elsewhere in the world. "

Pedroni agreed the concern of health workers. "We need to prevent outbreaks intrahospitales, and we must train staff on infection control and use of protective equipment for individuals, for example," detail.

Contracted severe

A serious case was added yesterday to the list of critically ill patients by infection of human influenza. This is Mario Chaparro Jimenez (52), who remains connected to a mechanical ventilator in the ICU of the Hospital Regional de Concepci?n. It was reported that developed the virus while he was interned by a brain stroke.

Last night, moved from Melipilla admitted to the Hospital Cl?nico UC, Ampuerto Eduardo Zamorano, 75, (pictured), who also was infected grave.

The counter Ismael Pavez, 40 years, Puerto Montt, is "very serious and life threatening" connected to an extracorporeal membrane oxygenation (ECMO) in the same hospital.

Sf, to give an answer on Your question:

ECMO is a method to treat severe respiratory failure (ARDS). ECMO stands for Extracorporeal Membrane Oxygenation. An ECMO machine is similar to a heart-lung machine. To initiate ECMO, cannulae are placed in large blood vessels to provide access to the patient's blood. Anticoagulant drugs are given to prevent blood clotting. The ECMO machine continuously pumps blood from the patient through a "membrane oxygenator" that imitates the gas exchange process of the lungs. Management of the ECMO circuit is done by a team of ECMO specialists in intensive care units.

To run an ECMO-system and before using it, the circuit will be primed with packed red blood cells and FFP (fresh frozen plasma). Losses of blood in ECMO-therapy situation are not rare, because of technical aspects (large cannulae, blood clotting) and because of bleeding complications due to anticoagulant drugs.

Rh negative blood group (Rh = rhesus factor) is rare (only about 15 % of the people are rhesus negative). Compatibility of packed red blood cells has to be tested in order to use only matched blood transfusions.

While stocks of certain packed red blood cells are running short from time to time, initiation and continuation of ECMO may become critical in certain situations.

There are some new technical methods of lung support going to be developed (iLA), for example:
